[Sleep apnea syndrome, snoring and uvulopalatopharyngoplasty].

Sleep-related closure of the upper airways is the key feature of the obstructive sleep apnoea syndrome (OSAS). Excessive daytime sleepiness and loud snoring are the major signs. It is not possible to differentiate patients with the single complaint of habitual snoring from patients with sleep apnoea by their history alone; polysomnography is a mandatory procedure. Recently, uvulopalatopharyngoplasty (UPPP) has been introduced as a surgical procedure to treat OSAS and excessive snoring. This paper presents the results of UPPP in eleven patients evaluated by means of postoperative polysomnograph

Elevated fasting insulin results in snoring: A view emerged from causal evaluation of glycemic traits and snoring.

BACKGROUND: Snoring and impaired glucose metabolism are common clinical manifestations and associated. The purpose of our study is to estimate the causal associations between snoring and glycemic traits. METHODS: We compared the weighted mean differences (WMD) for fasting insulin (FINS), glycosylated haemoglobin (HbA1c), fasting blood glucose (FBG) and 2 h-glucose post-challenge (2hGlu) levels between snorers and non-snorers by meta-analysis. Then, we obtained summary statistics from published GWAS of snoring and glycemic traits to perform bidirectional two-sample MR. Inverse variance weighti

Nasal symptoms increase the risk of snoring and snoring increases the risk of nasal symptoms. A longitudinal population study.

PURPOSE: Humans have a preference for nasal breathing during sleep. This 10-year prospective study aimed to determine if nasal symptoms can predict snoring and also if snoring can predict development of nasal symptoms. The hypothesis proposed is that nasal symptoms affect the risk of snoring 10 years later, whereas snoring does not increase the risk of developing nasal symptoms. METHODS: In the cohort study, Respiratory Health in Northern Europe (RHINE), a random population from Denmark, Estonia, Iceland, Norway, and Sweden, born between 1945 and 1973, was investigated by postal questionnaire

[Snoring in children. Algorithm for diagnostic approach].

BACKGROUND: Snoring in children is a prevalent symptom and may be an indicator of obstructive sleep apnoea. Despite its importance, there is no national guideline on its appropriate management. OBJECTIVE: To provide recommendations for the management of snoring in children and adolescents treated in a primary care setting. METHODS: A total of 16 national paediatric sleep experts were included in a Delphi process and formulated recommendations in the form of a step-wise work-up procedure. RESULTS: The following 8 steps were developed: (1) Identification of true cases of habitual snoring. (2)

The effect of singing on snoring and daytime somnolence.

The objective of the study is to compare the prevalence and severity of snoring and daytime somnolence amongst semiprofessional choir singers and non-singers. It is a cross-sectional comparative study and the setting is at a tertiary otorhinolaryngology referral centre. Adult singers were recruited from two mixed-gender choirs in London. The control group consisted of healthy volunteers who do not sing. The weight and height of all participants were measured by a single investigator. A questionnaire was completed by each subject, and the snoring habit section completed by their spouses or part

Somnoplasty for simple snoring--a pilot study.

INTRODUCTION: While most doctors believe they have a major role to play in breastfeeding promotion, and consider it worthwhile taking time to assist women to continue to breastfeed, it appears that gender and personal breastfeeding experience affect their attitude and confidence concerning breastfeeding issues. As doctors practicing in rural and regional areas may be expected to provide a greater degree of assistance and support for breastfeeding women, their views on these topics are of particular interest. This article reports the results of qualitative interviews with eight GP registrars fr

Assessment and management of the patient presenting with snoring.

BACKGROUND: Snoring is experienced regularly by approximately 30% of male adults, of whom one in seven will have obstructive sleep apnoea syndrome. Women experience snoring and apnoea at a rate of approximately half of that of men. OBJECTIVE: General practitioners have a key role in the management of snoring. Patients whose symptoms of excessive sleepiness and loud habitual snoring more than three times per week, should alert the GP to the presence of apnoeas. DISCUSSION: The co-existence of systemic essential hypertension, especially if more than two drugs are required for its control, or a

Radiofrequency versus LAUP for the treatment of snoring.

OBJECTIVE: To compare efficacy and tolerance of controlled temperature radiofrequency and laser-assisted uvulopalatopharyngoplasty (LAUP) for the treatment of snoring. METHODS: This was a prospective, nonrandomized study on 30 patients who snored. All patients had a sleep night study. Only patients with simple snoring or mild sleep apnea and soft palate obstruction were included. Efficacy was subjectively evaluated by the bed partner 2 months after the last treatment. Tolerance was estimated daily by the patient during 18 first postoperative days. Discomfort, pain, medication intake, diet, an

Imaging for the snoring and sleep apnea patient.

Upper airway imaging techniques have significantly advanced the understanding of the pathogenesis of obstructive sleep apnea and the biomechanical mechanisms by which therapeutic interventions for this disorder exert their effects. Both static and dynamic imaging studies have been used to examine the structure and function of the upper airway during wakefulness and sleep. These studies have highlighted the importance of the lateral pharyngeal walls in addition to the tongue and soft palate in modulating changes in upper airway caliber. Upper airway imaging has also been used to understand the

Patients' and partners' perceptions of uvulopalatopharyngoplasty for snoring.

OBJECTIVE: Uvulopalatopharyngoplasty (UPPP) has become a routine operation for snoring. Many centres have published their results. Few, if any, have questioned their patient's subjective feelings regarding the operation and more importantly, the patient's partner. METHOD: The study involved postal questionnaires and a retrospective case note review. Between the years 1988 and 1995, 125 patients underwent a UPPP for snoring at Derriford Hospital, Plymouth, England, a large district general hospital. Postal questionnaires were sent to all patients and their partners, asking them about their exp

Complications of palatoplasty for snoring or sleep apnoea.

A questionnaire was sent to consultant Otolaryngologists (483) throughout the UK to identify any mortality associated with uvulopalatoplasty and other forms of palatoplasties for snoring and/or obstructive sleep apnoea. The response rate was 76.8 per cent (371). Six intra- and post-operative deaths were reported and four were known to have obstructive sleep apnoea. Life-threatening morbidity occurred in at least seven patients (three known apnoeic), two required immediate tracheostomy and two were managed in the intensive care unit. This suggests that the apnoeic patients undergoing palatoplas

Treatment of sleep apnea and snoring in children.

Sleep apnea syndrome in children is usually caused by adenoidal and tonsillar hypertrophy. Here, we report on a typical case of obstructive sleep apnea syndrome, and also discuss recent indications for tonsillectomy in children. A 6-year-old boy came to our hospital suffering from serious obesity and somnolescence. He could not sleep lying down and thus started to sleep in a sitting position. Even during daytime he slept while standing, and exhibited hypersomnolescence. He was 133 cm tall and weighed 80 kg, and tonsillar hypertrophy was recognised. Consequently, tonsillectomy and a weight redu

[Surgical treatment of snoring and sleep apnea].

Twenty-one patients (four females and 17 males) greatly inconvenienced by snoring were subjected to UPP with CO2-laser technique. All patients were overweight (body-mass index average 30 kg/m2). Narrowing was found at the naso/oropharyngeal border in 71%. All had either abnormal uvula or soft palate. Snoring was reduced to a degree no longer socially troublesome in 17 patients. Three patients were still inconvenienced to some extent. Snoring was completely unchanged in only one patient. UPPP is not very difficult to perform, and we did not notice any sequelae of importance. We think that UPPP

Long-term results of uvulopalatopharyngoplasty for snoring.

Snoring is a disease of listeners and one which can cause significant disruption in an otherwise peaceful household. Uvulopalatopharyngoplasty (UPPP) was initially described in 1981 for the treatment of obstructive sleep apnea, and since that time only one large series has evaluated it as a treatment for snoring. Various therapeutic strategies have been described for the treatment of snoring, but UPPP seems to hold the most promise. A retrospective review was undertaken of over 100 patients undergoing UPPP since 1984 to evaluate the efficacy of the procedure in controlling the symptom of snori

Laser uvulopalatoplasty in treatment of habitual snoring.

Velopharyngeal sonorous snoring is best treated with uvulopalatopharyngoplasty (UPPP). To reduce surgical risks and minimize the morbidity, a simplified carbon dioxide laser uvulopalatoplasty (LUPP) was performed under local anesthesia. Among a total of 146 patients who had a LUPP performed, there was no significant bleeding or postoperative episodes of asphyxia. The procedure was well tolerated even by those patients with strong vomiting reflexes. The operation time was halved, as was the convalescence. Two patients (1.4%) developed scarring with nasal obstruction, but the impact of factors o

Snoring and the risk of ischemic brain infarction.

To determine if a history of snoring is a risk factor for brain infarction, I conducted a case-control study of risk factors for ischemic stroke using 177 consecutive male patients aged 16-60 (mean 49) years with acute brain infarction. For each patient I chose an age-matched (+/- 6 years) male control. Arterial hypertension, coronary heart disease, snoring (habitually or often), and heavy drinking (greater than 300 g/wk) were risk factors in the stepwise multiple logistic regression analysis. The odds ratio of snoring for brain infarction was 2.13. By McNemar's test this association increased

The surgical treatment of snoring: a patient's perspective.

An increasing number of loud snorers seek medical attention because of the social impact of snoring as well as its association with sleep apnea. Uvulopalatopharyngoplasty is reported to reduce or eliminate snoring in the majority of patients; however, little data are available to document the procedure's success. From February 1987 through August 1988, 125 patients underwent uvulopalatopharyngoplasty for habitual snoring; many of these patients had also documented sleep apnea. Of the 74 patients who responded to a postoperative questionnaire, 64 (86.48%) indicated that their snoring was either

[Surgical treatment of snoring: principle and technic].

Surgical treatment of snoring requires suppression both of the obstacle responsible for the nocturnal respiratory noise but also of the chronic hypoxia on which the sleep of these patients is grafted. The soft palate and its appendages (uvula and tonsillar pillars) are always involved. Precise definition of extent of palatal resection ensures success of operation and avoidance of nasal regurgitation of food or rhinolalia postoperatively. Nasal ventilation must be normal; a septoplasty is sometimes unavoidable and postoperative medical treatment of rhinitis necessary. The pharyngeal lumen must
