Challenges in managing neonatal ileal atresia in limited-resource settings: a case report.

Neonatal ileal atresia is a form of intestinal obstruction characterized by narrowing, complete closure, or absence of a segment of the ileum. This case involves a 5-day-old female neonate presented with abdominal distension and bilious vomiting. The neonate, delivered via spontaneous vertex birth weighing 2.9 kg, showed no fever and maintained stable vital signs during examination. Physical assessment revealed abdominal distension, hyper-tympanic areas, dullness in the lower quadrants, reduced bowel sounds, and a patent anus with an empty rectum. Imaging studies confirmed intestinal obstructi

Multiple system atrophy-cerebellar type: Diagnostic challenge in resource-limited settings case report.

KEY CLINICAL MESSAGE: This case report highlights the challenges of diagnosing MSA-C in resource-limited settings. MRI findings like the "hot cross bun" sign can be supportive, but the unavailability of advanced tools like seed amplification assay may delay diagnosis. Early diagnosis is crucial for proper symptom management. ABSTRACT: Multiple system atrophy is a rare neurodegenerative disorder affecting the pyramidal, autonomic, nigrostriatal, and cerebellar tracts. Multisystem atrophy should be considered in adults with progressive motor or autonomic dysfunctions. Clinical manifestations va

A Global Systematic Review of Open Heart Valvular Surgery in Resource-Limited Settings.

BACKGROUND: Many obstacles challenge the establishment and expansion of cardiac surgery in low- and middle-income countries, despite the unmet cardiac surgical needs. One challenge has been providing adequate follow-up care to monitor anticoagulation, manage morbidity, and prevent mortality. This systematic review describes outcomes after valvular cardiac surgery and focuses on strategies for prolonged follow-up care in resource-constrained settings. METHODS: Studies published between 2012 and 2022 were collected from Embase and the Cochrane Library. Article inclusion criteria were adolescent

Dry- down probe free qPCR for detection of KFD in resource limited settings.

Kyasanur Forest Disease is a tick-borne flavivirus is endemic in the Southern India. The recent expansion and resurgence of sporadic outbreaks in southern parts of country is the most important concern. Although only formalin inactivated vaccine is available for treatment with limited efficacy the early detection and timely identification is a only way to prevent spread of cases. If the disease can be identified prior to infection in humans like in forest areas from ticks and vectors the disease spread supposed to be managed quickly. Here we have standardized a single tube ready to use dry-dow

Teaching Trauma in Resource-Limited Settings: A Scoping Review of Pediatric Trauma Courses.

PURPOSE: Injury remains an important cause of death and disability globally, with 95% of all childhood injury deaths occurring in low- and lower-middle-income countries (LMICs). Pediatric trauma training, tailored to the resources in LMICs, represents an opportunity to improve such outcomes. We explored the nature of course offerings in pediatric trauma in resource-limited settings. METHODS: Seven databases were interrogated up to June 12, 2020, to retrieve articles examining pediatric trauma training in LMICs, as defined by the World Bank, without language restrictions. Penetrating esophageal injury: A diagnostic challenge in resource-limited settings; a case report.

INTRODUCTION: Penetrating injuries to the upper aerodigestive tract are potentially life-threatening, with significant morbidity and mortality. Although rare, the cervical esophagus is the most vulnerable part of the esophagus to penetrating injuries. Given the unique and condensed anatomy of the neck, penetrating injuries to the second zone of the neck pose a demanding challenge. Contention exists in diagnosing and managing penetrating esophageal injuries. CASE PRESENTATION: Herein is a case of a young male with a penetrating neck injury from the left lateral aspect with subsequent esophagea

Challenges and Solutions in translating sepsis guidelines into practice in resource-limited settings.

Sepsis and septic shock are major contributors to the global burden of disease, with a large proportion of patients and deaths with sepsis estimated to occur in low- and middle-income countries (LMICs). There are numerous barriers to reducing the large global burden of sepsis including challenges in quantifying attributable morbidity and mortality, poverty, inadequate awareness, health inequity, under-resourced public health, and low-resilient acute health care delivery systems. Context-specific approaches to this significant problem are necessary on account of important differences in populat

Pathogenesis, diagnostic challenges and treatment of zika virus disease in resource-limited settings.

The association of Zika virus (ZIKV) infection with congenital malformation and neurological sequelae has brought significant global concern. Consequently, the World Health Organization (WHO) declared it "a public health emergency of International concern" on 1 February, 2016. A critical review of its pathogenesis would lead to a better understanding of the clinical features and the neurological complications. This review is based on literature search in PubMed/Medline, Google Scholar and the WHO, http://www.who.int. This include all relevant articles written in English published through June

Management algorithms for cervical cancer screening and precancer treatment for resource-limited settings.

Management algorithms for screen-positive women in cervical cancer prevention programs have undergone substantial changes in recent years. The WHO strongly recommends human papillomavirus (HPV) testing for primary screening, if affordable, or if not, then visual inspection with acetic acid (VIA), and promotes treatment directly following screening through the screen-and-treat approach (one or two clinic visits). While VIA-positive women can be offered immediate ablative treatment based on certain eligibility criteria, HPV-positive women need to undergo subsequent VIA to determine their eligibi

Clinical trial laboratory principles for emerging infectious disease research in resource-limited settings.

For emerging and re-emerging viral diseases, the timing and location of outbreaks are typically unpredictable and may require rapid implementation of randomized controlled trials (RCTs) to evaluate safety and efficacy of candidate vaccines or therapeutics. In austere and hard-to-reach settings, establishing effective laboratory operations for RCTs requires considerable effort to address both anticipated and unforeseen challenges. Here, we describe key principles and practical challenges associated with effective research laboratory operationalization in the context of a double-blinded, placebo

Miniaturized subcutaneous cellular implants for sustained therapeutic protein delivery in resource-limited settings.

UNLABELLED: Cell encapsulation offers a promising strategy for sustained therapeutic protein delivery, obviating the need for repeated injections. Among potential implantation sites, the subcutaneous space is particularly attractive for its accessibility and amenability to minimally invasive procedures. However, performance of subcutaneous devices reported to date has been limited due to various challenges including foreign body response (FBR) and inadequate mass transfer. Moreover, typical encapsulation devices require surgeries for implantation and retrieval, limiting their potential use in

Congenital syphilis in neonates: Persistent diagnostic and treatment challenges in resource-limited settings.

Despite available guidelines, congenital syphilis (CS) continues to burden low- and middle-income countries (LMICs), with failures occurring across multiple points. Two prior case reports from India described symptomatic infants with delayed diagnoses resulting from absent or ignored antenatal screening. We add five neonates identified prospectively at birth at a tertiary care centre in northern India (September 2024-January 2026), born to mothers with reactive syphilis serology. All were asymptomatic, but maternal treatment was adequate in only two cases. Neonatal RPR was reactive in one infa

Clinical characteristics of thrombocytopenia in tropical diseases and management in resource-limited settings.

Platelets play a central role in the pathogenesis of tropical infectious diseases. Infections such as malaria, dengue, and leptospirosis are often accompanied by thrombocytopenia. While the platelet count is typically monitored as a measure of thrombocytopenia, other platelet indices may better determine bleeding severity. Platelet dysfunction in these conditions requires a nuanced approach to management. Prophylactic platelet transfusions have shown limited efficacy in preventing hemorrhagic complications and carry additional risks. These include transfusion-transmitted infections, logistical

Measuring the impact of medical education in resource limited settings: A scoping review.

The Lancet Commission on Global Surgery identified workforce training as a key area of investment to improve access to essential surgical care. Many non-governmental organizations and universities have attempted to contribute to training and upskilling of providers in resource limited settings through educational programs. However, there are no widely agreed upon metrics for measuring the long-term success of these programs. A scoping review was conducted to assess varying methods used to measure impact, with a specific interest in patient-level impact. This scoping review was conducted in fou

Delivering orthopaedics in Burundi: a model for humanitarian surgery in resource-limited settings.

Burundi remains one of the most socioeconomically challenged countries globally, facing profound limitations in healthcare infrastructure, workforce, and access. In this context, the Italian Medical Foundation for Central Africa (FIMAC) has conducted humanitarian orthopaedic missions for over two decades in Bubanza, addressing critical musculoskeletal conditions in both paediatric and adult populations. This essay provides a comprehensive overview of the operational, clinical, and ethical dimensions of these missions. Commonly treated pathologies include chronic osteomyelitis, neglected fractu

Nocturnal oximetry parameters as predictors of sleep apnea severity in resource-limited settings.

Nocturnal oximetry is an alternative modality for evaluating obstructive sleep apnea syndrome (OSAS) severity when polysomnography is not available. The Oxygen Desaturation (≥3%) Index (ODI3) and McGill Oximetry Score (MOS) are used as predictors of moderate-to-severe OSAS (apnea-hypopnea index-AHI >5 episodes/h), an indication for adenotonsillectomy. We hypothesised that ODI3 is a better predictive parameter for AHI >5 episodes/h than the MOS. All polysomnograms performed in otherwise healthy, snoring children with tonsillar hypertrophy in a tertiary hospital (November 2014 to May 2019) were
