Presbyacusis, hearing aids and aging.

This paper considers the factors of presbyacusis and age-related psychological changes in older adults in terms of their effects on the use or potential use of hearing aids as a means of improving aural communication. Data are presented on the prevalence of hearing impairment and the extent of hearing aid use in aging listeners. Four major classes of presbyacusis are described (sensory, neural, metabolic, and cochlear conduvtive) and a hearing aid prognosis is made for each type of disorder. It is shown that aging produces deficits in pure-tone thresholds, the speech reception threshold, and s

Wegener's granulomatosis with otological and nervous system involvement.

Wegener's granulomatosis usually presents to the otolaryngologist with nasal manifestations. Complications including otologic and central nervous system symptoms occur in 20-40 per cent of cases. Two such cases presented to our clinic with nasal manifestations initially, one subsequently developing otologic sequelae and one progressive sensory motor neuropathy. These patients were followed to determine the reversibility of the otologic and sensory motor neuropathy while on therapy. The otologic changes remain and this patient was required to use a hearing aid. The sensory motor neuropathy impr

Delivering a hearing and vision support intervention for people with dementia: looking "under the hood" of the SENSE-Cog trial pre- and during the COVID-19 pandemic.

PURPOSE: Acceptable hearing and vision rehabilitation is needed to improve quality of life for people with dementia (PwD) living at home. We examined delivery and uptake of SENSE-Cog, a complex sensory support intervention (SSI), within a European multicentre randomised controlled trial. MATERIALS AND METHODS: PwD with hearing and/or vision loss were randomised to an 18-week home-based SSI or sensory care-as-usual. A mixed-methods design included assessments at baseline, 18 and 36 weeks and qualitative interviews with Sensory Support Therapists (SSTs). Reporting followed TIDieR. COVID-19 rest

Inherited autonomic neuropathies.

Inherited autonomic neuropathies are a rare group of disorders associated with sensory dysfunction. As a group they are termed the "hereditary sensory and autonomic neuropathies" (HSAN). Classification of the various autonomic and sensory disorders is ongoing. In addition to the numerical classification of four distinct forms proposed by Dyck and Ohta (1975), additional entities have been described. The best known and most intensively studied of the HSANs are familial dysautonomia (Riley-Day syndrome or HSAN type III) and congenital insensitivity to pain with anhidrosis (HSAN type IV). Diagnos

Sensory re-education after nerve repair: aspects of timing.

The recovery of functional sensibility after nerve transection and repair is often disappointing. Here we address the timing of sensory re-education that aims at re-learning and modulating the changed sensory code from the hand after such an injury. Such training utilises the capacity for cortical functional re-modelling which characterises the young as well as the adult brain. Sensory re-education is traditionally not introduced until there is reinnervation in the hand, and such a late onset of training may be one explanatory factor for the poor functional results after nerve repair. Since fu

Sensory Trick in a Patient with Cervical Dystonia: Insights from Magnetoencephalography.

BACKGROUND: The proposed mechanisms for the sensory trick include peripheral sensory feedback to aid in correcting abnormal posture or movement. CASE REPORT: A 53-year-old woman with cervical dystonia underwent magnetoencephalography pre- and post-botulinum toxin injection and sensory trick, which was described as yawning. Study revealed connectivity between the left frontal and inferior frontal gyrus before yawning, which changed to the visual cortex and right middle frontal gyrus with yawning. Beta frequencies reduced and gamma frequencies increased after yawning. DISCUSSION: The increase

Neuropathy complication of antiretroviral therapy in HIV/AIDS patients.

OBJECTIVES: Significant pain from HIV-associated sensory neuropathy (HIV-SN) affects 40% of HIV-infected individuals treated with antiretroviral therapy (ART). The most salient symptom of the neuropathy is pain, which frequently is moderate-to-severe intensity, associated with reduced activities and physical function, sleep disruption, increased severity of depression, and anxiety. Yet, evidence for managing painful HIV-SN is poor. The purpose of this study was to verify by scientific evidence the neuropathy complication in HIV/AIDS patients to develop effective pain management strategies. ME

Approach to gait disorders and orthotic management in adult onset neuromuscular diseases.

In order to understand abnormal gait, this article will first review normal gait, discuss how neuromuscular diseases disturb gait patterns and review orthotic interventions. In normal gait, concentric contractions accelerate and eccentric contractions decelerate the limb. Neuromuscular gait disorders can be grouped into (1) proximal weakness, (2) distal weakness, (3) nonlength-dependent or generalized weakness, (4) asymmetric weakness, and (5) sensory disorders. Identification of gait disturbance type in neuromuscular diseases leads to the appropriate orthotic prescription since orthotic strat

Treatment of the neuromuscular complications of human immunodeficiency virus infection.

Neuromuscular disorders are increasingly being reported in individuals with human immunodeficiency virus (HIV) infection. The majority of these disorders resemble diseases occurring in HIV-seronegative patients and include inflammatory demyelinating polyneuropathies, multiple mononeuropathies, and polymyositis. In HIV-seronegative patients, these diseases are believed to be immune system-mediated. It is likely that similar pathogenic mechanisms are present in HIV-seropositive patients and reflect an altered immune system caused by viral infection and the loss of CD4 cells. Therapy is similar i

Do auditory brainstem implants favor the development of sensory integration and cognitive functions?

BACKGROUND: Information about the development of cognitive skills and the effect of sensory integration in children using auditory brainstem implants (ABIs) is still limited. OBJECTIVE: This study primarily aims to investigate the relationship between sensory processing skills and attention and memory abilities in children with ABI, and secondarily aims to examine the effects of implant duration on sensory processing and cognitive skills in these children. METHODS: The study included 25 children between the ages of 6 and 10 years (mean age: 14 girls and 11 boys) with inner ear and/or auditor

Cerebellar ataxia, neuropathy, vestibular areflexia syndrome due to RFC1 repeat expansion.

Ataxia, causing imbalance, dizziness and falls, is a leading cause of neurological disability. We have recently identified a biallelic intronic AAGGG repeat expansion in replication factor complex subunit 1 (RFC1) as the cause of cerebellar ataxia, neuropathy, vestibular areflexia syndrome (CANVAS) and a major cause of late onset ataxia. Here we describe the full spectrum of the disease phenotype in our first 100 genetically confirmed carriers of biallelic repeat expansions in RFC1 and identify the sensory neuropathy as a common feature in all cases to date. All patients were Caucasian and hal

AIDS-associated vacuolar myelopathy.

AIDS-associated vacuolar myelopathy (VM) is a common neurologic complication of AIDS. Pathologically, VM is characterized by vacuolization in the lateral and posterior columns of the thoracic spinal cord and has a striking similarity with the myelopathy of vitamin B12 deficiency. In autopsy series, 20% to 55% of patients with AIDS have evidence of spinal cord disease consistent with VM. The myelopathy usually manifests late in the course of HIV infection, with slowly progressive weakness of the lower extremities, gait disorder, sensory abnormalities in the legs, impotence in men, and urinary f

Efficacy of electronic travel aids for the blind and visually impaired during wayfinding.

Independent traveling remains challenging for blind and visually impaired (BVI) individuals. While the white cane is effective at detecting ground-level obstacles, it provides no information about elevated obstacles or object characteristics. Recent technologies have been designed to support navigation as well as object detection. In our study, we compared the performance of 13 BVI participants who separately used two secondary electronic travel aids (ETAs) versus cane use alone. One ETA was a camera-based mobility vest (NOA), and the other was an ultrasonic sensor-based wearable (BuzzClip). P

A deep learning approach for line-level Amharic Braille image recognition.

Braille, the most popular tactile-based writing system, uses patterns of raised dots arranged in cells to inscribe characters for visually impaired persons. Amharic is Ethiopia's official working language, spoken by more than 100 million people. To bridge the written communication gap between persons with and without eyesight, multiple Optical braille recognition systems for various language scripts have been developed utilizing both statistical and deep learning approaches. However, the need for half-character identification and character segmentation has complicated these systems, particular

Liver-innervating vagal sensory neurons are indispensable for the development of hepatic steatosis and anxiety-like behavior in diet-induced obese mice.

The visceral organ-brain axis, mediated by vagal sensory neurons, is essential for maintaining various physiological functions. Here, we investigate the impact of liver-projecting vagal sensory neurons on energy balance, hepatic steatosis, and anxiety-like behavior in mice under obesogenic conditions. A small subset of vagal sensory neurons innervate the liver and project centrally to the nucleus of the tractus solitarius, area postrema, and dorsal motor nucleus of the vagus, and peripherally to the periportal areas in the liver. The loss of these neurons prevents diet-induced obesity, and the
