[Evaluation of the efficacy of colistin/sulbactam combination on carbapenem-resistant Acinetobacter baumannii strains].

Acinetobacter baumannii strains, are opportunistic pathogens that cause severe nosocomial infections that are difficult to treat due to development of resistance to multiple antibiotics. As the antibiotic choices to be used in treatment are limited, combinations of a variety of antibiotics are used. The aims of this study were to identify the minimal inhibitory concentration (MIC) values of colistin and sulbactam against A.baumannii isolates and to determine the in vitro activity of colistin-sulbactam combination. A total of 50 A.baumannii strains isolated from different clinical specimens (32

Randomized comparison of sulbactam/cefoperazone with imipenem as empirical monotherapy for febrile granulocytopenic patients.

In a prospective, randomized, controlled trial, we compared sulbactam/cefoperazone with imipenem as empirical monotherapy for febrile, granulocytopenic patients; 101 patients received sulbactam/cefoperazone (2 g/4 g every 12 hours) and 102 patients received imipenem (500 mg every 6 hours). Documented infections were present in 40% of patients treated with sulbactam/cefoperazone (40 of 101) and in 39% of patients receiving imipenem (40 of 102). The number of pretherapy gram-positive pathogens (52 isolates) was twice the number of pretherapy gram-negative pathogens (26 isolates). The overall fav

A comparison of ampicillin plus sulbactam versus clindamycin and gentamicin for treatment of postpartum infection.

Thirty-six hospitalized patients, 18 in each of two groups, with postpartum upper genital tract infection were enrolled in a randomized, prospective study comparing treatment with sulbactam/ampicillin, to treatment with clindamycin/gentamicin. One (5.5%) clinical failure was reported in each group. Side effects were minimal in both groups and did not warrant discontinuation of treatment. The in vitro activity of ampicillin versus sulbactam/ampicillin (1:2) was evaluated and these data were compared with data from other drugs commonly used for aerobic and anaerobic infections. Sulbactam elimina

Sulbactam-durlobactam and cefiderocol combination treatment of Burkholderia cenocepacia-associated Fitz-Hugh-Curtis syndrome.

A 50-year-old woman with cystic fibrosis (CF) and lung transplantation developed persistent deep abscesses due to multidrug-resistant (MDR) Burkholderia cenocepacia, unresponsive to multiple antibiotics and surgeries. Combination therapy with sulbactam-durlobactam (SUL-DUR) and cefiderocol (FDC) along with surgical interventions led to clinical and radiographic stability. This report of a successful sulbactam-durlobactam and cefiderocol treatment highlights this targeted β-lactam-β-lactamase inhibitor combination as an option for refractory B. cepacia complex (BCC) infections.

Glutamine Potentiates Cefoperazone-Sulbactam against Klebsiella pneumoniae by Enhancing Cellular Uptake and ROS Production.

Antibiotic-resistant Klebsiella pneumoniae poses a serious global challenge to clinical practice and public health, highlighting an urgent need for effective therapeutic strategies. This study systematically evaluated the preclinical pharmacodynamics of cefoperazone-sulbactam (SCF) in combination with glutamine against K. pneumoniae. Clinically isolated strains were classified into three groups: 56 sensitive (S-KP), 62 multidrug-resistant but carbapenem-sensitive (MDR-KP), and 195 multidrug-resistant and carbapenem-resistant (CR-KP) isolates. Compared with SCF alone, the combination with gluta

Sulbactam-durlobactam plus ceftriaxone dosing and novel treatment regimens for Mycobacterium abscessus lung disease.

The guideline-based therapy achieves sputum culture conversion rates in 20%-34% of patients with Mycobacterium abscessus (MAB) lung disease (LD). Double-β-lactam combinations have been proposed to improve cure based on time-kill curves. We tested MICs followed by hollow fiber system model of MAB (HFS-MAB) exposure-effect studies with sulbactam-durlobactam administered every 8 h (q8h), q12h, and q24h, to identify target exposures. Next, the sulbactam-durlobactam target exposure plus ceftriaxone was administered in the HFS-MAB inoculated with three different MAB isolates, as was the sulbactam-du

Glutamine Potentiates Cefoperazone-Sulbactam Activity against Acinetobacter baumannii by Increasing Drug Uptake and ROS.

The combination of an antibiotic with a metabolic reprogramming agent is anticipated to emerge as a promising therapeutic strategy against antibiotic-resistant bacteria, although this hypothesis requires validation through preclinical pharmacodynamic studies. This study evaluated the preclinical pharmacodynamic profile of cefoperazone-sulbactam (SCF) combined with glutamine against 237 Acinetobacter baumannii clinical isolates, including 26 antibiotic-sensitive (S-AB), 8 multidrug-resistant (MDR-AB), and 203 carbapenem-resistant strains (CR-AB). Sulbactam-durlobactam improves cephalosporin and carbapenem susceptibility and time-kill effect against Mycobacterium abscessus.

Despite the long therapy duration, the treatment outcomes for Mycobacterium abscessus lung disease (MAB-LD) are very poor. β-Lactams are among the recommended drugs for the treatment of MAB-LD; however, they are prone to hydrolysis by MAB β-lactamase enzymes. Therefore, β-lactamase inhibitors have been developed to overcome this problem. Here, we performed minimum inhibitory concentration studies with a reference MAB strain (ATCC19977) and a collection of 63 clinical isolates for penicillin, cephalosporin, and carbapenems alone or in combination with β-lactamase inhibitors, sulbactam/durlobact

A rabbit model for bacterially induced preterm pregnancy loss: intervention studies with ampicillin-sulbactam.

We conducted experiments with a previously described rabbit model of Escherichia coli-induced preterm pregnancy loss. Does at 70% gestation were inoculated hysteroscopically with 0.2 ml of Escherichia coli (10(5) colony-forming units per milliliter) or saline solution. Animals were randomly assigned to either receive treatment with ampicillin-sulbactam (begun 1 to 2 hours before inoculation and continued for up to 7 days) or to receive no therapy. Animals were killed after delivery or after 7 days. Saline solution-inoculated animals had no pregnancy loss. Of the Escherichia coli-inoculated ani

Sulbactam-Durlobactam for Carbapenem-Resistant Acinetobacter baumannii in a Child: A Novel Application of Adult Pharmacokinetics.

There are limited antimicrobial options for the treatment of carbapenem-resistant Acinetobacter baumannii (CRAB). Sulbactam-durlobactam was recently approved for the treatment of CRAB in patients 18 years and older. However, CRAB infections can have mortality rates up to 80% in the intensive care unit. We describe the successful novel use of sulbactam-durlobactam in a pediatric patient using available adult pharmacokinetic data and preliminary pediatric dosing suggestions from a phase 1b trial.

Role of sultamicillin and ampicillin/sulbactam in the treatment of upper and lower bacterial respiratory tract infections.

The emergence of beta-lactamase-mediated resistance to beta-lactam antibiotics among key respiratory tract pathogens has threatened the usefulness of the beta-lactam agents familiar to physicians as being clinically effective and well tolerated. This article reassesses the clinical usefulness of ampicillin when administered in combination with the beta-lactamase inhibitor sulbactam, either intravenously or orally (as the mutual prodrug sultamicillin), in the treatment of upper and lower respiratory tract infections. Numerous clinical studies and several meta-analyses indicate that ampicillin/s
