Focal splenic disease demonstrated by ultrasound and computed tomography.

Twenty-five patients with focal splenic disease were analyzed retrospectively to determine the relative strengths and weaknesses of ultrasound and computed tomography (CT) in the detection and analysis of focal splenic lesions. Lesions were detected in all 25 patients by ultrasonography and in 22 of 25 patients by CT. Ultrasonography appears to be more sensitive in the detection of focal lymphoma within the spleen and may be slightly more sensitive in the detection of microabscesses. CT may offer more specific information regarding the nature of certain lesions, including the detection of cyst

Simple method of assessing splenic function in coeliac disease.

1. Erythrocytes with membrane abnormalities seen on interference contrast microscopy ('pitted erythrocytes') were counted in venous blood samples from patients with treated coeliac disease and dermatitis herpetiformis, and from control subjects and patients who had had an elective splenectomy. 2. The percentage of 'pitted erythrocytes' was compared with the 99mTc-labelled heat-damaged erythrocyte clearance, and with the splenic size computed from scintiscans. 3. There was a significant correlation between percentage 'pitted erythrocytes' and the above methods of measuring splenic function and

Rescue splenic artery embolization in an adult patient of sickle cell disease presented with acute splenic sequestration crisis.

BACKGROUND: Splenic sequestration crisis is a potentially fatal complication of sickle cell disease, mainly seen in young children. Only a few case series describe the acute splenic sequestration crisis in adults and its management, which primarily consists of supportive care and, in some cases, splenectomy. Splenic artery embolization has seldom been described in sickle cell disease. This is probably the first case in which an adult with sickle cell disease presented with an acute splenic sequestration crisis was managed successfully through splenic artery embolization. RESULTS: This 22-year

Splenic uptake of 99mTc-diphosphonate in sickle cell disease associated with increased splenic density on computerized transaxial tomography.

Splenic uptake of 99mTc-diphosphonate was found on the bone scan of a patient with sickle cell disease. No splenic activity was demonstrated on 99mTc-sulfur colloid liver-spleen scans and no evidence of splenic calcification was seen on plain abdominal radiographs. A computerized transaxial tomographic scan, however, did demonstrate a small spleen denser than other abdominal organs. The splenic accumulation of 99mTc-diphosphonate and the increased density on computerized transaxial tomography could be attributed to microscopic splenic calcifications and/or iron deposits in the spleen.

Comparative diagnostic performance of large language models and clinicians for splenic diseases in dogs and cats.

Splenic diseases in dogs and cats present significant diagnostic challenges, particularly in differentiating benign from malignant lesions using conventional clinical and imaging modalities. While histopathology remains the gold standard, artificial intelligence, especially large language models (LLMs), offers emerging diagnostic support capabilities. This two-center, retrospective diagnostic-accuracy study compared the performance of two state-of-the-art LLMs (ChatGPT-5 and Gemini 1.5 Pro) with experienced and novice veterinary clinicians in diagnosing splenic diseases in 38 dogs and cats tha

Splenic fine needle aspiration biopsy in the diagnosis of lymphoreticular diseases. A report of four cases.

Splenic fine needle aspiration biopsy (FNAB) is rarely performed in the United States for several reasons: there is a perception that there is an increased risk of bleeding, and rendering a diagnosis of lymphoma (the most common primary splenic neoplasm) by FNAB is controversial. During a three-year period, we saw 11 cases of splenic FNAB. We present four cases of lymphoreticular neoplasms occurring between 1988 and 1991 in which the spleen was the only or most accessible organ in which to establish a diagnosis by FNAB: one case of small cleaved cell lymphoma, two cases of immunoblastic lympho

An antibody reacting with splenic red pulp macrophages in the sera of patients with rheumatic diseases.

An antibody was detected in the sera of patients with certain rheumatic diseases that reacted with the cytoplasm of the splenic red pulp (SRP) cells of adult mice. This antibody was detected in the sera of all patients with mixed connective tissue disease (MCTD), 53% of patients with systemic lupus erythematosus (SLE), 42% with Sjögren's syndrome (SS), and 10% with rheumatoid arthritis (RA). However, this antibody was found neither in the sera from patients with other types of rheumatic diseases nor in healthy volunteers. The screening of this antibody may be useful in diagnoses of MCTD, SLE,

Gastro-splenic fistula, a rare presentation of sickle cell disease patient with splenic abscess: Case report and literature review.

INTRODUCTION: Hemoglobinopathies such as Thalassemia and Sickle Cell Disease are risk factors for splenic abscess formation. Gastro-splenic fistula is a rare complication of splenic abscess. In the literature, there are no reported cases of gastro-splenic fistula in patients with hemoglobinopathies. PRESENTATION OF CASE: We are presenting a rare case of a 15-year-old boy with undiagnosed sickle cell trait with splenic sequestration crisis as first presentation of his disease. This was complicated by splenic abscess formation. Gastro-Splenic fistula was not apparent in computed tomography and

Splenic histiocytic sarcoma: Disease progression from the perspective of pathophysiology.

Splenic histiocytic sarcoma (SHS) is a rare, aggressive hematological malignancy with unclear progression and management. Our case illustrates the progression and pathophysiological processes of SHS and provides key data for the diagnosis, treatment and management of SHS. A 60-year-old female with incidentally detected splenic mass (6.0 cm × 5.7 cm) underwent splenectomy, confirmed as SHS in 2020. Post-op imatinib therapy was given. In 2022, hepatic metastases (2.4 cm × 2.9 cm) with pancytopenia led to supportive care. Lesions enlarged to 4.3 cm × 2.7 cm, leading to multi-organ failure and dea

Light-Chain Deposition Disease Presenting with Spontaneous Splenic Rupture.

UNLABELLED: Light-chain deposition disease is a rare paraproteinaemia characterized by deposition of monoclonal light-chain immunoglobulins with a non-amyloid structure, most frequently affecting the kidney. The authors present the case of a 58-year-old man admitted due to haemorrhagic shock caused by spontaneous splenic rupture. Investigation showed coagulopathy, homogeneous hepatomegaly with elevated cholestasis enzymes but normal bilirubinaemia, and a nephrotic syndrome with monoclonal kappa light chains. Liver and bone marrow biopsies revealed light-chain deposition disease. Splenic ruptur

[Splenic marginal zone lymphoma complicated by cold agglutinin disease].

Splenic marginal zone lymphoma (SMZL) is a rare low-grade B-cell lymphoma accounting for less than 1% of lymphoid neoplasms and is often associated with autoimmune disorders. A 48-year-old woman presented with severe anemia due to steroid-refractory cold agglutinin disease (CAD), and was referred to our hospital for management of progressive systemic illness and high fever. On admission, she showed elevated serum soluble IL-2R and mild splenomegaly. PET/CT revealed FDG accumulation in the spleen and bone. She was pathologically diagnosed as having splenic marginal zone lymphoma by splenectomy

Successful Treatment of Hypersplenism in Wilson's Disease by Partial Splenic Embolization.

AIM: Hypersplenism can occur in patients with Wilson's disease (WD). Surgical splenectomy is a conventional treatment for this condition; however, emotional and neurological deterioration may follow splenectomy. In recent years, partial splenic embolization (PSE) has been increasingly performed as a nonsurgical alternative treatment for hypersplenism. The aim of this study was to evaluate the effectiveness and safety of PSE compared with splenectomy in the treatment of hypersplenism in WD patients. METHODS: Fifty WD patients with hypersplenism were randomly divided into two groups (group A an

[Acute splenic sequestration in children with sickle cell disease--an overview].

Acute splenic sequestration in children with sickle cell disease - an overview Acute splenic sequestration (ASS) is a life-threatening complication of sickle cell disease (SCD). The condition is important to recognize due to the fact that it can occur with previously unknown disease. ASS is one of the most common causes of death in children with SCD and is the result of blood suddenly getting congested in the spleen, resulting in splenomegaly, acute anemia, and hypovolemic shock. Timely and appropriate treatment is essential in preventing death. Episodes of ASS before one year of age are assoc

Splenic hypofunction and the spectrum of autoimmune and malignant complications in celiac disease.

BACKGROUND & AIMS: We investigated the prevalence of functional hyposplenism in autoimmune disorder (AID)-associated and complicated celiac disease (CD). In addition, because the association between hyposplenism and overwhelming infections caused by Streptococcus pneumoniae in patients with CD is well known, we investigated whether immunoglobulin (Ig)M memory B cells, which are responsible for protection against infections by encapsulated bacteria and require the spleen for their generation and/or survival, mirror the reduced splenic function in CD. METHODS: Peripheral blood samples were coll

[Crohn's disease complicated by multiple and recurrent aseptic splenic abscesses].

We report a case of multiple aseptic splenic abscesses occurring in a woman with Crohn's disease for three years. All microbiological samples were negative. The diagnosis was suspected on abdominal echography and CT scan and confirmed on histologic examination of the splenectomy specimen. The evolution was marked by recurrence of fever and inflammatory syndrome, associated to transitory morphological abnormalities of the accessory spleen. Aseptic splenic abscesses recurrence was suspected. This case allows us to consider aseptic splenic abscesses as an extra-intestinal manifestation and not as

Hepatic and splenic involvement in cat-scratch disease: imaging features.

Hepatosplenic involvement in cat-scratch disease, probably underdiagnosed, is characterized by multinodular lesions throughout the liver and spleen. Radiologic features of ultrasound, computed tomography, and magnetic resonance imaging are not specific. The key of the diagnosis relies on a history of cat or kitten contact. A specific serological test can confirm the diagnosis without invasive procedures such as biopsy.
