Ankle joint pressure change before and after subtalar joint arthrodesis in varus and valgus malalignment of the tibia.

Purpose: The compensation mechanism of subtalar joint in ankle with varus or valgus deformity is controversial and not well established. This biomechanical study aims to investigate how subtalar joint arthrodesis will affect the ankle joint pressure in varus and valgus malalignment of the tibia. Methods: Eight fresh-frozen human cadaver legs were tested in this study. A custom-made fixture was utilized and a total of 600N was applied to simulate weight-bearing. Intra-articular sensors (TeckScan) were inserted in the ankle joint to demonstrate the ankle joint pressure. Conditions include: Neutr

Spare the Talonavicular Joint! The Role of Isolated Subtalar Joint Fusion in the Treatment of Progressive Collapsing Foot Deformity.

Surgical management of progressive collapsing foot deformity continues to evolve. Previous studies have demonstrated that fusion of the talonavicular joint results in limited hindfoot motion and, therefore, may accelerate adjacent-joint arthrosis. Recent literature has supported using alternative arthrodesis constructs that spare the talonavicular joint, such as naviculocuneiform or isolated subtalar fusions, which may maintain some hindfoot motion through the talonavicular joint yet adequately address a patient's deformity. Concomitant reconstructive procedures may be used in addition to subt

Subtalar Joint Kinematics in Frontal-Plane Inversion-Eversion Using Weightbearing Cone-Beam CT.

BACKGROUND: The purpose of this study was to assess the subtalar joint's kinematics in in the frontal plane (inversion-eversion) using 3D weightbearing cone beam computed tomography by measuring both translational and angular range of motion. METHODS: In this cohort study, weightbearing cone beam computed tomography images were acquired of 41 subjects under weightbearing stress in 20 degrees inversion and 10 degrees eversion tilt, achieved with hard plastic wedges. The ankle was in neutral position, and the sole of the foot was entirely on the wedge. Interobserver and intraobserver reliabilit

Human in vivo midtarsal and subtalar joint kinematics during walking, running and hopping.

The interaction among joints of the midtarsal complex and subtalar joint is important for locomotor function; however, its complexity poses substantial challenges in quantifying the joints' motions. We determine the mobility of these joints across locomotion tasks and investigate the influence of individual talus morphology on their motion. Using highly accurate biplanar videoradiography, three-dimensional bone kinematics were captured during walking, running and hopping. We calculated the axis of rotation of the midtarsal complex and subtalar joint for the landing and push-off phases. A compa

A mouse model of ankle-subtalar joint complex instability induced post-traumatic osteoarthritis.

BACKGROUND: Ankle-subtalar joint complex instability is not uncommonly presented in the clinic, but symptoms and signs similar to other conditions can easily lead to its misdiagnosis. Due to the lack of appropriate animal models, research on ankle-subtalar joint complex instability is limited. The aims of the present study were to establish an animal model of ankle-subtalar joint complex instability in mice and to explore its relationship with post-traumatic osteoarthritis (PTOA). METHODS: Twenty-one male C57BL/6J mice were randomly divided into three groups: SHAM group (sham surgery group),

Bilateral Ankle and Subtalar Joint Fusion Secondary to Guillain Barré-Induced Foot Drop.

Guillain-Barré syndrome is a serious disorder that occurs when the body's immune system mistakenly attacks the peripheral nervous system. This leads to nerve inflammation and damage that can cause muscle weakness and/or paralysis, including foot drop. Therapy ranges from supportive measures, such as physical therapy, to surgical intervention. It appears from the published data that these patients, generally, have been left to their own devices, because no known cure is available, necessitating reliance on others for their daily activities and ambulation. We present a unique surgical option, wi

Subtalar joint axis in patients with symptomatic peritalar subluxation compared to normal controls.

BACKGROUND: The etiology of peritalar subluxation (PTS) is poorly understood and likely mutifactorial. An anatomic predisposition for posterolateral subluxation of the hindfoot has not been previously described or investigated. The aim of the current study was to describe the morphology of the subtalar joint axis (STJA) in patients with symptomatic PTS compared to normal controls. METHODS: We identified patients with symptomatic PTS who had undergone operative correction from hospital records. The angle of the axis of the posterior facet of the subtalar joint was made on simulated weight-bear

Retrospective analysis of the survivability of absorbable versus nonabsorbable subtalar joint arthroereisis implants.

Subtalar joint arthroereisis is a surgical modality that has been shown to be an effective procedure for flexible flatfoot in both pediatric and adult populations. Despite advances in understanding its mechanics and function, complication and implant removal rates remain as high as 30% to 40%. Analysis was performed to determine the survivability of 2 subtalar joint arthroereisis implants, absorbable and nonabsorbable, used alone and in combination with other procedures in both the adult and pediatric populations. The 95 total arthroereisis procedures were analyzed in several major categories:

Subtalar joint dynamics in progressive collapsing foot deformity: A cross-sectional progression model.

BACKGROUND: Peritalar subluxation (PTS) contributes to progressive collapsing foot deformity (PCFD), yet its pathophysiology remains unclear. This study analyzed subtalar joint (SJ) dynamics throughout deformity progression. METHODS: A retrospective review of 153 WBCT scans classified PCFD into four stages: (1) no PTS, (2) PTS without impingement, (3) PTS with sinus tarsi impingement (STI), and (4) PTS with subfibular impingement (SFI). Three-dimensional measurements of middle facet subluxation and incongruence, foot-ankle offset, SJ offset, and lateral process offset were compared between gr

Dynamic subtalar joint space measurement analysis following tibiotalar arthrodesis and total ankle replacement.

BACKGROUND: Tibiotalar arthrodesis and total ankle replacement are current standards for treating end-stage ankle osteoarthritis. Our clinical understanding of biomechanical causes and risk factors for subtalar secondary osteoarthritis development following surgical treatment is limited. The objective of this retrospective study was to investigate subchondral bone morphometric differences of individuals treated with a tibiotalar arthrodesis or ankle arthroplasty that may influence subtalar joint measurements during overground walking; results were compared between surgical groups, their unaffe

Arthroscopy of Foot and Ankle: Subtalar Joint Arthroscopy in Intra-articular Calcaneal Fractures.

The utilization of subtalar joint arthroscopy in intra-articular calcaneal fractures provides optimal visualization of articular surfaces for a more precise anatomical reduction, thus yielding better surgical outcomes. Current literature shows good functional and radiographic outcomes, fewer wound complications, and low incidence of post-traumatic arthritis with this technique than when utilizing an isolated lateral extensile incision of the calcaneus. As subtalar joint arthroscopy continues to grow in popularity and technological advancement, patients may benefit when surgeons incorporate thi

Changes in the subtalar joint alignment after supramalleolar osteotomy for varus ankle arthritis.

BACKGROUND: The subtalar joint may compensate for tibio-talar deformity, but what would happen to the joint after the deformity was corrected is not well known. Supramalleolar osteotomy (SMOT) is an effective procedure for the treatment of varus deformity of ankle arthritis. The objective of this study was to investigate the subtalar joint alignment pre and postoperatively following SMOT, and the factors which influenced the alignment of the subtalar joint. METHODS: Thirty-one patients with varus ankle arthritis (Takakura stage 2, 3a and 3b) who were treated using SMOT were retrospectively re

Subtalar joint pigmented villonodular synovitis misdiagnosed at the first visit: A case report.

BACKGROUND: Pigmented villonodular synovitis (PVNS) is an uncommon disease that usually occurs in large joints, and involvement of the subtalar joint is rare. The lack of comprehensive knowledge of subtalar joint PVNS could lead to misdiagnosis. CASE SUMMARY: We present a 64-year-old woman who, at her first visit, complained of discomfort in the right ankle when she walked. Based on the physical signs and X-ray report, the physician failed to make the suspected diagnosis of PVNS. Eighteen months later, the patient returned with a complaint of a mass in her right lateral malleolus with intermi

Characteristic Bone Morphology Change of the Subtalar Joint in Severe Varus Ankle Osteoarthritis.

The subtalar joint has a possible compensatory mechanism for supramalleolar deformities; the failure of this mechanism causes the progression of ankle osteoarthritis. However, the reason for this failure has not been fully elucidated. This study aimed to evaluate the characteristics of the morphologic changes in the subtalar joint in varus ankle osteoarthritis using computed tomography. The study included 30 patients with severe osteoarthritis (modified Kellgren-Lawrence classification grade ≥ 3; mean age: 68.5 years) and 30 patients without- or with early osteoarthritis (grade 0-1; mean age:

Does the subtalar joint compensate for ankle malalignment in end-stage ankle arthritis?

BACKGROUND: Patients with ankle arthritis often present with concomitant hindfoot deformity, which may involve the tibiotalar and subtalar joints. However, the possible compensatory mechanisms of these two mechanically linked joints are not well known. QUESTIONS/PURPOSES: In this study we sought to (1) compare ankle and hindfoot alignment of our study cohort with end-stage ankle arthritis with that of a control group; (2) explore the frequency of compensated malalignment between the tibiotalar and subtalar joints in our study cohort; and (3) assess the intraobserver and interobserver reliabil

Osteoid osteoma of the talus adjacent to the subtalar joint: a rare location and diagnostic challenge.

INTRODUCTION AND IMPORTANCE: Osteoid osteoma is a common benign bone tumor, but lesions located in the talus, especially those adjacent to the subtalar joint, are relatively rare and prone to misdiagnosis. Its clinical and imaging manifestations are often atypical, which can lead to delayed diagnosis and inappropriate conservative treatment. Accurate imaging evaluation and appropriate surgical management are critical for symptom relief and functional recovery. CASE PRESENTATION: We present a 20-year-old male with chronic left ankle pain that worsened at night and with weight-bearing. Initial

Extrapulmonary, Chronic Septic Arthritis From Mycobacterium tuberculosis in the Ankle and Subtalar Joints.

In the United States, rates of Mycobacterium tuberculosis infection have been declining for decades. Osteoarticular tuberculosis of the ankle is rarely observed. We present the case of a 65-year-old man who immigrated to the United States from India 24 years before the onset of symptoms. The patient initially reported atraumatic swelling and pain of the left ankle and foot and was treated for venous insufficiency. Later, the patient was referred to a nonsurgical orthopaedic clinic for additional workup and was found to have elevated inflammatory markers. MRI showed septic arthritis and osteomy
