Reforming 340B to Promote Program Integrity and Better Serve Vulnerable Populations: An American College of Physicians Policy Brief.

The United States pays some of the highest prices for prescription drugs in the world, with drug prices and overall spending increasing year-over-year. Ballooning prescription drug costs can result in low rates of medication adherence, negatively affecting public health and straining national health care systems and resources. The 340B Drug Pricing Program (340B Program), which allows health care safety-net settings to purchase prescription drugs at deep discounts, has long sought to function as a bulwark against increasing drug prices for institutions serving those most vulnerable in society.

Medical Respite to Support Patients Experiencing Homelessness and Cancer Treatment.

INTRODUCTION: People experiencing homelessness (PEH) face significant risks of early cancer-related mortality due to barriers of care for complex treatments. We describe our pilot study for PEH with advanced cancer undergoing treatment at a large safety-net hospital system. METHODS: We performed a retrospective case series of PEH with advanced cancer undergoing medical respite from 2020 to 2024. Descriptive statistics were used to describe patient characteristics and outcomes. RESULTS: Nineteen patients participated in medical respite program while undergoing cancer treatment. Patients were

Assessing multilevel barriers and facilitators to implementing strategies for cancer screening among Asian Americans in federally qualified health centers: a case study of a community-clinic partnership to improve care for safety-net patients.

BACKGROUND: Cancer screening disparities among medically underserved Asian Americans are well documented. Assessing determinants of success in implementing multilevel cancer screening strategies in safety-net settings is critical to improve screening and cancer equity. METHODS: Academic, clinic, and community partners established the Advancing Care Together (ACCT) formal network to implement multilevel strategies that promote cancer screening among low-income Chinese, Korean, and Vietnamese adults in Orange County, California. ACCT focused on breast, cervical, and colorectal cancer. From Augu

Outcomes of Rapid Restart Among People With Previously Diagnosed HIV at a Safety-Net HIV Clinic in San Francisco.

BACKGROUND: Little is known about outcomes of rapid restart of antiretroviral therapy (ART) among people with HIV relinking to care. SETTING: Safety-net HIV clinic in San Francisco. METHODS: Using electronic medical record data, we conducted a retrospective study of adults with HIV (≥18 years old, out of care, self-reported off ART) seen for rapid (same-day) restart of ART. Descriptive statistics summarized baseline sociodemographic and clinical variables. χ 2 and Fisher exact tests assessed associations between sociodemographic or clinical variables and 2 primary outcomes: (1) viral suppres

Lived Experiences of Families Navigating Safety Net Expansions and Retractions During the Pandemic: A Qualitative Study.

Objectives. To understand the experiences of families with low income in California with pandemic safety net support expansions and retractions, including barriers to program access. Methods. Using open-ended questions, we explored the self-reported experiences of pandemic-era safety net expansions and expirations between January and June 2023 among a group of caregivers of young children (n = 44). We used the Immersion-Crystallization technique to analyze the data, creating a codebook and identifying themes as they emerged. Results. We identified 4 main themes, including appreciation for safe

An evaluation of the Be Your Own Baby public awareness campaign.

OBJECTIVE: Assess the impact of the Be Your Own Baby (BYOB) public awareness campaign including population-level exposure, the effectiveness of ad platforms, and the effect of the campaign on family planning clinic attendance, the campaign's primary goal. DATA SOURCES: The study relied on administrative data on traffic and engagement from the campaign's website, population survey data measuring campaign exposure, and clinic attendance volumes from state-by-year restricted-use versions of the Office of Population Affairs' Family Planning Annual Reports (2006-2018). STUDY DESIGN: Bivariate ana

Treatment of Acute Cholecystitis at Safety-Net Hospitals: Analysis of the National Inpatient Sample.

Evidence supports index cholecystectomy as the quality of care for patients admitted with acute cholecystitis. We sought to examine the role of hospital safety-net status on whether patients received appropriate index procedures for cholecystitis. The National Inpatient Sample was queried for patients with acute cholecystitis. Proportion of Medicaid and uninsured discharges were used to define safety-net hospitals (SNHs). Multivariate logistic regression was used to calculate associations between the frequency of index cholecystectomy and prolonged length of stay (LOS), and the effect of SNH d

Laparoscopic Appendectomy: A Report on 1164 Operations at a Single-Institution, Safety-Net Hospital.

Laparoscopic appendectomy (LA) is the standard of care for the treatment of acute appendicitis. There is an ongoing debate regarding the optimal management of appendicitis, which led us to study outcomes after an appendectomy at a large safety-net hospital. We hypothesize that despite a high-risk population, LA remains a safe and effective treatment for acute appendicitis. A retrospective review was performed of all patients who underwent an appendectomy from 2011 to 2013. The primary end point was significant morbidity defined as a score of three or greater on the Clavien-Dindo scale of surgi

Who's boarding in the psychiatric emergency service?

INTRODUCTION: When a psychiatric patient in the emergency department requires inpatient admission, but no bed is available, they may become a "boarder." The psychiatric emergency service (PES) has been suggested as one means to reduce psychiatric boarding, but the frequency and characteristics of adult PES boarders have not been described. METHODS: We electronically extracted electronic medical records for adult patients presenting to the PES in an urban county safety-net hospital over 12 months. Correlative analyses included Student's t-tests and multivariate regression. RESULTS: 521 of 536

The health care safety net: money matters but savvy leadership counts.

The nation's health care safety net--heavily reliant on external funding and support--is uniquely vulnerable to shifting and often adverse market and policy conditions. While adequate funding is essential to ensuring safety net providers can care for low-income people, the Center for Studying Health System Change (HSC) has identified a number of other factors key to building and maintaining viable community safety nets. Throughout the four rounds of HSC's Community Tracking Study (CTS) site visits, researchers have found that strong political and organizational leadership, community support, c

State-Funded Health Insurance Expansion for Noncitizen Immigrants and Hospital Uncompensated Care.

IMPORTANCE: Undocumented and many noncitizen immigrants are largely excluded from federally funded health insurance programs, creating substantial gaps in health coverage and financial strain on safety-net hospitals that disproportionately serve these populations through uncompensated care. Several states have implemented state-funded health coverage expansions for immigrants, but the financial implications of these policies remain poorly understood. OBJECTIVE: To examine whether Health Benefits for Immigrant Seniors (HBIS) and Health Benefits for Immigrant Adults (HBIA) programs in Illinois

Opportunities and Challenges for the Federally Qualified Health Center Program: A Critical Review.

Federally Qualified Health Centers (FQHCs) were founded to improve access to primary health care for medically underserved areas and populations. Since their inception in the 1960s, FQHCs have grown to provide comprehensive primary care services to more than 31 million patients in 2023. In this review, we provide an overview of the role and growth of the FQHC program, summarize the existing evidence on its association with key outcomes, and discuss opportunities and challenges for the FQHC model. Evidence finds that FQHCs have played a crucial role in expanding access to primary care for low-i

Closing Gaps in Chronic Kidney Disease Detection: Evaluating At-Home Targeted Testing in a Safety-Net Population.

INTRODUCTION: Chronic Kidney Disease (CKD) affects 1 in 7 adults in the United States, yet 90% of those impacted remain unaware of their condition, and fewer than 20% of at-risk individuals are appropriately tested. Safety-net health care settings are disproportionately burdened by CKD, with a patient population enriched for CKD risk factors, social deprivation, and barriers to diagnostic testing which delay access to diagnosis and lifesaving interventions. The National Kidney Foundation partnered with a Federally Qualified Health Center (FQHC) to evaluate an approach to increase guideline-rec

Prevalence and Impact of Substance Use on Hospitalization and Post-Discharge Outcomes in Individuals with Congestive Heart Failure: Findings from a Safety-Net Hospital.

Introduction: Congestive heart failure (CHF) is a major global health challenge and the leading cause of hospitalization in the U.S., with disproportionately high 30-day readmission rates among low-income and minority communities. Social drivers of health and substance use both influence CHF outcomes, yet the effect of substance use on short-term readmissions remains understudied. This study evaluated the association between substance use and all-cause 30-day readmissions at a Safety-Net Community Hospital. Methods: A retrospective chart review was conducted among 500 adults admitted with CHF

The Association Between Hospital-Based Food Pantry Use and Subsequent Emergency Department Utilization Among Medicaid Patients With Diabetes.

We explored the association between the use of a hospital-based food pantry and subsequent emergency department (ED) utilization among Medicaid patients with diabetes in a large safety-net health system. Leveraging 2015-2019 electronic health record data, we used a staggered difference-in-differences approach to measure changes in ED use before vs after food pantry use. Food pantry use was associated with a 7.3 percentage point decrease per patient per quarter (95% confidence interval, -13.8 to -0.8) in the probability of subsequent ED utilization ( P = .03). Addressing food insecurity through

Engagement in mHealth-Prompted Self-Measured Blood Pressure Monitoring Among Participants Recruited From a Safety-Net Emergency Department: Secondary Analysis of the Reach Out Trial.

BACKGROUND: Hypertension, a key modifiable risk factor for cardiovascular disease, is more prevalent among Black and low-income individuals. To address this health disparity, leveraging safety-net emergency departments for scalable mobile health (mHealth) interventions, specifically using text messaging for self-measured blood pressure (SMBP) monitoring, presents a promising strategy. This study investigates patterns of engagement, associated factors, and the impact of engagement on lowering blood pressure (BP) in an underserved population. OBJECTIVE: We aimed to identify patterns of engageme

Resilience and diabetes self-management among African-American men receiving primary care at an urban safety-net hospital: a cross-sectional survey.

BACKGROUND: Resilience is the ability to adapt to adverse life events. Studies that explore diabetes self-management interventions integrating resilience in African-Americans with diabetes include few African-American men, who have higher diabetes-related mortality and complication rates compared to African-American women. DESIGN: We conducted a cross-sectional study of African-American men with uncontrolled diabetes living in diabetes hotspots. We measured resilience levels using the General Self Efficacy Scale (GSES), adherence to diabetes self-management behaviors using the Diabetes Self-M
