Purtscher-like retinopathy occurred in a 46-year-old Chinese male who received 5 mL of 2% lignocaine, retrobulbar injection, and underwent pterygium excision with conjunctiva graft. The visual acuity improved spontaneously from 20/120 on the first postoperative day to 20/20 within 8 weeks. The disc swelling, veinous tortuosity, retinal whitening, and hemorrhage resolved with time. However, relative afferent papillary defect and an arcuate visual field deficit persisted. The mechanisms of this Purtscher-like retinopathy are postulated.

OBJECTIVE: Acquired nystagmus may cause oscillopsia and in some cases decreased visual acuity. Such symptoms may be debilitating. We evaluated the efficacy of retrobulbar botulinum neurotoxin A in the visual rehabilitation of patients with acquired symptomatic nystagmus.
PATIENTS: Adults with acquired nystagmus from multiple sclerosis or brain-stem hemorrhage were recruited for this treatment study. Eligible patients were unable to perform visual tasks that they had performed prior to the onset of the nystagmus.
DESIGN AND INTERVENTION: A prospective study evaluated the results of the retrob

Five patients had trauma to the retina that occurred during cataract surgery. In two of the cases the injury was caused by perforation of the globe by the retrobulbar injection needle and in one case the retinal injury occurred when the superior rectus muscle bridle suture perforated the sclera. A stab incision for release of blood in an expulsive choroidal hemorrhage caused a retinal dialysis. Another case of retinal dialysis was caused by aspiration of solid vitreous after vitreous loss.

We report the successful removal of a retrobulbar foreign body using a transcranial approach in a 63-year-old patient with a penetrating injury to the left eye. Initial ocular examination revealed a corneoscleral laceration, hyphema, a traumatic cataract, and vitreous hemorrhage. Visual acuity consisted only of the perception of hand motion. Computed tomography demonstrated an orbital foreign body in the retrobulbar area. Emergency corneoscleral suturing, phacoemulsification of the cataract, and vitrectomy with posterior vitreous detachment were performed. Fifteen days after the emergency oper

A Purtscher-like retinopathy occurred in a 41-year-old woman who underwent the excision of a pterygium after a retrobulbar injection of 3 ml of lidocaine HCl. Visual acuity increased from 20/200 on the first postoperative day to 20/25, 12 weeks later. The exudates and the hemorrhages progressively disappeared. However, a relative, localized, paracentral scotoma and an afferent pupil defect still persist. The pathophysiology of this Purtscher-like retinopathy is discussed.

Retrobulbar haemorrhage (RBH) is a well described condition which is said to be a common cause of acute proptosis following trauma, but the evidence for this is not strong. The authors reviewed 186 publications on the subject, finding 82 cases of RBH related to trauma. This analysis suggests that in over half of the cases described, RBH was never proven conclusively. In the authors' experience RBH is not a common cause of acute proptosis following trauma and other causes need to be considered if patients are to be managed appropriately.

The authors report a case of nontraumatic orbital subperiosteal hematoma after general anesthesia with patient in the prone position. The patient, who was on aspirin, clopidogrel, and subcutaneous heparin, presented immediately after sacral ulcer debridement with acute bilateral vision loss and periorbital edema. While the OD improved with conservative management, the OS continued to have 20/200 vision, decreased color vision, afferent pupillary defect, and extraocular movement limitation after lateral canthotomy and cantholysis. Imaging revealed bilateral subperiosteal hematomas in the superi

Blepharoplasty is one of the most commonly performed surgeries for rejuvenation of the periorbital region, yet its complexity is often underappreciated. The surgeon must have a precise understanding of the underlying anatomy from skin to orbital rim and must recognize which components require surgery to achieve a desired aesthetic result in each individual. Understanding the limitations imposed by a particular patient's anatomy and conveying realistic aesthetic expectations are essential to avoid complications. The underpinning of successful blepharoplasty is an awareness of potential complica

A randomized, prospective, multicenter study evaluated the efficacy and safety of using collagen shields to deliver drugs after cataract surgery. Collagen shields saturated with an antibiotic and a steroid were placed in 90 eyes postoperatively. A control group of 93 eyes received the same drugs through a peribulbar/retrobulbar injection. One day after surgery, the shield group had significantly less corneal edema, conjunctival hemorrhaging, and postoperative pain and fewer corneal opacities. All symptoms except the conjunctival hemorrhaging disappeared by day seven. Our study suggests that us

"Guitar pick sign," also referred to as posterior globe tenting, is a radiological surrogate marker of tense orbit and profound vision loss. It is seen commonly in traumatic retrobulbar hemorrhage and carotico-cavernous fistula and less frequently in orbital cellulitis, subperiosteal abscess, and invasive fungal infections. We report a case series of Coronavirus disease-19-associated rhino-orbito-cerebral mucormycosis with guitar pick sign, of which none survived, and discuss the causative pathomechanisms, severity grade, and the clinical relevance of this unique radiological finding.

Introduction Postoperative eye patches are commonly used in orbital surgery to prevent bruising and edema, but there are only a few studies on their usefulness. We have performed bilateral orbital fat decompression as a surgical treatment for thyroid eye disease, which is very useful for relieving orbital pressure and improving functional and cosmetic outcomes. In this study, we compared the postoperative outcomes of patients with and without a postoperative compression eye patch and evaluated its effect on postoperative status based on bilateral orbital fat decompression. In endoscopic sinus surgery, the anterior ethmoidal artery (AEA) is usually identified as it traverses obliquely across the fovea ethmoidalis, posterior to the bulla ethmoidalis and anterior to or within the ground lamella's attachment to the skull base. Injury to the AEA may result in hemorrhage, retraction of the AEA into the orbit, and a retrobulbar hematoma. The resulting increase in intraorbital pressure may threaten vision. Waardenburg's syndrome (WS) is a rare congenital, autosomal dominantly inherited disorder, distinguished by characteristic facial features, pigmentation abnormalities

PURPOSE OF REVIEW: To examine the proposed mechanisms of vision-threatening injuries occurring secondary to orbital and facial trauma: traumatic optic neuropathy (TON), retrobulbar haemorrhage (RBH) and penetrating eye injury. To evaluate the evidence supporting different management options for traumatic vision-threatening injury.
RECENT FINDINGS: Despite considerable debate over the roles of surgical decompression and systemic steroid therapy for TON, these interventions have not been proved to be more effective than conservative management and there is limited evidence that the use of stero

Retrobulbar haemorrhage (RBH) occurs in a variety of situations. It can complicate facial fractures, orbital surgery and retrobulbar injections and can occur spontaneously. It is relatively uncommon and sight-threatening RBH is even less common. If not detected early enough it can lead to devastating loss of vision. We have collected five cases of acute RBH, following trauma, associated with a profound reduction in vision. In each case a permanent loss of vision was avoided using a lateral canthotomy and inferior cantholysis approach to obtain urgent orbital decompression.

UNLABELLED: The use of blunt cannulas for retrobulbar injection (retro) is recommended to reduce the risk of retrobulbar hemorrhage and globe perforation. Retro cannulas with blunt tips--so-called Atkinson needles--are commercially available. They can also be produced by being blunted on a sterile surface immediately before injection.
METHODS: In a prospective study, all retro cannulas (0.5 x 40 mm, Steriseal) used over 8 days in our hospital were collected after being blunted on a sterile Petri dish, followed by injection. Subsequently, scanning electron microscopic examination was performed
