Speech Changes After Second-Side Magnetic Resonance-Guided Focused Ultrasound (MRgFUS) Thalamotomy: Is Intraoperative Speech Evaluation Associated With Speech Outcomes?

Introduction Staged bilateral magnetic resonance-guided focused ultrasound (MRgFUS) thalamotomy is an emerging option for the treatment of bilateral essential tremor (ET). However, there is historical concern that permanent bilateral thalamic lesioning leads to high rates of speech complications. This study evaluates bilateral MRgFUS thalamotomy of the ventral intermediate nucleus (VIM) for essential hand tremor, focusing on the impact of intraoperative speech monitoring during second-side procedures and its association with speech-related adverse effects (AEs). Methods A retrospective review

Preliminary Speech Perception Performance Profiles of School-Age Children With Childhood Apraxia of Speech, Speech Sound Disorder, and Typical Development.

PURPOSE: Limited research exists assessing speech perception in school-age children with speech sound disorder (SSD) and childhood apraxia of speech (CAS); despite early evidence that speech perception may lead to error-prone motor planning/programming. In this study, we examine speech perception performance in school-age children with and without speech production deficits. METHOD: Speech perception was assessed using the Wide Range Acoustic Accuracy Scale to determine the just-noticeable difference in discrimination for three consonant-vowel syllable contrasts (/bɑ/-/wɑ/, /dɑ/-/gɑ/, /ɹɑ/-/w

Functional Connectivity and Speech Entrainment Speech Entrainment Improves Connectivity Between Anterior and Posterior Cortical Speech Areas in Non-fluent Aphasia.

BACKGROUND: Speech entrainment (SE), the online mimicking of an audio-visual speech model, has been shown to increase speech fluency in individuals with non-fluent aphasia. One theory that may explain why SE improves speech output is that it synchronizes functional connectivity between anterior and posterior language regions to be more similar to that of neurotypical speakers. OBJECTIVES: The present study tested this by measuring functional connectivity between 2 regions shown to be necessary for speech production, and their right hemisphere homologues, in 24 persons with aphasia compared to

Acoustic analyses on German vowels in read speech, recited speech, and singing.

On the continuum of hyper- and hypoarticulation, speech production is adjusted based on the environment and the listener to ensure intelligibility with minimal articulatory effort. The current study examines 13 German vowels produced by 16 professional female singers in read speech, recited speech, and singing. Stimuli consisted of ten original extracts from German classical art songs. The vowels' first two formants were analyzed, comparing across the three conditions. Furthermore, Euclidean distances were calculated for all vowels. Results generally indicate more hyperarticulation in recited

Speech-in-Noise Ability and Longitudinal Cortical Thinning in Speech-Processing Networks.

IMPORTANCE: Age-related hearing loss is a major modifiable risk factor for dementia, yet the neural mechanisms linking auditory dysfunction to brain aging and cognitive decline remain unclear. In particular, it is unknown whether peripheral hearing loss, central auditory processing deficits, or hearing aid use best predict neurodegenerative change in older adults. OBJECTIVE: To determine whether hearing thresholds, speech-in-noise ability, and hearing aid use are associated with longitudinal cortical atrophy and cognitive trajectories in cognitively normal older adults. DESIGN, SETTING, AND

Age-related changes in acoustic cue use for speech-in-speech perception.

Acoustic cues such as pitch and spatial location allow listeners to attend to a target speaker and ignore competing talkers, aiding speech recognition in background noise. Diminished ability to utilize acoustic cues for speech stream segregation may thus contribute to older adults' challenges hearing in noise. Adults aged 18-74 completed a speech-in-speech identification task with three conditions containing 1) only pitch cues (fundamental frequency), 2) only spatial cues (interaural time differences; ITDs), or 3) both pitch and spatial cues for segregating a target talker from competing talke

Progression of Motor Speech Function in Speakers With Primary Progressive Apraxia of Speech.

PURPOSE: Speakers with primary progressive apraxia of speech (PPAOS) have an insidious onset of motor speech planning/programming difficulties. As the disease progresses, the apraxia of speech (AOS) becomes more severe and a co-occurring dysarthria often emerges. Here, longitudinal data from speakers with phonetic- and prosodic-predominant PPAOS are used to characterize the progression of their motor speech impairment, including the development of dysarthria and mutism. METHOD: Data are presented from 52 speakers who had PPAOS at enrollment (i.e., progressive AOS in the absence of aphasia, co

Speech Perception Variability in Childhood Apraxia of Speech: Implications for Assessment and Intervention.

PURPOSE: Beyond hallmark production deficits characterizing childhood apraxia of speech (CAS), largely attributed to disruption(s) in speech motor planning, children with CAS often present with co-occurring speech perception and language difficulties. Thus, careful consideration of the potential for speech perception difficulties to have cascading downstream effects on intervention responsiveness and real-life functioning for some children with CAS is highly important. The purpose of this tutorial was to consider the impact of speech perception abilities in children with CAS, which carries imp

Clear speech adaptations in spontaneous speech produced by young and older adults.

The study investigated the speech adaptations by older adults (OA) with and without age-related hearing loss made to communicate effectively in challenging communicative conditions. Acoustic analyses were carried out on spontaneous speech produced during a problem-solving task (diapix) carried out by talker pairs in different listening conditions. There were 83 talkers of Southern British English. Fifty-seven talkers were OAs aged 65-84, 30 older adults with normal hearing (OANH), and 27 older adults with hearing loss (OAHL) [mean pure tone average (PTA) 0.250-4 kHz: 27.7 dB HL]. Phrase-level speech simulation with an airway modulation model of speech production.

Artificial talkers and speech synthesis systems have long been used as a means of understanding both speech production and speech perception. The development of an airway modulation model is described that simulates the time-varying changes of the glottis and vocal tract, as well as acoustic wave propagation, during speech production. The result is a type of artificial talker that can be used to study various aspects of how sound is generated by humans and how that sound is perceived by a listener. The primary components of the model are introduced and simulation of words and phrases are demon

Speech naturalness and prolonged-speech treatments for stuttering: further variables and data.

It is well known that unusual speech quality may result from stuttering treatments that are based on prolonged speech. However, empirical information concerning the speech quality associated with those treatments is lacking. The present study was designed to contribute such empirical information. Results indicated that speech quality assessments of posttreatment clients, using Martin, Haroldson, and Triden's (1984) speech naturalness scale, gave similar results regardless of whether they were based on monologues or conversations. The speech quality of those clients remained stable at the concl

The response of the apparent receptive speech disorder of Parkinson's disease to speech therapy.

Eleven patients with Parkinson's disease were tested for prosodic abnormality, on three tests of speech production (of angry, questioning, and neutral statement forms), and four tests of appreciation of the prosodic features of speech and facial expression. The tests were repeated after a control period of two weeks without speech therapy and were not substantially different. After two weeks of intensive domiciliary prosodic therapy, the prosodic abnormality score was improved, as were three of the four tests of recognition of the prosodic features of speech and facial expression, and two of t

Perceptual Ratings Predict Speech Inversion Articulatory Kinematics in Childhood Speech Sound Disorders.

PURPOSE: This study evaluated whether articulatory kinematics, inferred by articulatory phonology speech inversion neural networks, aligned with perceptual ratings of /ɹ/ and /s/ in the speech of children with speech sound disorders. METHOD: Articulatory phonology vocal tract variables were inferred for 5,961 utterances from 118 children and three adults, aged 2.25-45 years. Perceptual ratings were standardized using the novel 5-point PERCEPT Rating Scale and training protocol. Two research questions examined if the articulatory patterns of inferred vocal tract variables aligned with the perc

Red flags for speech impairment: who should we refer to speech therapy?

BACKGROUND AND OBJECTIVE: Speech disorders are a common presenting concern to paediatricians. Yet evidence to guide detection and speech sound therapy referral of cases at risk for persistent speech disorder is limited, with no normative English speech data published for over two decades. Here we describe speech development in a large, contemporaneous normative sample, to help clinicians identify errors of disordered speech from developmental errors (which are of lesser concern). We also examine differences in speech development over 20 years. METHODS: Children aged 2 years 0 months-12 years

Non-speech oral motor treatment for children with developmental speech sound disorders.

BACKGROUND: Children with developmental speech sound disorders have difficulties in producing the speech sounds of their native language. These speech difficulties could be due to structural, sensory or neurophysiological causes (e.g. hearing impairment), but more often the cause of the problem is unknown. One treatment approach used by speech-language therapists/pathologists is non-speech oral motor treatment (NSOMT). NSOMTs are non-speech activities that aim to stimulate or improve speech production and treat specific speech errors. For example, using exercises such as smiling, pursing, blow

Connected speech processes in developmental speech impairment: observations from an electropalatographic perspective.

This paper uses a combination of perceptual and electropalatographic (EPG) analysis to explore the presence and characteristics of connected speech processes in the speech output of five older children with developmental speech impairments. Each of the children is shown to use some processes typical of normal speech production but also to use a range of more unusual processes at word junctures, with both inter- and intra-speaker variability apparent in the data. The findings suggest that the traditional focus on single word production in the assessment of developmental speech disorders needs t
