BACKGROUND: The application of radiofrequency (RF) has been successfully used in the treatment of chronic pain conditions, including facet arthropathy, sacroiliac joint pain, groin pain, radicular pain, cervicogenic headaches, and phantom limb pain. Due to the neurodestructive effect of continuous RF ablation and possible deafferentation sequelae, only pulsed radiofrequency (PRF) has been applied to peripheral sensory nerves. There are no previous reports of successful PRF application to the sural nerve.
OBJECTIVES: To report on the successful use of PRF to the sural nerve for the treatment o

BACKGROUND: Damage to the sural nerve (SuN) may arise from surgical stripping or thermal ablation of the small saphenous vein (SSV).
OBJECTIVE: This study aims to demonstrate that visualisation of the SuN and its point of contact with the SSV ('risk point') using ultrasound imaging can be achieved in routine clinical practice.
TYPE OF STUDY: This is a cohort study.
PATIENTS: Fifteen normal subjects and five patients with chronic venous insufficiency (CVI) (two with a dilated, incompetent SSV).
METHOD: The SuN was identified using high-resolution ultrasound imaging using 14- and 18-MHz prob

BACKGROUND: Sural nerve biopsy (SNBx) is associated with multiple complications such as paresthesia, pain, or numbness in the sural nerve distribution at the site of biopsy and wound infection. An accurate idea of these adverse events would be useful while taking informed consent from patients.
AIMS: We conducted a prospective study to determine the extent of sensory deficits after SNBx.
SETTINGS AND DESIGN: It is a prospective, hospital-based (tertiary teaching hospital) study.
MATERIALS AND METHODS: All the patients who had SNBx between May 2003 and March 2004 were eligible for inclusion.

Sensory nerve conduction (NC) studies that were performed on patients with Guillain-Barre syndrome (GBS) were reviewed. These were compared with the sensory NC findings in patients with peripheral neuropathy of the axonal-loss type (axonopathies) and chronic inflammatory demyelinating polyneuropathy (CIDP). In GBS, the sural sensory nerve frequently showed normal NC findings compared to the median and ulnar sensory nerves. The reverse pattern was seen in axonopathies and CIDP, in which the sural nerve showed more abnormalities than the median and ulnar nerves. This pattern of sensory NC abnorm

A 43-year-old man developed progressive calf pain and paresthesias after a strain of the right gastrocnemius muscle. Physical examination revealed sensory loss in the distribution of the sural nerve with a positive Tinel's sign in the posterior calf in the region of his previous injury. Electrodiagnostic studies revealed prolongation of the sural distal latency with reduction of the sensory nerve action potential amplitude. Magnetic resonance imaging of the lower right leg demonstrated abnormal signal in the right medial gastrocnemius consistent with a subacute hematoma. Surgical exploration r

The sural nerve of a 31-year-old man with Lesch-Nyhan syndrome obtained at autopsy was studied histologically. Large, myelinated nerve fibers were reduced in number and no myelinated nerve fibers larger than 5 microns were seen. Diameter distribution of myelinated nerve fibers did not demonstrate a bimodal pattern. The density of myelinated nerve fibers was 5,530/mm2 and was decreased as compared to the controls. On electron microscopic examination, lipid-like inclusions were observed in the cytoplasm of some Schwann cells. The role of these inclusions could not be elucidated, but reduction of

We studied the prolonged injury discharge in axotomized myelinated (A) and unmyelinated (C) nerve fibres within the first 15 min following sural nerve transection of the rat. Immediately after axotomy, 18% (22/122) of the C fibres generated action potentials, but none of 73 A fibres. This discharge ceased during the first 2 min after axotomy in most of the C fibres. The discharge frequency in the first 15 s after nerve cut varied between 0.3 and 7.2 Hz (mean 2 Hz) and decreased to a mean of 0.3 Hz 4 min after nerve lesion. C fibres displaying initial discharge frequencies of > or = 2 Hz ceased

Regeneration after severe peripheral nerve injury is often poor. Knowledge of human nerve regeneration and the growth microenvironment is greatly lacking. We aimed to identify the regenerative proteins in human peripheral nerve by comparing the proteome before and after a transection injury. In a unique study design, we collected closely matched samples of naïve and injured sural nerve. Naïve and injured (two weeks after injury) samples were analyzed using mass spectrometry and immunoassays. We found significantly altered levels following the nerve injury. Mass spectrometry revealed that injur

Intact and injured cutaneous C-fibers in the rat sural nerve are cold sensitive, heat sensitive, and/or mechanosensitive. Cold-sensitive fibers are either low-threshold type 1 cold sensitive or high-threshold type 2 cold sensitive. The hypothesis was tested, in intact and injured afferent nerve fibers, that low-threshold cold-sensitive afferent nerve fibers are activated by the transient receptor potential melastatin 8 (TRPM8) agonist menthol, whereas high-threshold cold-sensitive C-fibers and cold-insensitive afferent nerve fibers are menthol insensitive. In anesthetized rats, activity was re

Neural fibrolipomatous hamartoma is a rare benign tumour commonly involving the median nerve. Other less frequently involved nerves include the ulnar, radial, brachial plexus, superficial peroneal nerve, inferior calcaneal nerve and median plantar nerve. Involvement of sural nerve has not been reported in the available literature so far. A three-year-old female child presented with a painless swelling over the posterolateral aspect of left leg with no associated motor or sensory deficits. Radiological investigations revealed a fat density lesion with interspersed neural element in the subcutan

BACKGROUND: The sural nerve is formed by the union of the medial and lateral cutaneous nerves of the leg that originate from the tibial and common peroneal nerves. Operative procedures and traumatic injuries to the popliteal fossa, leg, ankle and foot place the sural nerve and its branches at risk. The aim of this study was to describe the course, variations and some clinically significant relations of the sural nerve.
METHODS: The sural nerve was dissected in 30 lower limbs (leg-ankle-foot) of 15 cadavers. The specimens were measured, drawn and photographed.
RESULTS: In 18 specimens (60%) t

We present the case of a 29-year-old electric engineer with compression neuropathy of the left sural nerve due to occupational boots. Routine nerve conduction study of the sural nerve was normal. However, the sensory nerve action potential was not detected more than 3 cm distal to the lateral malleolus, although it returned to normal values after three years. Inching method of the sural nerve may be necessary for detecting compression sural neuropathy distal to the ankle.

To investigate the histological changes in the sural nerve by the damage to the centrally projected axon (CPA), we examined light-microscopic findings after dorsal rhizotomy. The L4-6 dorsal roots were cut unilaterally near the entry zone, with minimal laminectomy, in 27 anesthetized 8- to 10-week-old Sprague-Dawley rats. Animals were killed 1, 2, 4 and 8 weeks and 6 months after surgery, and morphometric results from the sural nerves on the side of the rhizotomy were compared with those from the unaffected side. There were no significant differences in total number, density, frequency distrib

The calibers and microtubular content of axons were studied in normal and regenerating fibers of the sural nerve from 17 to 122 days after a lesion of the sciatic nerve of young adult rats. During this period (70-175 days of age), the cross-sectional area of control myelinated axons almost doubled but that of nonmedullated axons did not change. In regenerating nerves, after 122 days of recovery, the cross-sectional area of myelinated fibers was still 38% below that of the normal side. In contrast, the regenerating nonmedullated population was richer in fine (less than 0.2 micron2) and in coars

Banded structures are reported to occur in the thickened perineurium of four normal human sural nerves (material from autopsy cases: one female and three males between 64 and 90 years old; three died of secondary complications of atherosclerosis, one died from cerebral contusion after an accident). The structures were found adjacent to typical perineurial cells as well as to fibroblast-like cells, and in association with an increased amount of basement membrane material. The pathologic significance of these structures is briefly discussed.

BACKGROUND: Endoscopic thoracic sympathectomy (ETS) may provide a permanent surgical cure for primary palmar hyperhidrosis. Unfortunately, some patients can experience intensive post-operative compensatory sweating (CS) that ultimately impairs quality of life. Sympathetic nerve reconstruction (SNR) may be used to counteract severe post-operative CS through the restoration of sympathetic pathways. In this case series, we describe the technical feasibility of a robot-assisted micro-peripheral nerve reconstruction method for achieving SNR in patients with post-operative CS.

OBJECTIVE: Autologous nerve grafts are used to treat severe peripheral nerve injury, but recovery of nerve function after grafting is rarely complete. Exogenous application of neurotrophic factors may enhance regeneration, but thus far the application of neurotrophic factors has been hampered by fast degradation after local application and unwanted side effects after systemic application. These problems may be overcome with the use of lentiviral (LV) vectors that direct sustained local transgene expression in cells.
METHODS: Human sural nerve segments were either submerged in or injected with

BACKGROUND: There are several documented variations of the sural nerve noted within the literature. This cadaveric study was conducted to provide a surgical perspective to either directly addressing the sural nerve, or avoiding it depending on the injury or pathology being surgically addressed.
METHODS: Twenty-five preserved cadaveric limbs were dissected from the popliteal fossa to the foot. Three locations of the course of the sural nerve data were analyzed. The first location (M1) was from the head of the fibula to the sural nerve. The second location (M3) was from 2.54 cm proximal to the

OBJECTIVE: The common fibular nerve (CFN) is the most frequently injured nerve in the lower limbs. Surgical management is necessary in approximately two thirds of patients and includes neurolysis, suture, graft repair, or nerve transfer. The distal sural nerve is the preferred donor for grafting, but it is not without complications and requires a second incision. We sought to study the surgical anatomy of the lateral sural cutaneous nerve (LSCN) with the aim of repairing CFN injuries through the same incision and as a potential source for grafting in other nerve injuries.
METHODS: The poplite
