Electric-Acoustic Stimulation After Reimplantation: Hearing Preservation and Speech Perception.

OBJECTIVE: Hearing preservation after cochlear implantation allows for fitting of acoustic and cochlear implant technologies in the same ear, known as Electric-Acoustic Stimulation (EAS). Cochlear implant recipients with EAS who experience an internal device failure face the additional risk of residual hearing loss during reimplantation. This report reviews the case of an EAS recipient with long-term hearing preservation and significant benefit who experienced a device failure and underwent cochlear reimplantation. PATIENT: Case study who presented with an internal device failure after nearly

Engaging the Articulators Enhances Perception of Concordant Visible Speech Movements.

Purpose This study aimed to test whether (and how) somatosensory feedback signals from the vocal tract affect concurrent unimodal visual speech perception. Method Participants discriminated pairs of silent visual utterances of vowels under 3 experimental conditions: (a) normal (baseline) and while holding either (b) a bite block or (c) a lip tube in their mouths. To test the specificity of somatosensory-visual interactions during perception, we assessed discrimination of vowel contrasts optically distinguished based on their mandibular (English /ɛ/-/æ/) or labial (English /u/-French /u/) postu

Do prereaders' auditory processing and speech perception predict later literacy?

Developmental dyslexia has frequently been linked to deficits in auditory processing and speech perception. However, the presence and precise nature of these deficits and the direction of their relation with reading, remains debated. In this longitudinal study, 87 five-year-olds at high and low family risk for dyslexia were followed before and during different stages of reading acquisition. The processing of different auditory cues was investigated, together with performance on speech perception and phonology and reading. Results show no effect of family risk for dyslexia on prereading auditor

Exploring dyslexics' phonological deficit III: foreign speech perception and production.

This study investigates French dyslexic and control adult participants' ability to perceive and produce two different non-native contrasts (one segmental and one prosodic), across several conditions varying short-term memory load. For this purpose, we selected Korean plosive voicing (whose categories conflict with French ones) as the segmental contrast and lexical stress as the prosodic contrast (French does not use contrastive lexical stress). We also used a French (native) segmental contrast as a control. Tasks were either auditory discrimination or repetition of CVCV nonsense words. Short-t

Cortical bases of speech perception: evidence from functional lesion studies.

Functional lesion studies have yielded new information about the cortical organization of speech perception in the human brain. We will review a number of recent findings, focusing on studies of speech perception that use the techniques of electrocortical mapping by cortical stimulation and hemispheric anesthetization by intracarotid amobarbital. Implications for recent developments in neuroimaging studies of speech perception will be discussed. This discussion will provide the framework for a developing model of the cortical circuitry critical for speech perception.

Language deficits in dyslexic children: speech perception, phonology, and morphology.

We investigated the relationship between dyslexia and three aspects of language: speech perception, phonology, and morphology. Reading and language tasks were administered to dyslexics aged 8-9 years and to two normal reader groups (age-matched and reading-level matched). Three dyslexic groups were identified: phonological dyslexics (PD), developmentally language impaired (LI), and globally delayed (delay-type dyslexics). The LI and PD groups exhibited similar patterns of reading impairment, attributed to low phonological skills. However, only the LI group showed clear speech perception defici

Acoustic-phonetic approach toward understanding neural processes and speech perception.

This review paper describes an "acoustic-phonetic" experimental approach aimed at understanding normal and abnormal speech perception processes from both a behavioral and an electrophysiologic perspective. First, we consider the relevant acoustic characteristics of speech and identify a set of acoustic-phonetic classes that represent the parameters most important for making an acoustic signal sound like speech. Second, we review what is known about the neurophysiologic representation of acoustic-phonetic speech parameters in animal and human subjects. Third, we describe how an acoustic-phoneti

[Predictive factors for speech perception in patients with cochlear implant].

Cochlear implant therapy is an epoch-making advance in artificial sensory organ transplants, but the positive effects on speech perception vary. Quantification theory type I, a multivariate analysis, was used to determine predictive factors for speech perception in patients with cochlear implants. Fifty-one postlingual deaf adults (18 male and 33 female, mean age, 53.4, mean duration of deafness, 8.6 years) were tested for speech perception three or more months after a Nucleus 22 channels cochlear implant. The cause of deafness in nine patients was labyrinthitis, ototoxicity in five, meningiti

Perception of pitch contours in single words in alaryngeal speech.

The perception and production of pitch contours were investigated in single words produced by two groups of alaryngeal speakers: tracheo-oesophageal (TE) and oesophageal (E) speakers. High quality tape-recordings of three tonal patterns by four oesophageal and eight tracheo-oesophageal speakers in monosyllabic words were judged by a group of six speech and language therapy listeners. The results indicated that tonal patterns can be produced with a relatively high level of reliability for both speaker groups. Some individual speakers from both groups approached predicted normal levels. These fi

Comparison of speech perception skills in young and old listeners.

A companion paper (Yanz. 1984, Ear Hear. 5, 64-71) has described an approach to the quantification of speech perception using measures derived from the theory of signal detection (TSD). The present experiment uses this approach to compare speech perception in young and old listeners in terms of the traditional word recognition score (%C), the ability to assess the accuracy of one's own identifications [P(A)], and the decision criterion used in that assessment (B). Monosyllables were presented in a background of broadband noise at two signal-to-noise ratios (S/N), 0 dB and 5 dB. Subjects record

The Influence of Sensorineural Hearing Loss on the Relationship Between the Perception of Speech in Noise and Dysarthric Speech.

PURPOSE: The ability to understand speech under adverse listening conditions is highly variable across listeners. Despite this, studies have found that listeners with normal hearing display consistency in their ability to perceive speech across different types of degraded speech, suggesting that, for at least these listeners, global skills may be involved in navigating the ambiguity in speech signals. However, there are substantial differences in the perceptual challenges faced by listeners with normal and impaired hearing. This study examines whether listeners with sensorineural hearing loss

[Vocal effectiveness in speech and singing: acoustical, physiological and perceptive aspects. applications in speech therapy].

OBJECTIVE: What is vocal effectiveness in lyrical singing in comparison to speech? Our study tries to answer this question, using vocal efficiency and spectral vocal effectiveness. MATERIAL AND METHODS: Vocal efficiency was mesured for a trained and untrained subject. RESULTS: According to these invasive measures, it appears that the trained singer uses her larynx less efficiently. Efficiency of the larynx in terms of energy then appears to be secondary to the desired voice quality. The acoustic measures of spectral vocal effectiveness of vowels and sentences, spoken and sung by 23 singers,

Effect of perceptive-auditory training on the classification of speech hypernasality.

PURPOSE: To analyze the effect of auditory-perceptual training by inexperienced speech-language pathologists in the classification of hypernasality in individuals with cleft lip and palate and compare their classification of hypernasality individually, with the gold standard evaluation, before and after this training. METHODS: Three inexperienced speech-language pathologists used a four-point scale to assess 24 high-pressure speech samples from individuals with cleft lip and palate, before and after auditory-perceptual training. The speech samples corresponded to six samples of each degree of

Hearing a voice in the noise: auditory hallucinations and speech perception.

BACKGROUND: It has recently been suggested that auditory hallucinations are the result of a criterion shift when deciding whether or not a meaningful signal has emerged. The approach proposes that a liberal criterion may result in increased false-positive identifications, without additional perceptual deficit. To test this hypothesis, we devised a speech discrimination task and used signal detection theory (SDT) to investigate the underlying cognitive mechanisms. METHOD: Schizophrenia patients with and without auditory hallucinations and a healthy control group completed a speech discriminati

Information in speech: observations on the perception of [s]-stop clusters.

A series of experiments is reported that investigated the pattern of acoustic information specifying place and manner of stop consonants in medial position after [s]. In both production and perception, information for stop place includes the spectrum of the fricative at offset, the duration of the silent closure interval, the spectral relationship between the frequency of the stop release burst and the following periodically excited formants, and the spectral and temporal characteristics of the first formant transition. Similarly, the information for stop manner includes the duration of silent

A Moderated Mediation Model Linking Speech-in-Noise Exposure, Speech Perception in Noise Ability, Loneliness, and Depression.

PURPOSE: Hearing loss is associated with elevated levels of loneliness and depression, but the factors accounting for these associations remain unclear. This study examined whether real-world hearing behaviors-time spent in speech-in-noise (SiN) environments and speech perception in noise (SPIN) ability-are associated with loneliness and depression in young adults with normal hearing, using a moderated mediation model. METHOD: Fifty-four participants with audiometrically normal hearing completed standardized assessments of SPIN ability (Quick Speech-in-Noise test), loneliness (UCLA Loneliness

Speech Perception in Noise and Medial Olivocochlear Reflex: Effects of Age, Speech Stimulus, and Response-Related Variables.

PURPOSE: The role of the medial olivocochlear system in speech perception in noise has been debated over the years, with studies showing mixed results. One possible reason for this could be the dependence of this relationship on the parameters used in assessing the speech perception ability (age, stimulus, and response-related variables). METHODS: The current study assessed the influence of the type of speech stimuli (monosyllables, words, and sentences), the signal-to-noise ratio (+5, 0, -5, and -10 dB), the metric used to quantify the speech perception ability (percent-correct, SNR-50, and

Duplex perception reveals brainstem auditory representations are modulated by listeners' ongoing percept for speech.

So-called duplex speech stimuli with perceptually ambiguous spectral cues to one ear and isolated low- versus high-frequency third formant "chirp" to the opposite ear yield a coherent percept supporting their phonetic categorization. Critically, such dichotic sounds are only perceived categorically upon binaural integration. Here, we used frequency-following responses (FFRs), scalp-recorded potentials reflecting phase-locked subcortical activity, to investigate brainstem responses to fused speech percepts and to determine whether FFRs reflect binaurally integrated category-level representation

Compensation in audiovisual speech perception: Discounting the pen in the mouth.

The articulation of speech segments is influenced by their phonetic context. Human speech perception seems to compensate for such coarticulatory effects, interpreting acoustic cues relative to their values expected in the current context. We test whether similar compensation is observed for visually presented, nonphonetic contexts (a pen in the mouth of a talker), as predicted by some accounts of perceptual compensation. In a series of perception experiments, we find that listeners compensate for the presence of a pen in the mouth of the talker, as long as the effects of the pen on the articul
