Intelligibility in Down syndrome: Effect of measurement method and listener experience.

BACKGROUND: Speech intelligibility is a global indicator of the severity of a speech problem. It is a measure that has been used frequently in research and clinical assessment of speech. Previous studies have shown that factors, such as measurement method and listener experience, can influence speech intelligibility scores. However, these factors of speech intelligibility assessment have not yet been investigated in people with Down syndrome (DS). AIMS: To compare the speech intelligibility scores in speakers with DS measured using two methods: orthographic transcription and visual analogue s

Measurement of Nasalance Scores Without Touching the Philtrum for Better Comfort During Speech Assessment and Therapy: A Preliminary Study.

OBJECTIVE: The Kay Pentax nasometer uses a separator plate that touches the philtrum of a patient to separate the nasal and oral sound energies for nasalance measurement. However, the separator plate can restrict the natural movement of the patient's upper lip and generate unpleasant pressure on the patient's philtrum. The present study was intended to measure nasalance scores without touching the philtrum for better comfort during speech assessment and therapy. METHODS: Nasalance scores of 10 males and 10 females having no speech disorders were measured under 4 levels (0, 5, 10, and 15 mm) o

Evidence for the distinction between "consonantal-/r/" and "vocalic-/r/" in American English.

We examine the distinction between "consonantal-r" and "vocalic-r" in American English, terms encountered in the speech pathology literature but rarely in phonetic studies. We review evidence from phonetics, phonology and therapy, and describe our own study which measured percentage rhoticity in pre- and post-vocalic /r/. We suggest that the evidence supports a view that there is no more variation between pre-vocalic and post-vocalic /r/ than found in many other consonants. We also evaluate the different transcription traditions for post-vocalic /r/ in American English (as a consonant or a vow

Hierarchical Temporal Structuring of Speech: A Multiscale, Multimodal Framework to Inform the Assessment and Management of Neuromotor Speech Disorder.

PURPOSE: Hierarchical temporal structuring of speech is the key to multiscale linguistic information transfer toward effective communication. This study investigated and linked the hierarchical temporal cues of the kinematic and acoustic modalities of natural, unscripted speech in neurologically healthy and impaired speakers. METHOD: Thirteen individuals with amyotrophic lateral sclerosis (ALS) and 10 age-matched healthy controls performed a story-telling task. The hierarchical temporal structure of the speech stimulus was measured by (a) 26 articulatory-kinematic features characterizing the

Auditory-Perceptual Features of Speech in Children and Adults With Down Syndrome: A Speech Profile Analysis.

Purpose The aim of this study was to determine how the speech disorder profiles in Down syndrome (DS) relate to reduced intelligibility, atypical overall quality, and impairments in the subsystems of speech production (phonation, articulation, resonance, and prosody). Method Auditory-perceptual ratings of intelligibility, overall quality, and features associated with the subsystems of speech production were obtained from recordings of 79 children and adults with DS. Ratings were made for sustained vowels (62 of 79 speakers) and short sentences (79 speakers). The data were analyzed to determine

The specific relation between perception and production errors for place of articulation in developmental apraxia of speech.

Developmental apraxia of speech is a disorder of phonological and articulatory output processes. However, it has been suggested that perceptual deficits may contribute to the disorder. Identification and discrimination tasks offer a fine-grained assessment of central auditory and phonetic functions. Seventeen children with developmental apraxia (mean age 8:9, years:months) and 16 control children (mean age 8:0) were administered tests of identification and discrimination of resynthesized and synthesized monosyllabic words differing in place-of-articulation of the initial voiced stop consonants

Traditional and phonological treatment for teaching English fricatives and affricates to Koreans.

Non-native speakers of English are being seen for accent reduction in speech-language pathology clinics although there is little evidence that treatment is effective. Two types of treatment were evaluated. Traditional articulation-based treatment was successful for 2 native Korean speakers in improving English fricatives and affricate production. A second treatment using phonologically based contrasts was also successful for the same sounds with 2 other Korean speakers. Some issues in foreign accent reduction are discussed.

Speech therapy for Parkinson's disease.

Twenty-six patients with the speech disorder of Parkinson's disease received daily speech therapy (prosodic exercises) at home for 2 to 3 weeks. There were significant improvements in speech as assessed by scores for prosodic abnormality and intelligibility' and these were maintained in part for up to 3 months. The degree of improvement was clinically and psychologically important, and relatives commented on the social benefits. The use of a visual reinforcement device produced limited benefit over and above that from prosodic exercises alone, except to patients with severe speech disorder.

Speech Intervention for Children With Cleft Palate Using Principles of Motor Learning.

PURPOSE: This is a pilot study to apply an articulatory kinematic speech intervention that uses the principles of motor learning (PML) to improve speech and resonance outcomes for children with cleft palate. It is hypothesized that (a) treatment that applies select PML during practice will improve production of treated phonemes, representing both active and inconsistent passive errors, at word level in children with cleft palate; (b) effects of practice on phonemes with active or inconsistent passive errors will generalize to untreated exemplars of treated phonemes; and (c) learning will be re

The Relationship Between Single-Word Speech Severity and Intelligibility in Childhood Apraxia of Speech.

PURPOSE: The purpose of this study was to investigate the association between perceived single-word speech severity and intelligibility in children with childhood apraxia of speech (CAS), with and without comorbid language impairment (LI), and to investigate the contribution of different CAS signs to perceived single-word speech severity and single-word intelligibility. METHOD: Thirty children with CAS, 18 with comorbid LI, completed the Goldman-Fristoe Test of Articulation-Second Edition (GFTA-2). Trained judges coded children's responses for signs of CAS and percent phonemes correct. Nine l

Resonant Voice: Perceptual and Acoustic Analysis After an Intensive Lessac Kinesensic Training Workshop.

OBJECTIVE: To investigate whether actors' and actresses' voices post an Intensive Lessac Kinesensic Training Workshop - ILKTW can be perceptually identified as more resonant, and to assess if the Acoustic Voice Quality Index (AVQI), the Acoustic Breathing Index (ABI), and their acoustic measures are able to indicate the classified voices as more resonant. METHODS: Eight vocally healthy English first-language actors and actresses, participants of the same ILKTW, were asked to sustain the vowel /a/ and to read a piece of the Rainbow Passage, pre and postworkshop, at a self-selected habitual fre

Dyslexia: the articulatory hypothesis revisited.

Articulatory disorders have been associated with developmental phonological dyslexia in the literature. However, very few information is available about the articulatory movements involved in speech production in dyslexic children. This study uses aerodynamic/acoustic data to explore how dyslexic children produce bilabial stops in French (/b, p/) within a sentence where they occurred in two positions and in three vowel environments. Average durations of articulatory closure and release were calculated in 10 phonological dyslexic children and two groups of age-matched and reading age-matched co

Dysphonia Interference in Schoolteachers' Speech Intelligibility in the Classroom.

UNLABELLED: "Among the most common occupations, schooteachers are the ones who experience the most changes throughout their career. Considering this, the present study aims to verify whether dysphonia in three different degrees may compromise the speech intelligibility of schoolteachers in the classroom. METHOD: Overall, 39 students, average age 10 years, randomly selected from a public school in the Federal District, Brazil (Distrito Federal, Brasil) performed a transcription task of 20 sentences spoken by four distinct female voices in a classroom, one with a control voice (normal), another

A Framework of Motoric Complexity: An Investigation in Children With Typical and Impaired Speech Development.

Introduction The current work presents a framework of motoric complexity where stimuli differ according to movement elements across a sound sequence (i.e., consonant transitions and vowel direction). This framework was then examined in children with childhood apraxia of speech (CAS), other speech sound disorders (SSDs), and typical development (TD). Method Twenty-four children (CAS, n = 8; SSD, n = 8; TD, n = 8), 5-6 years of age, participated in this study. The children produced words that varied in motoric complexity while transcription, acoustic, and kinematic data were collected. Multidime

Application of childhood apraxia of speech clinical markers to French-speaking children: A preliminary study.

PURPOSE: Childhood apraxia of speech (CAS) is difficult to diagnose because there is little agreement on objective clinical markers. Since studies of phonological development in French-speaking children are scarce, there are even fewer recognised markers in French as compared to English. This study aims to determine if a set of operationalised, quantitative measures derived from clinical markers of CAS in English corroborate with clinical CAS diagnosis in French-speaking children. This research contributes to improving differential diagnosis of CAS and phonological disorder cross-linguisticall

Improving alaryngeal speech intelligibility.

Laryngectomized patients using esophageal speech or an electronic artificial larynx have difficulty producing correct voicing contrasts between homorganic consonants. Voicing of a voiceless consonant is the most frequent listener misidentification. A therapy technique is described that emphasizes "pushing harder" on voiceless consonants to improve the intelligibility of alaryngeal speakers. Laryngectomy speech therapy programs should focus first on the production of voiceless consonants before attempting to effect voicing.

Implementing a Statistical Parametric Speech Synthesis System for a Patient with Laryngeal Cancer.

Total laryngectomy, i.e., the surgical removal of the larynx, has a profound influence on a patient's quality of life. The procedure results in a loss of natural voice, which in effect constitutes a significant socio-psychological problem for the patient. The main aim of the study was to develop a statistical parametric speech synthesis system for a patient with laryngeal cancer, on the basis of the patient's speech samples recorded shortly before the surgery and to check if it was possible to generate speech quality close to that of the original recordings. The recording made use of a represe

Identifying segmental and prosodic errors associated with the increasing word length effect in acquired apraxia of speech.

Purpose: Individuals with stroke-related apraxia of speech (AOS) plus aphasia tend to produce more speech errors with increasing word length. The Words of Increasing Length task (WIL) uses a 3-point scale to score word accuracy but penalises for error types that can arise either from language or motor impairment, reducing the test's sensitivity and specificity. The purpose here was to identify error types explaining variance in the WIL score, and those associated with AOS and word length.Method: Speech errors were perceptually identified on the WIL task for 51 Australian English-speaking adult
