BACKGROUND: Struma ovarii is a rare specific ovarian tumor. It is a highly differentiated monodermal teratoma with a malignant transformation rate as low as 5%. Struma ovarii is an infrequent type of teratoma arising from the ovary accounting for only 2% of all ovarian teratomas. Tumor-to-tumor metastasis is extremely rare. We herein describe a 67-year-old woman who underwent FDG PET/CT for initial staging of lung cancer. This imaging examination revealed a pelvic mass with peripheral FDG uptake, suggesting a metastatic tumor from lung cancer or a secondary malignancy. Examination of a surgical specimen confirmed metastatic lung adenocarcinoma within a struma ovarii, suggesting tumor-to-tumor metastasis.

BACKGROUND: Struma ovarii is a type of monodermal mature teratoma composed entirely or mainly of thyroid tissue, accounting for 1% to 3% of all ovarian teratomas and 0.3% to 1.0% of all ovarian tumors. Struma ovarii has elicited considerable interest because of its many unique features since Ludwig Pick first elucidated its relationship to teratoma in the early part of the 20th century.
