The Latarjet Procedure May Induce Pathokinematics with Posterior Humeral Head Subluxation: An Experimental Dynamic Radiostereometric Study.

BACKGROUND: The effectiveness of the Latarjet procedure in stabilizing the glenohumeral joint (GHJ) in the abducted and externally rotated position is well documented. However, evidence of its ability to restore the GHJ kinematics in other positions and without anterior-directed load is sparse. The purpose of this study was to evaluate the GHJ kinematics, throughout external rotation, following the Latarjet procedure in shoulders with 15% anterior glenoid bone loss. METHODS: Eight human donor arms were examined using dynamic radiostereometry during GHJ external rotation with anterior-directed

Innate immune response and implant loosening: Interferon gamma is inversely associated with early migration of total knee prostheses.

To allow prediction of the risk of loosening prior to surgery, we investigated the relationship between innate immune cytokine response via TLR2 stimulation and early migration of six different knee prostheses using RSA (radiostereometry). This study included 114 patients of a prospective RSA-cohort who received a total knee arthroplasty. Whole blood cytokine responses were obtained by ex vivo stimulation with tripalmitoyl-S-glycerylcysteine (Pam3Cys-SK4) for assessment of the TLR2 immune response. Early migration was calculated using the maximum total point motion (MTPM) 1 year post surgery.

Imaging Modalities to Assess Fracture Healing.

PURPOSE OF REVIEW: This review discusses imaging modalities for fracture repair assessment, with an emphasis on pragmatic clinical and translational use, best practices for implementation, and challenges and opportunities for continuing research. RECENT FINDINGS: Semiquantitative radiographic union scoring remains the clinical gold standard, but has questionable reliability as a surrogate indicator of structural bone healing, particularly in early-stage, complex, or compromised healing scenarios. Alternatively, computed tomography (CT) scanning enables quantitative assessment of callus morpho

Acetabular reconstruction in developmental hip dysplasia using reinforcement ring with a hook.

We followed prospectively 27 patients with severe acetabular bone-stock deficiencies due to developmental dysplasia of the hip. Mean preoperative patient age was 56 (34-78) years, mean Harris hip score was 31 (16-66) points and pre-operative mean acetabular angle of Sharp was 47 degrees (34 degrees - 61 degrees). Operative acetabular reconstruction was performed in 28 hips using reinforcement ring with a hook placed in the true acetabulum and autologous bone grafting. A 2-year clinical and radiological follow-up was available in 27 hips. Mean Harris hip score improved to 91 (70-100) points. Th

Peak loading during walking is not associated with fracture migration following tibial plateau fracture: A preliminary case series.

Tibial plateau fractures are common, but little evidence exists for their postoperative management, especially when recommending if patients should weight bear at all, partially, or as tolerated. In this study, we describe the loads passing through the fracture construct and the associated fracture migration over the first year following surgery. Nine patients were treated with open reduction and internal fixation and instructed to weight bear as tolerated. Fracture loading and migration were assessed at 2, 12, 26, and 52 weeks postoperative. Fracture loading was calculated as the knee joint r

Reduced wear in vitamin E-infused highly cross-linked polyethylene cups: 5-year results of a randomized controlled trial.

Background and purpose - Vitamin E-infused polyethylene is a relatively new material in joint arthroplasty; there are no long-term reports, and only few mid-term results. Using radiostereometric analysis (RSA), we primarily determined whether vitamin E-infused highly cross-linked polyethylene (HXLPE/VitE) acetabular cups show less wear than ultra-high molecular weight polyethylene (UHMWPE) acetabular cups at 5 years after total hip arthroplasty (THA). We also assessed whether wear rates correlate with increasing cup inclination angles or cup sizes.Patients and methods - This is a 5-year follow

Porous cups with and without hydroxylapatite-tricalcium phosphate coating: 23 matched pairs evaluated with radiostereometry.

Migration, wear, and presence of radiolucencies were studied in 23 matched pairs of patients operated with porous-coated acetabular cups with additional screw fixation. All implants had the same type of titanium fiber mesh. In each pair, one of the cups was plasma-sprayed with a coating consisting of 70% hydroxylapatite (HA) and 30% tricalcium phosphate (TCP). Radiostereometric analysis up to 2 years after the operation revealed smaller rotations around the horizontal axis in cups with HA/TCP coating. The migration of the cup center was not significantly influenced. Evaluation of femoral head

Cementless calcar-guided short stems: 24-month clinical outcomes and migration analysis.

Calcar-guided short stems aim to preserve proximal bone stock and reconstruct individual anatomy, but reduced diaphyseal anchorage raises concerns regarding initial stability. This prospective cohort study evaluated migration of the A2 calcar-guided short stem using high-precision radiostereometric analysis (RSA). Sixty patients (mean age 57.8 years) undergoing primary total hip arthroplasty were enrolled. Marker-based RSA was performed at baseline and at predefined intervals up to 24 months. The primary endpoint was axial subsidence (Ty) at 2 years. Secondary outcomes included three-dimension

Trochanteric fracture-implant motion during healing - A radiostereometry (RSA) study.

Cut-out complication remains a major unsolved problem in the treatment of trochanteric hip fractures. A better understanding of the three-dimensional fracture-implant motions is needed to enable further development of clinical strategies and countermeasures. The aim of this clinical study was to characterise and quantify three-dimensional motions between the implant and the bone and between the lag screw and nail of the Gamma nail. Radiostereometry Analysis (RSA) analysis was applied in 20 patients with trochanteric hip fractures treated with an intramedullary nail. The following three-dimensi

Abnormal kinematics of the distal radioulnar joint due to flat bone dysplasia in nail-patella syndrome: a 3D evaluation with dynamic radiostereometry.

Nail-patella syndrome is a genetic disease with different phenotypes often affecting nails and knees. We present a woman with nail-patella syndrome with instability and dysfunction of the distal radioulnar joint. This is a trait which, to our knowledge, has not previously been described in nail-patella syndrome. In the clinical examination, the ulna head subluxed from the distal radioulnar joint when the wrist was fully supinated, creating a dorsal visible indentation and a volar protrusion of the ulnar head. The distal radioulnar joint instability of the case patient was quantified with a 3D

The Effects of Resection Technique on Implant Migration in Single Radius Posterior-Stabilized Total Knee Replacement.

The purpose of the present study was to measure the effects of gap balancing and resection techniques on migration of a single total knee replacement implant design. A total of 23 patients (24 knees) were recruited on referral to either a surgeon performing gap balancing or a surgeon performing measured resection and followed prospectively. All patients received a fixed bearing, posterior stabilized total knee replacement implant of a single radius femoral component design with cement fixation, and all aspects of care outside of resection technique were identical. Patients underwent radiostere

Similar migration for medial congruent and cruciate-retaining tibial components in an anatomic TKA system: a randomized controlled trial of 60 patients followed with RSA for 2 years.

Background and purpose - There is an ongoing debate regarding the appropriate level of constraint in primary TKA. To provide increased stability and better kinematics, more resembling a natural knee, a medial congruent (MC) tibial insert has been introduced. To investigate potential impact on implant migration, we evaluated the cemented Persona TKA, randomizing between MC and cruciate-retaining (CR) tibial designs. Patients and methods - 60 patients with primary osteoarthritis were randomized to either the CR or MC tibial component. Radiostereometric analysis (RSA) examinations, evaluating tra

The association between tibial slope and posterior tibial tilting, not wear, in uncemented total knee arthroplasty : a secondary analysis of a five-year radiostereometry study.

AIMS: The primary objective of this study was to evaluate the influence of postoperative slope on the five-year tibial component migration and wear following uncemented cruciate-retaining condylar-stabilized total knee arthroplasty (TKA). Secondary objectives included the association between postoperative slope and clinical and functional outcomes. METHODS: This observational study was a secondary analysis of a randomized controlled trial of 93 TKAs on insert materials. Tibial component migration and insert wear were measured across three postoperative slope groups (small (≤ 5°), intermediate

RSA-measured inducible micromotion and interface modeling with finite element methods.

Osteolysis is the main cause of aseptic loosening and stem failure. The mechanism that leads to osteolysis is poorly understood; pressure generation caused by reversible stem micromotion may play an important role. We aimed to determine whether dynamically inducible micromotion occurs in vivo at the prosthesis-cement interface and to use these data to develop and confirm a finite element representation of this interface. Dynamically inducible micromotion was measured using radiostereometric analysis in 21 hips implanted with an Exeter stem, at 3 months and 12 months postoperatively, by changin

[Endoprosthetic wear analysis using virtual CAD-based radiographs].

The articulating parts of an endoprosthetic joint are subjected to constant wear, which results in loss of material. In peri-implant tissue, the wear debris particles cause aseptic inflammation with osteolysis and aseptic loosening. Due to the associated bone resorption-which can be considerable-"particle disease" represents a significant challenge for orthopedic surgeons. Continuous improvements in the material properties of polyethylene during the past decades have resulted in reduced wear and longer implant survival times. To evaluate wear properties in polyethylene liners after total hip a

Accuracy of EBRA-cup measurements after reconstruction of severe acetabular defects at revision THR.

Radiostereometric analysis (RSA) is the most accurate method of measuring component migration using radiographs but is restricted to use in prospective studies. Ein-Bild-Roentgen-analyze (EBRA)-Cup can be used retrospectively, but its accuracy to measure component migration following revision is unknown. This study aimed to determine the accuracy of EBRA-Cup measurements of uncemented acetabular component migration after revision total hip replacement (THR). The secondary aim was to compare the number of cases identified using EBRA-Cup and RSA as having proximally migrated above and below 1 mm

Three-dimensional displacement of the hip joint after triple pelvic osteotomy. A postmortem radiostereometric study.

Triple pelvic osteotomy reorients the acetabulum relative to the pelvis in order to improve acetabular coverage of the femoral head in cases of acetabular dysplasia. We undertook a radiostereometric analysis (RSA) on 6 osteotomized cadaver hips to determine the actual three-dimensional reorientation obtained. The centers of the femoral head were all translated posteriorly between 11 and 41 mm, and distally up to 13 mm. 4 were lateralized up to 8 mm, and 2 were medialized up to 5 mm. All acetabuli rotated anteriorly about the lateral to medial axis (X-axis), and 4 rotated outwards around the di
