Congenital hypothyroidism as a risk factor for hearing and parents' knowledge about its impact on hearing.

AIM: To evaluate the hearing of children with congenital hypothyroidism (CH) and to analyze the knowledge that parents' have on the possible auditory impacts of the disease. METHODS: A total of 263 parents/guardians were interviewed about aspects of CH and hearing. Audiological evaluation was performed on 80 participants, divided into two groups: with CH (n= 50) and without CH (n=30). Clinical and laboratory CH data were obtained from medical records, pure tone auditory thresholds and acoustic reflexes were analyzed. The auditory data was compared between groups. Student's t-test and Chi-squa

Hearing dysfunction in patients with Neuro-Sjögren: a cross-sectional study.

BACKGROUND: Sjögren's syndrome is an immunologically mediated disease with salivary and lacrimal gland destruction characterised by typical sicca symptoms of dry mouth and eyes. Awareness of extraglandular neurological manifestations such as polyneuropathy and affection of cranial nerves is rising. Hearing loss as consequence of involvement of the vestibulocochlear nerve presents a severe disability. The exact prevalence and nature of hearing dysfunction in patients with Neuro-Sjögren has been insufficiently evaluated to date. METHODS: Thirty patients with Sjögren's syndrome (ACR-EULAR classi

Hearing loss in vestibular schwannomas: analysis of cochlear function by means of distortion-product otoacoustic emissions.

OBJECTIVE: We investigated cochlear function in a group of patients affected by vestibular schwannoma (VS), by means of recording distortion-product otoacoustic emissions (DPOAEs). METHODS: Between January 1996 and January 2007, we observed 183 patients affected by unilateral VS. DPOAEs, compared to the corresponding hearing thresholds, were subjectively classified into three categories: "compatible" with hearing function, "cochlear" and "retro-cochlear". We also related the responses to some clinical variables (tumor size, intracanalicular tumor and radiologic appearance of the internal audi

Safety of audiology direct access for medicare patients complaining of impaired hearing.

BACKGROUND: Allowing Medicare beneficiaries to self-refer to audiologists for evaluation of hearing loss has been advocated as a cost-effective service delivery model. Resistance to audiology direct access is based, in part, on the concern that audiologists might miss significant otologic conditions. PURPOSE: To evaluate the relative safety of audiology direct access by comparing the treatment plans of audiologists and otolaryngologists in a large group of Medicare-eligible patients seeking hearing evaluation. RESEARCH DESIGN: Retrospective chart review study comparing assessment and treatme

Quantitative analysis of the visual vestibulo-ocular reflex using sinusoidal rotation in patients with peripheral vestibular disorders.

In order to investigate interaction of the vestibular and optokinetic systems in patients with peripheral vestibular disorders, the visual vestibulo-ocular reflex (V-VOR) gain in 49 patients with peripheral vestibular disorders was quantitatively measured by using pendular harmonic sinusoidal rotation with optokinetic stationary stimuli. The patients were rotated sinusoidally both with eyes closed and open at an amplitude of 240 degrees and at a frequency of 0.1 Hz (peak velocity 75.4%s). Comparing the vestibulo-ocular reflex (VOR) and V-VOR gains in the patients with definite Meniere's diseas

Pseudo-sudden deafness.

OBJECTIVES: We describe the symptom complex and management of a clinical entity we refer to as "pseudo-sudden deafness," which is an episode of acute otitis media that leads to sensorineural hearing loss with reduced speech discrimination. METHODS: We included 8 adult patients with audiometrically confirmed, asymmetric sensorineural hearing loss with decreased speech discrimination that presented after an episode of acute otitis media. Magnetic resonance imaging ruled out retrocochlear disease. Both physical examination and myringotomy helped confirm the diagnosis of serous otitis media (SOM)

[Objective auditory brainstem response threshold deficits in patients with cerebellopontine angle tumors].

The established criteria of auditory brainstem responses (ABR) such as JV latency, JI-V interpeak latency and interaural differences of latency or amplitude have been found to be sensitive for detecting tumors of the cerebellopontine angle if a response is present. However, the ABR can be absent in cases of acoustic neuromas because of desynchronization, even though pure-one audiometry indicates that responses should be present. This retrospective study compared the ABR and pure-tone thresholds in 234 cases with cerebellopontine angle tumors and a control group of 181 cases with sensory hearin

[Hearing disorders in industrial workers: clinical analysis of the cases observed in the audiological laboratory of the Lódź Institute of Occupational Medicine during the years 1976-1980].

The documentation of 697 cases with suspected, as suggested by regional health service units, occupational aetiology, was analysed. Within consultation those cases underwent complex audiological evaluation at the Laboratory of Audiology, using following examinations: pure tone threshold and suprathreshold audiometry, malingering tests, speech audiometry and electronystagmographic tests. The findings revealed that only 30% of subjects confirmed noise-induced hearing loss. 65.2% of those subjects exhibited cochlear localization, whereas in the remaining 34.8% the results of audiological tests sh

The natural progression of low-frequency hearing loss in patients who meet hybrid implant system candidacy criteria.

OBJECTIVES/HYPOTHESIS: To investigate the natural progression of hearing loss in patients with high-frequency hearing loss whose audiograms met the criteria for a hybrid cochlear implant (CI). STUDY DESIGN: Retrospective database review. METHODS: We retrospectively identified patients who met the criteria for a hybrid CI from our audiometric database. We also recorded the etiology of hearing loss in each patient and excluded patients with retrocochlear etiologies, those age <6 years, duration of observation <1 year, or those with a mixed or conductive hearing loss. We calculated the pre-CI p

Hearing rehabilitation outcomes in cochlear implant recipients with vestibular schwannoma in observation or radiotherapy groups: A systematic review.

Objectives: Vestibular schwannomas (VS) are rare benign tumours of the vestibular nerve that cause hearing loss. Management strategies include watchful waiting, radiotherapy or surgical resection. Historically, the presence of retrocochlear disease has been considered to be a contra-indication to cochlear implantation (CI). The aim of this systematic review is to assess hearing rehabilitation outcomes for CI recipients with VS, either sporadic or associated with neurofibromatosis type 2, whose tumours have been managed with either observation or radiotherapy. Methods: PubMed, Embase, and Cochra

Cost-Effective Evaluation of Asymmetric Sensorineural Hearing Loss with Focused Magnetic Resonance Imaging.

The poor sensitivity of audiometric brain stem response for small vestibular schwannomas (acoustic neuromas) creates a dilemma for the physician evaluating a patient with signs and symptoms of retrocochlear disease. Magnetic resonance imaging is recognized as the gold standard for the evaluation of these problems, but if a complete examination of the internal auditory canals and head is done on every patient, the cost is high. Although less expensive, screening with audiometric brain stem response risks missing up to 33% of small tumors. Therefore we developed a focused magnetic resonance imag

Management of vestibular schwannomas (acoustic neuromas): the value of neurophysiology for evaluation and prediction of auditory function in 420 cases.

OBJECTIVE: From 1978 to 1993, 1000 vestibular schwannomas were operated on at the Department of Neurosurgery at Nordstadt Hospital. The goal was to improve the chances of hearing preservation by recording auditory brain stem responses (ABRs). ABRs can be used for preoperative classification of cochlear nerve impairment and for prediction of the chances of hearing preservation. PATIENTS AND METHODS: In addition to the previously described audiometric testing, the patients underwent perioperative and intraoperative bilateral ABR recording at 100-dB condensation and rarefaction click stimulation

Electrically evoked hearing perception by functional neurostimulation of the central auditory system.

Perceptional benefits and potential risks of electrical stimulation of the central auditory system are constantly changing due to ongoing developments and technical modifications. Therefore, we would like to introduce current treatment protocols and strategies that might have an impact on functional results of auditory brainstem implants (ABI) in profoundly deaf patients. Patients with bilateral tumours as a result of neurofibromatosis type 2 with complete dysfunction of the eighth cranial nerves are the most frequent candidates for auditory brainstem implants. Worldwide, about 300 patients ha

Contrast enhancement of the labyrinth on MR scans in patients with sudden hearing loss and vertigo: evidence of labyrinthine disease.

The sudden onset of hearing loss and vertigo presents a difficult diagnostic problem. We describe the finding of labyrinthine enhancement on MR images in five patients with sudden unilateral hearing loss or vertigo or both and correlate the MR findings with audiologic and electronystagmographic studies. All patients were studied with T2-weighted axial images through the whole brain, contrast-enhanced 3-mm axial T1-weighted images through the temporal bone, and enhanced T1-weighted sagittal images through the whole brain. Cochlear enhancement, on the side of hearing loss only, was found in all

Scanning electron microscopy and immunoglobulins of the endolymphatic sac in normal human subjects and sensorineural deafness. With special reference to Menière's disease.

The pathophysiology of the endolymphatic sac (ES) in Meniere's disease was studied by scanning electron microscopy and staining of immunoglobulins in the intradural portion of the ES. The peroxidase-antiperoxidase method by means of paraffin sections was used for staining of immunoglobulins. First, subjects without hearing impairment and malformation of the temporal bone, ranging from a 7-month-old fetus to an 80-year-old adult, were investigated. All subjects, including fetuses, showed well-arranged epithelial cells by scanning electron microscopy. The epithelial cells in the proximal portion

ESR Essentials: diagnostic strategies in tinnitus-practice recommendations by the European Society of Head and Neck Radiology.

Tinnitus is common, with approximately 1/4 of the elderly population experiencing chronic tinnitus. While non-pulsatile tinnitus usually has no structural cause, pulsatile tinnitus is more likely to have an identifiable aetiology, and imaging plays a key role in the search for treatable and life-threatening causes. Since the characteristics of the tinnitus guide the diagnostic strategy, a detailed clinical assessment should always be performed before imaging is considered. In the setting of non-pulsatile tinnitus, imaging with MRI should only be performed if it is unilateral or asymmetric, or
