Conversion treatment for advanced intrahepatic cholangiocarcinoma: Opportunities and challenges.

The prevalence of intrahepatic cholangiocarcinoma (ICC) is increasing globally. Despite advancements in comprehending this intricate malignancy and formulating novel therapeutic approaches over the past few decades, the prognosis for ICC remains poor. Owing to the high degree of malignancy and insidious onset of ICC, numerous cases are detected at intermediate or advanced stages of the disease, hence eliminating the chance for surgical intervention. Moreover, because of the highly invasive characteristics of ICC, recurrence and metastasis postresection are prevalent, leading to a 5-year surviv

Impact of Pharmaceutical Prophylaxis on Radiation-Induced Liver Disease Following Radioembolization.

Radioembolization (RE) with yttrium-90 (90Y) resin microspheres yields heterogeneous response rates in with primary or secondary liver cancer. Radiation-induced liver disease (RILD) is a potentially life-threatening complication with higher prevalence in cirrhotics or patients exposed to previous chemotherapies. Advances in RILD prevention may help increasing tolerable radiation doses to improve patient outcomes. This study aimed to evaluate the impact of post-therapeutic RILD-prophylaxis in a cohort of intensely pretreated liver metastatic breast cancer patients; Methods: Ninety-three patient

Radioembolisation and portal vein embolization before resection of large hepatocellular carcinoma.

Resectability of hepatocellular carcinoma in patients with chronic liver disease is dramatically limited by the need to preserve sufficient remnant liver in order to avoid postoperative liver insufficiency. Preoperative treatments aimed at downsizing the tumor and promoting hypertrophy of the future remnant liver may improve resectability and reduce operative morbidity. Here we report the case of a patient with a large hepatocellular carcinoma arising from chronic liver disease. Preoperative treatment, including tumor downsizing with transarterial radioembolization and induction of future remn

STING agonist-loaded cationic radioactive microspheres enhance transarterial radioembolization of hepatocellular carcinoma via tumor immune activation.

Hepatocellular carcinoma (HCC) is a severe health condition that poses a significant threat to life, characterized by high incidence and mortality rates. Transarterial radioembolization (TARE) is expected to be a suitable method for the treatment of advanced HCC. Current radioactive microspheres used for internal radiation therapy in HCC treatment suffer from suboptimal embolic effects, a propensity for non-target embolization, and the potential for radiotherapy to adversely impact the tumor immune microenvironment. To address these limitations, we have engineered cationic quaternary ammonium

Yttrium-90 radioembolization treatment strategies for management of hepatocellular carcinoma.

As the third leading cause of cancer-related deaths worldwide, hepatocellular carcinoma (HCC) represents a significant global health challenge. This paper provides an introduction and comprehensive review of transarterial radioembolization (TARE) with Yttrium-90 (Y90), a widely performed transcatheter procedure for HCC patients who are not suitable candidates for surgery. TARE involves the targeted delivery of radioactive microspheres to liver tumors, offering a promising treatment option for managing HCC across various stages of the disease. By evaluating Y90 TARE outcomes across early, inter

Chemoembolization and radioembolization for metastatic disease to the liver: available data and future studies.

Hepatic metastatic disease includes tumors from colorectal, neuroendocrine, breast, melanocytes, kidney, and other primary sites. Tumor characteristic, liver function, and performance status are factors that need to be considered while deciding on treatment. Surgery offers the most optimal therapeutic option for such patients. However, due to the diffuse nature of disease and large tumor burden, a majority of the patients are not operable. Moreover, because the morbidity and mortality is associated with hepatic metastatic disease, it is intuitive to investigate and develop treatment options th

Current Treatment Paradigm and Approach to Advanced Hepatocellular Carcinoma.

Hepatocellular carcinoma (HCC) is one of the most common forms of primary liver cancer worldwide. Herein, we present a review article that provides a broad overview of the current landscape of HCC, including the etiology, potential risk factors, and molecular pathways that can serve as potential therapeutic targets. The risk factors tend to vary depending on the geographic distribution; hepatitis B-induced cirrhosis and HCC occur more frequently in Asia and Sub-Saharan Africa, whereas metabolic disorders are the culprits in Western Europe and the Americas. The exact molecular alterations that

Preoperative radioembolization versus chemoembolization and upfront resection for resectable hepatocellular carcinoma beyond the Milan criteria.

BACKGROUND: As the role of transarterial radioembolization (TARE) in resectable hepatocellular carcinoma (HCC) beyond Milan criteria remains undefined, this study compared preoperative TARE and chemoembolization (TACE) with primary resection to establish the optimal management strategy. MATERIALS AND METHODS: We retrospectively analyzed HCC patients who did not meet the Milan criteria and underwent primary liver resection, preoperative TACE, or preoperative TARE. Overall survival (OS), time to recurrence (TTR), and recurrence-free survival (RFS) were analyzed using Kaplan-Meier estimates and

Optimization of 99mTc-SPECT in the presence of 90Y for radioembolization.

BACKGROUND: 99mTc-macroaggregated albumin (MAA) imaging is part of the standard work-up procedure for radioembolization using 90Y microspheres. In certain scenarios, it may be warranted to visualize the distribution of 99mTc in co-presence of 90Y, for example when validating intra-procedural 99mTc-MAA imaging after 90Y-therapy to enable single-session radioembolization. Another instance involves additional 99mTc-MAA administration during the therapeutic procedure itself, e.g. when initial imaging reveals insufficient targeting of a specific liver segment. In these situations, crosstalk from 90

99mTc/90Y radiolabeled biodegradable gel microspheres for lung shutting fraction assessment and radioembolization in hepatocellular carcinoma theranostics.

Transarterial radioembolization (TARE) is a well-established clinical therapy for the treatment of patients with intermediate to advanced hepatocellular carcinoma (HCC) or those who are ineligible for radical treatment. However, commercialized radioactive microspheres still have some issues, such as high density, complicated preparation, non-biodegradability. Furthermore, the use of different radioactive microspheres during TARE and lung shunt fraction assessment has led to inconsistencies in biodistribution in certain cases. This study employed biodegradable hyaluronic acid (HA) as the backbo

Uveal melanoma: physiopathology and new in situ-specific therapies.

Uveal melanoma is the most common primary intraocular tumor in adults. It can arise from melanocytes in the anterior (iris) or posterior uveal tract (choroid and ciliary body). Uveal melanoma has a particular molecular pathogenesis, being characterized by specific chromosome alterations and gene mutations (e.g., GNAQ/GNA11; BAP1), which are considered promising targets for molecular therapy. Primary treatment of uveal melanoma includes radiotherapy (brachytherapy and charged-particle therapy), phototherapy (photocoagulation, transpupillary thermal therapy, and photodynamic therapy) and surgery
