[Osteochondrosis dissecans of concave joint surfaces: roof of shoulder joint, tibial plateau, distal tibia].

Typically an osteochondrosis dissecans occurs in the region of a convex articular surface, the most frequently localisation observed being the medial femoral condyle. Only few cases of an osteochondrosis dissecans in concave articular surfaces have been reported; these involved the tibial plateau, the distal tibia and the proximal surface of the navicular bone. Two more cases with an osteochondrosis dissecans in the proximal and distal tibial epiphysis are reported and additionally a hitherto unreported localisation in the glenoid fossa of the scapula. True osteochondrosis must be differentiat

Clinical Outcomes of Arthroscopic Treatment for Shoulder Joint Tuberculosis.

BACKGROUND: Shoulder joint tuberculosis is a rare form of extrapulmonary tuberculosis caused by Mycobacterium tuberculosis. Early diagnosis is challenging, and it can lead to severe symptoms in the later stages. Currently, there are few relevant studies on the treatment and diagnosis of this disease. This study aimed to assess the effects of early diagnosis and treatment of shoulder tuberculosis under arthroscopy. METHODS: Between January 2015 and January 2025, 18 patients with confirmed or clinically suspected shoulder tuberculosis underwent arthroscopic debridement and synovectomy after at

Antibiotic Spacers for Shoulder Periprosthetic Joint Infection: A Review.

Periprosthetic joint infection is a rare but potentially devastating complication of shoulder arthroplasty. The most conservative treatment approach is a two-stage revision involving interval placement of an antibiotic cement spacer. The purpose of this study was to contextualize the use of antibiotic spacers in the current treatment paradigm of shoulder periprosthetic joint infection and to review the history of shoulder spacers, the different types (eg, stemmed versus stemless and prefabricated versus handmade), the antibiotic composition and dosage, and their efficacy and complications.

Allergic reaction and metal hypersensitivity after shoulder joint replacement.

PURPOSE: Metal ion release may cause local and systemic effects and induce hypersensitivity reactions. The aim of our study is first to determine if implant-related hypersensitivity correlates to patient symptoms or not; second, to assess the rate of hypersensitivity and allergies in shoulder arthroplasty. METHODS: Forty patients with shoulder replacements performed between 2015 and 2017 were studied with minimum 2-year follow-up; no patient had prior metal implants. Each patient underwent radiographic and clinical evaluation using the Constant-Murley Score (CMS), 22 metal and cement haptens

Ultrasound of the shoulder joint: non "rotator cuff" lesions.

Most of the papers in the literature are on rotator cuff lesions. This paper will deal with a somewhat forgotten part of the shoulder, the joint and the nontendinous structures around the joint. Since 1985, we have performed ultrasound examinations of the shoulder. We have collected many cases and some of them will be performed. The shoulder joint can be partly examined by ultrasound. We can get some information about joint effusions, humeral head cartilage, some part of the labrum, loose bodies, ganglions and fractures around the joint. The acromio-clavicular joint is also part of the examina

Muscle and joint-contact loading at the glenohumeral joint after reverse total shoulder arthroplasty.

The purposes of this study were to determine the contributions of each shoulder muscle to glenohumeral joint force during abduction and flexion in both the anatomical and post-operative shoulder and to identify factors that may contribute to the incidence of glenoid component loosening/failure and joint instability in the shoulder after reverse shoulder arthroplasty (RSA). Eight cadaveric upper extremities were mounted onto a testing apparatus. Muscle lines of action were measured, and muscle forces and muscle contributions to glenohumeral joint forces were determined during abduction and flex

Assessment of periprosthetic joint infection in revision shoulder arthroplasty.

BACKGROUND: In those undergoing revision shoulder arthroplasty, the diagnosis of periprosthetic joint infection (PJI) remains challenging with many clinical and diagnostic variables being considered. The purpose of the study was to evaluate the predictive value of pertinent preoperative and intraoperative factors associated with eventual PJI as determined by the 2018 International Consensus Meeting (ICM) criteria. MATERIALS AND METHODS: All revision shoulder arthroplasties performed at a single tertiary referral medical center from July 2013 to January 2024 were retrospectively analyzed. Pati

Shoulder periprosthetic joint infection is associated with increased mortality.

BACKGROUND: Periprosthetic joint infections (PJIs) are a catastrophic complication after shoulder arthroplasty and may be associated with an increased mortality risk, as seen in hip and knee arthroplasty. Shoulder PJI organisms differ from hip and knee infections, as lower-virulence organisms, such as Cutibacterium acnes, are more commonly encountered. This study evaluated the association between shoulder PJI and mortality. METHODS: We retrospectively identified 411 patients who underwent revision shoulder arthroplasty from 2007 to 2020 at a single institution. 2018 International Consensus Me

Salvage of shoulder joint through simplification of wound management.

Infection is a common sequela of open dislocation of the shoulder joint. This typically leads to joint surface and humeral head necrosis, resulting in severe limitation of motion. Few cases of open and infected glenohumeral joints have been reported, and none had a good functional outcome with pain-free, near-full range of motion. Here we report the case of a 24-year-old man, who sustained a severe degloving injury of his left shoulder with an open glenohumeral joint, in a road traffic accident. We describe a treatment plan that salvaged near-full range of shoulder motion and resulted in pain-

[Animal model of humeral joint for shoulder arthroscopy training].

UNLABELLED: Shoulder arthroscopy used since sixties of the XXth century, becomes now dominating operative technique. Surgeon's skills are most important that's why perfect training model is still needed. Ideal training model for shoulder arthroscopy should combine best possible image of human anatomical structures, good accessibility and low price. Despite of great similarity, animal shoulder joint differs from human one. More spherical shape of animal glenoid gives better support for humeral head. As a result of different shape of glenoid, labrum is significantly less developed, and can actua

[Necrotizing fasciitis following therapeutic injection in a shoulder joint].

Necrotizing fasciitis is an inflammatory, rapidly progressive soft tissue infection usually caused by Streptococcus pyogenes or by a combination of aerobic and anaerobic microorganisms. Here we report the case of a patient who developed necrotizing fasciitis near the site of therapeutic injections. An orthopaedic surgeon in private practice had given the 74-year-old patient, who suffered from left shoulder pain, cortisone injections in his left shoulder joint. During the course of this therapy, the patient developed necrotizing fasciitis. Despite radical surgical debridement of the patient's b

[Final results after operative treatment of shoulder joint infections].

PURPOSE: In a consecutive case series the results of operative treatment of patients with shoulder joint infections following rotator cuff surgery, endoscopic subacromial decompression or subacromial injection were studied. PATIENTS AND METHODS: Between 1990 and 2001 26 patients developed a postoperative infection. 24 patients were available for a mean follow-up of 4.3 (1-12) years. The mean age of the study group was 59.8 (28-84) years. Patients were divided into three groups. Group I consisted of 9 patients with infection after rotator cuff repair, group II consisted of 4 patients with infe

Double-contrast computed tomographic arthrography of the shoulder joint.

Dislocation of the shoulder joint can cause post-traumatic injury to certain anatomical structures, which may lead to recurrent dislocation. The operative visualization of some of these abnormalities may be difficult and their identification preoperatively allows selection of the appropriate surgical approach and avoids the necessity for lengthy surgery. Fifty-one patients with a history of recurrent dislocation, single dislocation or suspected recurrent subluxation as a cause of pain were examined over a 3-year period by double-contrast computed tomographic (CT) arthrography. Thirty-five pati
