BACKGROUND: The terrorist attacks on the World Trade Center (WTC) of September 11, 2001 resulted in the deaths of 2,823 persons. They also generated a long-lasting burden of multiple physical and mental health illnesses among the cohort of 50,000 rescue workers who responded to the attacks and in the 400,000 residents and workers in nearby areas of New York City. A comprehensive health surveillance program was developed from the first months after the accidents and was further developed in the subsequent ears. Individual exposure and health data were stored in ad hoc databases and produced epi

Mass fatality incidents are always unexpected and put a sudden stress on local response agencies to cooperate and share resources to accomplish tasks that are outside their normal activities. Lines of legal authority are often conflicting when two or more agencies are statutorily in charge. Within the USA, the local coroner system is almost universally delegated as responsible for all involved tasks including body recovery, yet the coroner is almost always the smallest responding agency, with the smallest labour pool from which to draw and the least experience of formal Incident Command System

INTRODUCTION: This report addresses the development, implementation, and evaluation of a protocol designed to protect participants from inadvertent emotional harm or further emotional trauma due to their participation in the World Trade Center Evacuation (WTCE) Study research project. This project was designed to identify the individual, organizational, and structural (environmental) factors associated with evacuation from the World Trade Center Towers 1 and 2 on 11 September 2001.
METHODS: Following published recommended practices for protecting potentially vulnerable disaster research parti

INTRODUCTION: Early oxygenation is essential in a non-breathing scuba accident victim, but the need to exclude water has thus far prevented underwater ventilation, causing significant delay. The full-face mask (FFM) is a potential solution, but its safety and feasibility in this context has not been established. This is a preliminary study into the utility of FFMs for underwater ventilation.
METHODS: The tidal volume and peak inspiratory pressure delivered by the OTS Guardian and Neptune III FFMs were measured using a RespiTrainer Advance Airway Management Trainer and an open circuit scuba sy

This article is about the experiences gained from the largest full-scale exercise ever conducted in the State of Florida, specifically regarding the Orlando International Airport (MCO) venues. The exercise was centred on an airplane crashing into a hotel just outside of MCO property. The scenario clarified details regarding Incident Command and the unique jurisdictional responsibilities associated with a large-scale mass casualty incident. There were additional challenges with airline operations, walking wounded, and information sharing that provided valuable experiences toward enhancing emerg

OBJECTIVES: The objectives for this study were to determine the prevalence of use, safety and efficacy of different preparations of rescue medication used for prolonged seizures in children in the community and to use the information gained to inform good practice within the network.
METHODS: For all children in the EPIC area who had been prescribed epilepsy rescue medication for use in the community a questionnaire was completed by the clinician for each child relating to rescue medication prescribed, the epilepsy syndrome and seizure type. A questionnaire was also completed by the carers ab

Injured patients in crevasses who are suspected of having sustained spinal injuries should ideally be extricated after being immobilized in a horizontal position on a stretcher and having a cervical collar applied. Sometimes, however, horizontal stabilization is not possible, because the crevasse is too narrow, and the patient needs to be stabilized in a vertical position. In such cases an extrication device can be a useful adjunct. The Kendrick Extrication Device stabilizes the position of the body and maintains firm support of the head, neck, and torso. Therefore, the International Commissio

The use of non-conventional agents aimed at causing panic and terror among civilians has a long history. There have been uninterrupted threats and the use of biological and chemical weaponry from the time of early tribal conflicts to the Iran-Iraq war. The sole practical experience has come from the release of the nerve gas Sarin in a Tokyo subway (1994) and the inhalational anthrax discovered in Florida (2001). Drills that simulate scenarios of biological/chemical mass infestation have yielded valuable theoretical experience. This chapter reviews the main chemical and biological agents possib

BACKGROUND: Although numerous authors have described surgical experiences following major disasters, little is known regarding the needs of and barriers to care faced by surgeons during such disasters.
QUESTIONS/PURPOSES: We therefore (1) identified and compared recurrent interview themes essential to the disaster response following the 2010 Haiti earthquake; (2) determined the difference in reported disaster equipment management task difficulty between disaster-trained and untrained volunteers; and (3) approximated the quantity of various procedures performed.
METHODS: We conducted 14 inter

On August 29, 2005, Hurricane Katrina struck states along the Gulf Coast of the United States. In the days after the hurricane struck, approximately 750 evacuation centers were established in at least 18 states to accommodate more than 200,000 evacuees. State and local health departments, with assistance from CDC, initiated enhanced infectious disease surveillance and outbreak response activities, implemented by teams of public health and rescue workers, including military personnel. Outbreak monitoring included direct reporting of conditions of public health significance to public health agen

Direct in-water rescue is a safety-critical task in which towing technique and rescue-support equipment may influence both rescuer workload and task output. This study compared physical workload, towing performance and physiological economy across four mannequin towing techniques: Extended Arm Tow, Double Armpit Tow, "Sailor" Technique Tow and Rescue Tube Tow. Eighteen male water lifeguards (age 18.8 ± 1.3 years; V˙O2max 56.1 ± 5.0 mL·kg-1·min-1) completed randomized 50-m towing trials after an incremental swimming test to determine maximal oxygen uptake. Towing velocity, gas-exchange variable

In 2007, the United States successfully eliminated canine rabies virus variant. Globally, however, dogs remain the principal source of human rabies infections. Since 2007, three cases of canine rabies virus variant were reported in dogs imported into the United States, one each from India (2007), Iraq (2008), and Egypt (2015) (1-3). On December 20, 2017, a dog imported into the United States from Egypt was identified with rabies, representing the second case from Egypt in 3 years. An Egyptian-based animal rescue organization delivered four dogs from Cairo, Egypt, to a flight parent (a person s

OBJECTIVES: Identification, a cognitive process by which individuals think of themselves as similar to another person, may be associated with distress during traumatic events. This study examined the association of identification with psychological responses among disaster workers not directly exposed to an airline crash.
METHODS: Participants were 421 workers (aged 18-60 [M (SD) = 36.2 (9.9)], 86.4% male, 98.3% White, 71.8% married). Surveys at 2 months (Time 1; T1) and 7 (T2) months post-disaster assessed identification (i.e., extent to which participants identified victims as similar to th

This study explores sex/gender-related differences in ergonomic exposures and musculoskeletal disorders for 4090 working Syrian refugee children (>8-≤18 years) in the Bekaa Valley, Lebanon (n = 2107 males; n = 1983 females). Data was collected on demographic, occupational, and socioeconomic indicators and musculoskeletal disorders. Results revealed that children engaged in strenuous work. Ergonomic exposures differed by sex/gender, with girls more likely to engage in repetitive movements and boys in heavy lifting. Girls bore a double burden of work inside and outside their households and were

AIM: To calculate the first Austrian avalanche survival curve and update a Swiss survival curve to explore survival patterns in the Alps.
METHODS: Avalanche accidents occurring between 2005/06 and 2012/13 in Austria and Switzerland were collected. Completely buried victims (i.e. burial of the head and chest) in open terrain with known outcome (survived or not survived) were included in the analysis. Extrication and survival curves were calculated using the Turnbull algorithm, as in previous studies.
RESULTS: 633 of the 796 completely buried victims were included (Austria n=333, Switzerland n
