Periprosthetic humeral fractures associated with reverse total shoulder arthroplasty: incidence and management.

PURPOSE: The purpose of this study was to record the incidence and management of periprosthetic humeral fractures (PHF) using reverse total shoulder arthroplasty (RTSA) in our institution. METHODS: We performed a retrospective study of 203 RTSA implanted in 200 patients between 2003 and 2014. The mean follow-up was 78.82 months (range, 12-141). Mean age of the study cohort was 75.87 years (range, 44-88). There were only 25 male patients (12.5 %). We assessed the presence of periprosthetic humeral fractures studying the medical files and X-rays of all patients. RESULTS: We identified seven pe

Treatment of proximal humeral fractures with reverse shoulder arthroplasty in elderly patients.

BACKGROUND: Proximal humeral fractures in four or even only three parts, with metaphyseal hinge distances of <8 mm, represent a serious and widely debated problem. Reduction is complex and plating is often instable, especially in elderly patients. Failures, sometimes involving necrosis of the head, are frequent. Hemiarthroplasty has long been used for 3- or 4-part complex fractures, even in young patients, although often with sub-optimal results, due to reabsorption of tuberosities. This complication has partly been overcome with reverse shoulder prostheses which, although more invasive than p

[Miniinvasive technologies in treatment of intraarticular splittered fractures of the proximal shoulder].

The algorithm of diagnostics, miniinvasive ostheosynthesis and postoperative rehabilitation of patients with the splittered fractures of caput femori was worked out. Treatment results of 24 patients were analyzed. All patients were treated with the use of the original apparatus of the external osteal fixation. The method allows early functional treatment and limb function restoration in cases, when classic immersinal ostheosynthesis is contraindicated.

Acromial base fractures after reverse total shoulder arthroplasty: report of five cases.

BACKGROUND: Most series of reverse total shoulder arthroplasty (rTSA) have reported acromial fractures, but they have not been shown to alter reported outcomes in most series. We present 5 patients with acromial base fractures where the entire deltoid origin was displaced from its anatomic location. MATERIALS AND METHODS: Five patients with acromial base fractures after rTSA were identified and evaluated for functional outcomes and pain relief as well as results of fracture treatment. Three were treated operatively and 3 were treated nonoperatively. One nonoperative treatment eventually requi

[Reduction of glenoid rim fractures after primary shoulder dislocation in external rotation].

The treatment of anterior glenoid rim fractures depends on the size of the fracture and the articular surface involved. The operative treatment is open or arthroscopic refixation. In cases with small fragments and a stable shoulder nonoperative treatment is recommended. In patients with a primary shoulder dislocation immobilization in external rotation has been showed to improve the position of the displaced labrum on the glenoid rim. However, whether external rotation can reduce displaced glenoid rim fractures is not known. With the use of CT the repositioning of a glenoid rim fracture in a s

Bilateral Shoulder Dislocation with Fracture of Major Tubercles - A Case Report.

INTRODUCTION: Bilateral dislocations of the glenohumeral joint are most often posterior, secondary to convulsive seizures. We report a case of bilateral anterior dislocation with fracture of the two major tubercles following a very rare mechanism. CASE REPORT: A 52-year-old patient admitted at the emergency department for bilateral closed shoulder trauma following a tonic-clonic seizure. The patient had a blood pressure of 160/100 mm Hg and a clear consciousness. Symmetrical and bilateral clinical examination of the shoulders shows the signs of bilateral anterior dislocation of the shoulders.

Retching as an unusual cause of bilateral posterior shoulder fracture dislocation.

Bilateral posterior shoulder fracture dislocation is a debilitating injury and is quite rare. This injury pattern has been described in the literature as either secondary to seizure, electrocution or major trauma. We present a case of an elderly man who appears to have sustained the injury after an episode of retching and vomiting. With our patient witnessed closely throughout and no trauma, electrocution or seizure activity reported, we propose that the violent action of retching and vomiting not only has the potential to cause this injury pattern but also was the only credible cause in our p

Nontraumatic First Rib Fracture and Empyema Presenting as Shoulder Pain.

Fracture of the first rib is rare because of the anatomic protection provided by surrounding structures. Osseous fatigue caused by repetitive motion and antagonistic muscular contraction is postulated to contribute to nontraumatic first rib fracture, which is most often described in adolescent athletes in sports that involve repetitive overhead motion of the arm. In this report, we describe a 49-year-old man with an occupation involving repetitive overhead motion at the shoulder who was found to have a nontraumatic first rib fracture. The condition resolved with treatment.

Scapular Spine Stress Fracture as a Complication of Reverse Shoulder Arthroplasty.

We report a case of a stress fracture of the scapular spine which developed as a late complication of a reverse shoulder arthroplasty. After initially doing well after surgery, our patient developed pain and decreased shoulder function. A nondisplaced scapular spine fracture was noted on radiographs. Because reverse shoulder arthroplasty is a relatively new procedure in this country, radiologists may be unfamiliar with its potential complications. Stress or insufficiency fractures of the scapular spine and acromion are a potential complication of reverse shoulder arthroplasty, due to increased

Reverse total shoulder replacement for nonunion of a fracture of the proximal humerus.

Patients with pain and loss of shoulder function due to nonunion of a fracture of the proximal third of the humerus may benefit from reverse total shoulder replacement. This paper reports a prospective, multicentre study, involving three hospitals and three surgeons, of 35 patients (28 women, seven men) with a mean age of 69 years (46 to 83) who underwent a reverse total shoulder replacement for the treatment of nonunion of a fracture of the proximal humerus. Using Checchia's classification, nine nonunions were type I, eight as type II, 12 as type III and six as type IV. The mean follow-up was

Missed posterior shoulder dislocation with malunited proximal humerus fracture.

Posterior dislocation of the shoulder may be missed or neglected at initial presentation especially in developing countries. We present a case of 40-year-old Indian man who had 3-month missed posterior dislocation of the right shoulder along with malunited fracture of the anatomical neck of the humerus. Open reduction and stabilization with modified McLaughlin procedure was performed. Rotational osteotomy of proximal humerus had to be performed as supplementary procedure to keep the humeral head stable in glenoid cavity during functional range of movements. The patient had excellent result of

Humeral fracture between a total elbow and total shoulder arthroplasty.

This article presents a case of a 71-year-old woman with a humeral fracture between a cementless reverse total shoulder arthroplasty and a cemented total elbow arthroplasty and discusses our treatment plan. Surgical treatment was performed after the patient was informed of possible complications and the benefits of surgery including: early, complete restoration of arm anatomy, greater functional improvement of the adjacent joints, and increased risk of nonunion with nonoperative treatment. The fracture was comminuted and extended proximally around the shoulder prosthesis. Through the posterior

Anatomic and reverse shoulder prostheses in fracture sequelae of the humeral head.

OBJECTIVES: Arthroplasty for the proximal humerus problems secondary to fractures is troublesome, because of necrosis of the tubercles and the resulting insufficiency of the rotator cuff. The aim of this study was to investigate whether better results can be achieved with the differential use of anatomic and reverse shoulder prostheses, in comparison to the preoperative status. METHODS: Fifty-five patients with secondary fracture prostheses due to sequelae of fractures of the humeral head were followed. Anatomic prostheses were implanted in 36 cases (fracture sequelae types 1 and 2 according

Comminuted periprosthetic humeral fracture after reverse shoulder prosthesis.

Periprosthetic fractures represent a severe complication after joint replacement. A case of comminuted displaced humeral fracture around the stem of a SMR reverse shoulder prosthesis is reported. The patient was a 81-year-old lady who had had a total shoulder replacement 28 months previously. The surgical solution consisted of a partial revision of the modular implant with conservation not only of the glenoid component but also of the prosthetic humeral body, which was well fixed in the humeral metaphysis. The humeral stem was removed and a long uncemented revision stem was implanted providing

Concomitant fracture of the coracoid and acromion after direct shoulder trauma.

Fractures of the acromial process or coracoid process of the scapula are rare. We present a combined fracture of the coracoid and acromion after direct trauma to the shoulder. An anteroposterior radiography with the central x-ray beam angled 25 degrees cephalad or an axillary lateral radiograph may be needed to detect these fractures.

Missed posterior fracture dislocation of the shoulder. Closed reduction and pinning.

In four missed posterior shoulder dislocations, a trial of closed reduction was successful, but severe instability required further therapy. The glenohumeral joints were immobilized by 3 to 4 percutaneously inserted threaded pins. Removal of the pins after 4 to 6 weeks was followed by physiotherapy. After a follow-up of 4 to 42 months, the functional results of this simple procedure are good and there is no recurrence. Bending of the ends of the pins, a very careful follow-up to deal with pin migration, and adequate prevention of subsequent convulsions in epileptic patients are mandatory.

Irreducible Posterior Shoulder Dislocation With Concomitant Fracture of Both the Greater and Lesser Tuberosity: An Extremely Rare Shoulder Injury.

Posterior shoulder dislocation is a relatively rare injury representing only 5% of all shoulder dislocations. It is usually the result of a high-energy trauma or an epileptic seizure. Diagnosis is challenging with half of these injuries missed in the emergency room (ER). Often the dislocation is accompanied by a lesser tuberosity fracture as a result of the impact between the posterior glenoid and the proximal humerus. Additionally, fractures of the greater tuberosity or even the metaphysis are extremely rare, and their treatment remains challenging. We present a rare case of posterior locked
