Time of administration influences gastric inhibitory effects of ranitidine.

The overnight gastric secretion of patients with duodenal ulcer or oesophagitis and of healthy volunteers known to be gastric nocturnal hypersecretors was studied after administration of ranitidine at 1815 h or 2200 h. Patients with oesophagitis showed a significant therapeutic 'gain' after the earlier (1815 h) dosing, with gastric inhibition lasting an average of 5 h longer. A similar pattern was observed in the nine patients with duodenal ulcer, although three of these patients showed some degree of escape from the gastric inhibition after 0400 h following the 1815 h administration of raniti

Ranitidine does not affect chlormethiazole or indocyanine green disposition.

Cimetidine has been shown to inhibit hepatic mixed-function oxidase activity and to lower hepatic blood flow. It is not known whether these effects are related to its H2-receptor antagonism or to its intrinsic structure. Ranitidine is a more potent H2-receptor antagonist and differs structurally from cimetidine. In our study, ranitidine, 150 mg twice daily, had no effect on oral or systemic clearance of chlormethiazole, a sedative with a high clearance, and no effect on indocyanine green elimination.

The 'burn' of ranitidine recall: current insights and mitigation strategies.

Ranitidine was one of the most commonly used medications for the treatment of gastroesophageal reflux disease. On 1 April 2020, the US Food and Drug Administration requested all manufacturers to immediately withdraw ranitidine from the market because of concern of higher than acceptable levels of N-nitrosodimethylamine, a potential carcinogen. Herein, we highlight the reason for this recall, along with the effects of this recall on both the patients and healthcare practitioners, and offer insights on management strategies.

The effects of cimetidine, ranitidine, and famotidine on human neutrophil functions.

UNLABELLED: Neutrophil functions, which play an important role in the antibacterial host defense system, are inhibited by various anesthetics and surgical procedures. Histamine H2-receptor antagonists are perioperatively used as a prophylaxis against acid aspiration syndrome or stress ulceration. We examined the effect of cimetidine, ranitidine, and famotidine, at clinically relevant concentrations and at 10 and 100 times this concentration, on several aspects of human neutrophil function using an in vitro system. The three H2-receptor antagonists did not impair neutrophils' chemotaxis or phag

Case report: ranitidine-induced bradycardia in a patient with dextrocardia.

Although rare, bradycardia and other cardiac arrhythmias have been associated with the use of H2-receptor antagonists. Ranitidine is among the most frequently prescribed drugs. In this article, the authors observed a ranitidine-mediated sinus bradycardia in a man with dextrocardia (situs inversus) who had acute bleeding from a duodenal ulcer. The bradycardia was resolved after ranitidine was discontinued.

Pantoprazole is superior to ranitidine in the treatment of acute gastric ulcer.

BACKGROUND: Pantoprazole is a newly developed gastric H+/K(+)-adenosine triphosphatase inhibitor with a potent and long-acting inhibitory effect on gastric acid secretion. METHODS: In a double-blind multicenter study with 28 centers in Germany, pantoprazole (40 mg before breakfast) was compared with ranitidine (300 mg at bedtime) with regard to healing rates, time until healing, symptom relief, and tolerability. A total of 248 outpatients with benign gastric ulcer were included. RESULTS: The healing rates after 2, 4, and 8 weeks were 37%, 87%, and 97%, respectively, in the pantoprazole and 1

Use of i.v. cimetidine, ranitidine, and famotidine in 40 hospitals.

The drug experiences of 1200 patients receiving i.v. histamine H2-receptor antagonists were studied. Forty hospitals in five southeastern states participated in a drug-use evaluation (DUE). In addition to supplying background information, a pharmacist at each hospital used a standardized form to perform a concurrent review of 30 consecutive adult patients who had been started on i.v. cimetidine, ranitidine, or famotidine. In addition to the patient's age and the prescriber's medical specialty, specific DUE criteria included the reason given in the medical record for use; dosage regimen and adj

Ranitidine and antacids in the prevention of duodenal ulcer relapse in Greece.

The present study was undertaken to compare the efficacy of ranitidine 150 mg/nocte with that of antacids taken "on demand" in the prevention of recurrence of duodenal ulcer (DU). Two groups (A, B) of patients with DU recently healed by ranitidine or antacids were studied. Patients of Group A (110) healed by ranitidine were put on maintenance treatment with ranitide, and patients of Group B (32) healed by antacids were given antacids "on demand" for a period of one year. Endoscopy was performed at the end of the 6th and 12th months, as well as whenever symptoms appeared. Patients were followed

Implementing therapeutic interchange of intravenous famotidine for cimetidine and ranitidine.

The steps taken to implement a therapeutic interchange program for i.v. histamine H2-receptor antagonists and to determine the potential cost savings are described. A literature review conducted by pharmacists at a 273-bed nonteaching community hospital showed that i.v. famotidine was as safe and effective as i.v. cimetidine or ranitidine and that it was feasible to add famotidine to total parenteral nutrition (TPN) solutions. Because of famotidine's cost advantage, it was proposed that i.v. famotidine be used in place of specific dosage regimens of i.v. ranitidine or cimetidine and in TPN sol

A comparison of roxatidine acetate and ranitidine in gastric ulcer healing.

A randomised multicentre double-blind study was conducted to compare the efficacy and safety of roxatidine acetate 75 mg twice daily and ranitidine 150 mg twice daily in 295 patients with endoscopically confirmed gastric ulcers. Substantial reductions in ulcer diameters and healing rates of 85.6 and 88.2% for roxatidine acetate and ranitidine, respectively, were obtained after 8 weeks of treatment. There was no difference in healing rates between smokers and non-smokers in either group. The relief of day and night-time epigastric pain was comparable for both treatment groups, as was antacid ta

Effect of ranitidine and cimetidine on gastric ulcer healing and recurrence.

Fifty-eight patients with endoscopically confirmed benign gastric ulceration were randomly allocated to treatment with 150 mg ranitidine twice daily, placebo matching ranitidine twice daily, or 200 mg cimetidine three times daily and 400 mg at night. Patients were endoscoped at monthly intervals for up to 3 months, the endoscopist being unaware of the treatment. Significantly more ulcers (p less than 0.05) had healed after 2 months of ranitidine (14 of 18, 78%) and cimetidine (17 of 20, 85%) than with placebo (9 of 20, 45%; p less than 0.05) and after 3 months of ranitidine (15 of 18, 88%) and

Effect of cimetidine and ranitidine on carbamazepine and sodium valproate pharmacokinetics.

The pharmacokinetics of a single oral dose (400 mg) of carbamazepine and sodium valproate were compared in peptic ulcer patients before and after four weeks of a therapeutic course of either cimetidine (1 g/day, n = 6 subjects) or ranitidine (300 mg/day, n = 6 subjects). There was a small (up to 20%) but statistically significant decrease in oral clearance of carbamazepine after cimetidine treatment. A similar fall in sodium valproate clearance in five cimetidine-treated patients was accompanied by a significantly prolonged elimination half-life. No such trends were demonstrated during ranitid

[Effect of oxmetidine and ranitidine on reflux behavior in patients with esophagitis].

The behavior of reflux under oxmetidine and ranitidine has been examined by long-term-pH-metry in patients with endoscopic proved oesophagitis and pathological, gastro-oesophageal reflux (GER). Both H2-receptor antagonists lead to a significant decrease of the GER over the entire period of measurement of 18 hours. Statistical significant differences comparing both substances could not be found, so that as well oxmetidine as ranitidine seem to be appropriate for the therapy of reflux diseases.

Mania associated with ranitidine: a case report and review of literature.

H2 receptor antagonists can be associated with central adverse drug reactions (ADRs), like confusion, delirium, hallucinations, slurred speech or headaches. We report here a 'serious' case of severe mania leading to hospitalization in a 42-year-old alcohol-dependent man, 4 days after ranitidine introduction. Review of literature showed that this 'very rare' ADR occurs mainly in patients with predisposing factors: age, decrease in renal and/or hepatic function, polymedication, alcohol. Knowledge of this ADR can be particularly important for these drugs widely used as self-medication.

Agranulocytosis possibly caused by ranitidine in a patient with renal failure.

A 70-year-old Japanese woman with renal dysfunction under hemodialysis presented with vomiting and chill with fever. Over the previous 24 weeks she had been taking 75 mg of ranitidine after hemodialysis. Other medications taken were prednisolone, furosemide, alpha-calcidol, amlodipine and calcium carbonate. Before starting ranitidine, she had been treated with famotidine for about 2 years without complication. Hematological inspection on admission revealed agranulocytosis with WBC of 400/mm3. Ranitidine was discontinued and granulocyte colony-stimulating factor (G-CSF) was started. On Day 3, l

Nonprescription doses of ranitidine are effective in the relief of episodic heartburn.

BACKGROUND: Many Americans have heartburn or related symptoms monthly and >20% experience heartburn at least once per day. Although many self-treat episodic heartburn with nonprescription antacids, newer treatments that decrease gastric volume and increase the pH of refluxed material are proving effective and popular in relieving heartburn. AIM: To evaluate the safety and efficacy of low-dose regimens of ranitidine for the relief of heartburn. METHODS: Adults with at least a 3-month history of heartburn were eligible for this randomized, double-blind, parallel group, multicenter dose-ranging
