Effect of prone positioning and PEEP on respiratory mechanics in children undergoing scoliosis surgery.

BACKGROUND: Surgery for severe scoliosis (SS) is usually performed in the prone position. Changes in respiratory mechanics related to position and positive end expiratory pressure (PEEP) titration during anesthesia of SS are understudied. We aimed to investigate the effect of prone position and PEEP on the respiratory mechanics of scoliotic children undergoing spine surgery. METHODS: Prospective, crossover study performed in two pediatric hospitals (Montevideo, Uruguay-Centro Hospitalario Pereira Rossell- and Milano, Italy-Vittore Buzzi Children's Hospital). Shortly after intubation, pulmonar

The Effect of Waistbands on Intra-Abdominal Pressure and Respiratory Mechanics During Bodyweight Exercise.

Leahy, MG, Kipp, S, and Sheel, AW. The effect of waistbands on intra-abdominal pressure and respiratory mechanics during bodyweight exercise. J Strength Cond Res 38(12): 2047-2054, 2024-Athletic apparel is often designed with high-waisted, elastic bands around the abdomen to provide comfort and support during functional exercises. The abdominal musculature aids in both active respiration and trunk stability during the exercise, but whether high-waistband athletic garments affect pulmonary function and respiratory mechanics is unknown. Ten healthy women participated in a single-visit study. Sub

Asthma predictive index in relation to respiratory mechanics by impulse oscillometry in recurrent wheezers.

BACKGROUND: The identification of children who will have persistent asthma has become a focus of recent research. The aim of this study was to assess whether impulse oscillometry (IOS) has a diagnostic value to predict modified API (asthma predictive index) in pre-schoolers with recurrent wheezing. METHODS: Pre-school children aged 3-6 years with recurrent wheezing were enrolled. The study population was divided into two groups based on mAPI criteria. Lung function was assessed by IOS. RESULTS: 115 children were assessed; 75 (65.2%) of them were male. The median age was 39 months (min: 36, m

Diaphragm remodeling and compensatory respiratory mechanics in a canine model of Duchenne muscular dystrophy.

Ventilatory insufficiency remains the leading cause of death and late stage morbidity in Duchenne muscular dystrophy (DMD). To address critical gaps in our knowledge of the pathobiology of respiratory functional decline, we used an integrative approach to study respiratory mechanics in a translational model of DMD. In studies of individual dogs with the Golden Retriever muscular dystrophy (GRMD) mutation, we found evidence of rapidly progressive loss of ventilatory capacity in association with dramatic morphometric remodeling of the diaphragm. Within the first year of life, the mechanics of br

Accuracy of pulmonary auscultation to detect abnormal respiratory mechanics: a cross-sectional diagnostic study.

Pulmonary auscultation is a method used in clinical practice for the evaluation and detection of abnormalities relating to the respiratory system. This method has limitations, as it depends on the experience and hearing acuity of the examiner to determine adventitious sounds. In this context, it's important to analyze whether there is a correlation between auscultation of lung sounds and the behavior of the respiratory mechanical properties of the respiratory system in patients with immediate postoperative cardiac surgery.

On the respiratory mechanics measured by forced oscillation technique in patients with systemic sclerosis.

BACKGROUND: Pulmonary complications are the most common cause of death and morbidity in systemic sclerosis (SSc). The forced oscillation technique (FOT) offers a simple and detailed approach to investigate the mechanical properties of the respiratory system. We hypothesized that SSc may introduce changes in the resistive and reactive properties of the respiratory system, and that FOT may help the diagnosis of these abnormalities. METHODOLOGY/PRINCIPAL FINDINGS: We tested these hypotheses in controls (n = 30) and patients with abnormalities classified using spirometry (n = 52) and pulmonary vo

Impact of lung remodelling on respiratory mechanics in a model of severe allergic inflammation.

We developed a model of severe allergic inflammation and investigated the impact of airway and lung parenchyma remodelling on in vivo and in vitro respiratory mechanics. BALB/c mice were sensitized and challenged with ovalbumin in severe allergic inflammation (SA) group. The control group (C) received saline using the same protocol. Light and electron microscopy showed eosinophil and neutrophil infiltration and fibrosis in airway and lung parenchyma, mucus gland hyperplasia, and airway smooth muscle hypertrophy and hyperplasia in SA group. These morphological changes led to in vivo (resistive

Differences of respiratory mechanics in mechanical ventilation of acute respiratory distress syndrome between patients with COVID-19 and Influenza A.

BACKGROUND: Whether COVID-19-induced acute respiratory distress syndrome (ARDS) should be approached differently in terms of mechanical ventilation therapy compared to other virus-induced ARDS is debatable. Therefore, we aimed to ascertain whether the respiratory mechanical characteristics of COVID-19-induced ARDS differ from those of influenza A induced ARDS, in order to establish a rationale for mechanical ventilation therapy in COVID-19-induced ARDS. METHODS: This was a retrospective cohort study comparing patients with COVID-19-induced ARDS and influenza A induced ARDS. We included intens

Ventilatory management of patients with acute respiratory distress syndrome and optimization of lung mechanics.

Acute respiratory distress syndrome (ARDS) is a form of acute lung injury characterized by increased alveolar-capillary permeability and non-cardiogenic pulmonary edema. Mechanical ventilation remains the cornerstone of supportive care, but ventilatory support itself can worsen lung injury. Current guidelines recommend focusing on minimizing ventilator-induced lung injury through volume and pressure limitation (lung-protective ventilation). In selected patients, noninvasive strategies such as high-flow nasal cannula and noninvasive ventilation may prevent intubation. For patients requiring inv

Respiratory Mechanics and Diaphragmatic Dysfunction in COPD Patients Who Failed Non-Invasive Mechanical Ventilation.

BACKGROUND: Although non-invasive mechanical ventilation (NIV) is the gold standard treatment for patients with acute exacerbation of COPD (AECOPD) developing respiratory acidosis, failure rates still range from 5% to 40%. Recent studies have shown that the onset of severe diaphragmatic dysfunction (DD) during AECOPD increases risk of NIV failure and mortality in this subset of patients. Although the imbalance between the load and the contractile capacity of inspiratory muscles seems the main cause of AECOPD-induced hypercapnic respiratory failure, data regarding the influence of mechanical de

Pulmonary mechanics and gas exchange characteristics in uncommon etiologies of acute respiratory distress syndrome.

BACKGROUND: Common causes of acute respiratory distress syndrome (ARDS) include pneumonia, aspiration, non-pulmonary sepsis and trauma. Little is known about pulmonary mechanics and gas exchange in less-common etiologies of ARDS, which comprises 12-23% of cases. Our hospital's ARDS quality assurance database contained a substantial number of these cases. This descriptive study examines the pulmonary mechanics, and gas exchange characteristics of this diverse cohort of ARDS subjects. METHODS: Between March 2010 and April 2017 we identified 94 subjects with less common etiologies of ARDS who ha

The effects of airway pressure release ventilation on respiratory mechanics in extrapulmonary lung injury.

BACKGROUND: Lung injury is often studied without consideration for pathologic changes in the chest wall. In order to reduce the incidence of lung injury using preemptive mechanical ventilation, it is important to recognize the influence of altered chest wall mechanics on disease pathogenesis. In this study, we hypothesize that airway pressure release ventilation (APRV) may be able to reduce the chest wall elastance associated with an extrapulmonary lung injury model as compared with low tidal volume (LVt) ventilation. METHODS: Female Yorkshire pigs were anesthetized and instrumented. Fecal pe

Influence of mucolytic therapy on respiratory mechanics in patients with chronic obstructive pulmonary disease.

Mucus hypersecretion is a hallmark of chronic obstructive pulmonary disease (COPD) and is regarded as contributing to airflow limitation in these patients. The value of mucolytic therapy is still a matter for debate. We investigated the short-term influence of an inhalative mucolytic drug mercapto-ethane sulfonate (Mesna) on the lung function, respiratory mechanics and viscous respiratory load in patients with COPD in a randomized, placebo controlled, double-blind, cross-over study in 10 patients with moderate, stable COPD. Respiratory mechanics in very low birth weight infants during continuous versus intermittent gavage feeds.

This study was designed to determine whether respiratory mechanics in stable, very low birth weight infants changed after replacing intermittent feeds with continuous feeding. We measured static respiratory system compliance, respiratory system resistance, functional residual capacity (FRC), and tidal volume immediately before feeds and at 20, 60, and 120 min after feeds, and again the next day on continuous feeds. Patients selected for enrollment into the study needed to fulfill the following criteria: 1) birth weight and postnatal weight < 1,500 g, 2) no need for mechanical ventilation, posi

How stiff is the armadillo? A comparison with the allometrics of mammalian respiratory mechanics.

Static respiratory mechanics were examined in the armadillo (Dasypus novemcinctus) and compared with allometric relationships newly derived for adult mammals from values in the literature. Normalised by body weight, chest wall compliance (Cw) in the armadillo is lower than predicted. Lung compliance (Cl) is also low in the armadillo, however it is appropriately matched to the resting lung volume (Vr) (ie. Cl/Vr infinity Mass0.0) and the ratio of Cw/Cl is appropriate for the size of the animal. Respiratory system resistance is high in the armadillo, presumably because of smaller airways associa

Effect of two tidal volumes on oxygenation and respiratory system mechanics during the early stage of adult respiratory distress syndrome.

PURPOSE: To study the effect of two tidal volumes on gas exchange, lung mechanics, and hemodynamics in 12 patients with acute respiratory distress syndrome (ARDS) within the first 72 hours of mechanical ventilation. METHODS: Tidal volume (VT) was increased by 40% from the initial value at fixed positive end-expiratory pressure (PEEP) and matched minute ventilation by adjusting the respiratory rate (RR) of the ventilator. Initial VT and RR were 592 +/- 42 mL and 19 +/- 1 min-1, respectively. High VT amounted to 825 +/- 54 mL with a RR of 12 +/- 1 min-1. RESULTS: We found that at high VT (1) t
