Giant ureteral calculi are defined as stones greater than 5 cm in length or circumference. These giant calculi can cause blockage of the ureter, dilation of the kidney and also decreased kidney function if not treated in time. The patient in this report presented with complaints of bilateral episodic pain of the bilateral lumbar region. Kidney, ureter and bladder (KUB) X-ray test showed a large bilateral ureteral stone about 14 cm in length and 106 g weight in the left ureter and 3 cm longitudinal diameter in the right ureter and also a staghorn stone in the left upper collecting system. There

We have undertaken a detailed retrospective study of urinary calculi in the Bristol clinical area from 1950 to 1978. Most calculi in children are detected in the upper urinary tract and the majority are associated with urinary infection. There were two predominant aetiological groups. First the children under 5 years of age, usually male, with a Proteus infection and triple phosphate calculi of the staghorn type in the renal pelvis and calyces. Of the 7 children with a sterile urinary tract in this age group, 4 were discovered to have a metabolic cause for the their calculi. These included 2 p

To describe a concept of ideal 'puncture zone' as against any single ideal 'puncture tract' for percutaneous nephro-lithotomy (PCNL) and present our results. Through this narrative, we aim to reduce the gaps in inter-understanding of an erstwhile description of ideal tract and real-life puncture making. The puncture zone principle was applied for our novel puncture making technique during PCNL. The largest imaginary cone that can fit into a respective calyx, with its tip in the pelvis defines the 'puncture zone' for that calyx. This concept allows fine-tuning of the ideal puncture tract based

A case of renal adenoma associated with staghorn calculus is reported. The patient was a 52-year-old man. In 1973, he underwent an ileal conduit because of tuberculous contracted bladder and impediment in renal function. At that time, small calculi were already present in the right kidney. Later, in the left kidney, calculi developed into staghorn calculus. An ureteral calculus was also present in the right side and contracted kidney was observed. In 1983, he suffered from continuous left abdominal pain and macroscopic hematuria. Left nephrolithotomy was performed on August 23, 1983. During su

Two more indications are described for temporary ex vivo perfusion of kidneys with revascularization of these organs as autografts to orthotopic or heterotopic locations. One of the patients had staghorn calculi which were removed from a solitary kidney. The other patient had both kidneys autografted in the course of a surgical procedure on an extensive abdominal aortic aneurysm.

A man in his 50s presented with right flank pain and intermittent haematuria for 15 days. He was evaluated and diagnosed to have bilateral staghorn renal calculi with left poorly functioning kidney. He underwent right kidney stone clearance followed by laparoscopic left simple nephrectomy. Postoperatively, he developed recurrent urinary tract infections. On evaluation, non-contrast CT of the abdomen revealed calculus in left ureteral stump and he was diagnosed to have ureteric stump syndrome. He underwent ureteroscopy which revealed multiple stump calculi and complete stone clearance was achie

Four patients with 5 nonopaque renal calculi composed of uric acid were examined by ultrasound. The calculi varied in size from a 1.5 X 1.5-cm intrapelvic stone to a staghorn calculus measuring 4 cm. All stones were satisfactorily imaged by ultrasound, allowing a diagnosis of nephrolithiasis to be made with confidence in each case. In 2 patients with poor excretion on urography, the diagnosis was not suspected prior to the ultrasound examination. The authors feel that ultrasound has great potential value in the investigation of nonopaque filling defects of the renal pelvis and in patients with

A 71-year-old female presented with recurrent episodes of right flank pain and fever for the past several years. No pathogens were detected in the urine culture. Abdominal X-ray revealed staghorn calculi of the right kidney. Computerized tomography revealed pneumopelvis and staghorn stones of the right kidney. A nephrectomy was indicated based on the results of intravenous pyelography (IVP) and renogram which revealed a right non-functioning kidney. The adhesion of the right kidney with the duodenum and a transverse colon was so strong that a nephrectomy, a wedge resection of the duodenum and

INTRODUCTION: In this study, we aimed to evaluate the safety and efficacy of the percutaneous nephrolithotomy procedure performed with adult-sized instruments in pediatric cases with staghorn kidney stone.
METHODS: We retrospectively evaluated the efficacy and safety of 94 percutaneous nephrolithotomy procedures performed during 15 years in a single center for 82 pediatric patients with staghorn calculi using adult-sized instruments (24-Fr nephroscope). Stone free status was defined as complete clearance of the stones or the presence of insignificant residual stones of <3 mm in diameter.

The authors propose a classification of ex situ surgery of the kidney and present their clinical experience of 20 renal autotransplantations over a period of 6 years: 16 were performed for renovascular hypertension with 2 failures and 4 for recurrent staghorn calculi, with 1 stone recurrence. Based on their experience and on the literature they discuss the present status and the future implications of ex situ surgery of the kidney.

Shock wave lithotripsy (ESWL) remains the most common treatment for renal calculi. In this article, recent literature pertaining to ESWL monotherapy of renal calculi was reviewed, with the goal of improving ESWL results through better case selection. When selecting the optimal surgical approach for a patient, multiple factors must be considered. Factors to consider include stone-related factors (size, number, composition and location), renal anatomical factors, and patient-related factors. Each of these factors is presented in detail, with the discussion limit to non-staghorn renal calculi. Ch

The first Danish experience with Extracorporeal Shock Wave Lithotripsy (ESWL) using a second generation Lithotriptor (Siemens Lithostar) is reported. 306 patients underwent 392 treatments for 363 stones. There were 339 renal calculi including 5 staghorn calculi and 54 ureteral calculi. Treatments were performed under local analgesia (82%) or epidural or general anesthesia (18%) when invasive procedures had to be done in connection with the treatment. Stone fragmentation was achieved with 2487 +/- 1262 shocks. The first months stone clearance rate was 45%; 26% had fragments less than 6 mm; 29%

PURPOSE Anatrophic nephrolithotomy performed via open surgery involves incising the renal parenchyma along an avascular plane to remove a large, complex renal stone. We determined the feasibility of performing laparoscopic anatrophic nephrolithotomy in a survival porcine model. Furthermore, we present a novel technique of creating a staghorn calculus in the porcine model. MATERIALS AND METHODS After developing the technique in 3 pigs the survival study was performed in 10 consecutive animals. The procedure comprised 2 aspects. 1) We developed an animal model for staghorn calculi by retrograde

Urinary calculi can induce urothelial cellular abnormalities comparable with those of malignancy; this was found in 11 out of 62 lithiasis cases. Severe cellular changes, comparable with those of carcinoma in situ, may be seen in the epithelium adjacent to a calculus. Squamous metaplasia was frequently observed in cases with staghorn stones in the renal pelvis. The abrasive effect of the calculus may result in many multinucleated cells in the sediment. When the calculi are removed the cytological atypia and the observed multinucleation disappeared, and none of these 62 patients developed uroth

BACKGROUND: Percutaneous nephrolithotomy (PCNL) is the gold-standard for complex renal stones treatment in the pediatric population. While the miniaturization of PCNL reduces the risk of bleeding, it can hinder surgical and functional outcomes. The aim of the study was to assess the safety and feasibility of semi-closed-circuit vacuum-assisted Mini-PCNL (vmPCNL) in pediatric patients.
METHODS: From January 2017 to December 2018, we prospectively collected data on consecutive vmPCNLs from two European tertiary referral centers. The procedure was performed with the ClearPetra® (Well Lead Medica

We dissolved the cystine calculi incarcerated in the ureter by irrigation with tromethamine-E via percutaneous nephrostomy. Case 1: A 25-year-old man with a past history of cystinuria and staghorn calculi in the right kidney, suffered from obstructive renal failure caused by the incarceration of a stone (18 x 15 mm) in the left middle ureter. After recovering promptly from renal failure by left percutaneous nephrostomy, he received continuous irrigation with tromethamine-E through an angiographic catheter percutaneously placed in the ureter. The ureteral calculus was reduced by 70% in size aft

PURPOSE OF REVIEW: Preoperative planning in staghorn stones treated with percutaneous nephrolithotomy (PCNL) can improve efficiency and minimize complications. We summarized the current evidence on imaging features to guide the selection of optimal access for managing staghorn stones.
RECENT FINDINGS: Although the upper pole may offer the most favorable route for stone clearance, it may carry a higher risk of complications in certain situations. In such cases, the middle-calyx may be a safer and equally efficient alternative. Multiple access may still be needed in certain cases. Care should b
