Obstructive sleep apnea syndrome (OSAS) in children is often caused by obstruction of the upper airway due to hypertrophy of the adenoids and palatine tonsils. Between October 1988 and December 1991, 50 children (34 males, 16 females) visited our department due to attacks of sleep apnea and underwent adenotomy or adeno-tonsillectomy. Respiratory monitoring during sleep was performed before and after operation, and the usefulness of the surgery was evaluated. Before operation, 27/ 50 children (54.0%) were diagnosed as having OSAS. Their age distribution showed peaks at the ages of 4 and 5 years

OBJECTIVE: To study the characteristics of the sleep apnea syndrome in the aged.
METHODS: Forty-two males who were suspicious of the sleep apnea syndrome (SAS) were detected with 7-hour polysomnography (PSG).
RESULTS: The body mass index (BMI) and neck circumference showed no significant difference in the aged and middle aged groups. The apnea/hypopnea index (AHI) and microarousal index (MAI) were significantly lower, and the minimum oxygen saturation (SaO2) significantly higher in the aged than those in the middle aged (P < 0.05). Age was negatively correlated to AHI (r = -0.329, P < 0.05).

Obstructive sleep apneas result from pharyngeal collapse during sleep. Upper airways may be investigated using either static or dynamic techniques during wakefulness or sleep. In OSAS patients, imaging has different goals: to describe the anatomy of the pharynx and the sequence of the pharyngeal collapse, to try to select patients that will benefit from surgery, and to analyze the results of different treatments.

Sleep apnea syndrome is characterized by recurrent complete or partial upper airway obstructions during sleep and recognized as an important risk factor for cardiovascular and cerebrovascular diseases. Several eye diseases have been associated with sleep apnea syndrome. Due to floppy eyelids often a chronic conjunctivitis occurs. A leaking mask used for apnea treatment may also induce a conjunctivitis. The lids may show an upper lid ptosis, lower lid ectropium, blepharochalasis, or trichiasis. Corneal findings include infectious keratitis, dry eye, recurrent erosion, keratoconus, and progressi

The number of patients suffering from sleep apnea syndrome(SAS) have been increased. In recent years, many therapeutic approaches such as nasal CPAP, dental appliance, upper airway surgery, pharmacological agents, have been used for the treatment of SAS. The various drugs(acetazolamide, some tricyclic antidepressant, progesterone, theophylline, aminophylline and so on) have been used for pharmacotherapy of SAS. Acetazolamide have been established as the best medication. But all approaches without acetazolamide, could not shown their satisfactory effects on SAS. In this article, drug therapy fo

A RECENTLY DEFINED SYNDROME: Sleep apnea syndrome is defined as 5 episodes of apnea or 10 episodes of hyopnea per hour of sleep; the cardinal symptom is excessive diurnal somnolence. Two factors could explain sleep apnea syndrome: hypoxemia and fragmented sleep. ROLE OF HYPOXEMIA: Early studies focused on hypoxemia, but few took into account the effect of subjective complaints of somnolence. Further investigations based on objective measurements introduced a certain degree of complexity distinguishing between sleepiness and the capacity for voluntary vigilance. FRAGMENTARY SLEEP: More recent w

We studied sleep architecture and sleep apnea pattern in patients with postpolio syndrome (PPS). Ten patients with clinical signs of PPS underwent polysomnographic recording for two consecutive nights. Although sleep efficiency and proportions of sleep stages were within the normal range, sleep architecture was disrupted owing to sleep apnea. Patients with bulbar involvement had more frequent sleep apnea (mean sleep apnea index, 11.09) than patients without (apnea index, 5.88). The former also had significantly more central apnea, which occurred more commonly during non-rapid-eye-movement (NRE

As daytime deficits in sleep apnea/hypopnea syndrome (SAHS) correlate poorly with earlier definitions of arousals, we investigated the relationship between microarousals and sleepiness in 63 patients with SAHS. There was a poor correlation between mean sleep latency and microarousal frequency. To determine whether arousals not detected by current definitions could account for some of the residual variance in normal subjects we used sound to induce "arousals" that were detected by rises in blood pressure but produced no visible EEG change. Such autonomic arousals produced an increase in sleepin

The dynamics of cerebral blood flow velocity (CBFV) during sleep were investigated in the right middle cerebral artery of 10 patients with sleep apnea syndrome (SAS) (mean age, 37 years) and 10 healthy control subjects (mean age, 32 years) throughout the entire sleep period. A computer-assisted pulsed (2 MHz) transcranial Doppler ultrasonography system was modified for continuous long-term and on-line recording of cerebral hemodynamics. Concurrently, simultaneous polysomnography, continuous BP recordings, and measurement of the end-expiratory carbon dioxide were undertaken. CBFV showed compara

Regular, continuous snoring usually does not have any sleep disturbing effects; however, discontinuous snoring may be associated with increased upper airway resistance and leads to recurrent arousals, sleep fragmentation and excessive daytime sleepiness. Loud snoring, recurrent obstructive apneas and oxygen desaturation during sleep with excessive daytime sleepiness are the hallmarks of the obstructive sleep apnea syndrome (OSAS). A tentative diagnosis of disturbed breathing during sleep can be made from a careful history and confirmed by a sleep study. Continuous positive airway pressure (CPA

The imaging techniques of the upper airway (UA) now permit a description of the characteristics of pharyngeal collapse during the course of obstructive apnoea. The start is oropharyngeal with active movements anterior and posterior of the soft palate and a falling back of the tongue. The extension occurs almost systematically towards the hypopharynx. A displacement of the hyoid bone and of the cervical spine is noted synchronously with thoracoabdominal movements. These imaging techniques of UA show the occurrence of passive pharyngeal collapse during certain types of central apnoea. In snorers

Twenty patients with sleep apnea syndrome were examined polysomnographically with and without an Esmarch sleep apnea appliance. The relationship between the therapeutic effect of the device and dentofacial morphologic characteristics of the patients were analyzed with the use of diagnostic casts and cephalometric radiographs. The apnea index or number of apneic episodes per hour decreased significantly (p < 0.0001) after insertion of the appliance. The diagnostic casts and cephalometric analyses revealed that the device was most effective for patients who exhibit a tendency toward micrognathia

The aim of this work was to study the existence of special characteristics in the sleep apnoea syndrome (SAS) discovered following a stay on the Intensive Care Unit. This retrospective study of 25 casenotes of SAS patients who were resuscitated has enabled a comparison with 182 SAS patients who have never had acute respiratory failure. The intensive care consisted of controlled ventilation, following intubation, in a clinical context of acute respiratory failure with major problems of conscious level. The diagnosis of SAS was made using conventional or computerised polysomnography, or a comput

The results of the treatment of the obstructive sleep apnea syndrome with tracheostomy (68 patients), continuous positive airway pressure (67 patients) or uvulopalatopharyngoplasty (14 patients) have been presented. The best results were after tracheostomy or during continuous positive airway pressure application, worse results gave uvulopalatopharyngoplasty.

Snoring usually is trivial and unimportant, but it can turn into a social or medical problem. Obesity, hypertension and heart disease are more frequent among snorers than among nonsnorers, and especially snorers with hypersomnia during the day are at risk. Hypersomnia in association with snoring usually signifies obstructive sleep apnea. Increased resistance in the upper airways, together with negative inspiratory pharyngeal pressure and muscular hypotonia during deep non-REM and REM sleep, lead to collapse of the pharynx, hypoxia and hypercapnia. Only after arousal from sleep does muscle tone

Sleep-related closure of the upper airways is the key feature of the obstructive sleep apnoea syndrome (OSAS). Excessive daytime sleepiness and loud snoring are the major signs. It is not possible to differentiate patients with the single complaint of habitual snoring from patients with sleep apnoea by their history alone; polysomnography is a mandatory procedure. Recently, uvulopalatopharyngoplasty (UPPP) has been introduced as a surgical procedure to treat OSAS and excessive snoring. This paper presents the results of UPPP in eleven patients evaluated by means of postoperative polysomnograph
