Self-Adjusted Amplification Parameters Produce Large Between-Subject Variability and Preserve Speech Intelligibility.

The current study used the self-fitting algorithm to allow listeners to self-adjust hearing-aid gain or compression parameters to select gain for speech understanding in a variety of quiet and noise conditions. Thirty listeners with mild to moderate sensorineural hearing loss adjusted gain parameters in quiet and in several types of noise. Outcomes from self-adjusted gain and audiologist-fit gain indicated consistent within-subject performance but a great deal of between-subject variability. Gain selection did not strongly affect intelligibility within the range of signal-to-noise ratios teste

How do families of children with Down syndrome perceive speech intelligibility in Turkey?

Childhood verbal apraxia has not been identified or treated sufficiently in children with Down syndrome but recent research has documented that symptoms of childhood verbal apraxia can be found in children with Down syndrome. But, it is not routinely diagnosed in this population. There is neither an assessment tool in Turkish nor any research on childhood verbal apraxia although there is a demand not only for children with Down syndrome but also for normally developing children. The study examined if it was possible to determine oral-motor difficulties and childhood verbal apraxia features in

Direct tongue neurotization: the effect on speech intelligibility in patients with Möbius syndrome.

BACKGROUND: Möbius syndrome is a disorder characterized by developmental impairment of cranial nerve VII, VI, often XII, and other cranial nerves. Facial reanimation in such patients restores the ability of some motion and of limited emotional expression. In one-fourth of these patients, hypoglossal involvement results in severe speech impairment due to tongue atrophy and lack of voluntary mobility. Bilabial incompetence due to facial paralysis further deteriorates speech capability. Direct tongue neurotization has been used by the senior author (J.K.T) to improve tongue function and speech in

Computerized measurement of speech intelligibility. II. Normative data from a 'closed response' test.

A Danish Four Alternative Forced Choice (4AFC) test was presented to normal-hearing and cochlearly hearing-impaired listeners in two background noises: a modulated, speech-spectrum-shaped noise and an 8-persons babble noise. At the presentation to the hearing-impaired subjects the test signals were given a frequency-dependent amplification in accordance with the POGO-fitting method. Normative data are presented which indicate that 4AFC is a valid test in spite of lack of homogeneity between the alternatives allowed. On the other hand, a learning effect is demonstrated which affects the reliabi

Speech sound production, speech intelligibility, and oral-motor outcomes of preterm children: Are they different from full term children?

We studied the speech sound abilities of preterm (PT) children. Thirty-one PT and twenty-nine full term (FT) children were recruited. Speech abilities were assessed in single word, story retelling, oral-motor, and intelligibility. PT group had poorer outcomes (Mean = 25.77, SD = 17.19) than FT ones (Mean = 5.9, SD = 4.97) for single word (p < 0.001). They obtained poorer results (Mean = 9.65, SD = 7.85) than FT peers (Mean = 2.95, SD = 3.34) in story retelling (p = 0.002) and intelligibility (Man-Whitney U = 89.50, p = 0.02). They obtained lower values for diadochokesis/patuku/(p < 0.001), iso

Impact of clear, loud, and slow speech on scaled intelligibility and speech severity in Parkinson's disease and multiple sclerosis.

PURPOSE: The perceptual consequences of rate reduction, increased vocal intensity, and clear speech were studied in speakers with multiple sclerosis (MS), Parkinson's disease (PD), and healthy controls. METHOD: Seventy-eight speakers read sentences in habitual, clear, loud, and slow conditions. Sentences were equated for peak amplitude and mixed with multitalker babble for presentation to listeners. Using a computerized visual analog scale, listeners judged intelligibility or speech severity as operationally defined in Sussman and Tjaden (2012). RESULTS: Loud and clear but not slow condition

Preschool speech intelligibility and vocabulary skills predict long-term speech and language outcomes following cochlear implantation in early childhood.

Speech and language measures during grade school predict adolescent speech-language outcomes in children who receive cochlear implants (CIs), but no research has examined whether speech and language functioning at even younger ages is predictive of long-term outcomes in this population. The purpose of this study was to examine whether early preschool measures of speech and language performance predict speech-language functioning in long-term users of CIs. Early measures of speech intelligibility and receptive vocabulary (obtained during preschool ages of 3-6 years) in a sample of 35 prelingual

An examination of the effects of broadband air-conduction masker on the speech intelligibility of speech-modulated bone-conduction ultrasound.

Ultrasound can be heard by bone-conduction, and speech-modulated bone-conducted ultrasound (BCU) delivers the speech information to the human ear. One of the recognition mechanisms is the demodulation of the signals. Because some of the profoundly deaf can also hear speech-modulated BCU, another mechanism may also contribution to the recognition of speech-modulated BCU. In this study, eight volunteers with normal hearing participated. The intelligibilities of speech-modulated BCU were measured using a numeral word list under masking conditions. Because the masker can mask the demodulated sound

Evaluation of the effect of speech-rate slowing on speech intelligibility in noise using a simulation of cochlear hearing loss.

The effect of digital processing, which slows the speed of speech (speech-rate) without changing its pitch, has been examined. The processing is intended to make speech communication easier by allowing more time for cognitive processing when the listening situation is difficult, for example, when listening to a foreign language, or when the user has a hearing loss. The speech-rate slowing makes use of a pitch-synchronous partial expansion of the waveform in the time domain. The processing was evaluated using a simulation of hearing loss which has been shown to lead to reduced intelligibility f

Investigating Acoustic Correlates of Intelligibility Gains and Losses During Slowed Speech: A Hybridization Approach.

Purpose This exploratory study sought to identify acoustic variables explaining rate-related variation in intelligibility for speakers with dysarthria secondary to multiple sclerosis. Method Seven speakers with dysarthria due to multiple sclerosis produced the same set of Harvard sentences at habitual and slow rates. Speakers were selected from a larger corpus on the basis of rate-related intelligibility characteristics. Four speakers demonstrated improved intelligibility and three speakers demonstrated reduced intelligibility when rate was slowed. A speech analysis resynthesis paradigm termed

Objective assessment of cleft lip and palate speech intelligibility using articulation and hypernasality measures.

Assessment of intelligibility is required to characterize the overall speech production capability and to measure the speech outcome of different interventions for individuals with cleft lip and palate (CLP). Researchers have found that articulation error and hypernasality have a significant effect on the degradation of CLP speech intelligibility. Motivated by this finding, the present work proposes an objective measure of sentence-level intelligibility by combining the information of articulation deficits and hypernasality. These two speech disorders represent different aspects of CLP speech.

Effects of loud and amplified speech on sentence and word intelligibility in Parkinson disease.

PURPOSE: In the two experiments in this study, the author examined the effects of increased vocal effort (loud speech) and amplification on sentence and word intelligibility in speakers with Parkinson disease (PD). Methods Five talkers with PD produced sentences and words at habitual levels of effort and using loud speech techniques. Amplified sets of sentences and words were created by increasing the intensity of habitual stimuli to the level of loud stimuli. Listeners rated the intelligibility of the 3 sets of sentences on a 1-7 scale and transcribed the 3 sets of words. RESULTS: Both loud

Simulation of the effects of loudness recruitment on the intelligibility of speech in noise.

This experiment simulated the threshold elevation and loudness recruitment associated with three different types of hearing loss: moderate flat (condition R2), severe flat (condition R3), and moderate-to-severe sloping (condition RX). This was done to allow an examination of the effects of these factors on the intelligibility of speech, in isolation from other factors that are normally associated with cochlear hearing loss, such as reduced frequency selectivity. The speech was presented at a fixed input level of 65 dB SPL, against a background of a noise whose spectrum was shaped to match the
