Impacts of a multipronged initiative with community HIV clinics to support retention and re-engagement in HIV care.

BACKGROUND: Despite advances in HIV treatment, gaps in care retention threaten the individual health of people with HIV (people) and public health efforts to end the HIV epidemic. OBJECTIVE: This project aimed to identify and address gaps in retention and support re-engagement in care. METHODS: A multipronged initiative at five community HIV clinics and community-based organizations (CBOs) included patient, healthcare professional (HCP), and community-focused interventions. Patient-oriented interventions included instructional videos for patients to view before appointments and conversation

Barriers and facilitators to medical care retention for pediatric systemic lupus erythematosus in South Africa: a qualitative study.

BACKGROUND: Systemic lupus erythematosus (SLE) is a life-threatening, chronic, autoimmune disease requiring long term subspecialty care due to its complex and chronic nature. Childhood-onset SLE (cSLE) is more severe than adult-onset, and the cSLE population in South Africa has been reported to have an even higher risk than patients elsewhere. Therefore, it is critical to promptly diagnose, treat, and manage cSLE. In this paper, we aim to describe and evaluate barriers and enablers of appropriate long-term care of cSLE South Africa from the perspective of caregivers (parents or family members)

The impact of neighborhood socioeconomic status on retention in care and viral suppression among people living with HIV.

Our study combined publicly available neighborhood socioeconomic status (nSES) data from the U.S. Census and clinical data to investigate the relationships between nSES, retention in care (RIC) and viral suppression (VS). Data from 2275 patients were extracted from 2009 to 2015 from a midwestern infectious diseases clinic. RIC was defined as patients who kept ≥ 3 visits and VS as an average viral load <200 copies/mL during their index year of study. Logistic regression models provided estimates for neighborhood-level and patient-level variables. In multivariable models, patients living in zip …

Postpartum urinary retention and its associated obstetric risk factors among women undergoing vaginal delivery in tertiary care hospital.

INTRODUCTION: Postpartum urinary retention (PUR) is not an uncommon consequence of bladder dysfunction after vaginal delivery. Despite its ubiquity, morbidity and discomfort brought to the postpartum women, it's a poorly discussed condition. Patients with Overt PUR are unable to void spontaneously within 6h of vaginal delivery and those with Covert PUR are able to void spontaneously but have a post void residual bladder volume (PVRV) of >150mL. Reported incidences of PUR vary widely ranging from 1.7 %-17.9 %. AIM AND OBJECTIVES: To identify the prevalence and obstetric risk factors for PUR af

Retention in HIV Care and Viral Suppression: Individual- and Neighborhood-Level Predictors of Racial/Ethnic Differences, Florida, 2015.

The objective of this study was to estimate racial/ethnic differences in retention in HIV care and viral suppression and to identify related individual and neighborhood determinants. Florida HIV surveillance records of cases aged ≥13 years diagnosed during the years 2000-2014 were analyzed. Retention in care was defined as evidence of ≥2 or more laboratory tests, receipts of prescription, or clinical visits at least 3 months apart during 2015. Viral load suppression was defined as a viral load of <200 copies/mL for the last test in 2015. Multi-level logistic regressions were used to estimate a…

Severe mental illness at ART initiation is associated with worse retention in care among HIV-infected Ugandan adults.

OBJECTIVE: The impact of severe mental illness (SMI) on retention in HIV care remains uncertain. We aimed to measure the association between SMI at antiretroviral therapy (ART) initiation and subsequent retention in care in HIV-infected Ugandan adults. METHOD: We conducted cohort study of 773 patients who initiated ART between January 2005 and July 2009 at the Butabika HIV clinic in Kampala, Uganda. SMI was defined as any clinically diagnosed organic brain syndrome, affective disorder or psychotic disorder. We used Kaplan-Meier and Cox proportional hazards analysis to evaluate the association…

Retention in HIV care and immunovirological outcomes among migrants and non-migrants: A 24-year cohort study in Barcelona.

BACKGROUND: Migration affects HIV outcomes; even with universal antiretroviral therapy (ART), migrant people living with HIV (PLWH) in Spain show worse immunovirological responses and retention in care. METHODS: A retrospective longitudinal study of 8398 PLWH followed from 2000 to 2023 was conducted at Hospital Clínic of Barcelona; 4566 (54%) were migrants. Mixed-effects and Cox models assessed immunovirological outcomes, retention, and loss to follow-up. RESULTS: Migrants were younger (median 34 vs 37) and more often male (92% vs 85%) and transgender (11% vs 1%). The baseline median cluster…

Retention in care of HIV-exposed infants and associated infant and maternal factors in Teso Sub Region, Eastern Uganda.

INTRODUCTION: Globally, about 2.1 million children less than 15 years old get infected, and 110,000 die due to HIV/AIDS-related causes. Approximately 50% of HIV-exposed infants (HEI) are retained in Early Infants Diagnosis (EID) services for follow-up testing within the recommended timeframe of 18 months after birth. However, the retention of exposed infants who depend on the adults along their 18-month care cascade remains unknown. This study aimed to assess the retention in care of HEIs at the critical monitoring points of their care cascade and to establish factors associated with infant an…

Improving estimators of HIV mortality and retention in care by linking clinical cohort data with national registries in Mexico.

INTRODUCTION: Loss to follow-up (LTFU) bias mortality estimates in cohort studies. This study aimed to estimate the corrected incidence of LTFU, retention in care, and mortality rates in people with HIV (PWH) in an HIV/AIDS Clinic in Mexico City, by linking clinic and national registry data, and to identify characteristics associated with LTFU. METHODS: We linked adult PWH who met lost to follow-up criteria while enrolled at a Mexico City clinic with the national death registry, a national administrative HIV database and a clinical national registry in May 2019 and January 2021. We defined lo…

The determinants of staff retention after Emergency Obstetrics and Newborn Care training in Kenya: a cross-sectional study.

INTRODUCTION: Kenya's maternal mortality ratio is relatively high at 342/100,000 live births. Confidential enquiry into maternal deaths showed that 90% of the maternal deaths received substandard care with health workforce related factors identified in 75% of 2015/2016 maternal deaths. Competent Skilled Health Personnel (SHP) providing emergency obstetric and newborn care (EmONC) in an enabling environment reduces the risk of adverse maternal and newborn outcomes. The study objective was to identify factors that determine the retention of SHP 1 - 5 years after EmONC training in Kenya. METHODS

A mixed-methods approach to understanding barriers to postpartum retention in care among low-income, HIV-infected women.

Optimal retention in HIV care postpartum is necessary to benefit the health and wellbeing of mothers and their infants. However, postpartum retention in HIV care among low-income women is suboptimal, particularly in the Southern United States. A mixed-methods study was conducted to identify factors associated with postpartum retention in care among HIV-infected women. Participants (n=35) were recruited during pregnancy at two county clinics and completed self-report demographic and psychosocial surveys. Twenty-two women who returned for a postpartum appointment completed a semi-structured inte…

Aligning definitions with realities: a qualitative study on the complexities of measuring retention in HIV care in the global context.

BACKGROUND: Retention in HIV care is associated with higher rates of antiretroviral treatment adherence and viral suppression, as well as lower risk of AIDS-related morbidity and mortality. However, the multidimensional nature of retention complicates measurement standardisation, limiting comparability and global evaluation. OBJECTIVES: This study explored how HIV stakeholders define and assess retention, aiming to develop a patient-centred and conceptually robust understanding to inform research and practice. METHODS: We conducted a qualitative study using interpretive description methodolo

Development and implementation of a pharmacy introductory cancer care program: Impacts on employee knowledge, engagement and retention.

IntroductionProvision of education and training is essential when transitioning staff into specialist practice areas within pharmacy. While many professional organisations provide high-quality cancer/oncology education programs, they are non-specific to the policies, workflows, and resources of service providers. Furthermore, effective training and education programs can be a tool to increase staff engagement and retention. Our organisation developed a pharmacy Introduction to Cancer Care (ICC) program to address these needs.MethodThe ICC program utilised online learning modules and a two-day

Clinically-enhanced digital health program for respiratory care associated with better medication use and retention.

Digital health platforms for asthma self-management have demonstrated promise in improving clinical and quality of life outcomes. However, few studies have examined such an approach in a real-world, fully remote setting. As such, we evaluated the benefit of an evidence-based digital self-management platform for asthma-both on its own and when integrated into an established virtual clinical service. We compared six-month outcomes of a digital self-management program plus virtual clinical oversight, called a therapeutic resource center, (DP + TRC) with a digital self-management-only (DP) program
