[Gastric volvulus: diagnosis and management].

Gastric volvulus is defined as an abnormal rotation of all or part of the stomach around one of its axes. It is a diagnostic emergency and therapeutic challenge because in acute forms it may lead to gastric strangulation with a high risk of ischemia and necrosis. Organoaxial and mesentericoaxial volvulus are distinguished according to the direction of rotation. The most common cause of gastric volvulus is hiatal hernia, but the principal predisposing factor is ligamentous laxity. The diagnosis is suspected when erect chest radiograph images show a high air-fluid level in the chest. Moreover a

Intrathoracic Gastric Volvulus Following Laparoscopic Fundoplication: A Case Report and Review of the Literature.

Gastric volvulus is a fatal complication that rarely occurs after Nissen fundoplication with organoaxial volvulus being the most common type. We present a case of a 36-year-old male with a surgical history of Nissen fundoplication with hiatal repair three years prior to presentation. He presented with shortness of breath, abdominal pain, and obstipation. CT scan showed no passage of the oral contrast towards the stomach. The stomach is seen completely herniated into the chest suggesting rotation along its long axis worrisome for organoaxial volvulus. Intraoperative findings were a strangulated

[Recurrent gastric volvulus resolved surgically after therapeutic percutaneous endoscopic gastrostomy in Duchenne muscular dystrophy].

A 20-year-old man with Duchenne muscular dystrophy (DMD) with recurrent gastric volvulus underwent percutaneous endoscopic gastrostomy (PEG). Four months later, he developed vomiting and consciousness disturbance. CT revealed gastric volvulus recurrence along the gastrostomy axis. Endoscopic repositioning failed and fistula perforation necessitated emergency surgery. The upper position of the stomach was twisted counter-clockwise and revolved on the gastrostomy axis sliding between the lower stomach and abdominal wall. The fistula showed necrotic perforation and was thus resected. The anterior

Acute Organoaxial gastric volvulus: A massive problem with a twist-case report.

INTRODUCTION: Gastric volvulus (GV) is a rare and life threatening condition if not treated promptly or wrongly diagnosed. The main complication of gastric volvulus is foregut obstruction. The extreme rotation can cut off blood supply to the stomach and even distal organs, which can lead to ischemia and necrosis of the affected area. PRESENTATION OF CASE: We report a case of a 41yo female that complained of severe abdominal pain, nausea and vomiting for approximately 3days after eating a large meal. The patient didn't have any flatus or bowel movements in the last 24h. CT of the abdomen and p

A Rare Twist: Case Study on Gastrohepatic Volvulus.

A gastric volvulus occurs when the stomach rotates around itself, creating a closed-loop obstruction. This can result in a life-threatening condition in which supporting vasculature can be obstructed and lead to ischemia, necrosis, and perforation. A gastrohepatic volvulus is an exceedingly rare type of gastric volvulus in which the stomach rotates in relation to the liver, specifically around the gastrohepatic ligament. We present a case of a 39-year-old female with a history of congenital malrotation and developmental delay who presented to the emergency room for nausea and emesis. Physical

Gastric volvulus with necrosis and gangrene associated with wandering spleen: A rare case report from Syria.

Gastric volvulus and wandering spleen are two rare and uncommon conditions. Gastric volvulus is characterized by the rotation of the stomach around itself, causing a closed obstruction. It was first described in the 19th century and is classified according to the axis, etiology, or chronicity. A wandering spleen is defined by the absence of one or all of the splenic ligaments, which occurs either for congenital or acquired reasons. An enlarged spleen may contribute to the wandering spleen and, in turn, lead to gastric volvulus and infarction of the stomach, requiring emergency surgical interve

Gastric volvulus associated with wandering spleen in a child.

Wandering spleen is an uncommon entity in childhood and has been described only rarely in association with gastric volvulus. Wandering spleen and gastric volvulus were diagnosed in a 5-year-old boy who presented with acute abdominal pain and distension. Intraoperatively, normal ligamentous connections between the stomach, spleen, and posterior abdominal wall were absent. Developmental anomalies that result in wandering spleen may lead to hypermobility of the stomach and a predisposition to gastric volvulus. In such patients, prophylactic gastropexy should be considered.

Hiatal hernia with gastric volvulus and perforation: a rare complicated entity.

Gastric volvulus with perforation is a rare and potentially catastrophic complication of hiatal hernia. Gastric volvulus complicates ~4% of hiatal hernia cases. We present a case of a 71-year-old male patient who presented with severe progressive generalized abdominal pain associated with absolute constipation, nausea, and vomiting ongoing for 8 days before admission. An abdominal computed tomography scan demonstrated a large hiatal hernia with organoaxial gastric volvulus and pneumoperitoneum around the stomach, denoting gastric volvulus with perforation. During laparotomy, the abdomen was fo

Acute gastric volvulus in a patient with trisomy 21.

Acute gastric volvulus is a torsion of the stomach by more than 180° and a life-threatening condition. We present a 50-year-old male patient with acute abdominal pain who has Down syndrome/trisomy 21. Computed tomography showed a significant distended stomach with features of a severe gastric volvulus. Emergency operation in form of reduction and gastropexy was performed. We are not aware of any similar cases published in the English literature, where as gastric volvulus occurred in a patient with Down syndrome.

[Laparoscopic therapy of gastric volvulus associated with hiatal hernia in the elderly patient].

Gastric volvulus is an abnormal rotation of one portion of the stomach around itself that may lead to gastric obstruction or an impairment of the vascular supply to the stomach leading to strangulation. This may present acutely or with chronic intermittent symptoms. Acute gastric volvulus is an emergency situation in which there is a closed loop obstruction and vascular compromise that requires urgent treatment. In the chronic type, unspecific abdominal pain is common. Chronic gastric volvulus must be treated to avoid complications, but there is not a standard treatment for this condition. Min

Boerhaave's syndrome in a patient with an upside down stomach: A case report.

INTRODUCTION: Spontaneous esophageal perforation, or Boerhaave's syndrome, is a life-threating condition which usually requires emergent surgery. An upside down stomach is defined as a gastric volvulus in a huge supradiaphragmatic sac. In general, this condition can result in ischemia and perforation of the stomach. This is the first report of a patient with Boerhaave's syndrome and an upside down stomach. CASE PRESENTATION: A 79-year-old woman presented with sudden epigastric pain following hematemesis. Evaluation of the patient showed both an esophageal perforation and an upside down stomac

Laparoscopic gastropexy for chronic gastric volvulus.

Gastric volvulus is an infrequently encountered clinical entity characterized by abnormal rotation of the stomach. Surgical treatment by anterior gastropexy is indicated for acute and chronic gastric volvulus. Recently, laparoscopic surgery has been used in abdominal surgery. We report the case of a patient with chronic gastric volvulus who was treated by laparoscopic gastropexy and evaluated by upper gastrointestinal series and endoscopic appearance of the stomach. The laparoscopic approach is safe and effective for patients and is considered applicable to surgery for other upper gastrointest

Laparoscopic-guided gastropexy for intermittent gastric volvulus.

A 12-year-old girl with acute gastric volvulus associated with severe scoliosis and neurological impairment did well initially with nasogastric tube decompression. To prevent recurrence, anterior gastropexy was performed using percutaneous sutures through the abdominal wall and stomach with the gastroscope in place. Simultaneous laparoscopic guidance was used to ensure that the stomach was fixed in a nonrotated position. One year later, volvulus has not recurred. This method of gastropexy is indicated only in children with intermittent gastric volvulus who do not have an underlying abnormality

[Upside-down stomach and hiatal hernia].

Hiatal hernias represent a special variant of diaphragmatic hernia in which there is a transdiaphragmatic migration of the stomach through the esophageal hiatus. There are 4 types admitted--sliding (type I), paraesophageal (type II), mixt-combining elements for both previous types (type III) and complex (type IV) and represents at most 5-15% from all hiatal hernias. Upside-down stomach is a special form of organoaxial volvulus of the entire stomach in a sac of mixt hiatal hernia (type III) or complex (type IV). There are asymptomatic forms, but the majority of the patients present signs of gas

Ultrasonography of gastric volvulus: "peanut sign".

We report a case of chronic gastric volvulus in which ultrasonography (US) was useful. An 81-year-old woman was hospitalized due to vomiting, and upper gastroduodenoscopy revealed that the stomach was spirally twisted and constricted. An upper gastrointestinal barium study demonstrated an organoaxial-mesenteroaxial combined type gastric volvulus. US showed constriction between the dilated upper stomach body and the lower stomach body similar to a "peanut". Thereafter, the patient's vomiting stopped and follow-up US demonstrated that the constriction of the stomach was loosened. Therefore, we b

Gastric volvulus, Borchardt's Triad, and Endoscopy: A Rare Twist.

The authors describe a case of gastric volvulus, which is a rare cause of gastric outlet obstruction. An 85-year-old man presented with nausea, vomiting, and epigastric pain. Admission abdominal radiograph demonstrated a grossly distended stomach with air-fluid levels. Multiple attempts at nasogastric tube placement failed. Endoscopy revealed a fluid-filled, tortuous stomach with a paraesophageal hernia, and the operator was unable to locate or pass the scope through the pylorus. Traditionally Borchardt's triad is believed to be diagnostic for acute gastric volvulus and consists of unproductiv
