An autopsy case of meningeal carcinomatosis with parenchymal invasion through the cranial and spinal nerve roots.

Meningeal carcinomatosis is a well-known complication of malignant neoplasms. We report a case of meningeal carcinomatosis of 2 months' duration in a 22-year-old man, in whom the initial symptom was gradually worsening headache. Postmortem examination revealed infiltrating adenocarcinoma of the stomach. Carcinoma cells showed diffuse spread to the subarachnoid space of the brain and spinal cord. In many places, subarachnoid tumor cells had infiltrated to the cranial and spinal nerves. Moreover, carcinoma cells in the nerve roots extended to the parenchyma of the brain and spinal cord beyond th

Minimally invasive lateral approaches for the treatment of spinal tumors: single-position surgery without the "flip".

Although primary tumors of the spine and neural elements are rare, metastatic disease to the spine is quite common. Traditionally, surgical treatment for spinal tumor patients involves open decompression with or without stabilization. The single-position minimally invasive (MIS) lateral approach, which has been recently described over the recent decade, allows simultaneous access to the anterior and posterior columns with the patient positioned in the lateral decubitus position. Herein, we review the application of single-position MIS lateral surgery for the treatment of spinal neoplasm. The a

Neurofibromatosis type 2--the importance of serial imaging of the brain and spinal cord.

Meningiomas and schwannomas are relatively common central nervous system neoplasms, but patients harbouring multiple meningiomas or schwannomas are rare. We present the case of a 27 year-old patient with Neurofibromatosis type 2 with multiple associated intracranial meningiomas and spinal cord neurofibromas at various levels. Patients with Neurofibromatosis type 2 should be followed up for life with serial magnetic resonance imaging of the brain and spinal cord to detect new and recurrent tumours at these sites.

Brown-Sequard syndrome associated with Horner syndrome following cervical disc herniation.

INTRODUCTION: Brown-Sequard syndrome (BSS) has been reported in patients with various spinal pathologies, including spinal traumatic injuries, spinal cord neoplasms, epidural hematomas and spinal cord ischemia. Pure BSS caused by cervical disc herniation is very rare. CASE PRESENTATION: We report a rare case of cervical disc herniation presenting as BSS associated with Horner syndrome (HS), which has not been reported up to now. A prompt diagnosis by magnetic resonance imaging (MRI), followed by spinal cord decompression was performed. A postoperative rapid improvement of the neurological def

Paraganglioma of the cauda equina: a case report.

This case report describes a paraganglioma of the cauda equina in a 63 year old woman. Very few examples of paraganglioma have been recorded in this site. Histologically these neoplasms may have considerable similarity with ependymoma, the most common neoplasm of the lower spinal cord, and the diagnosis can be easily missed unless special techniques are employed. The clinical and pathological data obtained from this and the other reported examples suggests that paragangliomas of the cauda equina are benign, slowly growing neoplasms. In contrast to ependymomas and to paragangliomas elsewhere, t

Spinal infections: clinical and imaging features.

Spinal infections represent a group of rare conditions affecting vertebral bodies, intervertebral discs, paraspinal soft tissues, epidural space, meninges, and spinal cord. The causal factors, clinical presentations, and imaging features are a challenge because the difficulty to differentiate them from other conditions, such as degenerative and inflammatory disorders and spinal neoplasm. They require early recognition because delay diagnosis, imaging, and intervention may have devastating consequences especially in children and the elderly. This article reviews the most common spinal infection

Imaging diagnosis--spinal cord histiocytic sarcoma in a dog.

A 12-year-old mixed breed dog was presented for evaluation of progressive paraparesis and ataxia. Magnetic resonance (MR) imaging was performed and identified multifocal intradural spinal cord mass lesions. The lesions were hyperintense in T2-weighted sequences, isointense to mildly hyperintense in T1-weighted sequences with strong contrast enhancement of the intradural lesions and spinal cord meninges. Spinal cord neoplasia was suspected. A diagnosis of intramedullary spinal cord histiocytic sarcoma, confined to the central nervous system, was confirmed histopathologically. Spinal cord histio

A molecular array for 10-second diagnosis of common spinal tumor types with picosecond infrared laser mass spectrometry.

BACKGROUND: Improving the surgical outcomes for commonly occurring spinal neoplasms of extradural and intradural extramedullary origins requires precise intraoperative diagnosis provided by highly trained neuropathologists. METHODS: Through a retrospective study of n = 319 patient specimens, verified where appropriate by learning curve analysis to be sufficient for statistically significant observations, we aimed to assess the utility of 10-second picosecond infrared laser mass spectrometry (MS; PIRL-MS) for non-subjective diagnosis of major spinal tumor types of metastatic carcinoma, schwann

[Intraspinal extradural tumors].

CLINICAL ISSUE: Intraspinal extradural lesions represent the most frequent group of spinal tumors and comprise a broad spectrum of primary and secondary neoplasms as well as intradural extramedullary tumors with secondary extradural extension. Because even small epidural lesions can cause significant neurological deficits through compression of the spinal cord or cauda equina, a prompt and accurate diagnosis is essential. RADIOLOGICAL STANDARD PROCEDURES: Contrast-enhanced magnetic resonance imaging (MRI) is the imaging modality of first choice as it enables a reliable assessment of the tumor

Pediatric intramedullary spinal cord tumors: special considerations.

Intramedullary spinal cord tumors (IMSCTs) of the pediatric population are rare and comprise thirty-five percent of intraspinal neoplasms. Low-grade astrocytomas predominate; ependymomas increase in frequency with ascending age and become the most frequent IMSCT in adults. Gangliogliomas are very rare in adults but comprise nearly thirty percent of tumors in children under three years of age. The cervical spine is the region of the spine most affected. Pain is the most common presenting symptom with weakness, gait deterioration, torticollis also frequently reported. Hydrocephalus occurs with g

Intralesional surgery of primary tumors of the anterior cervical column.

The study involves a series of 10 primary neoplasms of the anterior cervical column (body and transverse processes) surgically treated by intralesional excision and followed-up after 2-10 years. The series included 6 benign and 4 malignant neoplasms. Among the benign tumors 4 cases were classified Enneking stage II (2 osteoid osteomas, 1 fibrous dysplasia, 1 osteoma): after simple excision none of the cases revealed symptoms or signs of recurrence at long-term follow-up. The other 2 benign forms were stage III (giant cell tumor and aneurysmal cyst): both of the cases were treated by extracapsu

Ganglioneuroblastoma of the cauda equina.

A 39-year-old lady presented with low back pain and neurogenic claudication. Magnetic resonance imagining revealed an intradural neoplasm in the cauda equina region. The patient underwent lumbar laminectomy and total excision of the neoplasm. Biopsy showed it to be a ganglioneuroblastoma, which is rare in the spinal canal and so far does not appear to have been reported in the region of the cauda equina. Its management is discussed.

Imaging of Neoplastic Disorders of the Spinal Cord: Part 2.

This article addresses uncommon primary spinal cord tumors, neoplasms of the conus medullaris, filum terminale, cauda equina region, and secondary neoplastic involvement of the spinal cord and CSF spaces. Although many primary intramedullary tumors discussed are individually rare, secondary neoplastic disease, including intramedullary metastases and leptomeningeal dissemination is encountered more frequently in clinical practice and represents a major cause of rapidly progressive neurological symptoms. Imaging appearances across these entities often overlap with common primary tumors and non-n

[Primitive neuroectodermal tumor of the cauda equina. Report of a case].

Primitive neuroectodermal tumors are malignant neoplasms whose classification has been controversial. Spinal primitive neuroectodermal tumors are rare and at the cauda equina are even less common. We report the case of a 40 year-old man with a history of progressive back pain and gait difficulty. After the diagnosis of a primitive neuroectodermal tumor of the cauda equina, the patient presented seeding to the intracranial subarachnoid space followed by a poor outcome. We review the literature of primary spinal primitive neuroectodermal tumors.

Computed tomographic evaluation of the retroperitoneum in infants and children.

With many retroperitoneal diseases in infancy and childhood, computed tomography will give information unavailable by any other single imaging technique. This maxim applies particularly to the adrenal, where the percolating nature of neuroblastoma with its propensity toward spinal involvement is exquisitely shown with computed tomography. Renal neoplasms and trauma can be illustrated effectively with computed tomography, as can pancreatic disease and spinal and paraspinal inflammatory and neoplastic disease. In addition to diagnostic studies, computed tomography may be used as guidance for inv

Disc protrusion in the child. Particular features and comparison with neoplasms.

Lumbar intervertebral disc herniation, although common in adults, is infrequent in the young, and especially in patients under 17 years old. In this work we review clinical data pertaining to two pediatric groups of patients whose main complaint was low back pain and/or sciatica, trying to identify factors that might contribute to their earlier referral and to the differential diagnosis of protruded disc and spinal neoplasm in this population. Group A comprises 17 youngsters diagnosed as having lumbar herniated nucleus pulposus and group B, 16 children with neoplasms of the lower thoracic and

[Extradural angiolipoma of the thoracic spine: a case report and literature review].

BACKGROUND: Spinal angiolipomas (SA) are rare primary tumors accounting for 0.14-1.2% of all spinal cord and spine tumors and approximately 3% of extradural spinal neoplasms (<0.1% of all primary CNS tumors). Histologically, SA consists of mature adipocytes and abnormally developed vessels. Clinical presentation of this tumor is due to severe spinal cord compression. MATERIAL AND METHODS: The authors present a 33-year-old patient with severe spinal cord compression who underwent resection of Th6-Th8 spinal angiolipoma. Spinal intradural malignant peripheral nerve sheath tumor at the foramen magnum in a non-neurofibromatosis child.

Primary spinal intradural malignant peripheral nerve sheath tumors (MPNSTs) are rare neoplasms, especially in children with a non-neurofibromatosis background. Scarce pediatric data exist with regard to such tumors. A 4-year-old child with a history of spastic limb weakness was operated for a foramen magnum spinal lesion (intradural and extradural) with imaging suggestive of schwannoma; the histopathology, however, was that of a MPNST. He underwent redo-surgery for recurrent infiltrating lesion, became ventilator dependent, and expired. Along with this case, we briefly discuss the relevant lit

Lateral retrovascular approach to the upper cervical spine.

The authors describe the surgical anatomy of lateral approach to the upper cervical spine. The approach is retrovascular retropharyngeal extraoral allowing for anterolateral exposure of the upper cervical column, and it may be enlarged posteriorly on the homolateral posterior hemiarch of C1 and C2, and/or inferiorly on the lower cervical column. Absolute indications to this approach are anterolateral fusion with transarticular screws C1-C2 and excision of skeletal neoplasms adjacent to the vertebral artery. For other indications such as the treatment of anterior neoplasms and spinal cord decom
