Toe spatiotemporal differences between transition steps when ascending shorter flight stairways of different heights.

BACKGROUND: Studies have typically treated the first and second floor-to-stair transition steps (TS1 and TS2) as one stride. However, because the foot is devoid of plantar cutaneous input from the stair surface at TS1, these steps may have different toe spatiotemporal profiles, and resultantly, different susceptibilities to a trip and/or a fall. This study compared vertical toe clearance, forward velocity, and their respective variability magnitudes between TS1 and TS2 when ascending stairs of different heights. METHODS: Twenty young adults (seven males and 13 females) (21.68 ± 2.49 years; 16

Ambulation Control System Design for a Hybrid Knee Prosthesis.

Prosthetic knees available to individuals with transfemoral amputation seek to restore functional ability to the user. Passive prosthetic knees are lightweight but can restore only limited, dissipative ambulation activities whereas active knees can provide energy to restore additional ambulation activities such as stair climbing and standing up from a chair. Semi-active prosthetic devices aim to only power a subset of activities and use passive components and control when that power is not necessary. Here, we outline an ambulation control system for a lightweight Hybrid Knee prosthesis that is

Influences of lumbar disc herniation on the kinematics in multi-segmental spine, pelvis, and lower extremities during five activities of daily living.

BACKGROUND: Low back pain (LBP) is a common problem that can contribute to motor dysfunction. Previous studies reporting the changes in kinematic characteristics caused by LBP present conflicting results. This study aimed to apply the multisegmental spinal model to investigate the kinematic changes in patients with lumbar disc herniation (LDH) during five activities of daily living (ADLs). METHODS: Twenty-six healthy subjects and 7 LDH patients participated in this study and performed level walking, stair climbing, trunk flexion, and ipsilateral and contralateral pickups. The angular displace

Enhancements and On-Site Experimental Study on Fall Detection Algorithm for Students in Campus Staircase.

Campus stairwells, characterized by their crowded nature during certain short periods of time, present a high risk for falls that can lead to dangerous stampedes. Accurate fall detection is crucial for preventing such accidents. However, existing research lacks a detection model that balances high precision with lightweight design and lacks on-site experimental validation to assess practical feasibility. This study addresses these gaps by proposing an enhanced fall recognition model based on YOLOv7, validated through on-site experiments. A dataset on campus stairwell falls was established, cap

Impact of Self-Contained Breathing Apparatus (SCBA) Weights on Firefighter's Kinematics During Simulated Firefighter Tasks.

Firefighters face a multitude of hazards in their line of duty, with overexertion being one of the foremost causes of injuries or fatalities. This high risk is often exacerbated by the burden of carrying a heavy self-contained breathing apparatus (SCBA). This study aims to explore the impact of SCBA weight on firefighters' musculoskeletal joint movements. Six firefighters participated in this study, performing four simulated firefighting tasks under three different SCBA weight conditions. A hybrid inverse kinematics approach was employed to analyze the kinematic data from two participants. The

Estimation of Heart Rate Recovery after StairClimbing Using aWrist-Worn Device.

Heart rate recovery (HRR) after physical exercise is a convenient method to assesscardiovascular autonomic function. Since stair climbing is a common daily activity, usually followedby a slow walking or rest, this type of activity can be considered as an alternative HRR test. The present study explores the feasibility to estimate HRR parameters after stair climbing usinga wrist-worn device with embedded photoplethysmography and barometric pressure sensors.A custom-made wrist-worn device, capable of acquiring heart rate and altitude, was used to estimatethe time-constant of exponential decay t,

Comparison of fixed and mobile-bearing total knee arthroplasty in terms of patellofemoral pain and function: a prospective, randomised, controlled trial.

BACKGROUND: Despite growing evidence in the literature, there is still a lack of consensus regarding the use of the mobile-bearing (MB) design total knee arthroplasty (TKA). METHODS: In a prospective, comparative, randomised, single centre trial, 106 patients with end-stage osteoarthritis of the knee were randomised to either an MB or fixed-bearing (FB) group to receive posterior stabilised (PS)-TKA using a standard medial parapatellar approach and patellar resurfacing with follow-up (FU) for 5 years. The primary outcome was anterior knee pain (AKP) during the chair rise test and the stair cl

Design optimization of a total hip prosthesis for wear reduction.

Aseptic loosening from polyethylene debris is the leading cause of failure for metal-on-polyethylene hip implants. The accumulation of wear debris can lead to osteolysis, the degradation of bone surrounding the implant components. In the present study, a parametric three-dimensional finite element model of an uncemented total hip replacement prosthesis was constructed and implanted into a femur model constructed from computed tomography (CT) scan data. Design optimization was performed considering volumetric wear as an objective function using a computational model validated in a previous stud

The influence of alignment on the musculo-skeletal loading conditions at the knee.

BACKGROUND AND AIM: High tibial osteotomies attempt to recreate physiologically normal joint loading. Previous studies have discussed the influence of mal-alignment on the distribution of static loads to the medial and lateral compartments of the knee. The aim of this study was to determine the influence of mal-alignment on the tibio-femoral loading conditions during dynamic activities. MATERIAL AND METHODS: Using a musculo-skeletal model of the lower limb, which had been previously validated with in vivo data, in this study we modified the alignment of the knee in four patients, from a norma

Muscle Contributions to Balance Control During Amputee and Nonamputee Stair Ascent.

Dynamic balance is controlled by lower-limb muscles and is more difficult to maintain during stair ascent compared to level walking. As a result, individuals with lower-limb amputations often have difficulty ascending stairs and are more susceptible to falls. The purpose of this study was to identify the biomechanical mechanisms used by individuals with and without amputation to control dynamic balance during stair ascent. Three-dimensional muscle-actuated forward dynamics simulations of amputee and nonamputee stair ascent were developed and contributions of individual muscles, the passive pro

The Only Way Is Up: Active Knee Exoskeleton Reduces Muscular Effort in Quadriceps During Weighted Stair Ascent.

Firefighters consistently rank stair ascent with gear, which can weigh over 35 kg, as their most demanding activity. Weighted stair climbing requires dynamic motions and large knee torques, which can cause exhaustion in the short term, and overuse injuries in the long term. An active knee exoskeleton could potentially alleviate the burden on the wearer by injecting positive energy at key phases of the gait cycle. Similar devices have reduced the metabolic cost for various locomotion activities in previous studies. However, no information is available on the effect of active knee exoskeletons o
