Combined aplasia of sphenoid, frontal, and maxillary sinuses accompanied by ethmoid sinus hypoplasia.

OBJECTIVE: To report the case of a woman who had combined aplasia of sphenoid, frontal, and maxillary sinuses accompanied by ethmoid sinus hypoplasia. METHODS AND RESULTS: A 47-year-old woman presented with complaints of headache, nasal obstruction, and postnasal drainage. She had no previous history of either facial trauma or systemic diseases affecting the skeletal system. She had a history of functional endoscopic sinus surgery twice in an other institution. However, the surgeries did not significantly alter her symptoms. Coronal and axial computed tomographic scans of the nose and the par

[Intra-sphenoidal prolapse of the internal carotid artery. Computed tomography of 300 sinuses].

OBJECTIVES: Endonasal sphenoidotomy is a high risk operation: fatal complications due to direct lesion to the internal carotid artery have been reported. We conducted a prospective analysis of carotid prolapse within the sphenoid cavity in order to draw attention to this dangerous anatomic variant. METHODS: Tomodensitometry of the sinuses was performed in 150 patients (mean age 35 years; range 20-70) with inflammatory nasal symptoms of less than one year duration (mean 5.2 months). Cases with benign or malignant tumours, polyposis or a past history of sinus trauma were excluded. Transversal s

The sphenoidal sinuses: radiographic patterns of normal development and abnormal findings in infants and children.

Developmental patterns of sphenoidal sinuses were determined from lateral radiographs of 1702 patients from newborn to 15 years of age. Pneumatization of the conchal sinuses was seen as early as 6 months of age; 90% showed sphenoidal sinus pneumatization by 4 years of age; 100% by 10 years of age. Extension of pneumatization into the basisphenoid may be evident after 4 years of age, and 95% showed basisphenoidal pneumatization after age 12. An apparent lack of pneumatization of the sphenoidal sinuses in patients over 10 should stimulate investigation for sphenoidal sinus disease. Although isol

Anatomical variation in the sphenoidal sinuses in patients with chronic rhinosinusitis: A CT scan study.

UNLABELLED: Computerized tomography (CT) of the skull base region has become an indispensable tool for endoscopic sinonasal surgery. OBJECTIVES: Fundamental knowledge of the sinus anatomy is crucial for preoperative surgical planning. The aim of this research was to evaluate associations between the anatomical variations sphenoidal sinuses and chronic rhinosinusitis (CRS) by using CT. METHODS: A descriptive study was performed on patients with CRS, who were referred to the department of radiodiagnosis and imaging for CT scanning. Parameters were compared between the study and control groups.

Assessment of the relation of the optic nerve to the posterior ethmoid and sphenoid sinuses by computed tomography.

OBJECTIVE: The aim of this study was to observe the relationship of the sphenoid sinus and posterior ethmoid cells with the optic nerve. MATERIAL AND METHODS: 34 CT scans (68 sides) of paranasal sinuses of patients older than 18 years were retrospectively reviewed. Images were assessed in two planes--axial and coronal. We observed the position and relationship of the optic nerve to the posterior ethmoid and sphenoid sinuses, bony dehiscence and protrusion of the optic nerve into sinuses, and pneumatization of the anterior clinoid process. RESULTS: The most frequent position of optic nerve (O

Meningitis following relapsing painful ophthalmoplegia in aspergillus sphenoidal sinusitis: a case report.

We report the case of a 58-year-old woman in whom relapsing painful ophthalmoplegia related to a mycetoma of the sphenoid sinus gave origin to meningitis with markedly depressed glucose levels in the cerebrospinal fluid. Surgical exeresis of the mycetoma allowed aetiological diagnosis (aspergillosis) and--together with antimycotic therapy--led to durable clinical response.

The relationship between the sphenoid and the posterior ethmoid sinuses and the optic nerves in Turkish patients.

The sphenoid and the posterior ethmoid sinuses are surrounded by more vital structures than any other sinus. With the widespread acceptance and expanding role of endoscopic sinus surgery, a proper understanding of the anatomy of the sphenoid and the posterior ethmoid sinuses is achieved. We reviewed 100 CT studies of the paranasal sinuses belonging to patients examined for a chronic inflammatory disease of the nasal cavity and the paranasal sinuses. The relationship between the optic nerves and the paranasal sinuses is classified into four discrete categories. Type 1 with a proportion of 64% i

Diagnosis of Acute Isolated Sphenoid Sinusitis Based on a Positive Kernig Sign in a Pediatric Patient.

Acute isolated sphenoid sinusitis in children is relatively rare, often presents with atypical symptoms, and may result in intracranial complications; therefore, early diagnosis is essential. We report the case of a 14-year-old girl who presented with a headache and a positive Kernig sign. Computed tomography (CT) demonstrated fluid retention in the sphenoid sinus, and lumbar puncture showed no abnormalities in the cerebrospinal fluid (CSF). Based on these findings, she was diagnosed with acute isolated sphenoid sinusitis without intracranial complications. However, the patient showed a poor r

Central skull base osteomyelitis secondary to invasive aspergillus sphenoid sinusitis presenting with isolated 12th nerve palsy.

Skull base osteomyelitis is a potentially life-threatening infection, usually seen in elderly immunocompromised patients secondary to malignant otitis externa (MOE) caused by Pseudomonas. Central or atypical skull base osteomyelitis often poses a diagnostic challenge as they present as head-ache with or without cranial nerve palsy often without any obvious source of infection. Although the incidence of fungal skull base osteomyelitis is increasing central skull base osteomyelitis due to invasive fungal sinusitis presenting with isolated hypoglossal nerve palsy has not been reported in the lite

Isolated abducens nerve palsy following upper respiratory infection in a patient with ipsilateral chronic sphenoid sinusitis.

The abducens nerve, which is vulnerable because of its complex anatomy at the skull base, is seldom affected by acute or severe sphenoid sinusitis. Notably, abducens nerve palsy following asymptomatic chronic rhinosinusitis (CRS) in a healthy young individual after a mild upper respiratory infection (URI) remains undocumented in the literature. Herein, we report a case of acute unilateral abducens neuropathy in a healthy 35-year-old woman with CRS in the ipsilateral sphenoid sinus, following a mild URI 2 weeks earlier. She presented with sudden-onset diplopia, was afebrile, and had normal seru

Miller Fisher variant of Guillain-Barré syndrome masquerading as acute sphenoid sinusitis with orbital apex syndrome.

Miller Fisher syndrome (MFS), a variant of Guillain-Barré syndrome, is a rare disorder typically characterized by a triad of ataxia, areflexia, and ophthalmoplegia, which may have a highly variable clinical presentation. We report a case of MFS in a 45-year-old female presenting with sphenoid sinusitis and sixth nerve palsy. She underwent endoscopic sphenoid sinusotomy without improvement, had postoperative deterioration, was diagnosed with MFS, and was treated with intravenous immunoglobulin with complete response. Because of the potential severity of Guillain-Barré syndrome, great vigilance

Infectious aneurysm of the intracavernous carotid artery occurring concomitantly with sphenoid sinusitis; an autopsy case report.

A previously healthy 56-year-old woman presented with right-sided ophthalmic pain and diplopia following headache and fever. A neurological examination revealed 3rd and 6th right cranial nerve palsies. Brain magnetic resonance imaging (MRI) and 3D-computed tomography (CT) angiography (CTA) showed right-sided sphenoid sinusitis, cavernous sinus thrombophlebitis, and aneurysms in the right intracavernous carotid artery and in a portion of internal carotid-posterior communicating artery. We diagnosed her condition as cavernous sinus syndrome with an infectious aneurysm secondary to sphenoiditis;

[Maxillary and sphenoid sinusitis complicated by acute inflammatory optic neuritis in a 12-year-old patient].

Acute inflammatory optic neuritis (retrobulbar optic neuritis) is an uncommon but described complication of acute sinusitis. It is due to a close anatomic relationship between the sinus and the optic nerve. Inflammatory mechanism is dominant with a possible bilateral neuritis, especially among children. In the reported case, as usual in childhood, visual acuity loss has been particularly severe, and recovery slow (it takes usually 6 weeks) but almost complete. Primary treatment is medical, with antibiotics and corticosteroids that must be prescribed for a prolonged time.

The simultaneous occurrence of a fungal ball in maxillary and sphenoid sinuses: a case report and review of the literature.

A fungal ball is most frequently found in the maxillary sinus, though it can also occur in the sphenoid, ethmoid, and frontal sinuses. This study aims to demonstrate the pathological conditions and clinical features of a patient with unilateral left maxillary and sphenoid sinus fungal balls. A 52-year-old woman arrived with complaints of headache, facial pain, and post-nasal discharge. An endonasal endoscopic examination was unremarkable. However, the plain paranasal sinuses computerized tomography scan (CT scan PNS) of the patient revealed two simultaneous opacities occupying both the maxilla

Three-Dimensional Image Evaluation of the Relationship Between the Diagonal Dimensions of the Sella and Sphenoid Sinuses.

OBJECTIVE: The sphenoid sinus is a key anatomic structure in transsphenoidal procedures due to its central location and proximity to vital structures. A comprehensive understanding of the dimensional relationship between the sphenoid sinus and sella is essential for advancing transsphenoidal surgery and enhancing its safety. Therefore, this study aims to quantitatively analyse the relationship between the volume and linear dimensions of these structures using specialized radiologic software on adult CT images. METHODS: A total of 220 paranasal sinus computed tomography scans were evaluated. S
