Case report: Laboratory diagnosis methods on brain abscess pathogens caused by anaerobic species and Streptococci constellatus.

Brain abscess (BA) represents a severe, intracranial infection associated with high morbidity and mortality. Multiple pathogens may contribute to BA development, some of which are difficult to detect using conventional diagnostic methods alone. With the wide utilization of metagenomic next-generation sequencing (mNGS), more experiences are concluded in authenticity practice except for being regarded as a comprehensive method for detecting all pathogens. Here, we report a severe case of BA in a patient diagnosed by CT imaging, who subsequently underwent surgical removal of the abscess. To ident

A Rare Case of Blood Culture-Negative Isolated Pulmonic Valve Endocarditis Causing Empyema.

Right-sided native valve infective endocarditis (IE) refers to IE involving the tricuspid or pulmonic valve. The most common factors causing right-sided IE include intravenous drug use, intracardiac device, and central venous catheters. Isolated pulmonic valve IE has only been reported in less than 2% of all IE cases. We present a unique case of a patient with a history of poor oral hygiene found to have isolated blood culture-negative pulmonic valve IE, who subsequently developed empyema positive for Streptococcus constellatus.

Antimicrobial activity of silver diamine fluoride on human periodontitis microbiota.

Silver diamine fluoride (SDF) has been used in management of dentinal hypersensitivity and dental caries. This in vitro study evaluated the antimicrobial effects of SDF on subgingival microorganisms from severe human periodontitis lesions. Subgingival biofilm specimens from 24 adults with severe periodontitis were mixed in vitro with 19% or 38% SDF or left untreated (n = 24 per group) and then inoculated on enriched Brucella blood agar with anaerobic incubation. Selected red- and orange-complex periodontal pathogens were phenotypically identified in the subgingival specimens, including Porphyr

Progressive unilobar cavitating lung disease despite medical therapy: Diagnostic limitations and definitive surgical management.

Cavitating lung lesions are a common radiological finding with a broad differential diagnosis, including infectious, malignant and inflammatory causes. Management of infective lesion is often challenging, particularly when disease progresses despite appropriate medical therapy. We report a case of progressive, unilobar cavitating lung disease in a 59-year-old man related to Aspergillus colonization and Streptococcus constellatus infection, despite prolonged optimal antibacterial and antifungal treatment. Initial bronchoscopic investigations identified Aspergillus hyphae, but repeated conventio

Identification of clinically relevant viridans streptococci by analysis of transfer DNA intergenic spacer length polymorphism.

The utility of PCR analysis of transfer DNA intergenic spacer length polymorphism (tDNA-ILP) for the identification to the species level of clinically relevant viridans streptococci was evaluated with a collection of reference strains of 15 species of the salivarius, anginosus, mitis and mutans rRNA homology groups. PCR products generated by using fluorescent, outwardly directed, consensus tDNA primers were analysed by electrophoresis on denaturating polyacrylamide gels and by laser fluorescence scanning. Eleven species showed specific and distinct tDNA patterns: Streptococcus cristatus, Strep

Chronic lacrimal canaliculitis - the answer to a three-year history of red eye.

Chronic lacrimal canaliculitis is a rare infection of the lacrimal system, and can lead to misdiagnosis due to its overlapping presentation to other common entities. The authors report a case of lacrimal canaliculitis with a three-year history of recurrent unilateral red eye and mucopurulent discharge. Here, we describe the clinical course, surgical details, and microbial analysis of canaliculitis infection.

Osteomyelitis of the mandible secondary to malignant infantile osteopetrosis in an adult.

Malignant infantile osteopetrosis (MIOP), an autosomal-recessive disorder, is extremely rare, presenting early in life with extreme sclerosis of the skeleton and reduced activity of osteoclasts. It was first described by Albers Schonberg in 1904. Disease manifestations include compensatory extramedullary haematopoiesis at sites such as the liver and spleen, hepatosplenomegaly, anaemia and thrombocytopaenia. Neurological manifestations can also occur due to narrowing of osseous foramina resulting in visual impairment, hearing loss, facial palsy and hydrocephalus. In addition, growth retardation

Comparing Matrix-Assisted Laser Desorption Ionization-Time of Flight Mass Spectrometry and Phenotypic and Molecular Methods for Identification of Species within the Streptococcus anginosus Group.

The heterogeneity of members of the Streptococcus anginosus group (SAG) has traditionally hampered their correct identification. Recently, the group was subdivided into 6 taxa whose prevalence among human infections is poorly described. We evaluated the accuracy of the Rapid ID32 Strep test, matrix-assisted laser desorption ionization-time of flight mass spectrometry (MALDI-TOF MS), and a PCR multiplex method to identify 212 SAG isolates recovered from human infections to the species and subspecies level by using multilocus sequence analysis (MLSA) as the gold standard. We also determined the

Spontaneous septic arthritis of the pubic symphysis in an afebrile patient.

Septic arthritis is a rare infection usually involving the knee or hip but can infrequently affect less obvious joints such as the pubic symphysis.Risk factors for septic arthritis include joint repair or replacement surgery, systemic infection, intravenous recreational drug use, and alcoholism. We present the case of a 48-year-old man with a final diagnosis of septic arthritis of the pubic symphysis who had no risk factors besides alcoholism. The presentation was unusual in that the patient was afebrile, and the infection seemed to be spontaneous. The infecting pathogen was identified as Strept

Virulence factors of Streptococcus anginosus - a molecular perspective.

Streptococcus anginosus together with S. constellatus and S. intermedius constitute the Streptococcus anginosus group (SAG), until recently considered to be benign commensals of the human mucosa isolated predominantly from oral cavity, but also from upper respiratory, intestinal, and urogenital tracts. For years the virulence potential of SAG was underestimated, mainly due to complications in correct species identification and their assignment to the physiological microbiota. Still, SAG representatives have been associated with purulent infections at oral and non-oral sites resulting in absces

[A Case of Esophageal Submucosal Abscess Originating from a Peritonsillar Abscess].

We report the case a 54-year-old male patient who was admitted to our hospital with a sore throat and evidence of a high inflammatory reaction. Computed tomography revealed a peritonsillar abscess extending from the inferior right tonsil to the cervical esophagus, and thickening of the esophageal wall. The pharyngeal abscess was drained through an intraluminal incision in the mesopharynx. Culture of the abscess fluid showed growth of Streptococcus constellatus, one of the organisms of the Streptococcus milleri group. We diagnosed the patient as having phlegmonous esophagitis secondary to perit

Metagenomic next-generation sequencing identified a brain abscess caused by mixed oral anaerobe infection: A case report.

Fusobacterium vincentii brain abscesses are relatively rare. Here, we report our treatment of an anaerobic brain abscess caused by a mixed infection of Parvimonas micra, Streptococcus constellatus, Fusobacterium vincentii, and Bacteroides heparinolyticus diagnosed by metagenomic next-generation sequencing (mNGS). This is the first reported case of Fusobacterium vincentii in a brain abscess. This case highlights the possibility that oral anaerobic microbes can cause a brain abscess and demonstrates that mNGS has the potential to be deployed to provide rapid infection diagnosis and rationalize a

[A case of spinal epidural abscess associated with retropharyngeal abscess].

Spinal epidural abscesses are known to occur associated with retropharyngeal abscess, but such cases are few in the literature. We treated a 72-year-old woman who reported pain in the back of the neck. Computed tomography (CT) showed a retropharyngeal abscess extending to the upper neck through the carotid space on the left side and an magnetic resonance imaging (MRI) showed a spinal epidural abscess without cervical vertebral osteomyelitis. The abscess was assumed to reach the epidural space along the nerve root through the intervertebral foramen. Since tonsillitis appeared to cause the retro

Sinogenic Subdural Empyema in an Adolescent: Multifocal Disease, Decompressive Craniotomy, and Complete Recovery.

Sinogenic subdural empyema is a rare and life-threatening complication of bacterial sinusitis. Non-specific symptoms tend to make early diagnosis and management challenging. We report the case of a previously healthy 16-year-old male who presented with a one-week history of upper respiratory symptoms and headache followed by a rapidly progressive neurologic deterioration. Multifocal subdural empyema was confirmed via multiparametric MRI. Cultures taken during neurosurgical intervention identified a polymicrobial etiology, including Staphylococcus aureus, Staphylococcus epidermidis, Streptococc

Biological characterisation of group F streptococci causing human infections.

Fifty five strains of group F streptococci (GFS) isolated from clinical samples of 53 patients with diverse diseases, were characterised by a battery of physiological tests. Among them 37 (67%) were Streptococcus anginosus F, 14 (25%) were Strep. intermedius, two of which were mannitol positive and 4 (7%) were Strep. constellatus. Determination of minimum inhibitory concentrations of penicillin, erythromycin, ampicillin, gentamicin and cefazolin showed that despite their biochemical heterogeneity, all strains were uniformly susceptible to these antibiotics. Except for bacteraemia, other diseas

Infected hepatic cyst complicating urinary sepsis.

Congenital hepatic cysts are a common disorder. Usually they are asymptomatic and do not have to be treated. However, some serious complications can occur. We report here the case of an 86-year old patient who has been treated by a percutaneous drainage for an infected solitary hepatic cyst due to urinary sepsis. She was admitted to the Department of Internal Medicine for epigastric pain with fever and chills. The patient was treated for a urinary tract infection 3 weeks ago by her General Practitioner. On admission, blood tests showed 21 620 neutrophils per microliter with a C-reactive protei
