Complications in Patients Undergoing Spinal Fusion After THA.

INTRODUCTION: Patients with lumbar spine and hip disorders may, during the course of their treatment, undergo spinal fusion and THA. There is disagreement among prior studies regarding whether patients who undergo THA and spinal fusion are at increased risk of THA dislocation and other hip-related complications. QUESTIONS / PURPOSES: Is short or long spinal fusion associated with an increased rate of postoperative complications in patients who underwent a prior THA? PATIENTS AND METHODS: A retrospective study of New York State's Department of Health database (SPARCS) was performed. SPARCS ha

Recidivism Rates After Smoking Cessation Before Spinal Fusion.

Tobacco use has documented negative effects on perioperative complications and clinical outcomes. Smoking cessation before spinal surgery may improve clinical outcomes. The goal of this study was to determine the recidivism rate after smoking cessation before spinal fusion. A prospective observational study was performed at the University of Kansas Medical Center between 2006 and 2011. All patients with serum-confirmed nicotine cessation before spinal fusion surgery were eligible. Smoking status was determined with questionnaires at 3 months, 6 months, and 1 year postoperatively. All reported

Paraspinal muscle vasculature contributes to posterolateral spinal fusion.

STUDY DESIGN: Study of posterolateral fusions in a rabbit model. OBJECTIVES: To characterize the contribution of paraspinal musculature to the healing of posterolateral spinal fusions in a rabbit model. SUMMARY OF BACKGROUND DATA: Previous studies have demonstrated that successful spinal arthrodesis requires vascular ingrowth from adjacent decorticated bone. In other areas of the body, such as the tibia, vascular ingrowth from the surrounding musculature has also been shown to be important. The role of the surrounding paraspinal musculature in spinal fusions has yet to be assessed. METHODS:

Assessing observational studies of spinal fusion and chemonucleolysis.

STUDY DESIGN: Literature review and survey of spine surgeons. OBJECTIVES: To identify reasons for variation in results among observational studies of spinal surgery. SUMMARY AND BACKGROUND DATA: Orthopedic treatments are often evaluated by observational studies rather than randomized controlled trials. The value of observational studies is debated. METHODS: A literature search was performed to find several observational studies that compared the same spinal surgeries. Possible confounders for these studies were identified by a survey of spinal surgeons. Study characteristics from these arti

Can autologous bone culture predict spinal fusion capacity?

The capacity of the individual patient to initiate osteoblast proliferation as a predictor for successful lumbar spinal fusion has not yet been reported. The objectives of this study were, first, to analyze the relationship between in vitro osteoblast proliferation and clinical bony fusion in the individual patient in order to predict the fusion outcome and, second, to measure the effect of preoperative tobacco smoking on osteoblast proliferation. Sixty-one patients (mean age 46 years) underwent posterolateral lumbar fusion in the period 1994-1995. Thirty-eight patients received CD pedicle scr

The advantages of intrathecal opioids for spinal fusion in children.

Two groups of 40 homogeneous patients (ASA physical status (1-2)) with idiopathic scoliosis undergoing spinal fusion with CD instrumentation were studied prospectively. Group A (intrathecal) received a mixture of morphine and sufentanil administered intrathecally at the level of L3-L4 after the induction of anaesthesia. Group B (control) had inhalation and intravenous narcotic anaesthesia. The use of intrathecal opioids resulted in a significant reduction of blood pressure without the use of any hypotensive agents and produced prolonged postoperative analgesia. There was no adverse effect on s

The Fate of the Neuromuscular Hip After Spinal Fusion.

BACKGROUND: The goals of treatment of neuromuscular scoliosis are to achieve a balanced spine and level pelvis with most constructs including pelvic fixation. However, the pelvis can become a stiff "end vertebra" that prevents compensatory mechanisms to adjust to hip deformities in this patient population. The purpose of this study is to determine the frequency of hip pathology and surgery after spinal fusion in this patient population. METHODS: We performed a retrospective chart and radiographic review of cerebral palsy patients who underwent posterior spinal fusion (PSF) at our institution

A rethink of fusion surgery for lumbar spinal stenosis.

Lumbar spinal stenosis is one of the most common spinal disorders in elderly patients, how to choose decompression alone or decompression plus fusion is still controversial. It was reported that the rate of decompression alone for lumbar stenosis was decreased, whereas the rate of decompression plus fusion was increased recently. Two recent multicenter RCTs papers published in the same issue of N Engl J Med compare the outcomes of decompression alone or decompression plus fusion for lumbar spinal stenosis currently. We combined the results from these two studies using the Stata software, we fo
