Regeneration and remyelination promoting effects of spinal cord stimulation following spinal cord injury: A scoping review.

INTRODUCTION: Spinal cord injury (SCI) disrupts crucial neural pathways and causes severe neurological deficits that impact the quality of life. Although preliminary research has shown some promise of Spinal Cord Stimulation (SCS) in promoting axonal regeneration and remyelination, a clear understanding of the pertinent literature remains lacking. This scoping review consolidates evidence on the effects of SCS in promoting axonal growth and myelin development, preserving spared tissue integrity, and facilitating functional recovery following SCI. METHODS: Pre-clinical studies from 1946 to 202

Transcutaneous spinal cord stimulation effects on spasticity in patients with spinal cord injury: A systematic review.

CONTEXT: Spasticity is one of the most prevalent impairments following spinal cord injury (SCI). It can lead to a decrease in the patient's functional level. Transcutaneous spinal cord stimulation (tSCS) has demonstrated motor function improvements following SCI. No systematic reviews were published examining the influences of tSCS on spasticity post-SCI. OBJECTIVES: This review aimed to investigate the effects of tSCS on spasticity in patients with SCI. METHODS: PubMed, SCOPUS, PEDro, CINAHL, MEDLINE, REHABDATA, AMED, and Web of Science databases were searched until June 2021. The Physiothe

Epidural spinal cord stimulation as an intervention for motor recovery after motor complete spinal cord injury.

Spinal cord injury (SCI) commonly results in permanent loss of motor, sensory, and autonomic function. Recent clinical studies have shown that epidural spinal cord stimulation may provide a beneficial adjunct for restoring lower extremity and other neurological functions. Herein, we review the recent clinical advances of lumbosacral epidural stimulation for restoration of sensorimotor function in individuals with motor complete SCI and we discuss the putative neural pathways involved in this promising neurorehabilitative approach. We focus on three main sections: review recent clinical results

Spinal cord stimulation and intrathecal baclofen therapy for patients with severe spasticity after spinal cord injury.

RATIONALE: Spasticity is one of the main complications after the spinal cord injury (SCI). Most commonly, severe cases of spasticity are treated surgically with intrathecal baclofen therapy (ITB). Spinal cord stimulation for chronic pains (SCS) serves as an alternative for ITB. Both methods have their benefits and limitations. This study is aimed at development of a personalized SCS and ITB treatment algorithm for patients with severe cases of spasticity after SCI. MATERIALS AND METHODS: The paper analyzes the treatment results of 66 patients with severe spasticity after SCI (50 men and 16 wo

Electrical spinal cord stimulation must preserve proprioception to enable locomotion in humans with spinal cord injury.

Epidural electrical stimulation (EES) of the spinal cord restores locomotion in animal models of spinal cord injury but is less effective in humans. Here we hypothesized that this interspecies discrepancy is due to interference between EES and proprioceptive information in humans. Computational simulations and preclinical and clinical experiments reveal that EES blocks a significant amount of proprioceptive input in humans, but not in rats. This transient deafferentation prevents modulation of reciprocal inhibitory networks involved in locomotion and reduces or abolishes the conscious percepti

The use of spinal cord stimulation in pain management.

SUMMARY Pain is a complex behavior process, the anatomy and physiology of which is not completely understood, and is subject to continuous exploration and research. Following on the heels of Melzack and Wall's gate control theory of pain (1965), Shealey et al., in 1967, were the first to implant stimulation electrodes over the dorsal columns in an attempt to provide relief for patients with chronic, intractable pain. Since then, significant strides in both the technological and therapeutic sides have facilitated the evolution of spinal cord stimulation (SCS) in the management of a variety of p

Spinal cord stimulation as a tool for physiological research.

The use of spinal cord stimulation for alleviation of disabilities due to motor neuron lesions has provided the opportunity to explore a new approach to measurement of spinal cord physiology. Externalized leads of epidural electrodes provide the possibility of recording evoked spinal cord activity, while both externalized or implanted leads can be used to study cortical evoked responses and twitches induced by spinal cord stimulation. The use of such electrophysiological techniques can be expected to expand greatly the applicability of the technique for alleviating motor disabilities, through

Computational modeling of interferential stimulation of the spinal cord.

Objective.Interferential stimulation uses multiple independent groups of electrodes to apply high-frequency currents with a small-frequency offset. At a specific region(s) in space, superposition of the high-frequency currents creates low-frequency amplitude modulation that can drive neural activation. Therefore, with interferential spinal cord stimulation (IF-SCS), it may be possible to focus stimulation on target areas while avoiding stimulation of non-target areas that could produce unwanted side effects. In this study, we used a comprehensive computational modeling approach to evaluate the

A Comprehensive Review of Spinal Cord Stimulator Infections.

Spinal cord stimulator (SCS) is approved to treat various pain conditions and is commonly seen in the chronic pain patient population. Due to the nature of the device and its location, infections associated with SCS have a particularly high morbidity. According to post-market data and medical device reports, 87% of patients receiving SCS implants were given perioperative antibiotics as the implantable neurostimulator or receiver pocket serve as the most common sites of infection. The most common antibiotics for surgical prophylaxis given are first-generation cephalosporins (cefalexin, cefazoli

Technique for steering spinal cord stimulator electrode.

BACKGROUND: Spinal cord stimulation is an established technology for management of chronic low back and leg pain when other surgical options have failed or are not feasible. Precise placement of the paddle-style electrode relative to the patient's distribution of pain can be difficult because of anatomic variation and the inherent limitations of electrode design. OBJECTIVE: To describe a surgical technique for adjustment of spinal cord stimulator epidural electrode location during placement or revision to optimize stimulation coverage. METHODS: We devised a method using a malleable disposabl

The efficacy of spinal cord stimulation for chronic pain.

A prospective study was undertaken to evaluate the efficacy of spinal cord stimulation (SCS) in the management of chronic pain syndrome. The study included all patients who underwent this procedure at the Royal Melbourne Hospital and the Melbourne Private Hospital over a period of two years. A total of 29 patients were managed by the end of June 1996. These patients were carefully screened by a neurosurgeon (JVR) and a psychiatrist. Of these, 26 patients had a follow up evaluation at the end of August 1996. From the group of 29 patients, four patients failed to obtain any relief during the tri

Spinal cord stimulation in the treatment of spasmodic torticollis.

This report presents our observations in 63 patients undergoing chronic spinal cord stimulation for treatment of spasmodic torticollis. In this series there were 23 patients (36.5%) who demonstrated marked improvement, characterized by no evidence of torticollis, full range of motility of the head and neck and no pain. Moderate improvement was found in 20 patients (31.8%) who showed minimal residual torticollis, but had full motility and no pain. There were 5 patients (7.9%) considered mildly improved who demonstrated decrease in their torticollic position, spasms and pain, but retained some e

Transcutaneous Spinal Stimulation Modulates Spinal Reflex Circuit Excitability in Persons with Spinal Cord Injury.

Background: Transcutaneous spinal stimulation (TSS) is a noninvasive stimulation approach to modulate spinal reflex circuit excitability after spinal cord injury (SCI) Posterior root muscle (PRM) reflexes can be used to characterize the change in excitability of spinal reflex circuits after TSS; these responses are likely influenced by stimulus parameters. Methods: We compared PRM reflex responses to 3 TSS conditions: single-site continuous (SS-CONT), single-site burst (SS-BURST), and dual-site continuous (DS-CONT). Stimulation (frequency: 50 Hz, intensity: 80% soleus reflex threshold[RT]) was

Thoracic spinal subdural hematoma after spinal cord stimulation trial: a case report.

BACKGROUND: Spinal subdural hematoma (SSDH) is a rare but potentially serious condition characterized by a compressive accumulation of blood within the thecal sac. It may result from trauma, surgery, or underlying vascular abnormalities, often associated with anticoagulant use. In this case, SSDH was diagnosed post-implantation of a spinal cord stimulation (SCS). Current estimates suggest that a neuraxial (including epidural) hematoma following SCS occurs in about 0.32% of cases, with no reported sub/intradural pathology from uncomplicated SCS procedures. CASE DESCRIPTION: We report a case of
