Beyond the regulatory radar: knowledge and practices of rural medical practitioners in Bangladesh.

BACKGROUND: Informal and unregulated rural medical practitioners (RMPs) provide healthcare services to about two-thirds of people in Bangladesh, although their service is assumed to be substandard by qualified providers. As the RMPs are embedded in the local community and provide low-cost services, their practice pattern demands investigation to identify the shortfalls and design effective strategies to ameliorate the service. METHODS: We conducted a cross-sectional study in 2015-16 using a convenient sample from all 64 districts of Bangladesh. Personnel practising modern medicine, without an

Access and coverage of malaria services in Haiti in the context of elimination: a scoping review of the literature.

INTRODUCTION: Haiti is on the verge of possibly eliminating malaria, an endemic parasitic infection primarily caused by Plasmodium falciparum on the island of Hispaniola. Owing to its associated morbidity and mortality, malaria is a leading public health priority in Haiti. This scoping review aims to identify the scope of research on access and coverage of malaria surveillance, diagnosis, and treatment in Haiti in the elimination setting. METHODS: We compiled, analyzed, and synthesized the literature on access and coverage of antimalarial services in Haiti in the elimination setting based on

Traditional Chinese medicines and their active ingredients sensitize cancer cells to TRAIL-induced apoptosis.

The rapidly developing resistance of cancers to chemotherapy agents and the severe cytotoxicity of such agents to normal cells are major stumbling blocks in current cancer treatments. Most current chemotherapy agents have significant cytotoxicity, which leads to devastating adverse effects and results in a substandard quality of life, including increased daily morbidity and premature mortality. The death receptor of tumor necrosis factor-related apoptosis-inducing ligand (TRAIL) can sidestep p53-dependent pathways to induce tumor cell apoptosis without damaging most normal cells. However, vari

Quality assessment of selected co-trimoxazole suspension brands marketed in Nairobi County, Kenya.

INTRODUCTION: Quality of medicines in both developed and developing countries is sometimes compromised due to infiltration of counterfeit, substandard or degraded medicines into the markets. It is a public health concern as poor quality medicines endanger public health where patients are exposed to chemical toxins and/or sub-therapeutic doses. This could lead to reduced treatment efficacy and promote development of drug resistance. Co-trimoxazole, a fixed dose combination of sulfamethoxazole and trimethoprim, is a broad spectrum for bacterial diseases and is also used as a prophylaxis for oppo

Severe postpartum haemorrhage at a large referral hospital in Uganda: A prospective observational pilot study.

BACKGROUND: Postpartum haemorrhage (PPH) is a leading cause of maternal mortality, near-misses and morbidity in Uganda and globally. Kawempe National Referral Hospital (KNRH), Uganda's largest obstetric referral hospital, receives many obstetric emergencies, including PPH, from lower-level health facilities. Little is known about the outcomes and management of severe PPH at KNRH. This study aimed to map the occurrence, profile and management challenges of severe PPH at KNRH. METHODS: A prospective hospital-based observational pilot study was conducted between 5th April and 30th May 2023 at KN

National maternal mortality ratio in Egypt halved between 1992-93 and 2000.

Two surveys of maternal mortality conducted in Egypt, in 1992-93 and in 2000, collected data from a representative sample of health bureaus covering all of Egypt, except for five frontier governorates which were covered only by the later survey, using the vital registration forms. The numbers of maternal deaths were determined and interviews conducted. The medical causes of death and avoidable factors were determined. Results showed that the maternal mortality ratio (MMR) had dropped by 52% within that period (from 174 to 84/100,000 live births). The National Maternal Mortality Survey in 1992-

Improving the quality of care for mothers, newborns, and children in ten hospitals of the Republic of Tajikistan.

BACKGROUND: Tajikistan has accomplished reductions in maternal, newborn, and child mortality over the past decades through targeted policies and interventions. Challenges remain in providing quality healthcare due to limited resources, geographic barriers, and inadequate infrastructure. We aimed to evaluate the impact of a quality improvement (QI) initiative implemented in ten district hospitals from 2021 to 2024 to improve maternal, newborn, and childcare to accelerate progress towards achieving the Sustainable Development Goals. METHODS: A baseline assessment was conducted in 2021, with an

Identifying and combating the impacts of COVID-19 on malaria.

BACKGROUND: The COVID-19 pandemic has resulted in millions of infections, hundreds of thousands of deaths and major societal disruption due to lockdowns and other restrictions introduced to limit disease spread. Relatively little attention has been paid to understanding how the pandemic has affected treatment, prevention and control of malaria, which is a major cause of death and disease and predominantly affects people in less well-resourced settings. MAIN BODY: Recent successes in malaria control and elimination have reduced the global malaria burden, but these gains are fragile and progres

Malaria in the Greater Mekong Subregion: heterogeneity and complexity.

The Greater Mekong Subregion (GMS), comprised of six countries including Cambodia, China's Yunnan Province, Lao PDR, Myanmar (Burma), Thailand and Vietnam, is one of the most threatening foci of malaria. Since the initiation of the WHO's Mekong Malaria Program a decade ago, malaria situation in the GMS has greatly improved, reflected in the continuous decline in annual malaria incidence and deaths. However, as many nations are moving towards malaria elimination, the GMS nations still face great challenges. Malaria epidemiology in this region exhibits enormous geographical heterogeneity with My

Outbreak of acute hepatitis B virus infection associated with exposure to acupuncture.

BACKGROUND: The most common risk factors for acute hepatitis B virus (HBV) infection are sexual contact, injection drug use and perinatal, or nosocomial exposure. Acupuncture, used in China for over 2,500 years, has been gaining popularity as an alternative medical therapy in the western world, but when associated with poor infection control practices, is also a risk for blood-borne infections. OBJECTIVE: To describe the outbreak investigation following detection of two cases of acute HBV infection associated with acupuncture services from the same provider within four months of symptom onset

Pharmaceutical quality of artemether-lumefantrine in Rwanda's public health supply chain: a cross-sectional post-market surveillance study.

BACKGROUND: Substandard and falsified (S&F) antimalarial medicines undermine malaria control efforts across sub-Saharan Africa, with an estimated 19% of antimalarials failing to meet quality standards and contributing to 40,000-160,000 preventable deaths annually. ACTs, particularly artemether-lumefantrine (AL), are the cornerstone of malaria treatment, yet their effectiveness is threatened by poor pharmaceutical quality and emerging drug resistance. This study evaluated the pharmaceutical quality of AL circulating in Rwanda's public health supply chain to assess compliance with international

Forecasting Asparaginase Need and Cost for Childhood Cancer Using ACCESS FORxECAST.

PURPOSE: Asparaginase (ASN) is a critical component of pediatric ALL protocols. Until recently, ASN was available in three formulations: native Escherichia coli, PEGylated E. coli (PEG), and Erwinase, with native E. coli typically more accessible in low- and middle-income countries (LMICs). Short shelf lives, intermittent availability, and concern for substandard formulations in LMICs have created a need for proactive ASN demand estimates. METHODS: We adapted FORxECAST, a pediatric cancer drug forecasting model, to focus on ASN for pediatric ALL. The model is adaptable to user data and defaul

Microbial Quality, Risk Analysis, and Molecular Detection of Escherichia coli O157 and Salmonella spp. in Fresh Raw Beef Sold in Ho Central Market, Ghana.

Beef is a prominent dietary component in many developing countries, including Ghana, due to its rich protein, energy, and fat contents. However, it has become a significant source of foodborne pathogens, including Escherichia coli O157 and Salmonella, which can lead to severe gastrointestinal infections. This study investigated food hygiene practices and the microbial safety of beef sold at the Ho Central Market in Ghana. Sixty-three beef handlers and processing facilities in 25 shops were conveniently sampled using a structured questionnaire. Twelve beef samples were also collected in duplica

Community-level perceptions of housing services for people with opioid use disorder: a qualitative analysis of community stakeholders in the HEALing communities study.

BACKGROUND: Housing insecurity, or the inability to afford or maintain residence in a safe and quality place, is a barrier to accessing and staying in substance use treatment and/or recovery support services. There is a gap in knowledge in how housing insecurity influences substance use disorder (SUD) prevalence, and the role housing security plays in SUD treatment initiation and retention. The HEALing Communities Study (HCS) is a large multisite trial aiming to reduce overdose risk through the implementation of evidence-based interventions delivered by community coalitions to reduce overdose

Do chain pharmacies perform better than independent pharmacies? Evidence from a standardised patient study of the management of childhood diarrhoea and suspected tuberculosis in urban India.

INTRODUCTION: Pharmacies and drug stores are frequently patients' first point of care in many low-income and middle-income countries, but their practice is often poor. Pharmacy retailing in India has traditionally been dominated by local, individually owned shops, but recent years have seen the growth of pharmacy chains. In theory, lower-powered profit incentives and self-regulation to preserve brand identity may lead to higher quality in chain stores. In practice, this has been little studied. METHODS: We randomly selected a stratified sample of chain and independent pharmacies in urban Beng

Analysis of the nature and contributory factors of medication safety incidents following hospital discharge using National Reporting and Learning System (NRLS) data from England and Wales: a multi-method study.

INTRODUCTION: Improving medication safety during transition of care is an international healthcare priority. While existing research reveals that medication-related incidents and associated harms may be common following hospital discharge, there is limited information about their nature and contributory factors at a national level which is crucial to inform improvement strategy. AIM: To characterise the nature and contributory factors of medication-related incidents during transition of care from secondary to primary care. METHOD: A retrospective analysis of medication incidents reported to

Systemic treatments for metastatic cutaneous melanoma.

BACKGROUND: The prognosis of people with metastatic cutaneous melanoma, a skin cancer, is generally poor. Recently, new classes of drugs (e.g. immune checkpoint inhibitors and small-molecule targeted drugs) have significantly improved patient prognosis, which has drastically changed the landscape of melanoma therapeutic management. This is an update of a Cochrane Review published in 2000. OBJECTIVES: To assess the beneficial and harmful effects of systemic treatments for metastatic cutaneous melanoma. SEARCH METHODS: We searched the following databases up to October 2017: the Cochrane Skin G

Cancer: a looming public health crisis in Africa-a call to accelerate equitable access to medicines.

The global incidence of cancer is on the rise, with Africa experiencing a particularly steep increase in both cancer incidence and mortality. This upward trend is driven by a combination of factors, including population growth, poverty, aging, urbanization, and the adoption of lifestyles that increase the risk of cancer. It is projected that from 2022 to 2045, the incidence of cancer in Africa will increase by 106.8%, while cancer mortality will rise by 111.7% if no action is taken. In contrast, Europe is expected to see increases of only 22.5% in incidence and 32.2% in mortality. Despite glob
