UNLABELLED: The dehiscence of the anterior semicircular canal is a new clinical entity, it is necessary to consider this when a gusher occurs during stapedial surgery.
CASE REPORT: We report a rare case of dehiscence of the anterior canal associated with otosclerosis. This was revealed by a gusher during the initial stapedectomy. At 3 months, an obliteration of the dehiscence was necessary by middle cranial fossa approach (persistant vestibular symptoms), seven months later. Revision stapedial surgery resulted in complete closure of the air-bone-gap.
CONCLUSION: High resolution CT scans of t

The authors present the case of a man who had superior semicircular canal dehiscence syndrome in addition to chronic otitis media. This case is atypical because the patient coincidentally had middle ear and mastoid disease, which previously had been treated surgically. The prior ear surgery delayed the diagnosis of superior semicircular canal dehiscence syndrome and increased the complexity of the repair of the superior semicircular canal dehiscence. Superior semicircular canal dehiscence syndrome is a recently recognized syndrome resulting in acute or chronic vestibular symptoms. The diagnosi

There are many known causes of vertigo, but many cases remain unexplained. Sound-induced, pressure-induced, or positional vertigo caused by bony dehiscence of the superior semicircular canal into the middle cranial fossa is a newly described etiology of vertigo. Three case studies of patients with CT evidence and surgical confirmation of bony dehiscence of the superior semicircular canal with variable presentations are described. The history, symptoms, CT findings, vestibular studies, and method of surgical repair are presented. Two patients had disabling vertigo and one had no vestibular symp

Benign paroxysmal vertigo and nystagmus are induced not only by the posterior but also by the horizontal semicircular canal. Benign positional nystagmus of the horizontal canal is more often observed than was previously thought. In 10 patients we analyzed the characteristics and the variability of nystagmus which accompanies positional vertigo of the horizontal canal. There are two forms of nystagmus: primary-geotropic, most often paroxysmal nystagmus (7 patients), and primary-apogeotropic, non-paroxysmal nystagmus (3 patients). Interestingly, in 2 patients with the primary-apogeotropic form t

A subset of patients with acoustic neuromas and useful hearing have tumors that are inadequately approached by both middle fossa and retrosigmoid techniques. The enhanced retrosigmoid technique combines the hearing preservation of posterior semicircular canal ablation to achieve lateral internal auditory canal exposure with the ample cerebel-lopontine angle exposure of the standard retrosigmoid technique.

We report the clinical features and results of quantitative eye-movement testing in 13 patients with episodic positional vertigo and nonfatiguing direction-changing horizontal positional nystagmus (beating to the right with the head turned to the right and beating to the left with the head turned to the left). The benign history and lack of associated neurologic findings support a peripheral localization of the lesion. This syndrome probably represents a horizontal semicircular canal variant of benign positional vertigo. Free-floating debris in one horizontal canal may explain many of the clin

Fenestration of the horizontal semicircular canal enables the otologic surgeon to restore hearing in those patients with congenital conductive deafness who are not candidates for stapedectomy, ossicular reconstruction, or tympanoplasty. Since the development of stapedectomy, much of the finesse technique of fenestration surgery has been lost, and many otologic surgeons today are unfamiliar with the fenestration operation. The surgical technique for creating a permanently patent fenestra in the horizontal semicircular canal is described. The causes of failure in fenestration surgery are reviewe

BACKGROUND: A vestibular implant can partially restore vestibular function by providing motion information through implanted electrodes. During vestibular implantation, various obstructions of the semicircular canals, such as protein deposits, fibrosis, and ossification, can be encountered. The objective was to explore the relationship between preoperative imaging and intraoperative findings of semicircular canal obstruction and to develop surgical strategies for dealing with obstructions of the semicircular canal(s) in patients eligible for vestibular implantation.
METHODS: Patients undergoi

In the vertebrate inner ear, the ability to detect angular head movements lies in the three semicircular canals and their sensory tissues, the cristae. The molecular mechanisms underlying the formation of the three canals are largely unknown. Malformations of this vestibular apparatus found in zebrafish and mice usually involve both canals and cristae. Although there are examples of mutants with only defective canals, few mutants have normal canals without some prior sensory tissue specification, suggesting that the sensory tissues, cristae, might induce the formation of their non-sensory comp

Inflight and post-landing "immunity" to the "coriolis sickness susceptibility test", observed during the Skylab M131 experiment, suggests that the otolith organs play a major role in space motion sickness (SMS). This view is supported by the report that ocular counter-torsion asymmetries correlate with SMS incidence and severity. Further data indicate that sensory-motor adaptation to microgravity includes a process whereby central interpretation of otolith signals is biased from "tilt" toward translation. However, unexpected responses to linear acceleration suggest the importance of gravicepto

OBJECTIVE: The purpose of this study was to determine the accuracy of high resolution CT (HRCT) in the detection of facial canal dehiscence and semicircular canal fistula, the preoperative evaluation of both of which is clinically very important for ear surgery.
MATERIALS AND METHODS: We retrospectively reviewed the HRCT findings in 61 patients who underwent mastoidectomy at Yamagata University between 1989 and 1993. The HRCT images were obtained in the axial and semicoronal planes using 1 mm slice thickness and 1 mm intersection gap.
RESULTS: In 46 (75%) of the 61 patients, the HRCT image-b

BACKGROUND AND PURPOSE: Superior semicircular canal dehiscence (SSCD) is an otologic condition characterized by a bony roof defect of the superior semicircular canal, which can result in auditory and vestibular symptoms. Given the proposed associations between elevated intracranial pressure and SSCD development, as well as the established relationship between obesity and intracranial hypertension, we hypothesized that a higher body mass index (BMI) would be associated with an increased radiographic prevalence of SSCD.
MATERIALS AND METHODS: We evaluated consecutive adult patients who underwen

The phylogenetic relationships of early catarrhines (Old World monkeys and apes) are contentious, particularly regarding pliopithecoids, which are considered a clade of Miocene stem catarrhines more derived than most of their Oligocene counterparts. However, their debated affinities and the uncertain phylogenetic relationships of the Oligocene catarrhine Saadanius leave their systematic position unresolved. Notably, Saadanius displays a mosaic morphology, coupling primitive craniodental traits with a fully ossified ectotympanic (as opposed to the annular/incompletely ossified ear tube of stem

OBJECTIVE: Radiologic superior semicircular canal dehiscence (SSCD) is not an uncommon finding, while many patients remain asymptomatic. We aimed to highlight minor head trauma as a risk factor for evolution into a clinically significant SSCD syndrome and provide the audiovestibular findings.
METHODS: A retrospective chart review study in a tertiary referral medical center was conducted. Patients were identified from the Province of Ontario Workplace Safety and Insurance Board database, 1998-2025. Each patient underwent a comprehensive assessment comprising history, physical examination, audi

The syndrome of the third mobile window of the semicircular canals (or Minor's syndrome) is a degeneration of the bone wall of the upper semicircular canal, first described by L. Minor in 1998. This is a rare lesion of the inner ear, which is characterized by hearing impairment and vestibular manifestations. The presence of bone wall degeneration of the semicircular canal in 50% of patients can be detected during a standard CT scan of the temporal bone (in 1 mm increments) in the frontal (coronary) projection or in the oblique coronary projection in the PC plane. Minor syndrome occurs in only
