Cervical osteophytes and respiratory failure. An unusual case of upper airway obstruction.

An unusual case is presented in which large cervical osteophytes caused upper airway obstruction. The presenting features of acute-on-chronic respiratory failure and cor pulmonale were alleviated by permanent tracheostomy.

Tract pain because of cervical spondylotic myelopathy: A case series.

Spinal cord compression may lead to pain that is sometimes directed to areas far below the compression level. In certain cases, it may present as sciatica pain, knee pain or low back pain (LBP). These types of pain are called tract pain or funicular pain. Tract pain because of cervical spondylotic myelopathy (CSM) may lead to delays in the diagnosis and treatment of CSM in some cases, and sometimes unnecessary medical and surgical treatments. This study evaluated the results of four patients who presented to the outpatient clinic with complaints of LBP accompanying CSM findings. This study aim

Rectilinear bone scanning: differentiation between metastases and degenerative spinal disease.

Criteria have been developed for radioisotope scanning of bone with an eight-colour-display rectilinear scanner, to differentiate metastases from degenerative spinal disease. If the area of increased uptake is two or more colours "hotter" than normal, this is probably due to metastases even if degenerative spinal disease coexists. The positivity of a scan due to degenerative spinal disease is a function of the degree of sclerosis adjacent to the intervertebral discs and apophyseal joints; it is not related to the degree of osteophyte formation.

The position and shape of osteophyte formations at canine vertebral endplates and its influence on radiographic diagnosis.

The relationship between the risk of missing osteophytes in lateral radiographs and their position was determined in a sample of German shepherd dogs (GSDs, n = 42) and dachshunds (DHs, n = 36). Only 10% of osteophytes which were not detected on radiographs were located in the ventral median area (position 1) and classified as type 1 (small spur with narrow base). The highest risk for being affected next to the intervertebral foramen and/or the dorsal area of the vertebral endplate (position 3) was noticed in the lumbar segment (95% CI: 5.2-9.8% in DHs; 11.9-17.7% in GSDs). The minimum risk of

The development of dysphagia and dysphonia due to anterior cervical osteophytes.

Cervical osteophytes may cause dysphagia by compressing the esophagus and may cause dysphonia by compressing the larynx and inferior laryngeal nerve. The occurrence of dysphagia and dysphonia due to cervical osteophytes has rarely been reported in literature. In this article, a case, in which the multiple cervical osteophytes were found to be the cause of dysphagia and dysphonia, was evaluated by imaging methods and electrophysiological evaluation of swallowing and the case was discussed in the light of relevant literatures.

[Dysphagia and cervical bony spurs].

A 74-year-old man had acute complete dysphagia due to esophageal compression cause by a degenerative osteophye on C4-C5. Functional improvement was immediate after surgical removal of the osteophyte via an anterior cervical approach. Reports in the literature show that vertebral hypertrophic spurs causing dysphagia result from bony degeneration or idiopathic causes (diffuse idiopathic skeletal hyperosteosis: DISH). Surgery is mandatory if medical care fails and dysphagia is complete. Discetomy-graft is indicated for degenerative lesions and simple removal of the bony spur for ossification of t

Hyperostosis of lumbar spinous process: a radiological feature in a young acromegalic.

Hyperostosis of the spinous process may be associated with diffuse idiopathic skeletal hyperostosis, Baastrup's disease or acromegaly. Radiological differential diagnosis between them are discussed. No case of hyperostosis of the spinous process has been described clinically or radiologically in young acromegalics. This case represents another manifestation of spinal involvement of acromegaly. Coincidentally, the enthesopathy could also be seen clearly in the middle and proximal phalanges in this young patient. Physicians treating spinal diseases should be aware of chronic backache due to the

Cervical myelopathy caused by C3-C4 spondylosis in elderly patients: a radiographic analysis of pathogenesis.

STUDY DESIGN: A radiographic analysis of elderly patients with cervical spondylotic myelopathy, particularly those with involvement of the C3-C4 level. OBJECTIVES: To elucidate the potential contributors to the higher incidence of pathology at C3-C4 in the elderly. SUMMARY OF BACKGROUND DATA: In this study, the elder patients showed a greater predilection for involvement of the C3-C4 lesion compared with their younger counterparts. No previous study has addressed C3-C4 pathology in elderly patients. METHODS: This study included 18 patients, 10 men and 8 women, with cervical spondylotic myel

[Surgical possibilities in the alleviation of degenerative vertebral instability].

The indications for operative treatment of degenerative lumbar instability are spondylolisthesis, spinal stenosis and degenerative disc disease. It is important that the indication is based upon a thorough clinical and radiological assessment of the patient. Psychometric tests should be included in the preoperative evaluation. The surgical technique consists for most of the cases of a dorso-lateral fusion. To increase the fusion rate, a pedicular segmental instrumentation system is used. The clinical results for the treatment of degenerative lumbar instability are in more than 80% acceptable.

Sex differences in spinal osteoarthritis in humans and rhesus monkeys (Macaca mulatta).

STUDY DESIGN: Retrospective study of male and female spinal osteoarthritis, characterized by lateral spine thoracolumbar radiographs, in humans and nonhuman primates. OBJECTIVE: To characterize differences in prevalence and vertebral distribution of spinal osteoarthritis between men and women, between male and female macaques, and between the 2 phylogenetically related genera. SUMMARY OF BACKGROUND DATA: Naturally occurring spinal osteoarthritis manifests similarly in humans and rhesus macaques. Other types of osteoarthritis particularly of the knee and hip have revealed sex differences in h

Pseudochoreoathetosis secondary to progressive spondylotic cervical myelopathy.

Pseudochoreoathetosis is a rare movement disorder associated with loss of proprioception. Culprit lesions may occur at any point between the cerebral cortex and the peripheral nerve. Seldom is the underlying cause reversible or prone to improvement. An elderly man presented to our tertiary centre with choreoathetoid movements secondary to spondylotic subaxial cervical myelopathy. His myelopathy fulminated and he was emergently treated with posterior decompressive neurosurgery. Unexpectedly, his choreoathetoid movements improved significantly post-operatively. There are a multitude of reports o

Anatomy and clinical significance of the uncinate process and uncovertebral joint: A comprehensive review.

INTRODUCTION: The uncinate process and its associated uncovertebral articulation are features unique to the cervical spine. This review examines the morphology of these unique structures with particular emphasis on the regional anatomy, development and clinical significance. MATERIALS AND METHODS: Five electronic databases were utilized in the literature search and additional relevant citations were retrieved from the references. A total of 74 citations were included for review. RESULTS: This literature review found that the uncinate processes and uncovertebral articulations are rudimentary

Degenerative cervical spondylosis: clinical syndromes, pathogenesis, and management.

Degenerative changes in the cervical spinal column are ubiquitous in the adult population, but infrequently symptomatic. The evaluation of patients with symptoms is facilitated by classifying the resulting clinical syndromes into axial neck pain, cervical radiculopathy, cervical myelopathy, or a combination of these conditions. Although most patients with axial neck pain, cervical radiculopathy, or mild cervical myelopathy respond well to initial nonsurgical treatment, those who continue to have symptoms or patients with clinically evident myelopathy are candidates for surgical intervention.
