BACKGROUND: Spinal muscular atrophy is a progressive neurodegenerative disorder that can be treated with intrathecal antisense oligonucleotide therapy (nusinersen). However, administration is often complicated by posterior spinal fusion and neuromuscular scoliosis, necessitating a transforaminal approach.
PURPOSE: To assess the safety profile of the transforaminal approach for intrathecal access.
DATA SOURCES: Searches of the PubMed, Web of Science, and SCOPUS databases.
STUDY SELECTION: Thirteen articles were selected based on inclusion of transforaminal access and appropriate clinical inf

Post-dural puncture headache is one of the most undesirable complications of spinal anaesthesia. Previous pairwise meta-analyses have either compared groups of needles or ranked individual needles based on the pooled incidence of post-dural puncture headache. These analyses have suggested both the gauge and needle tip design as risk-factors, but failed to provide an unbiased comparison of individual needles. This network meta-analysis compared the odds of post-dural puncture headache with needles of varying gauge and tip design. We searched randomised controlled trials in medical databases. Th

OBJECTS: We report a case of epidural cerebrospinal fluid (CSF) leak after lumbar puncture caused by CSF dissecting into the spinal epidural space. The incidence of this phenomenon may be higher than suspected, although most cases may remain asymptomatic.
MATERIALS AND METHODS: A 4-year-old girl with new-onset seizure underwent a diagnostic lumbar puncture, the results of which were normal; 3 h later, she began experiencing severe low-pressure headaches and lower back pain, bilateral lower extremity weakness, numbness, and pain, and urinary retention when upright. Spinal MRI demonstrated exte

This study examined the incidence of failed spinal anesthesia and postdural puncture headache using a 27-gauge Whitacre and a 27-gauge Quincke needle in patients undergoing elective inpatient orthopedic procedures. The overall rate of failed spinal anesthesia was 8.5% [95% confidence interval (CI) = 4.6%-12.4%] (n = 17) in the Quincke group (n = 199) and 5.5% [95% CI = 2.3%-8.7%] (n = 11) in the Whitacre group (n = 199). This difference was not statistically significant. The overall incidence of postdural puncture headache (PDPH) was 0.8%; 1.1% [95% CI = 0%-2.4%] (n = 2) in the Quincke group a

BACKGROUND: Postdural puncture headache (PDPH) is a known potential complication of lumbar drain placement. Data regarding incidence of PDPH after lumbar drain placement varies, with a range of 0% to 47%. Furthermore, spinal drain placement has been associated with a significantly increased risk of debilitating neurologic complications.
CASE REPORT: In this article, we describe the case of a patient who developed PDPH, subdural hematoma, and abducens nerve palsy after lumbar drain. The patient was treated successfully with epidural blood patch. To our knowledge, this is the first case report

Lumbar puncture (LP) to extract cerebral spinal fluid (CSF) to quantify analytes relevant to brain health can lead to post-dural puncture headache (PDPH). Incidences vary across studies. This prospective memory clinic study assessed the incidence and intensity of PDPH after protocolised extraction and post-procedural rest. Patient and procedural risk factors for PDPH were explored. In 724 patients or healthy volunteers we performed 1147 procedures with intravenous infusion of 500 ml of 0.9% saline, LP with 22-gauge non-cutting pencil point needles, CSF collection (13.5-15 ml) without negative

BACKGROUND: Accidental dural puncture during epidural lead insertion for Spinal Cord Stimulation (SCS) is a recognized surgical complication that may lead to cerebrospinal fluid (CSF) leakage and subsequent postdural puncture headache (PDPH). The optimal technical approach to prevent CSF leakage remains controversial. This study aimed to evaluate a simple and efficient intraoperative technique for managing accidental dural puncture during SCS lead placement.
MATERIALS AND METHODS: A retrospective review was conducted of the medical records and imaging studies of all patients who underwent SCS

BACKGROUND: Puncture training with simulation models has emerged as a critical method for transmitting puncture skills, improving success rates, and minimizing injuries. Yet, obstacles such as proper material for ultrasound guidance, restricted options of 3D printing resources, and available substances to simulate human skin and muscle still hinder the production of simulation models that closely replicate clinical practice. This study aimed to develop a selective laser melting (SLM), 3D-printed simulation model that replicated the spine and skin contours of patients with spinal scoliosis.

In this prospective single-arm study of 50 pediatric patients with acute lymphoblastic leukemia (ALL), we evaluated the clinical performance of a novel bioimpedance spinal needle system in 152 intrathecal treatment lumbar punctures (LP) of these patients. The system detects in real-time when the needle tip reaches the cerebrospinal fluid (CSF) in the spinal canal. The success was defined as getting a CSF sample and/or administering the intrathecal treatment with one needle insertion. Incidence of traumatic LP (TLP) was defined as ≥ 10 erythrocytes/µL of CSF. Post-procedural complications were

BACKGROUND: Lumbar puncture is particularly challenging in patients with abnormal spinal anatomy, such as severe scoliosis or obesity. Conventional ultrasound‑assisted neuraxial techniques have two fundamental limitations: inaccurate projection of the probe's geometric center onto the skin when the probe is tilted and difficulty replicating the exact probe angle during needle insertion. These limitations often result in multiple needle passes, prolonged procedure time, and increased patient discomfort and complication risk. We developed an ultrasound‑assisted technique designed to standardize

OBJECTIVE: This study aimed to assess magnetic resonance imaging (MRI) findings of the lumbar spine in patients with chronic post-dural-puncture headache.
BACKGROUND: Post-dural-puncture headache is a common complication after dural puncture usually resolving within 14 days. However, a chronic course (>14 days persistence) is increasingly being recognized and may lead to tremendous symptom burden. While current research shows that MRI of the head is mostly unremarkable in chronic post-dural-puncture headache, lumbar imaging signs have not yet been systematically studied.
METHODS: This is a r

Spinal anesthesia using a pencil-point needle in older patients is a commonly performed procedure. Post-dural puncture headache (PDPH) is a complication that can happen following spinal anesthesia due to cerebrospinal fluid (CSF) leakage, but its incidence in elderly patients is rare. While PDPH presents with typical symptoms and resolves with conservative management, in older patients, it can happen along with intracranial subdural hematoma (SDH), a rare but potentially life-threatening complication due to the persistent CSF leak. We report a rare case of a 73-year-old woman who presented wit

BACKGROUND: Adhesive arachnoiditis has been described as a deteriorating neurological complication after neuraxial blockade; however, few pieces of literatures have reported minor cases that resemble peripheral neuropathy.
CASE PRESENTATION: A 29-year-old nulliparous woman underwent an emergent cesarean delivery under spinal anesthesia at the second and third lumbar interspace (L2/3) without any specific concerns. Subsequently, she developed left L5 and sacral first (S1) radiculopathy that persisted for 2 months. Although the neurological findings more likely indicated peripheral neuropathy,

BACKGROUND: Surgical time-out is essential for patient safety, requiring patients to remain conscious to verify their identity and surgical site. In China's high-volume operating rooms, this step is crucial to prevent errors. Spinal anesthesia often induces anxiety, making sedation necessary. An ideal sedative must provide effective anxiolysis while allowing rapid recovery. This study compares remimazolam and midazolam to meet these dual demands and assess their cardiorespiratory safety.
METHODS: This prospective, randomized controlled trial compared remimazolam and midazolam for sedation in

Post-dural puncture headache is becoming a major health problem around the globe. Even though it becomes the reason for increased maternal morbidity, there is still a great scarcity of research in Eastern Ethiopia. This study aimed to assess the prevalence and associated factors of post-dural puncture headache in patients undergoing cesarean sections following spinal anesthesia at Hiwot Fana Specialized University Hospital. An institution-based cross-sectional study was conducted among 291 patients from November 20/2024, to January 20/2025. An adjusted odds ratio presented the strength of asso
