Role of steroids in conservative treatment of parapharyngeal and retropharyngeal abscess in children.

PURPOSE: To characterize the clinical features and outcomes of pediatric patients with retropharyngeal (RPA) or parapharyngeal abscesses (PPA) managed only with medical treatment and showing the importance of early symptoms and imaging studies in the diagnosis of deep neck space infections (DNIs) in children. METHODS: A retrospective analysis of all patients diagnosed with RPA and PPA between 2007 and 2017 was performed in Hospital Universitario Central de Asturias. RESULTS: 30 children were identified, with 11 RPA and 19 PPA. 23 children (76.7%) were under 5 years old, and all were treated

[Retropharyngeal abscess due to a congenital pyriform sinus fistula in a teenager].

Pyriform sinus fistulas are rare anomalies of the branchial arches. The most common form of presentation in children and adolescents is acute suppurative thyroiditis and/or recurrent lateral cervical abscesses. However, fistulas can manifest atypically. The rarity of this pathology and the atypical clinical presentation can delay the diagnosis increasing the risk of recurrent infections and complications. We present the unusual case of a 13-year-old teenager with retropharyngeal abscess due to a congenital pyriform sinus fistula successfully treated by endoscopic electrocautery.

Confluent Retropharyngeal, Lateral Pharyngeal, and Peritonsilar MRSA Abscess in an Infant.

Neck abscesses such as retropharyngeal, peritonsilar, and lateral pharyngeal are well described, typically cause a characteristic illness, and have a known epidemiology. We present a rare occurrence of case of confluent, mixed retropharyngeal, lateral pharyngeal, and peritonsilar abscess in a 9-month-old female infant. The symptoms at presentation were very mild and not expected in association with this extensive an abscess. The causative organism was methicillin-resistant Staphylococcus aureus.

Transoral negative-pressure catheter drainage of a retropharyngeal and mediastinal abscess.

PURPOSE: A retropharyngeal abscess (RPA) is an extremely rare entity in adults that has a tendency to spread vertically and cause a mediastinal abscess. Traditionally, immediate aggressive drainage is recommended via a transcervical or transthoracic approach for the treatment of a retropharyngeal abscess with mediastinal extension. Here, we present a case of a retropharyngeal and mediastinal abscess using a transoral negative-pressure catheter drainage approach. PATIENTS AND METHODS: A 24-year-old woman was admitted with a 4-day history of severe sore throat and painful swallowing. Computed t

Retropharyngeal abscess in adults: five case reports and review of the literature.

Retropharyngeal abscesses are rare in adults. They occur mostly in immunocompromised patients or as a foreign body complication. We report 5 cases of retropharyngeal abscess collected in the ENT Department of CHU Mohammed VI of Marrakech, during a two-year period (December 2008 to December 2009). Local trauma by foreign body ingestion was the aetiology in four patients. The presenting symptoms, for all patients, were fever, odynophagia, torticollis, and trismus, and the clinical examination showed bulging of the posterior wall of the oropharynx. The radiography of cervical spine showed prevert

Peritonsillar abscess with parapharyngeal and retropharyngeal involvement: incidence and intraoral approach.

CONCLUSION: This study illustrates common sites of infection seen in peritonsillar abscesses with involvement of the pharyngeal space and retropharyngeal space. Abscesses behind and/or inferior to the tonsil were encountered more frequently than expected. In these cases, the drainage had to be placed in the inferior pole of the tonsil and these types were frequently seen in older patients. OBJECTIVES: The aim of this study was to assess to what extent abscesses spread in patients with peritonsillar abscess and to determine to what extent pus can be drained intraorally. PATIENTS AND METHODS:

Retropharyngeal abscess complicated with torticollis: case report and review of the literature.

Retropharyngeal abscess commonly develops among infants and small children, and is associated with the severe inflammation of the retropharyngeal lymph nodes located in the retropharyngeal space. Retropharyngeal abscess causes cervical pain, swelling, contracture of the neck, and in rare cases inflammatory torticollis, all of which result from an inflammatory process that irritates the cervical muscles, nerves or vertebrae. Here we report a rare case of retropharyngeal abscess with a complication of torticollis. A 4-year-old girl suffered from severe retropharyngeal abscess spreading through t

Retropharyngeal abscess: a clinical experience at the University College Hospital Ibadan.

BACKGROUND: Infection within the retropharyngeal space could progress on to an abscess formation resulting into retropharyngeal abscess (RPA), which can either be acute or chronic. RPA can be a life-threatening emergency, with potential for airway compromise and other catastrophic complications. This review is aimed at highlighting our experience with patients with a diagnosis of retropharyngeal abscess. METHODS: Retrospective review of thirty patients with a confirmed diagnosis of retropharyngeal abscess, who were admitted and managed within an eleven-year period (1993 to 2003) in the Otolar

[Non-dystonic torticollis. A report of a case secondary to retropharyngeal abscess].

INTRODUCTION: There are many causes of acquired non-dystonic torticollis: cervical bony anomalies, nasopharyngeal infections, tumours of the spinal cord, and posterior fossa, ocular, vestibular and gastrointestinal disorders. In children, non-dystonic is commoner than dystonic torticollis, except for in adverse reactions to drugs. Palatopharyngeal lesions due to a rigid object being impacted in the mouth (pencil-injury) are common in childhood. Many are not dangerous and require no specific treatment, although there is a risk of perforation of the pharyngeal wall and of a retropharyngeal absce

Internal carotid artery narrowing in children with retropharyngeal lymphadenitis and abscess.

BACKGROUND AND PURPOSE: Our purpose was to describe the association between narrowing of the internal carotid artery (ICA) and retropharyngeal abscess in children. METHODS: Neck CT scans from 13 consecutive children with suppurative retropharyngeal lymphadenitis and abscess were evaluated retrospectively for asymmetric ICA diameters at the level of the abscess. Clinical status at the time of illness was established via a chart review. Twenty control CT scans obtained from pediatric patients with normal imaging findings were evaluated prospectively to determine symmetry and size of the ICA. R

Retropharyngeal abscess revealing Granulomatosis with polyangiitis GPA (Wegener's): a case report.

INTRODUCTION: A retropharyngeal abscess (RPA) is an infection that develops in the deep spaces of the neck, more common in children under five due to their prominent lymph nodes. However, it can also occur in adults. To our knowledge, Case reports of RPA revealing Granulomatosis with polyangiitis (GPA) (Wegener's) have yet not been reported. CASE PRESENTATION: We present a case of a 62-year-old woman, originally from West Africa, with symptoms of sore throat, shortness of breath and otalgia. Imaging was compatible with RPA. Surgical drainage involved endoscopic and oral procedures with biopsi

Varicella Infection Complicated by Group A Beta-Hemolytic Streptococcal Retropharyngeal Abscess.

An unimmunized 19-month-old child presented with a retropharyngeal abscess and coincident varicella infection. The abscess resolved with operative drainage. This is the first published report of this connection, although varicella is known to be associated with abscesses in general. Practitioners should be aware that cervical abscesses may complicate varicella infections.

Clinical and radiological predictors of retropharyngeal abscesses in deep neck infections.

BACKGROUND: Retropharyngeal abscesses (RPAs) are severe deep neck infections that require rapid intervention to prevent life-threatening complications. This study aimed to evaluate the clinical, laboratory, and radiological risk factors that predict the need for intensive care unit (ICU) admission and surgical drainage in adult patients with RPAs. METHODS: A retrospective review was conducted on patients treated for RPAs at our center between January 2013 and December 2023. Of the 64 initial cases, 57 adults were included in the final analysis after excluding pediatric patients and incomplete

Retropharyngeal Abscess With Mediastinal Extension: A Case Series and Review of the Literature.

Deep neck infections are common in infants and occur in several anatomic subsites including the retropharyngeal space. Retropharyngeal abscesses are significant given their propensity for mediastinal extension and can have life-threatening sequelae. We present 3 cases of retropharyngeal abscess with mediastinal extension in infants. In one case, an incompletely vaccinated 10-month-old boy presented with cough, rhinorrhea, and fever. Despite antibiotic treatment, he developed Horner's syndrome and hypoxia. A computed tomography (CT) scan showed a C1-T7 retropharyngeal abscess. He underwent tran

Spontaneous Cervical Spondylodiscitis With Retropharyngeal Abscess and Bacteremia: A Case Report.

Retropharyngeal abscess is a deep neck infection, rarely reported in adults. Nevertheless, when it occurs, it is mostly in immunocompromised patients and it can have life-threatening complications such as airway obstruction. On the other hand, more insidious complications can develop, such as mediastinitis, spinal osteomyelitis, and epidural abscess which represent an emergency medical condition when the patient develops neurologic symptoms. All must be diagnosed early and treated promptly. Spinal infection is an ancient disease, yet, morbidity remains significant despite developments in surgi

Awake tracheostomy in a child with respiratory distress due to retropharyngeal abscess.

Awake tracheostomy in a child with respiratory distress is an emergency life-saving procedure when risk of airway loss after induction of general anaesthesia is greater due to difficult anatomy. A 10-year-old boy presented three days after removal of a foreign body in the throat under general anaesthesia. Over the subsequent days, the patient had a progressively increasing visible swelling in the neck, stridor and respiratory distress. An urgent X-ray and computed tomography scan of the neck revealed a retropharyngeal abscess compressing the trachea. Due to anticipated difficulty in airway man
