Sensory Function Improves With Subthalamic Nucleus Stimulation in Parkinson's: A Network Analysis.

OBJECTIVE: Sensory dysfunction is common in Parkinson disease (PD) and may impair motor function and quality of life. Although subthalamic nucleus deep brain stimulation improves motor symptoms, it also modulates sensory function. This study aimed to identify stimulation sites and structural brain networks associated with improvements in sensory decision making. MATERIALS AND METHODS: Data from 21 patients with PD treated with subthalamic nucleus deep brain stimulation were retrospectively analyzed. Sensory decision making was assessed using the rubber hand illusion. Measures included proprio

Heterogeneity of Subthalamic Nucleus Neuronal Responses to Voluntary Movements in Parkinsonian Patients.

The subthalamic nucleus (STN), an important part of the motor control system, represents a prime target for deep brain stimulation (DBS) in patients with Parkinson's disease (PD). The STN is involved in several pathways regulating the preparation and execution of voluntary movements. However, the neural mechanisms providing motor control in the STN remain unclear. We analysed the single-unit activity and entire spiking activity of the STN in 21 PD patients who underwent standard-of-care DBS implantation procedures. We asked patients to perform similar motor tasks during microelectrode and EMG

Stimulation-Evoked Resonant Neural Activity in the Subthalamic Nucleus Is Modulated by Sleep.

BACKGROUND: Deep brain stimulation is a treatment for advanced Parkinson's disease and currently tuned to target motor symptoms during daytime. Parkinson's disease is associated with multiple nocturnal symptoms such as akinesia, insomnia, and sleep fragmentation, which may require adjustments of stimulation during sleep for best treatment outcome. OBJECTIVES: There is a need for a robust biomarker to guide stimulation titration across sleep stages. This study aimed to investigate whether evoked resonant neural activity (ERNA) is modulated by sleep. METHODS: We recorded local field potentials

Impact of subthalamic nucleus stimulation on urinary dysfunction and constipation in Parkinson's disease.

OBJECTIVE: The effect of subthalamic nucleus (STN) deep brain stimulation (DBS) on urinary dysfunction and constipation in Parkinson's disease (PD) is variable. This study aimed to identify potential surgical and nonsurgical variables predictive of these outcomes. METHODS: The authors used the Movement Disorder Society-Unified Parkinson's Disease Rating Scale (MDS-UPDRS) Part I to assess urinary dysfunction (item 10) and constipation (item 11) preoperatively and at 6-12 months postoperatively. A multiple linear regression model was used to investigate the impact of global cerebral atrophy (GC

Bilateral deep brain stimulation of the subthalamic nucleus in primary Meige syndrome.

OBJECTIVE Subthalamic nucleus deep brain stimulation has been shown to be effective in reducing symptoms of primary Meige syndrome. However, assessments of its efficacy and safety have been limited to several case reports and small studies. METHODS The authors performed a retrospective study to assess the efficacy and safety of bilateral subthalamic nucleus stimulation in 15 patients with primary Meige syndrome who responded poorly to medical treatments or botulinum toxin injections. Using the movement and disability subscores of the Burke-Fahn-Marsden Dystonia Rating Scale, the authors evalua

Reduced cortical innervation of the subthalamic nucleus in MPTP-treated parkinsonian monkeys.

The striatum and the subthalamic nucleus are the main entry points for cortical information to the basal ganglia. Parkinson's disease affects not only the function, but also the morphological integrity of some of these inputs and their synaptic targets in the basal ganglia. Significant morphological changes in the cortico-striatal system have already been recognized in patients with Parkinson's disease and in animal models of the disease. To find out whether the primate cortico-subthalamic system is also subject to functionally relevant morphological alterations in parkinsonism, we used a comb

Localization and function of dopamine receptors in the subthalamic nucleus of normal and parkinsonian monkeys.

The subthalamic nucleus (STN) receives a dopaminergic innervation from the substantia nigra pars compacta, but the role of this projection remains poorly understood, particularly in primates. To address this issue, we used immuno-electron microscopy to localize D1, D2, and D5 dopamine receptors in the STN of rhesus macaques and studied the electrophysiological effects of activating D1-like or D2-like receptors in normal and 1-methyl-4-phenyl-1,2,3,6-tetrahydropyridine (MPTP)-treated parkinsonian monkeys. Labeling of D1 and D2 receptors was primarily found presynaptically, on preterminal axons

Deep brain stimulation of the subthalamic nucleus improves pain in Parkinson's disease.

BACKGROUND: In Parkinson's disease (PD), chronic pain is a common symptom which markedly affects the quality of life. Some physiological arguments proposed that Deep Brain Stimulation of the Subthalamic Nucleus (STN-DBS) could improve pain in PD. METHODS: We investigated in 58 PD patients the effect of STN-DBS on pain using the short McGill Pain Questionnaire and other pain parameters such as the Bodily discomfort subscore of the Parkinson's disease Questionnaire 39 and the Unified Parkinson's Disease Rating Scale section II (UPDRS II) item 17. RESULTS: All pain scores were significantly imp

Effect of frequency on subthalamic nucleus deep brain stimulation in primary dystonia.

BACKGROUND: Subthalamic nucleus deep brain stimulation (DBS) is an alternative target choice for treating primary dystonia, but little is known about the most effective programming parameters. OBJECTIVE: Here we prospectively evaluate the effect of low versus high frequency subthalamic nucleus DBS in patients with predominantly cervical or upper extremity primary dystonia. METHODS: Seven patients were stimulated at low frequency stimulation (60 Hz) for the first three months and then switched to high frequency stimulation (130 Hz) until month six. Severity of dystonia was determined by a bli

Enhanced low-frequency oscillatory activity of the subthalamic nucleus in a patient with dystonia.

BACKGROUND: Local field potentials were recorded from the subthalamic nucleus (STN) in a patient with dystonia to further elucidate disease-specific aspects of basal ganglia oscillatory activity. METHODS: STN local field potentials and electromyograms (EMGs) from dystonic muscles were recorded to provide an estimate of the power spectra and coherence between the STN activity and EMG. RESULTS: STN power spectra revealed a distinct peak at approximately 7 Hz in our patient. This finding is similar to the pallidal activity seen in dystonic patients but clearly different from the subthalamic bet

Involvement of the subthalamic nucleus in impulse control disorders associated with Parkinson's disease.

Behavioural abnormalities such as impulse control disorders may develop when patients with Parkinson's disease receive dopaminergic therapy, although they can be controlled by deep brain stimulation of the subthalamic nucleus. We have recorded local field potentials in the subthalamic nucleus of 28 patients with surgically implanted subthalamic electrodes. According to the predominant clinical features of each patient, their Parkinson's disease was associated with impulse control disorders (n = 10), dyskinesias (n = 9) or no dopaminergic mediated motor or behavioural complications (n = 9). Subthalamic nucleus stimulation in Parkinson's disease is associated with a risk of fixed epiglottis.

Deep brain stimulation (DBS) of the subthalamic nucleus (STN) alleviates motor disability of patients with Parkinson's disease (PD). Mental changes and other adverse events are common, but typically transient. Severe complications such as intracerebral haemorrhage or infection are rare, but 6 of 73 patients who underwent STN-DBS died of pneumonia, cardiac failure or pulmonary embolism. We describe a patient with PD who had sudden respiratory difficulty due to a fixed epiglottis after STN-DBS. This symptom was confirmed to be related to STN stimulation on fibre-optic examination of the larynx.

Hypomania as an adverse effect of subthalamic nucleus stimulation: report of two cases.

Mania following subthalamic nucleus (STN) deep brain stimulation (DBS) is well described and obvious to both the patient and their physician. The authors describe two patients who developed hypomania following STN-DBS but were unaware of their mood disturbance. Two Parkinson's patients with no previous mood disorders received bilateral STN electrodes. Both experienced dramatic improvement in their motor function and neither complained of any side effects. Their families reported detrimental hypomanic behaviour. Readjusting the stimulation parameters resolved the hypomania with continued motor

Bilateral subthalamic nucleus stimulation results in reversal of alopecia in Parkinson's disease.

Alopecia is a rare but important side effect of anti-parkinsonian medications. Our patient was a 72-year-old man with advanced Parkinson's disease (PD) who received levodopa and anti-cholinergic drugs and whose head had become almost completely bald. As bilateral subthalamic nucleus (STN) stimulation produced improvement in his motor symptoms, his drug dosages were reduced postoperatively. At 8 months after surgery, hair again covered his entire head. Our study presents a new aspect of the benefit of STN stimulation with regard to drug-induced non-motor symptoms in patient with PD.

Unilateral subthalamic nucleus lesioning: a safe and effective treatment for Parkinson's disease.

The present study, the largest in the literature, was performed to assess the effectiveness and safety of unilateral subthalamic nucleus (STN) lesioning for Parkinson's disease (PD). From August 1999 to September 2000, 21 consecutive patients evaluated pre- and postoperatively by a single examiner were operated. Levodopa intake and dyskinesia, Hoehn & Yahr, Schwab & England and UPDRS motor scores were recorded. Stereotactic CT and MRI and the effects of macrostimulation were used to determine STN coordinates. A single radiofrequency lesion was made (60-75 degrees C/60"). Concomitant ipsilatera

Subthalamic nucleus lesions induce deficits as well as benefits in the hemiparkinsonian rat.

Lesions of the subthalamic nucleus can restore some imbalances in motor output of the basal ganglia induced by nigrostriatal dopamine depletion, and have been proposed as a potential therapy for Parkinson's disease. Although there is substantial supporting evidence from experimental studies in both rats and primates, there is less information on the effects of subthalamic lesions alone. In order to characterize potential side effects, the present study evaluates the behavioural effects of unilateral excitotoxic lesions of the subthalamic nucleus in rats that have previously received either uni

Effects of subthalamic nucleus lesions in a putative model of tardive dyskinesia in the rat.

The effects of bilateral excitotoxic lesions of the subthalamic nucleus on vacuous chewing movements induced by chronic neuroleptic therapy were examined in the rat. Fluphenazine decanoate (25 mg/kg i.m.q 3 weeks x 24 weeks) induced vacuous chewing movements, as previously described. This response was suppressed to control levels in animals tested 1-3 weeks following bilateral infusion of quinolinic acid (100 nmol/1 microliter per side) into the subthalamic nucleus. Subthalamic nucleus lesions resulted in increased locomotion and sniffing in neuroleptic-naive animals, but these responses were
