Selective dorsal rhizotomy from indication to rehabilitation: a worldwide survey.

PURPOSE: Selective dorsal rhizotomy (SDR) is a neurosurgical treatment used worldwide to reduce spasticity. The procedure has undergone many changes since its introduction in the early 1900s, and currently, different centers vary in many aspects of the procedure. We surveyed centers on different continents regarding SDR indications, surgical techniques, and postoperative rehabilitation. METHODS: Ten centers worldwide with SDR experience participated in an online survey preparing for a pre-conference workshop in 2022. The main topics were patient characteristics, the selection process, surgery

Long-Term Outcome of Selective Dorsal Rhizotomy for Spastic Children.

Given the concerns associated with the ageing process in adults with cerebral palsy (CP) and the significant number of children who have undergone selective dorsal rhizotomy (SDR) in the recent decades, there is an important clinical need to evaluate both the short- and long-term impacts of this invasive and irreversible neurosurgical procedure. To gain insight in the health condition, the International Classification of Functioning, Disability and Health (ICF) model was used. The ICF serves as a biopsychosocial model for evaluating individuals with disabilities, providing a comprehensive pers

Computed Tomography Guidance for Percutaneous Glycerol Rhizotomy for Trigeminal Neuralgia.

BACKGROUND: Percutaneous glycerol rhizotomy (PGR) is a well-described treatment for trigeminal neuralgia; however, the technique in using surface landmarks and fluoroscopy has not drastically changed since being first introduced. In this paper, we describe a protocol for PGR using computed tomography (CT) guidance based on an experience of over 7 yr and 200 patients. OBJECTIVE: To introduce an approach for PGR using CT guidance and, in doing so, demonstrate possible benefits over the traditional fluoroscopic technique. METHODS: Using a standard CT scanner, patients are placed supine with hea

Effectiveness of repeat glycerol rhizotomy in treating recurrent trigeminal neuralgia.

BACKGROUND: Percutaneous glycerol rhizotomy (GR) is used to treat trigeminal neuralgia (TN), with satisfactory pain relief lasting 2 to 3 years in most patients after the first intervention. The efficacy of subsequent GRs, however, has not been studied. OBJECTIVE: To compare the pain relief and durability achieved by the first GR with those obtained after subsequent GRs in a retrospective cohort of TN patients. METHODS: Between 1998 and 2010, 548 patients with TN underwent 708 GRs. After exclusions, 430 initial GRs (GR1) and 114 subsequent GRs (GR2+) were compared in terms of initial pain re

Percutaneous trigeminal ganglion balloon compression rhizotomy: experience in 27 patients.

PURPOSE: Percutaneous ganglion balloon compression (PBC) is a minimally invasive procedure for treatment of trigeminal neuralgia. MATERIALS AND METHODS: Twenty-seven (19 female and 8 male) patients, who presented with classical symptoms of trigeminal neuralgia, were included. Age ranged from 34 to 91 years (median 62 years), 33 procedures were performed. Duration of the symptoms ranged from 1 year to 30 years (median 5 years). RESULTS: After the procedure, pain relief was reported in 25 (93%) patients. In two patients, the pain remained the same. The pain free period ranged from 2 to 74 mont

Percutaneous balloon rhizotomy for trigeminal neuralgia using three-dimensional fluoroscopy.

BACKGROUND: Percutaneous balloon rhizotomy is one of the standard techniques for the treatment of trigeminal neuralgia. However, there have been well-reported complications from cannulating the foramen ovale (FO). We describe a novel technique for cannulating the FO using 3-dimensional (3D) rotational fluoroscopy. METHODS: Three-dimensional rotational fluoroscopy is used to reconstruct the skull base. The optimal working projection is thus generated to best visualize the FO. When the optimal working projection is not anatomically feasible, for example, in a patient with severe cervical spondy

Predicting functional change from preintervention measures in selective dorsal rhizotomy.

OBJECT: In this investigation the authors attempted to predict change in function following selective dorsal rhizotomy (SDR) and intensive physical therapy in patients with spastic diplegic cerebral palsy (CP) based on multidomain preintervention measures. METHODS: Data pertaining to 22 children with CP were collected before the SDR and again 20 months afterward. Although equations for predicting change in gait speed and function (such as the Gross Motor Function Measure) were derived, the 95% confidence interval (CI) widths were too broad to make accurate predictions that were clinically use

Percutaneous retrogasserian glycerol rhizotomy for trigeminal neuralgia: technique and expectations.

OBJECT: In the management of trigeminal neuralgia (TN), physicians seek rapid and long-lasting pain relief, together with preservation of trigeminal nerve function. Percutaneous retrogasserian glycerol rhizotomy (PRGR) offers distinct advantages over other available procedures. The aim of this report was to provide details of the PRGR procedure and its expected outcome. METHODS: The authors reviewed their experience with PRGR in 1174 patients to evaluate the procedural technique, results, and complications. Although it is clear that TN is not a static disorder but one characterized by remissi

Spinal deformities after selective dorsal rhizotomy for spastic cerebral palsy.

OBJECT: Spinal deformities are significant problems in children with spastic cerebral palsy. The treatment of their spasticity by selective dorsal rhizotomy (SDR) may worsen or improve these problems. The purpose of this study was to determine the incidence of and change in degree of thoracolumbar scoliosis, thoracic kyphosis, and lumbar lordosis in children who have undergone SDR. METHODS: A retrospective review was conducted of children younger than 18 years of age, who had undergone SDR at British Columbia's Children's Hospital from 1987 to 2001 and in whom preoperative and postoperative s

Posterior fossa partial trigeminal rhizotomy: an alternative to microvascular decompression.

Doctor Walter Dandy was a pioneer in the surgical treatment of trigeminal neuralgia. The Frazier-Spiller operation had been the standard operation for trigeminal neuralgia. Doctor Dandy pioneered the approach through the posterior fossa. He sectioned the lower-most 30 percent of the trigeminal nerve and found that trigeminal neuralgia was relieved. Anesthesia dolorosa did not develop, and the patients had only minimal sensory loss. Dandy's operation was not widely accepted, but microvascular decompression as described by Jannetta was developed by application of Dandy's principles. In twenty pa

Selective Dorsal Rhizotomy for the Treatment of Spastic Hemiplegic Cerebral Palsy.

Background Selective dorsal rhizotomy (SDR) can remove spasticity in cerebral palsy (CP). Spastic hemiplegia is associated with spasticity in the upper and lower limbs on one side. Only a single report described the outcome of SDR specifically in patients with spastic hemiplegic CP. The effect of SDR on spastic hemiplegia requires further investigation. Objectives To analyze the outcomes of motor functions, the quality of life, and satisfaction of patients who received SDR for the treatment of spastic hemiplegia. Methods A total of 29 children and 1 adult who received SDR were surveyed. The su

CyberKnife radiosurgical rhizotomy for the treatment of atypical trigeminal nerve pain.

OBJECT: Patients with atypical trigeminal neuralgia (TN) have unilateral pain in the trigeminal distribution that is dull, aching, or burning in nature and is constant or nearly constant. Studies of most radiosurgical and surgical series have shown lower response rates in patients with atypical TN. This study represents the first report of the treatment of atypical TN with frameless CyberKnife stereotactic radiosurgery (SRS). METHODS: Between 2002 and 2007, 7 patients that satisfied the criteria for atypical TN and underwent SRS were included in our study. A 6-8-mm segment of the trigeminal n

Effect of neonatal spinal transection and dorsal rhizotomy on hindlimb muscles.

The purpose of this study was to elucidate the effect of deafferentation on spinal motoneurons. We studied the effects of spinal cord transection and/or dorsal rhizotomy upon the contractile properties of EDL and soleus muscle, as well as on the number of motoneurons corresponding to these muscles. Neonatal Wistar rats were randomly divided into four groups in which spinal midthoracic section (T8-T10), unilateral dorsal lumbar rhizotomy (L3-S2) or both procedures were performed on the second postnatal day (PND2). Another group served as unoperated control. At 2 months of age, the animals were

Treatment of occipital neuralgia by partial posterior rhizotomy at C1-3.

To minimize the sensory loss associated with intradural posterior rhizotomy for medically refractory occipital neuralgia, partial sectioning of the upper cervical posterior rootlets was performed in 11 patients. The ventrolateral aspect of each posterior rootlet from C-1 to the upper portion of C-3 was divided at the root entry zone. In three patients with bilateral neuralgia, the procedure was performed on both sides, for a total of 14 partial rhizotomy procedures in the 11 patients. This resulted in satisfactory preservation of scalp sensation in all cases. Pain within the territory of the g

Retrogasserian glycerol rhizotomy and its selectivity in the treatment of trigeminal neuralgia.

In the treatment of trigeminal neuralgia, the possibility of obtaining a selective effect on different trigeminal branches by glycerol rhizotomy was studied. Transcutaneous electrical stimulation was used to quantify sensory impairment. An attempt was made to obtain a localized neurotoxic effect of the glycerol on the different trigeminal branches by keeping the patient's head in different positions during and for one hour after glycerol injection. The amount of glycerol injected varied according to the estimated size of the trigeminal cistern and/or to which branch that was involved. The stud

Effects of selective dorsal rhizotomy on gait in children with cerebral palsy.

We wished to examine the effects of selective dorsal rhizotomy (SDR) on the gait patterns of children with cerebral palsy (CP). Nineteen ambulatory children underwent preoperative and 1-year postoperative gait analyses that included clinical assessment of joint range of motion (ROM) and muscle tone, three-dimensional motion analysis, and dynamic electromyography (EMG). The children were divided into two groups: independent ambulators (n = 11) and dependent ambulators (requiring a walking aid) (n = 8). Improvements in joint passive ROM and a reduction in lower extremity spasticity were noted. I
