Obesity and type 2 diabetes mellitus (T2DM) are widespread health concerns that often coexist, contributing to increased cardiometabolic risks and premature death. Despite advancements in both lifestyle interventions and medical treatments, achieving lasting weight reduction and stable glycemic control remains a major clinical challenge. This review examines findings from the semaglutide treatment effect in people with obesity 1 (STEP) 1-5 trials, which assessed the effectiveness, safety, and long-term outcomes of once-weekly semaglutide 2.4 mg for weight management in adults. These trials inc

Obesity is an increasingly growing public health issue affecting a substantial proportion of the population in the United States. This condition increases the risk for multiple disease processes, including cardiometabolic disorders, osteoarthritis, gout, respiratory issues, reproductive problems, and some cancers. Glucagon-like peptide-1 (GLP-1) receptor agonists got their first US Food and Drug Administration (FDA) approval in 2005 in the form of exenatide for type 2 diabetes mellitus (T2DM) management. Thereafter, GLP-1 analogs have been increasingly studied as a noninvasive strategy for wei

Alzheimer's disease (AD) is a neurodegenerative disorder characterized by progressive cognitive decline. Glucagon-like peptide-1 receptor agonists (GLP-1RAs) have shown neuroprotective potential, but the mechanisms underlying these effects remain incompletely understood. This study investigated whether semaglutide, a long-acting GLP-1RA, ameliorates AD-like phenotypes in APP/PS1 mice and explored associated changes in neuroinflammatory signaling and blood-brain barrier (BBB) integrity. Eight-month-old amyloid precursor protein/presenilin 1 (APP/PS1) transgenic mice were treated with semaglutid

CONTEXT: In a 28-week clinical trial of patients with type 2 diabetes (T2D), tirzepatide 15 mg (a dual GIP/GLP-1 receptor agonist) and semaglutide 1 mg (a GLP-1 receptor agonist) both improved fasting glucose, with tirzepatide showing superior efficacy. The longitudinal metabolic mechanisms underlying this difference remain incompletely defined.
OBJECTIVE: To compare the longitudinal metabolic effects of tirzepatide and semaglutide using model-based analysis of fasting measures, elucidate underlying mechanisms, and explore strategies to enhance outcomes in low responders.
METHODS: A mechanis

BACKGROUND: Although semaglutide is widely used due to its efficacy in glycemic control and weight reduction, cases of the associated adverse drug reactions (ADRs), including kidney injury, have gradually increased. This systematic review aimed to summarize reported cases and case series of semaglutide-associated renal injury and their clinical and laboratory features.
METHODS: Case reports and case series on ADRs associated with semaglutide published in English and Chinese were systematically retrieved using relevant keywords from PubMed/MEDLINE, Web of Science, Embase, the Cochrane Library,

The therapeutic dominance of semaglutide in managing diabetes, obesity, and cardio-kidney-metabolic diseases has spurred an influx of generics, biosimilars, and compounded pharmacosimilars. However, shared nomenclature does not guarantee therapeutic equivalence. This analysis introduces the ABCDE framework (author-proposed conceptual checklist) to differentiate reference r-DNA semaglutide from synthetic or informally marketed copies. Evaluation rests on five pillars: (A) approved status, where regulatory authorization does not inherently imply interchangeability; (B) bioavailability, noting th

Semaglutide, a GLP-1 receptor agonist, is FDA-approved for managing type 2 diabetes (T2D) and reducing cardiovascular risk. Its off-label use in weight management and other conditions has grown, prompting a review of its benefits and risks. This review evaluates evidence on semaglutide's effects, highlighting its therapeutic potential beyond approved indications. Studies from 2021-2024 were reviewed via PubMed, ScienceDirect, and Google Scholar. Semaglutide showed promise in managing PCOS-related obesity, insulin resistance, and demonstrated renoprotective effects in diabetics and chronic kidn

OBJECTIVE: Randomized clinical trials show that subcutaneous semaglutide is modestly superior to dulaglutide in reducing HbA1c and body weight, but no trial has compared their effectiveness on hard cardiovascular outcomes. This study aimed to examine whether semaglutide and dulaglutide differ in cardiovascular effectiveness.
RESEARCH DESIGN AND METHODS: This new-user, active-comparator cohort study used nationwide population-based Danish healthcare data to emulate a target trial of adults with type 2 diabetes receiving standard care who initiated subcutaneous semaglutide compared with dulaglu

Semaglutide is a glucagon-like-peptide-1 receptor agonist (GLP-1 RA) that is approved for the treatment of type 2 diabetes mellitus (T2DM) and obesity. Common adverse drug reactions (ADR) of semaglutide include nausea, vomiting, abdominal pain, constipation, and diarrhea, which are often dose-dependent in nature. Select ADRs that are less common, but may result in more significant concerns, include development of acute pancreatitis, gallbladder and biliary tract diseases, acute kidney injury, and ileus. Limited clinical literature exists at present regarding management of GLP-1 RA overdose, pa

Rhabdomyolysis is a clinical syndrome characterized by the breakdown of skeletal muscle, leading to the release of creatine kinase (CK), myoglobin and electrolytes into the circulation. This can result in acute kidney injury and other complications. Common etiologies include trauma, medications and metabolic disorders. We report a case of semaglutide-associated rhabdomyolysis in a 36-year-old obese male who presented with bilateral lower extremity pain, cramping and dark urine 6 days after increasing his weekly dose from 1.7 mg to 2.4 mg. The dose had been titrated weekly from 0.25 mg to 1.0 m

The precise mechanism of action of semaglutide in obese patients remains uncertain, although it is evident that the drug confers a significant cardiac benefit. An excessively high-fat diet has been demonstrated to result in an increased expression of solute carrier family 27 member 2 (Slc27a2/FATP2) on cardiomyocyte membranes. This is associated with a number of adverse effects, including lipid deposition in cardiomyocytes, elevated levels of inflammation and oxidative stress, impaired cardiac function, decreased cardiomyocyte viability, and increased apoptosis. The administration of semagluti

Semaglutide, as a glucagon-like peptide-1 receptor agonist (GLP-1 RA), was approved for glucose control in type 2 diabetes mellitus in 2017 and approved for weight loss in 2021 by the U.S. Food and Drug Administration (FDA). No psychiatric adverse effect associated with semaglutide has been reported so far. Here we report two cases of semaglutide-associated depression. One is a middle-aged man with no previous history of depression who developed depressive symptoms about 1 month after taking semaglutide. The other one is a middle-aged woman with recurrent depressive disorder whose symptoms als

PRESENTATION: Non-arteritic ischaemic optic neuropathy (NAION) is a common cause of acute, painless monocular visual disturbance in older adults. This case discusses a woman in her 70s with vascular risk factors who presented with a three-day history of progressive left sided visual disturbance. Examination demonstrated a left relative afferent pupillary defect and inferior quadrantopia.
DIAGNOSIS: Haematological investigations and neuroimaging were unremarkable. Formal ophthalmological assessment demonstrated superior optic disc swelling with an inferior visual field defect, consistent with

Ischemic colitis associated with glucagon-like peptide-1 (GLP-1) receptor agonists is an exceptionally rare condition, with only a few cases reported in the literature. We present the case of a 46-year-old woman treated with oral semaglutide (Rybelsus®) for weight loss who experienced three episodes of abdominal pain and hematochezia temporally associated with drug exposure. The third episode, which occurred after semaglutide was reintroduced, led to a confirmed diagnosis based on computed tomography, rectosigmoidoscopy, and histopathological findings consistent with ischemic colitis. The use of glucagon-like peptide-1 receptor agonists (GLP-1 RAs) has expanded rapidly worldwide for the treatment of obesity, accompanied by a parallel increase in medication errors and accidental overdoses. Gastrointestinal symptoms are the most commonly reported adverse effects in cases of supratherapeutic exposure, whereas neurologic manifestations remain poorly characterized. We report a case of a 50-year-old man with obesity receiving weekly semaglutide 2.4 mg who accidentally administered a total dose of 9.6 mg due to a pen-device handling error. Two days after the injection, he develope

Semaglutide, a glucagon-like peptide-1 receptor agonist, has demonstrated clinically meaningful weight-loss effects in individuals with type 2 diabetes and high body mass index, and dental practitioners and physicians often encounter patients who take these medications. Semaglutide is known to be associated with several systemic side effects, including delayed gastric emptying, belching, reflux and nausea, nutritional changes, and gastrointestinal discomfort. Systemic effects secondarily manifest in the oral cavity and, as a result, individuals taking semaglutide may present with xerostomia (d

This report describes a female patient in her 60s with an immune-mediated necrotising myopathy (IMNM) occurring after semaglutide use. She was found to have progressive dysphagia, dysphonia, proximal muscle weakness and creatine kinase elevation over a 4-month course. Her symptoms were preceded by a single dose of semaglutide. Her work-up revealed features consistent with necrotising myopathy with inflammatory infiltrate on biopsy, and she responded to immunosuppression with corticosteroids and intravenous immunoglobulin (IVIG). She gradually improved and successfully returned to her baseline

To our knowledge, this is the first case reported in literature, regarding an increase in tumoral biomarkers during a treatment with Semaglutide. A 61-year-old patient was referred to our gynecological consultation due to suspected adnexal cysts following an MRI performed for a functional defecation disorder. The patient was on Semaglutide treatment. Gynecological examination and transvaginal ultrasound were normal, with no evidence of adnexal cysts. Tumor biomarker testing revealed a significant increase in CA 19-9 and a mild elevation in CA-125. In an asymptomatic patient, these findings rai

Alzheimer's disease (AD) and metabolic syndrome often occur together, sharing characteristics such as insulin resistance, dyslipidemia, and chronic inflammation. Metabolic dysfunction frequently precedes cognitive decline, indicating that early intervention might alter the disease's progression. We investigated whether the GLP-1 receptor agonist semaglutide (SMGL) influences metabolic impairment and AD pathology in an AD mouse model. Male and female 5xFAD and wild-type (WT) mice on regular (RD) or high-fat diets (HFD) were administered SMGL for 13 weeks. SMGL-treated groups exhibited significa
