Stillbirth and neonatal mortality in Jordan: findings from Jordan stillbirths and neonatal deaths surveillance system.

BACKGROUND: Stillbirth and neonatal mortality declined significantly in high- and some middle-income countries since 2000 because of the significant improvements in obstetric and neonatal care. Yet, stillbirth and neonatal mortality rates remain high in many low- and middle-income countries. The main reason for low progress in reducing such stillbirths and neonatal deaths in Jordan is the scarcity of data on causes and contributing factors leading to these deaths. This study aimed to determine the rates, causes, and risk factors of stillbirth and neonatal mortality in Jordan. METHODS: An elec

Did smallpox cause stillbirths? Maternal smallpox infection, vaccination, and stillbirths in Sweden, 1780-1839.

While there is strong evidence that maternal smallpox infection can cause foetal loss, it is not clear whether smallpox infections were a demographically important cause of stillbirths historically. In this paper, we use parish-level data from the Swedish Tabellverket data set for 1780-1839 to test the effect of smallpox on stillbirths quantitatively, analysing periods before and after the introduction of vaccination in 1802. We find that smallpox infection was not a major cause of stillbirths before 1820, because most women contracted smallpox as children and were therefore not susceptible du

Antenatal care, identification of suboptimal fetal growth and risk of late stillbirth: findings from the Auckland Stillbirth Study.

INTRODUCTION: Stillbirth remains an important public health problem in Australia and New Zealand. The role that antenatal care plays in the prevention of stillbirth in high-income countries is unclear. METHODS: Cases were women with a singleton, late stillbirth without congenital abnormality, booked to deliver in the Auckland region and born between July 2006 and June 2009. Two controls with ongoing pregnancies were randomly selected at the same gestation at which the stillbirth occurred. Data were collected through interview-administered questionnaires and from antenatal records. RESULTS: O

Diagnostic evaluation to identify infection-attributable stillbirth.

OBJECTIVE: To characterize stillbirth evaluations, including the frequency and yield of investigations for infections causing stillbirth. STUDY DESIGN: Retrospective cohort of stillbirths at three university-affiliated perinatal centers from 2017 to 2022. The primary outcome was adherence to American College of Obstetrics and Gynecology core stillbirth evaluation recommendations (placental pathology, fetal autopsy, and fetal genetic testing). We further characterized the prevalence and yield of specific testing to evaluate for infection-attributable stillbirth etiologies. RESULTS: The cohort

Placenta and Umbilical Cord Abnormalities in Stillbirth.

This is a review of the critical role of placental and umbilical cord abnormalities in stillbirth, emphasizing their strong association with adverse pregnancy outcomes. We discuss key lesions-maternal and fetal vascular malperfusion, infectious, inflammatory, genetic, and multiple gestation-related abnormalities-detailing their biology, clinical presentation, and pathologic characteristics. We further highlight the importance of thorough placental examination for determining the cause of stillbirth and guiding future pregnancy management. Standardized criteria, such as the Amsterdam guidelines

Identifying risk of stillbirth using machine learning.

BACKGROUND: Previous predictive models using logistic regression for stillbirth do not leverage the advanced and nuanced techniques involved in sophisticated machine learning methods, such as modeling nonlinear relationships between outcomes. OBJECTIVE: This study aimed to create and refine machine learning models for predicting stillbirth using data available before viability (22-24 weeks) and throughout pregnancy, as well as demographic, medical, and prenatal visit data, including ultrasound and fetal genetics. STUDY DESIGN: This is a secondary analysis of the Stillbirth Collaborative Rese

Stillbirth at term: Does size really matter?

Placental dysfunction has a deleterious influence on fetal size and is associated with higher rates of perinatal morbidity and mortality. This association underpins the strategy of fetal size evaluation as a mechanism to identify placental dysfunction and prevent stillbirth. The optimal method of routine detection of small for gestational age (SGA) remains to be clarified with choices between estimation of symphyseal-fundal height versus routine third-trimester ultrasound, various formulae for fetal weight estimation by ultrasound, and the variable use of national, customized, or international

Addressing Stillbirth in India Must Include Men.

BACKGROUND: Millennium Development Goal 4, to reduce child mortality, can only be achieved by reducing stillbirths globally. A confluence of medical and sociocultural factors contribute to the high stillbirth rates in India. The psychosocial aftermath of stillbirth is a well-documented public health problem, though less is known of the experience for men, particularly outside of the Western context. Therefore, men's perceptions and knowledge regarding reproductive health, as well as maternal-child health are important. METHODS: Key informant interviews (n = 5) were analyzed and 28 structured

Elevated outdoor temperatures and risk of stillbirth.

BACKGROUND: The causes of stillbirth are poorly understood, including whether elevated outdoor temperatures increase risk. We assessed the relationship between elevated ambient temperatures and risk of stillbirth by gestational age and cause of death during warm months in a temperate region. METHODS: We performed a case-crossover study of 5047 stillbirths in continental Quebec, Canada, between the months of April through September from 1981 to 2011. Using data on maximum daily temperatures adjusted for relative humidity, we estimated associations with stillbirth, comparing temperatures before

Altered fetal growth, placental abnormalities, and stillbirth.

BACKGROUND: Worldwide, stillbirth is one of the leading causes of death. Altered fetal growth and placental abnormalities are the strongest and most prevalent known risk factors for stillbirth. The aim of this study was to identify patterns of association between placental abnormalities, fetal growth, and stillbirth. METHODS AND FINDINGS: Population-based case-control study of all stillbirths and a representative sample of live births in 59 hospitals in 5 geographic areas in the U.S. Fetal growth abnormalities were categorized as small (<10th percentile) and large (>90th percentile) for gesta

Predisposing factors associated with stillbirth in Tanzania.

OBJECTIVE: To determine whether specific medical conditions and/or fetal compromise during labor are associated with fresh stillbirth (FSB), and whether absent fetal heart rate (FHR) before delivery can increase risk of FSB. METHODS: An observational cohort study was conducted at three university referral hospitals in Tanzania between January and September 2013. Maternal, labor, and neonatal characteristics were recorded for all deliveries. FSB was defined as an Apgar score of 0 at 1 and 5minutes, with intact skin and suspected death during labor or delivery. RESULTS: Among 15 305 deliveries

Infectious causes of stillbirth: a clinical perspective.

Untreated infection may cause stillbirth by several mechanisms, including direct fetal infection, placental damage, and severe maternal illness. Many bacteria, viruses, and protozoa have been associated with stillbirth. In developed countries, up to 24% of stillbirths have been attributed to infection, although with increased availability of sophisticated diagnostics and rigorous screening, it appears likely that higher numbers may actually be associated with infection. In developed countries, ascending bacterial infection is usually the most common infectious cause of stillbirth, with a numbe

Prenatal smoking and risk of intrapartum stillbirth.

The purpose of this study was to examine the association between prenatal smoking and intrapartum stillbirth by the use of a cohort of singleton births in Missouri from 1978 through 1997. Overall, the authors identified a total of 7,325 counts of stillbirth, yielding a stillbirth rate of 4.4 per 1,000. The timing of the occurrence of the stillbirth to onset of labor was specified in 85.6% (n = 6,273). Of these, 1,070 (17.0%) occurred intrapartum. Smoking mothers were 50% more likely to experience intrapartum fetal death as compared with nonsmoking gravidas (adjusted hazard ratio = 1.5; 95% con

Predictors of stillbirth in sub-saharan Africa.

OBJECTIVE: To describe the incidence and predictors of stillbirth in a predominantly human immunodeficiency virus (HIV)-infected African cohort. METHODS: Human Immunodeficiency Virus (HIV) Prevention Trials Network (HPTN) 024 was a randomized controlled trial of empiric antibiotics to reduce chorioamnionitis-related perinatal HIV transmission. A proportion of HIV-uninfected individuals were enrolled to reduce community-based stigma surrounding the trial. For this analysis, only women who gave birth to singleton infants were included. RESULTS: Of 2,659 women enrolled, 2,434 (92%) mother- chil

Outcome of resuscitation following unexpected apparent stillbirth.

There are few data to inform a decision to resuscitate babies who are unexpectedly stillborn. The outcome for 42 successfully resuscitated stillborn children, of whom 62% survived to be discharged home, is reported. Of the survivors, a poor outcome with severe disability was found in 23% (including one postneonatal death), equivocal outcome was found in 15% (two mild hypertonia; two with mild hemiplegia and no associated other disability) and 62% were free of any impairment at follow up 20 months to 8 years later. In 39 (93%) fetal problems had been identified and the resuscitation team was pr

Congenital leukaemia: an unusual cause of stillbirth.

Three cases of congenital leukaemia associated with hydrops and intrauterine death are described. The leukaemic infiltrates, which involved both the infants and the placentas, were of monocytic, lymphoblastic, and myeloblastic types, respectively. The general lack of recognition of the condition is probably due to inadequate histological examination of the stillborn infant and the placenta.

Grief, Bereavement, and Mental Health After Stillbirth.

Following stillbirth, profound grief is well-documented, and co-occurring mental health struggles are common. Distinguishing normal grief from mental health conditions is crucial yet difficult, especially when losses are traumatic. Strong social and emotional support promotes integration and adjustment, but stigma surrounding stillbirth often negatively impacts the degree of support families receive. Many bereaved parents prefer peer-based and animal-assisted supports over conventional clinical services, which may reflect gaps in clinician training in perinatal bereavement care. Accordingly, a

Postpartum Hemorrhagic Morbidities with Livebirth versus Stillbirth.

OBJECTIVE: ACOG publications on stillbirth or postpartum hemorrhage (PPH) do not consider stillbirth as a risk factor for postpartum hemorrhagic morbidity. This study aimed to ascertain the likelihood of composite maternal hemorrhagic outcome (CMHO) among individuals who delivered vaginally with livebirth versus a stillbirth. STUDY DESIGN: This was a retrospective cohort study of all parturients greater than 20 weeks gestation who delivered vaginally at a single level IV site within 24 months. Demographic differences and baseline PPH risks were analyzed. CMHO included any of the following: es
