Care Wounds: Precarious Vulnerability and the Potential of Exposure.

What does it mean to offer care when the act of caring is wounding to its giver? For peer specialists-individuals with lived experience as patients in the psychiatric system-this question shapes how they use their own histories to provide support for individuals experiencing psychiatric crisis. Peer support is unique in the way it draws on empathetic resonance and depends on carefully deployed vulnerability; where one connects with others through the recognition of shared experience and mutual hurt. For peers, care works when this guidance, reassurance, and "being with"-all of which draw upon

Community care for adults with Williams syndrome: how families cope and the availability of support networks.

A study of the adjustment difficulties and needs of 70 adults with Williams syndrome found that the majority continued to live at home and remained heavily dependent on their families for their self-care. Twenty-nine families (41.4%) had had no contact with a social worker in the preceding 2 years, and 34 out of the 48 families whose children still lived at home (70.8%) had no access to respite care. Advice regarding benefits, and appropriate living and occupational arrangements for the adults was also patchy. Despite progressive medical problems, and high rates of behavioural and emotional di

"We Really Help, Taking Care of Each Other": Older Homeless Adults as Caregivers.

OBJECTIVES: Many older homeless adults maintain contact with family. We conducted a qualitative study examining the role of family caregiving for older homeless adults. METHOD: We conducted semi-structured qualitative interviews with a sample of 46 homeless participants who reported spending at least one night with a housed family member in the prior 6 months. RESULTS: A total of 13 of 46 older adult participants provided caregiving. Themes included (a) the death of the care recipient led to the participant's homelessness; (b) feeling a duty to act as caregivers; (c) providing care in exchan

A clinical description of strategies to address traumatic brain injury experienced by homeless patients at Baltimore's medical respite program.

BACKGROUND: Medical respite programs provide a safe place for people experiencing homelessness to recover from an acute illness or injury. Many patients in respite programs have experienced a traumatic brain injury (TBI) that impedes their ability to complete the self-management tasks necessary to recover from an acute medical condition. Patients with brain injuries may also have behavioral problems that are difficult to manage in a medical respite setting. OBJECTIVE: This paper describes the experiences of one medical respite program in screening, assessing, and treating patients experiencin

Development and Implementation of a Medical Respite Program for People Experiencing Homelessness: An Analysis of a Cross-sectoral Partnership.

People with lived experience of homelessness (PWLEH) undergo worse health outcomes despite frequent acute care use, often because their functional and social needs remain unmet. In response, a homeless shelter and the provincial health authority formed a partnership to implement a medical respite program. We describe this collaboration by conducting a qualitative study, interviewing people involved in planning and implementing the program (n=25). Thematic analysis was performed. Stakeholders were motivated by a common desire to address the health inequities experienced by PWLEH, and frontline

Improving post-hospital care for people who are homeless: Community-based participatory research to community-based action.

This article discusses how community-based participatory research (CBPR) on hospital care transitions in New Haven, Connecticut led to the development of a new medical respite program to better serve patients who are homeless. Key insights include: • Homelessness is an important driver of hospital utilization and must be addressed in efforts to decrease hospital readmissions. • Hospitals and community organizations often serve a shared patient/client base and can work together to develop innovative programs that are beneficial to all parties. • Community-based participatory research methods ar

The impact of a family-centered case management approach.

Two urban programs of family-centered case management services were established for families of children with both a developmental disability and a chronic health condition. These are children who present significant caregiving demands due to the long-term and severe nature of their disabilities. Thirty-two mothers were interviewed within the family's first month in the project and reinterviewed approximately one year later. The results indicate that more families received respite care, nursing services, training in the care of the child, educational services and transportation to school at fo

An examination of assessment arrangements and service use for older people in receipt of care management.

With anticipated greater demand for formal care services globally, this article examines the sociodemographic and health characteristics of frail older people in receipt of community support. Data were collected from audits of case files of older people receiving care management at two time points during which two government policy initiatives were implemented to promote greater standardization in health and social care provision for older people in England. Findings at Time 2 revealed that there were higher levels of physical and mental impairment and more health care assessments undertaken.

Stubborn Families: Logics of Care of a Family Member with Borderline Personality Disorder.

This study conducted in-depth, largely unstructured interviews with 31 involved family members in a metropolitan area of the United States (US) Midwest on their experiences of BPD in a close relative. Narrative analysis employing concepts from anthropology (the logic of care and family assemblage) was used to examine the nature and quality of care practices and identify human, environmental, and cultural supports needed for family recovery. Findings indicate that these US family caregivers provided intensive and extensive care over the long term. They acted in situations of risk to their relat

Preferences for palliative care services for patients with advanced cancer from the demand-supply perspective: A multicenter discrete choice experiment.

BACKGROUND: Palliative care plays a critical role in improving quality of life for patients with life-limiting illnesses. Despite increasing global policy attention, the design and uptake of palliative-care services remain constrained by limited understanding of how different stakeholders, particularly healthcare providers and patients, value service attributes. OBJECTIVE: To examine similarities and differences in preferences for palliative-care services between providers and patients with advanced cancer from a dual supply-demand perspective. The developing role of children's nurses in community palliative care.

This article addresses some of the contemporary issues in relation to providing palliative care for children in Ireland, particularly focusing on the creation of the specialist palliative care nursing outreach posts. The recent publication of an Irish national policy on palliative care for children with life-limiting illness is welcome news for paediatric nurses in Ireland. Children are valued members of society and have a right to receive quality health care and support appropriate to meet both children and the families' precise needs. However, community services for children with life-limiti

Quality care for ill children: AIDS-specialized foster family homes.

Children with AIDS and related complexes present a new and serious challenge to the child welfare field's foster care system. At this stage of the world's knowledge of AIDS and treatment capacity for its victims, the children are stigmatized as plague bearers, their viability is unknown, and few, if any, will ever have a home to return to. This article describes a special foster care demonstration in behalf of AIDS infants.

Insights From Wise Warriors: A Needs Assessment and Preventive Parenting Program Recommendations From Parents of Children With Spinal Muscular Atrophy.

Spinal muscular atrophy (SMA) is a neuromuscular disease that significantly affects children's motor function, including the ability to breathe and swallow, and was the leading genetic cause of infant death until 2016. SMA affects not only children but entire families. SMA parents experience overall poorer mental health compared with parents of children without disabilities, impacting caregiving. However, their experiences and needs remain understudied. This study aimed to identify parent needs and recommendations for a parenting program. Semistructured interviews were conducted with 32 parent

Short breaks services for children with autistic spectrum disorders: factors associated with service use and non-use.

Short break services in a UK county were studied using a postal survey of 256 families with a child with an autistic spectrum disorder (ASD). Results confirmed high degrees of stress and low levels of informal support for all families, but no significant difference in the informal support available to non-users as compared to users of short break services. Robinson & Stalker's (1990) 10-point dependency scale showed a significant difference in dependence and more difficult behaviors between children of users and non-users. However, a large number of non-users had children with high dependence

Navigating the poverty of heroin addiction treatment and recovery opportunity in Kenya: access work, self-care and rationed expectations.

Drawing on the analyses of qualitative interview accounts of people who inject heroin in Kenya, we describe the narration of addiction treatment access and recovery desire in conditions characterised by a 'poverty of drug treatment opportunity'. We observe the performance of addiction recovery narrative in the face of heavy social constraints limiting access to care. Fee-based residential rehabilitation ('rehab') is the only treatment locally available and inaccessible to most. Its recovery potential is doubted, given normative expectations of relapse. Treating drug use is a product of tightly
