During the past 19 years, 229 autopsies of intracranial hemmorrhages were performed at the Department of Pathology, and 23 cases of posterior fossa subdural hemorrhages were discovered (10.0%). 25 cases of posterior fossa subdural hemorrhages (2 of our own treated surgically and 23 untreated found at autopsy) are described in detail. 2 cases treated surgically responded favorably to surgical treatment. The mechanism of hemorrhage into posterior fossa subdural spaces is investigated and reconsidered following a review of the literature. The necessity for CT is emphasized for early diagnosis of

We present a case of refractory infantile subdural empyema secondary to pneumococcal meningitis. Initial treatment was with antibiotics and multiple burr hole drainage of the subdural space. As there was no significant improvement with this standard procedure, craniectomy, and juxtapositioning of the temporalis muscle was conducted and the patient recovered promptly.

Dynamic biologic modeling was used to calculate the transfer rate constant for gadolinium-diethylenetriaminepenta-acetic acid (Gd-DTPA) and capillary permeability in the outer membrane of chronic subdural hematomas and effusions. Following intravenous Gd-DTPA injection, Gd concentrations in the subdural fluid and in timed arterial blood samples were measured by ion-coupled plasma emission spectrometry in 53 chronic subdural hematomas and 18 chronic subdural effusions. The capillary surface area in outer membrane was assessed morphometrically. Transfer rate constants for subdural hematomas and

X-linked myotubular myopathy is a severe congenital myopathy that can involve multiple organs. We report on a 10-month-old boy who manifested X-linked myotubular myopathy with subdural hemorrhage. The diagnosis of X-linked myotubular myopathy was based on typical muscle pathology and MTM1 missense mutation. The patient had undergone no traumatic episodes or bleeding diathesis. Axial growth acceleration is known to occur in X-linked myotubular myopathy, potentially leading to dolichocephaly. In our patient, an enlarged subdural space apparently stretched the bridging veins, increasing susceptib

BACKGROUND: Recurring subdural empyema despite adequate surgical drainage and antibiotic treatment is still a life-threatening disease. This is mainly due to poor diffusion of the antibiotic into the subdural space.
CASE DESCRIPTION: We report 2 cases of recurring subdural empyema due to Streptococcus, after repeat surgery and high-dose antibiotic treatment including beta-lactamines and vancomycin. Both patients showed marked clinical and radiologic improvement after introduction of linezolid. There was no drug-related adverse effect despite 36 and 90 days of treatment.
CONCLUSION: Off-label

Spinal subdural haematomas are a rare cause of spinal cord or cauda equina compression. These are usually seen in association with lumbar puncture or coagulopathy. Spontaneous spinal subdural haematomas are even rarer. This report presents one such case diagnosed by magnetic resonance imaging (MRI) together with a review of the literature. The patient presented with low back pain of acute onset after minimal trauma. MRI showed high signal intensity on both T1-weighted and T2-weighted sequences in the posterior subdural space indicative of a subacute lumbar subdural haematoma. There was no hist

Chronic subdural hematomas (cSDH) are increasingly prevalent, particularly among the elderly, and often require surgical intervention as the standard treatment. The placement of drains following surgical evacuation of cSDH is widely recognized as an effective strategy to reduce recurrence rates and improve clinical outcomes. However, the optimal location for drain placement remains a topic of debate. This study aims to comparatively evaluate the efficacy and safety of subgaleal/subperiosteal drainage (SGPD) and subdural drainage (SDD) techniques in patients with cSDH. This study followed PRISM

BACKGROUND: Juvenile myelomonocytic leukemia (JMML) is a rare childhood hematopoietic disorder typically presenting with hepatosplenomegaly, lymphadenopathy, pallor, fever, and cutaneous findings. The authors report the first case, to our knowledge, of JMML presenting in a pediatric patient with a subdural hematoma.
CASE DESCRIPTION: A 7-month old male with recurrent respiratory infections and a low-grade fever presented with a full fontanelle and an increasing head circumference and was found to have chronic bilateral subdural collections. Abusive head trauma, infectious, and coagulopathy wo

BACKGROUND: Chronic subdural hematoma (CSDH) is one of the more frequent pathologic entities in daily neurosurgical practice. Historically, CSDH was considered progressive recurrent bleeding with a traumatic cause. However, recent evidence has suggested a complex intertwined pathway of inflammation, angiogenesis, local coagulopathy, recurrent microbleeds, and exudates. The aim of the present review is to collect existing data on pathophysiology of CSDH to direct further research questions aiming to optimize treatment for the individual patient.
METHODS: We performed a thorough literature sear

Subdural hematomas were evacuated in 41 newborns during the first 4 days after birth. Subdural hematomas due to birth trauma may be subdivided into supratentorial, subtentorial, and suprasubtentorial. The location was frequently bilateral. The clinical course and diagnosis are described. Evacuation of the subdural hematoma was performed by subdural taps, linear craniotomies through the coronal and lambdoid sutures, and, in some cases, by combined approaches. The results were favorable in 68.3% of cases.

OBJECTIVE AND IMPORTANCE: Intrathecal baclofen delivery for the treatment of spasticity has been used for almost 20 years with a great deal of success. A wide variety of complications and pitfalls have been described. This report details a novel complication involving inadvertent and initially unrecognized canalization of the subdural space with the spinal catheter, which ultimately resulted in an overdose.
CLINICAL PRESENTATION: An intrathecal pump system was implanted in a 15-year-old girl with spasticity. This initially resulted in a lack of therapeutic effect. The diagnostic workup ultima

Chronic subdural hematoma (CSDH) is traditionally described as a post-traumatic blood collection within the subdural space; however, both its anatomical localization and pathophysiology have been increasingly questioned. Ultrastructural and histopathological evidence demonstrates that no true subdural space exists under physiological conditions and that CSDH originates instead within the dural border cell (DBC) layer, a mechanically fragile and biologically active meningeal interface. Accordingly, chronic "subdural" hematoma may be more accurately interpreted as an intradural border cell lesio

CASE: A 73-year-old woman, after spinal surgery, presented with symptomatic spinal subdural extra-arachnoid hygroma (SSEH) because of a fall on the third postoperative day. The hygroma was diagnosed by magnetic resonance imaging (MRI). Lumbar puncture was performed under local anesthesia, after which the leg pain disappeared immediately. MRI obtained immediately after puncture and 1 week later confirmed disappearance of the hygroma.
CONCLUSION: Although dural transection is mentioned in most of the reports on treatment of symptomatic postoperative SSEH, we were able to treat this entity by ep

BACKGROUND AND OBJECTIVES: Unintentional subdural block, while attempting an epidural anesthetic, is a rare but known complication. The authors describe a radiologically confirmed case of an unintentional combined epidural and subdural block while attempting to perform an epidural block for transurethral resection of prostate in a middle-aged man.
METHODS: Loss of resistance to air injection was used for identifying the epidural space prior to catheter placement. Following the epidural injection of 20 mL of a 1.5% lidocaine and 0.5% bupivacaine mixture, the patient developed rapid, intense, p

The dura-arachnoid junction is examined in normal animals and in animals subjected to subdural infusion of blood immediately prior to death, simulating acute subdural hemorrhages. The Norwegian landrace pig is used as the experimental animal. Horseradish peroxidase (HRP) has been added to the injected blood to serve as a macromolecular tracer. The material is studied by light and electron microscopy. Special attention is given to the level of the induced subdural cleavage plane, the total distribution of the infused blood, and the natural sites of drainage. The dura-arachnoid junction, identif

We describe a patient who received an apparently uneventful extradural block in labour but developed rapid extension of neural block within minutes of receiving her first incremental dose 2 h later. Computed contrast tomography revealed radio-opaque dye within both the subdural and subarachnoid spaces, but none within the extradural space. This case report demonstrates that subdural spread of low-dose local anaesthetics is not always clinically distinguishable from extradural analgesia and that the arachnoid membrane may subsequently perforate with potentially serious consequences.

An AIDS patient with a subdural empyema as a complication of candida sinusitis is presented. Clinical features resembled those of invasive aspergillosis or phycomycosis. Microbiological diagnosis was established by blood cultures, sinus cultures, and histopathological examination. An aggressive surgical approach first to the sinuses and then to the subdural space, together with fluconazole produced a satisfactory result.

It is unusual to encounter a case where subdural haematoma (effusion and internal hydrocephalus coexist and are progressive. Often, when the two diseases are concomitantly present, the pathology of one complicated the other. The pathology of subdural haematoma (effusion), complicated by internal hydrocephalus, and problems involved in the diagnosis and treatment of such cases are described, with particular reference to three such cases treated by the authors, and a review of the literature. When cerebral angiography is used as a follow-up study for postoperative subdural haematoma (effusion),

BACKGROUND: A subdural injection is a rare complication of neuraxial anesthesia where medication is accidentally injected into the potential space between the dura mater and arachnoid mater of the spinal cord. Subdural blocks are typically underdiagnosed due to their variable presentation; failed early detection can lead to significant long-term consequences.
CASE REPORT: This is a unique case of a 33-year-old parturient who presented for epidural placement during which a 3 mL subdural injection of preservative-free 1.5% lidocaine with 1:200,000 epinephrine test dose led to rapid clinical det
