Risk of Intussusception After Rotavirus Vaccination.

BACKGROUND: In 2013, the German Standing Committee on Vaccination (Ständige Impfkommission, STIKO) recommended rotavirus (RV) vaccination for all infants while stating that this mildly increased the risk of intussusception, a potentially life-threatening event. Since this recommendation was issued, multiple observational studies on this topic designed as self-controlled case series (SCCS) have been published. The SCCS design is particularly suitable for the study of rare adverse effects of medications. METHODS: We systematically searched the literature for SCCS studies on the risk of intussus

Rotavirus vaccination in Central American children.

Rotavirus is the leading cause of acute diarrhea in children younger than five years of age around the world. Severe dehydration and mortality rates are higher in developing countries, especially those from Latin America, Africa, and Asia. The vaccine has been introduced in the national immunization programs of more than half of Latin American countries, and impact data from some of these nations has been already published. The two rotavirus vaccines, the 2-dose monovalent (RV-1) and the 3-dose pentavalent (RV-5) vaccine, have been available in the market to all Central American countries. Rot

Rotavirus gastroenteritis complicated with toxic megacolon.

UNLABELLED: Rotavirus is a leading cause of gastroenteritis in young children, which may indicate hospitalization due to dehydration and electrolyte imbalance. Most cases are self-limited with good prognosis. The association between rotavirus and toxic megacolon has never been mentioned in the literature. We report a case of toxic megacolon secondary to rotavirus gastroenteritis. CONCLUSION: Toxic megacolon can occur in patients with rotavirus gastroenteritis. An abdominal radiograph should be taken for patients with rotavirus gastroenteritis who have systemic toxicity and persistent abdomina

A live attenuated human rotavirus vaccine.

Rotavirus infections are the leading cause of severe gastroenteritis in young children worldwide. This review provides a summary of the development of a live oral rotavirus vaccine, Rotarix (GlaxoSmithKline, Rixensart, Belgium) developed from a single protective G1P[8] human strain. To attenuate the wild-type virus, the original isolate, 89-12, was passaged multiple times in tissue culture. Large safety and efficacy trials that have included more than 100,000 children around the world have shown the vaccine is safe and not associated with intussusception. The vaccine is highly immunogenic and

Rotavirus vaccines: targeting the developing world.

For the past 2 decades, rotavirus infection, the most common cause of severe diarrhea in children, has been a priority target for vaccine development. This decision to develop rotavirus vaccines is predicated on the great burden associated with fatal rotavirus disease (i.e., 440,000 deaths/year), the firm scientific basis for developing live oral vaccines, the belief that increased investment in development at this time could speed the introduction of vaccines in developing countries, and the appreciation that implementation of a vaccine program should result in a measurable decrease in the nu

Neonatal hypocalcaemia associated with rotavirus diarrhoea.

UNLABELLED: We observed an association between rotavirus diarrhoea and hypocalcaemia in several patients and therefore started a prospective evaluation with measurement of calcium levels in all patients with rotavirus infection during a period of 8 months. We report on 54 infants with rotavirus gastro-enteritis. Serum concentrations of sodium, potassium, and total and ionized calcium were measured on admission. If hypocalcaemia was detected, total and ionized calcium were measured every day until recovery. Calcium was supplemented as calcium gluconate which was added to milk. Out of 54 newborn

The rotavirus cVLPVP8∗ vaccine candidate induces cross-genotype/heterotypic neutralizing antibody responses against human rotavirus G9P[4].

Live oral rotavirus (RV) vaccines show variable efficacy and safety profiles, particularly in low- and middle-income countries (LMICs). We have developed a chimeric hepatitis B core parenteral virus-like particle displaying VP8∗P[8] RV vaccine antigen (cVLPVP8∗) capable of inducing VP8∗- specific serum neutralizing antibodies (nAbs). The immunogenicity of cVLPVP8∗ and its potential to induce cross-genotype/heterotypic nAbs was investigated following subcutaneous administration of low, medium, and high doses in mice. The cVLPVP8∗ vaccine elicited potent VP8∗-specific humoral responses, includin

Concurrent oral poliovirus and rhesus-human reassortant rotavirus vaccination: effects on immune responses to both vaccines and on efficacy of rotavirus vaccines. The US Rotavirus Vaccine Efficacy Group.

Interference between oral poliovirus vaccine (OPV) and monovalent (RRV-S1) and tetravalent (RRV-TV) rhesus-human rotavirus vaccines was evaluated. Serum antibody responses to OPV and rotavirus vaccines and efficacy of rotavirus vaccines were compared among control and vaccine groups stratified by number of concurrent OPV and rotavirus vaccinations received. Neutralizing antibody titers to poliovirus type 1 tended to rise more steeply in placebo than RRV-TV recipients, but there were no significant differences in seroprevalence or in geometric mean titers (GMTs) of antibodies to types 1, 2, or

Bovine rotavirus: A narrative review of rotavirus a biology, vaccination strategies, and regional insights from Morocco.

Group A rotavirus commonly infects cattle and results in gastrointestinal disorders that are responsible for heavy losses among these animals and within the livestock industry in general. In Morocco, the G10P[14] and G6P[5] genotypes are frequently observed; however, limited data contribute to restricted vaccine choice and disease control. Preventive measures include effective colostrum management practices, hygiene, and vaccination of pregnant cows to pass on passive immunity to newborn calves. Nevertheless, the effectiveness of vaccines in Morocco has not been ascertained because there has b

Acute viral gastroenteritis following rotavirus vaccine implementation in Venezuela: Is rotavirus still a cause for concern?

INTRODUCTION: Acute gastroenteritis (AGE) remains a major public health concern for the pediatric population. Diarrheal surveillance in Venezuela following the implementation of the rotavirus vaccines has been discontinuous, resulting in a lack of knowledge of the true epidemiological burden. This study investigated retrospectively the occurrence of enteropathogenic virus infections and potential changes in the etiological pattern of diarrheal disease in Venezuelan children during the post-vaccination period. METHODOLOGY: Stool samples from 150 children with AGE and 148 controls under five ye

Impact of rotavirus vaccine on paediatric rotavirus hospitalisation and intussusception in New Zealand: A retrospective cohort study.

BACKGROUND: Rotavirus results in a significant burden of hospitalisations and deaths globally. Rotavirus vaccine has been used in New Zealand since July 2014. The aim of this study was to assess the safety and effectiveness of RotaTeq® vaccine in New Zealand between 2006 and 2016. METHODS: A national cohort study of 723,695 children aged less than 6 years was carried out using linked administrative datasets. Study outcomes were hospitalisation for intussusception, rotavirus, and all-cause gastroenteritis. Intussusception hospitalisation rates were calculated from 2006 to 2016, and rotavirus a

The burden of rotavirus diarrhea in Khanh Hoa Province, Vietnam: baseline assessment for a rotavirus vaccine trial.

BACKGROUND: In Vietnam, rotavirus is seen as a priority disease because studies have demonstrated that >50% of children hospitalized for treatment of diarrhea have rotavirus as the pathogen. To anticipate the availability of new vaccines, we have examined our field area in Nha Trang, Khanh Hoa Province, Vietnam, as a potential site to conduct a field trial of a future rotavirus vaccine. METHODS: Data from a population census, incidence rates of diarrhea from a previous cholera vaccine trial and hospitalization rates from computerized records collected from the 2 main hospitals in the province

Rotavirus vaccines: new drug. Rotavirus infection: one case of severe diarrhoea prevented per 100 vaccinated infants.

(1) Each year in France, diarrhoea caused by rotavirus kills about 10 infants under three years of age and leads to about 210 hospitalisations per 100 000 children under 5 years of age. (2) Two new oral rotavirus vaccines designed for immunisation during infancy are marketed or shortly to be marketed in France for the first time; one is based on a human strain and the other on both bovine and human strains. (3) The vaccine based on a human strain has been tested in a double-blind placebo-controlled trial involving about 63 000 infants. It had no statistically significant impact on mortality bu

Rotavirus vaccines: progress and new developments.

INTRODUCTION: Rotavirus is the primary cause of severe acute gastroenteritis among children under the age of five globally, leading to 128,500 to 215,000 vaccine-preventable deaths annually. There are six licensed oral, live-attenuated rotavirus vaccines: four vaccines pre-qualified for global use by WHO, and two country-specific vaccines. Expansion of rotavirus vaccines into national immunization programs worldwide has led to a 59% decrease in rotavirus hospitalizations and 36% decrease in diarrhea deaths due to rotavirus in vaccine-introducing countries. AREAS COVERED: This review describes

New approaches in oral rotavirus vaccines.

Rotavirus is the leading cause of severe dehydrating diarrhea worldwide, and affects primarily developing nations, in large part because of the inaccessibility of vaccines and high rates of mortality present therein. At present, there exist two oral rotaviral vaccines, Rotarix™ and RotaTeq™. These vaccines are generally effective in their actions: however, associated costs often stymie their effectiveness, and they continue to be associated with a slight risk of intussusception. While different programs are being implemented worldwide to enhance vaccine distribution and monitor vaccine adminis

Opsoclonus-myoclonus syndrome following rotavirus gastroenteritis.

Opsoclonus-myoclonus syndrome (OMS) is a rare neurologic disorder characterized by opsoclonus, myoclonus, ataxia and behavioral disturbance. In the pathogenesis, an autoimmune process with infectious or paraneoplastic trigger has been suggested. We describe the case of a 22-month-old girl with OMS following rotavirus gastroenteritis. Rotavirus should be considered in the differential diagnosis of OMS in children.
