Regulation of sarcomere formation and function in the healthy heart requires a titin intronic enhancer.

Heterozygous truncating variants in the sarcomere protein titin (TTN) are the most common genetic cause of heart failure. To understand mechanisms that regulate abundant cardiomyocyte (CM) TTN expression, we characterized highly conserved intron 1 sequences that exhibited dynamic changes in chromatin accessibility during differentiation of human CMs from induced pluripotent stem cells (hiPSC-CMs). Homozygous deletion of these sequences in mice caused embryonic lethality, whereas heterozygous mice showed an allele-specific reduction in Ttn expression. A 296 bp fragment of this element, denoted

Proteolytic degradation of atrial sarcomere proteins underlies contractile defects in atrial fibrillation.

Atrial fibrillation (AFib) is the most common cardiac rhythm disturbance, often treated via electrical cardioversion. Following rhythm restoration, a period of depressed mechanical function known as atrial stunning occurs, suggesting that defects in contractility occur in AFib and are revealed upon restoration of rhythm. This project aims to define the contractile remodeling that occurs in AFib. To assess contractile function, we used a canine atrial tachypacing model of induced AFib. Mass spectrometry analysis showed dysregulation of contractile proteins in samples from AFib compared with sin

Small molecule drugs to improve sarcomere function in those with acquired and inherited myopathies.

Muscle weakness is a hallmark of inherited or acquired myopathies. It is a major cause of functional impairment and can advance to life-threatening respiratory insufficiency. During the past decade, several small-molecule drugs that improve the contractility of skeletal muscle fibers have been developed. In this review, we provide an overview of the available literature and the mechanisms of action of small-molecule drugs that modulate the contractility of sarcomeres, the smallest contractile units in striated muscle, by acting on myosin and troponin. We also discuss their use in the treatment

Sarcomeric gene mutations in phenotypic positive hypertrophic cardiomyopathic patients in Indian population.

HCM is a monogenic cardiac disorder with a high risk of sudden cardiac death, heterogeneous phenotypic expression and genetic profile. HCM is expressed as autosomal dominant in fashion with the prevalence of 1:500 in the general population. The main objective of the current study was to unravel the mutation status in sarcomeric genes in urbanizing Pune population. HCM patients were recruited from Bharti hospital and Poona hospital and research centre, Pune after being screened by 2-D echocardiography. DNA was extracted from whole blood samples and PCR amplification was performed for selected e

Drugging the Sarcomere, a Delicate Balance: Position of N-Terminal Charge of the Inhibitor W7.

W7 is a sarcomere inhibitor that decreases the calcium sensitivity of force development in cardiac muscle. W7 binds to the interface of the regulatory domain of cardiac troponin C (cNTnC) and the switch region of troponin I (cTnI), decreasing the binding of cTnI to cNTnC, presumably by electrostatic repulsion between the -NH3+ group of W7 and basic amino acids in cTnI. W7 analogs with a -CO2- tail are inactive. To evaluate the importance of the location of the charged -NH3+, we used a series of compounds W4, W6, W8, and W9, which have three less, one less, one more, and two more methylene grou

Differential contributions of sarcomere and mitochondria-related multigene variants to the endophenotype of hypertrophic cardiomyopathy.

BACKGROUND: Hypertrophic cardiomyopathy (HCM) is a multigenic disease that occurs due to various genetic modifiers. We investigated phenotype-based clinical and genetic characteristics of HCM patients using comprehensive genetic tests and rare variant association analysis. METHODS: A comprehensive HCM-specific panel, consisting of 82 nuclear DNAs (nDNAs: 33 sarcomere-associated genes, 5 phenocopy genes, and 44 nuclear genes linked to mitochondrial cardiomyopathy) and 37 mitochondrial DNAs (mtDNAs), was analyzed. Rare variant analysis was performed to determine the association of specific gene

The value of electrocardiography and echocardiography in distinguishing Fabry disease from sarcomeric hypertrophic cardiomyopathy.

BACKGROUND: Screening for Fabry disease is sub-optimal in non-specialised centres. AIM: To assess the diagnostic value of electrocardiographic scores of left ventricular hypertrophy and a combined electrocardiographic and echocardiographic model in Fabry disease. METHODS: We retrospectively reviewed the electrocardiograms and echocardiograms of 61 patients (mean age 55.6±11.5 years; 57% men) with Fabry disease and left ventricular hypertrophy, and compared them with those from 59 patients (mean age 44.8±18.3 years; 66% men) with sarcomeric hypertrophic cardiomyopathy. Six electrocardiography

Echocardiographic features of Fabry cardiomyopathy-Comparison with hypertrophy-matched sarcomeric hypertrophic cardiomyopathy.

BACKGROUND: The concept of "red flags" has been particularly useful in the etiologic diagnosis of cardiomyopathies such as Fabry disease, as early detection is often essential for treatment response and outcomes. The present study sought to describe the echocardiographic features that may differentiate Fabry cardiomyopathy from sarcomeric hypertrophic cardiomyopathy (HCM). METHODS: Forty patients with left ventricular (LV) hypertrophy were prospectively included and divided into two groups: the Fabry group (20) and the sarcomeric HCM group (20). The two groups were matched for LV hypertrophy

Diabetes with heart failure increases methylglyoxal modifications in the sarcomere, which inhibit function.

Patients with diabetes are at significantly higher risk of developing heart failure. Increases in advanced glycation end products are a proposed pathophysiological link, but their impact and mechanism remain incompletely understood. Methylglyoxal (MG) is a glycolysis byproduct, elevated in diabetes, and modifies arginine and lysine residues. We show that left ventricular myofilament from patients with diabetes and heart failure (dbHF) exhibited increased MG modifications compared with nonfailing controls (NF) or heart failure patients without diabetes. In skinned NF human and mouse cardiomyocy

Incident Atrial Fibrillation Is Associated With MYH7 Sarcomeric Gene Variation in Hypertrophic Cardiomyopathy.

Background Although atrial fibrillation (AF) is common in hypertrophic cardiomyopathy (HCM) patients, the relationship between genetic variation and AF has been poorly defined. Characterizing genetic subtypes of HCM and their associations with AF may help to improve personalized medical care. We aimed to investigate the link between sarcomeric gene variation and incident AF in HCM patients. Methods and Results Patients from the multinational Sarcomeric Human Cardiomyopathy Registry were followed for incident AF. Those with likely pathogenic or pathogenic variants in sarcomeric genes were inclu

Evidence of autoantibodies against cardiac troponin I and sarcomeric myosin in peripartum cardiomyopathy.

Peripartum cardiomyopathy (PPCM) is a major cause of pregnancy-related maternal heart failure that develops towards the end of pregnancy or in the months following delivery. In small retrospective case series, autoimmune responses in the pathogenesis of PPCM have been proposed upon identification of autoantibodies (AABs) to cardiac antigens. However, their clinical and prognostic relevance still remain unclear. In this study, we evaluated the presence of circulating AABs against cardiac sarcomeric myosin (MHC) and troponin I (TnI) in the sera of PPCM patients and in relation to clinical presen

Significance of sarcomere gene mutations analysis in the end-stage phase of hypertrophic cardiomyopathy.

End-stage hypertrophic cardiomyopathy (ES-HC) has an ominous prognosis. Whether genotype can influence ES-HC occurrence is unresolved. We assessed the spectrum and clinical correlates of HC-associated mutations in a large multicenter cohort with end-stage ES-HC. Sequencing analysis of 8 sarcomere genes (MYH7, MYBPC3, TNNI3, TNNT2, TPM1, MYL2, MYL3, and ACTC1) and 2 metabolic genes (PRKAG2 and LAMP2) was performed in 156 ES-HC patients with left ventricular (LV) ejection fraction (EF) <50%. A comparison among mutated and negative ES-HC patients and a reference cohort of 181 HC patients with pre

Cardiac myosin binding protein-C: a novel sarcomeric target for gene therapy.

Through its ability to interact with both the thick and thin filament proteins within the sarcomere, cardiac myosin binding protein-C (cMyBP-C) regulates the contractile properties of the myocardium. The central regulatory role of cMyBP-C in heart function is emphasized by the fact that a large proportion of inherited hypertrophic cardiomyopathy cases in humans are caused by mutations in cMyBP-C. The primary dysfunction in cMyBP-C-related cardiomyopathies is likely to be abnormal myofilament contractile function; however, currently, there are no effective therapies for ameliorating these contr

The ultrastructure of cat fusimotor endings and their relationship to foci of sarcomere convergence in intrafusal fibres.

1. Six muscle spindle poles, five from experiments in which foci of sarcomere convergence had been observed during stimulation of fusimotor axons, were serially sectioned for light and electron microscopy. Every somatic motor terminal was studied in ultrathin sections at several levels.2. In all six poles static gamma axons, or presumed static gamma axons, supplying the static bag(2) fibre and/or chain fibres had no terminations on the dynamic bag(1) fibre. In five poles, the dynamic bag(1) fibre was selectively innervated by dynamic gamma or beta axons save in one case where a dynamic gamma a

Determining Proximities of Sarcomeric Proteins in Living Skeletal Muscle Cells Using FRET Imaging.

The technique of fluorescence resonance energy transfer (FRET) has been used in this study to determine the proximities (efficiencies, i.e., E%) of interacting proteins in sarcomeres in living cultured embryonic chicken skeletal muscle cells to draw up a FRETsome of a sarcomere in mature myofibrils. During this study, two known Z-band proteins, FATZ (also called myozenin), and telethonin, were found to be actin binding proteins. The Efficiency of the F-actin and telethonin pair was measured to be 15%, while the F-actin and FATZ pair was 18%. The known F-actin and alpha-actinin interactions res

Myosin light chain proteins cooperatively promote sarcomere growth in fast-twitch muscle.

Muscle becomes stronger by expanding sarcomeres from the cell's edge to its center. Here we investigate how Mylpf (Myosin Light Chain Phosphorylatable Fast) abundance impacts sarcomere growth. The two zebrafish Mylpf genes (mylpfa and mylpfb) are exclusively expressed in fast-twitch muscle, with mylpfa expressed more abundantly than mylpfb. Mutations in both genes cause full mRNA and protein loss, and loss of sarcomere formation. The degree of sarcomere reduction in fast-twitch muscle is predicted by Mylpf protein dosage using a high-slope Hill equation. The sarcomere defect may be driven by t

MRI-derived wall thickness heterogeneity in hypertrophic cardiomyopathy and in carriers of sarcomeric gene mutations.

AIMS: Hypertrophic cardiomyopathy (HCM) shows variable left ventricular hypertrophy, yet current diagnostic criteria rely on absolute wall thickness (WT) cut-offs that may miss early or subtle phenotypes, particularly in sarcomere mutation carriers. OBJECTIVES: To determine whether wall thickness standard deviation (WTSD), reflecting WT heterogeneity, improves identification of HCM and mutation carriers. METHODS AND RESULTS: Cardiac magnetic resonance was performed in 382 healthy controls, 297 patients with HCM, 82 sarcomere mutation carriers without overt hypertrophy, and 180 patients with

Activity-Dependent Sarcomere Remodeling in C. elegans Muscle Correlates with Mechanical Vulnerability in Dystrophy.

Sarcomere networks (formed by lateral connections between myofibrils) are essential for force distribution in striated muscle. Yet, whether these networks remodel to meet changing mechanical demands or contribute to muscle pathology is unclear. Using C. elegans body-wall muscles, we show that sarcomeres form interconnected networks via actin-, myosin-, and actinin-rich junctions that dynamically remodel in response to locomotor activity. Swimming, which imposes greater mechanical demands, enhances sarcomere branching compared to crawling. In dystrophin-deficient dys-1(eg33) mutants, impaired l
