Efficacy and safety of ultrasound-guided peripheral nerve blocks in management of chronic resistant migraine.

BACKGROUND: Migraine is a common primary headache disorder with different treatment modalities emerging as ultrasound guided peripheral nerve blocks. We compared the efficacy and safety of ultrasound guided bilateral sphenopalatine ganglion (SPG) block versus bilateral greater occipital nerve (GON) block, in chronic resistant migraine patients and controls. METHODS: This study was an interventional randomized controlled trial, including 53 patients, 22 in sphenopalatine ganglion arm, 21 greater occipital nerve arm and 10 in sham group. All patients were assessed initially by headache diary (f

Postdural Puncture Headaches in Pediatric Patients: A Review of Options When Repeated Epidural Blood Patches Do Not Work.

We present the case of an adolescent with refractory postdural puncture headache (PDPH), whose symptoms resolved with a sphenopalatine ganglion (SPG) nerve block using a J-tip style catheter. Our patient was treated with multiple modalities, including conservative and medical management, multiple epidural blood patches, and different nerve blocks. We discussed different treatments for the PDPH, why each modality did not work, and why our SPG block with a J-tip catheter possibly provided a better sympathetic block in a patient with intractable PDPH for two weeks.

Neurolysis of the Sphenopalatine Ganglion for Refractory Facial Pain in a Paediatric Cancer Patient: A Case Report.

Facial pain associated with oncologic pathologies in pediatric patients presents significant diagnostic and therapeutic challenges with a considerable impact on quality of life. In this context, chemical neurolysis of the sphenopalatine ganglion emerges as a promising therapeutic option. We present the case of a 9-year-old girl with embryonal rhabdomyosarcoma who experienced severe facial pain resistant to conventional treatment. The pain was described as continuous, disabling, and neuropathic in nature, affecting basic functions such as eating and sleeping. Pharmacological management with opi

The Role of IR in Pediatric Pain and Palliation.

Pediatric chronic pain is a significant problem that is often underreported and therefore not always adequately addressed. Causes include anatomic variants, trauma, local inflammation, and malignancy. Nerve blocks and cryoneurolysis can aid as an adjunct to pain management for these patients and significantly improve their quality of life. This article will describe specific procedures in which interventional radiology can aid in several specific procedures for pediatric pain management: splanchnic cryoneurolysis, sphenopalatine ganglion nerve block/cryoneurolysis, inferior alveolar nerve bloc

Update and Advances on Post-dural Puncture Headache.

This document provides an overview of post-dural puncture headache (PDPH), covering its historical perspective, anatomy and physiology of cerebrospinal fluid (CSF), pathophysiology, risk factors, diagnosis, and treatment options. PDPH is a common complication of dural puncture, characterized by a postural headache due to CSF leakage. The understanding of CSF and dural anatomy has evolved over time, leading to advancements in diagnosing and managing PDPH. Treatment options range from conservative measures to epidural blood patch, intrathecal catheter, and regional techniques like sphenopalatine

Combined Occipital Nerve and Sphenopalatine Ganglion Neuromodulation for Refractory Craniofacial Pain with Trigeminal Autonomic Features: A 12-Year Follow-Up (Case Report).

Craniofacial complex regional pain syndrome (CRPS) is a rare, diagnostically challenging condition characterized by sympathetically maintained pain, allodynia, and autonomic features in the craniofacial region. Its overlap with trigeminal autonomic cephalalgias (TACs) creates significant diagnostic complexity. We report the case of a 41-year-old male with a 12-year history of severe right-sided hemicranial and facial pain with prominent autonomic features (facial edema, ptosis, lacrimation, allodynia) following plastic surgery in occipital area, in a patient with a prior history of CRPS. Atypical facial pain after cranioplasty: A too perfect design?: Dolor facial atípico asociado a craneoplastia: ¿un encaje demasiado perfecto?

Cranioplasty is a procedure routinely performed in neurosurgery. It is associated with significant morbidity and several types of postsurgical complications. The most common are infections, bone flap resorption and hematomas. Atypical facial pain has not been documented yet as a potential postoperative complication. We present a case of atypical facial pain reported at inmediate postoperative period after cranioplasty. The pain was refractory to medical treatment and sphenopalatine ganglion block. Eventually, the pain totally disappeared after surgical revision of the cranial implant.

Postoperative effects of bilateral sphenopalatine ganglion blockade in septorhinoplasty operations; double-blind randomized clinical trial.

OBJECTIVE: We aimed to investigate the effect of bilateral sphenopalatine ganglion blockade (SPGB) on the main postoperative complications in septorhinoplasty operations. METHODS: In this randomized, controlled, prospective study, 80 cases planned for Septorhinoplasty operations under general anesthesia were included in the study. The cases were divided into two groups; SPGB was performed with 2 mL of 0.25% bupivacaine bilaterally 15 min before the end of the operation in the SPGB group (Group S, n = 40). In the control group (Group C, n = 40), 2 mL of 0.9% NaCl solution was applied into both

Postdural puncture headache in the obstetric population: a new approach?

BACKGROUND AND OBJECTIVES: The gold standard for the treatment of postdural puncture headache (PDPH) is the epidural blood patch (EBP). Regional techniques-sphenopalatine ganglion block (SPGB), greater occipital nerve block (GONB) and trigger point infiltration (TPI)-can also be used for the treatment of PDPH. Our objective was to evaluate the efficacy of these peripheral nerve blocks (PNBs) in the treatment of PDPH. METHODS: A retrospective study was conducted including all patients with PDPH in the obstetrics department of our institution between April 2016 and December 2017. Data were retr

Transfacial transpterygomaxillary access to foramen rotundum, sphenopalatine ganglion, and the maxillary nerve in the management of atypical facial pain.

Post-traumatic atypical facial pain syndromes are refractory to medical therapy and thus challenging to treat. Some of these patients have a facial causalgia syndrome that may include autonomic as well as trigeminal fibers as the anatomic mediators. A procedure that may be of both diagnostic and therapeutic benefit is a nerve block in the region of the foramen rotundum. This allows access to both the maxillary nerve and the sphenopalatine ganglion. A simple technique developed to perform this procedure is described, and the results in a series of six patients are presented.

Transnasal spread of bupivacaine into the pterygopalatine fossa following endoscopically assisted cotton swab placement: a cadaveric study.

BACKGROUND: There are conflicting data on the efficacy of transnasal topical anesthetic approaches intended to achieve a pterygopalatine ganglion block, specifically regarding the extent to which local anesthetics reach the pterygopalatine fossa. This cadaveric study aims to determine whether bupivacaine can reach the pterygopalatine fossa following topical administration near the sphenopalatine foramen using endoscopically assisted cotton ball placement. METHODS: Nine fresh cadavers underwent topical nasal administration of a solution containing bupivacaine, methylene blue, and iodine contra

Local anesthetics as a therapeutic tool for post COVID-19 patients: A case report.

BACKGROUND: Post COVID-19 syndrome (PC-19S) appears to be independent of acute illness severity and humoral response. The involvement of the autonomic nervous system (ANS), expressed by dysautonomia, has been hypothesized as a contributor. Several studies have associated the therapeutic effects of local anesthetics (LA) to their action on the ANS. To the best of our knowledge, this is the first report of therapeutic injections with LA linked to clinical improvement in a patient with PC-19S. PATIENT CONCERNS AND DIAGNOSES: This was a 54-year-old-man with postCOVID-19 symptoms lasting 14 weeks,

Ganglion Cyst of the Temporomandibular Joint Mimicking Auriculotemporal Neuralgia.

Ganglion cysts within the temporomandibular joint (TMJ), although uncommon, typically present with swelling, pain, trismus, and difficulty with mastication. The authors report an unusual case of a ganglion cyst in the TMJ of a 52-year-old man who presented with chief complaints of severe headaches and dizziness that had not subsided following treatment with medication, trigger point injections, or sphenopalatine ganglion blocks. The cyst appeared as a nonenhancing, T2 hyperdensity adjacent to the left TMJ condyle on magnetic resonance imaging, supported by the presence of chronic erosion and r

Rhinorrhea is decreased in dogs after nasal application of botulinum toxin.

At this time no effective long-term therapy exists for the excessive secretion of vasomotor rhinitis. Because rhinorrhea is under parasympathetic control, it was theorized that botulinum toxin--a powerful and long-acting cholinergic blocker that has been successful in the treatment of dystonia--might be useful in blocking the cholinergic control of rhinorrhea. Four male mongrel dogs were studied. Fifty units of type A botulinum toxin was soaked into sterile gauze, which was then packed into the left nasal cavity of each dog for 1 hour. Saline-soaked gauze was similarly introduced into the righ

A case report: Infrazygomatic approach neurocryoablation of the sphenopalatine ganglion under cone beam computer tomography guidance in treating refractory headaches.

INTRODUCTION: The sphenopalatine ganglion (SPG) drives the cranial autonomic symptoms associated with trigeminal autonomic cephalalgias, migraine, and trigeminal neuralgia. Infrazygomatic approach SPG block serves as the gold standard for both prognostic and therapeutic intervention for management of refractory headache with autonomic symptoms. However, this intervention often provides only short-term relief. Identification of novel interventions that provide more durable relief is imperative. CASE: An 84-year-old female with chronic headache featuring autonomic cranial symptoms that failed b
