BACKGROUND: Facial synkinesis (FS) is a common and disabling sequelae of facial nerve injury, yet no validated, condition-specific patient-reported outcome measure (PROM) exists to assess its functional and psychosocial effects. This study aimed to develop and validate a reliable, patient-centered instrument for individuals with FS.
METHODS: This prospective, multi-phase validation study (2023-2025) was performed at a tertiary academic medical center in accordance with established recommendations for PROM development and reported following guidelines for observational studies. The process inc

BACKGROUND: Treatment selection for postparalysis facial synkinesis (PPFS) is a multifaceted decision for patients. This study aimed to identify patient factors influencing treatment choice between chemodenervation with onabotulinumtoxinA alone and synkinesis surgery.
METHODS: PPFS patients presenting between 2009 and 2024 were grouped into those choosing only chemodenervation and those choosing synkinesis surgery. Excluded were untreated patients and those without qualifying photographs at least 16 months after paralysis onset. Electronic Facial Paralysis Assessment was used to quantify synk

BACKGROUND: Oral-ocular synkinesis is a frequent sequela of idiopathic facial paralysis (IFP), yet objective, dynamic methods to quantify associated palpebral fissure changes remain limited.
OBJECTIVE: To objectively measure palpebral changes in IFP using automated facial tracking.Design Type: Case-control.
METHODS: A novel, video-based facial tracking software assessed facial expressions in 12 controls and 30 IFP cases with synkinesis at least 12 months from onset of IFP. Palpebral fissure dimensions were measured at rest and during smile, laugh, and pucker. Bilateral differences across exp

Oculomotor synkinesis is an unusual phenomenon that encompasses various presentations with few associations reported in the literature. It can occur with or without a previous setting of oculomotor nerve injury. The authors present the first reported case of bilateral congenital trochlear-oculomotor synkinesis in a 5-month-old infant. [J Pediatr Ophthalmol Strabismus. 2024;61(5):e50-e53.].

Background: While there has been great interest in offering selective neurectomy (SN) to patients with nonflaccid facial palsy (NFFP), postoperative outcomes are inconsistent. Objective: To assess overall SN outcome in NFFP patients and to examine correlation between preoperative factors and SN outcome. Methods: SN cases were retrospectively identified between 2019 and 2021. Patient factors and facial function were assessed using chart review, the Facial Clinimetric Evaluation (FaCE), the electronic clinician-graded facial function tool (eFACE), and an automated computer-aided facial assessmen

INTRODUCTION: Synkinesis is a disabling sequelae of facial palsy, which worsens facial asymmetry and diminishes the patients' quality of life. Their physiopathology is partially known, but cannot explain all the synkinesis.
MATERIAL AND METHODS: We report a literature review of the state of the art concerning the knowledge on synkinesis physiopathology, as well as their management.
RESULTS: It is accepted that the physiopathological mechanism of synkinesis is mixed. The phenomena of cerebral plasticity, aberrant nerve regrowth, hyperexcitability of the facial nerve nucleus and ephaptic nerve

Oculomotor synkinesis is the involuntary movement of the eyes or eyelids with a voluntary attempt at a different movement. The chemokine receptor CXCR4 and its ligand CXCL12 regulate oculomotor nerve development; mice with loss of either molecule have oculomotor synkinesis. In a consanguineous family with congenital ptosis and elevation of the ptotic eyelid with ipsilateral abduction, we identified a co-segregating homozygous missense variant (c.772G>A) in ACKR3, which encodes an atypical chemokine receptor that binds CXCL12 and functions as a scavenger receptor, regulating levels of CXCL12 av

Facial synkinesis is an involuntary activation of muscles innervated by the zygomatic or mandibular branch of the facial nerve in conjunction with voluntary activation of the other branch. It appears frequently after recovery from peripheral facial nerve paralysis. We report 10 patients with facial synkinesis following Bell's palsy with a mean duration of synkinesis of 7 +/- 4 years before treatment with periorbital injections of Botulinum toxin type A. 9 had marked subjective and objective improvement starting a few days after injection and lasting 4-9 months. The results suggest a useful tre

Background: Facial synkinesis can lead to unintended activation of perinasal muscles, contributing to dynamic nasal obstruction. Objective: To determine whether targeted botulinum toxin injections, compared to no treatment, reduce new ipsilateral nasal obstruction symptoms in patients with facial synkinesis, as measured by patient-reported outcomes and the Nasal Obstruction Symptom Evaluation (NOSE) scale. Methods: A retrospective review was conducted of patients presenting with ipsilateral nasal obstruction and synkinesis at a multidisciplinary facial nerve clinic. Botulinum toxin (2-2.5 unit

Background: Selective neurectomy (SN) typically leaves cut nerve endings to be either free-floating or buried in facial muscles. Regenerative peripheral nerve interfaces (RPNIs) use autologous skeletal muscle grafts to provide a nonfacial muscle target for reinnervation. Objective: To evaluate the effectiveness of RPNI surgery with SN for improving postoperative facial function through botulinum toxin use and facial movement metrics. Methods: This was a retrospective cohort study comparing patients with synkinesis undergoing SN with and without RPNI surgery. Outcomes included assessing botulin

Ocular-oral synkinesis refers to the unintended contraction of the perioral muscles during voluntary eye closure. This phenomenon can either be congenital or occasionally develop during recovery from facial palsy. In this study, we report a case of ocular-oral synkinesis without facial palsy after trauma.

BACKGROUND: Patients with severe oro-ocular synkinesis often present with concomitant inefficient smile excursion on the affected site. In theory, oculo-zygomatic nerve transfer may decrease synkinesis and improve smile by redirecting nerve fibers to their target muscle. The aim of this study was to explore the feasibility of nerve transfer in human cadavers between a caudal branch innervating the orbicularis oculi to a cephalad branch innervating the zygomaticus major muscles.
METHODS: Eighteen hemi-faces were dissected. Reach for direct coaptation of a caudal nerve branch innervating the or

OBJECTIVES/HYPOTHESIS: Using surface electrostimulation, we aimed to use facial nerve mapping (FNM) in healthy subjects and patients with postparetic facial synkinesis (PPFS) to define functional facial target regions that can be stimulated selectively.
STUDY DESIGN: Single-center prospective cohort study.
METHODS: FNM was performed bilaterally in 20 healthy subjects and 20 patients with PPFS. Single-pulse surface FNM started at the main trunk of the facial nerve and followed the peripheral branches in a distal direction. Stimulation started with 0.1 mA and increased in 0.1 mA increments. Th

OBJECTIVE: This paper presents a rare case of oculostapedial synkinesis.Case reportAfter partial resolution of an idiopathic facial palsy, a male patient presented with persistent distortion of hearing when blinking and closing his eye. Audiometry findings were unremarkable, and cross-sectional imaging of the facial nerve revealed no abnormalities apart from an incidental contralateral meningioma. Initial conservative management, with referral to a specialist physiotherapist, failed to resolve the symptoms. The patient subsequently opted for surgical intervention, and underwent a transmeatal t

BACKGROUND: Facial synkinesis describes a sequela of transient facial nerve injury that can occur following viral reactivation syndromes such as Bell's palsy or following traumatic facial nerve injury. Facial synkinesis is frequently characterized by muscle hypertonicity, involuntary co-contraction of facial muscles, and spasm and can have severe functional and esthetic consequences. Although this condition is not curable, several treatment options exist; however, there is currently a lack of consensus regarding the appropriate timeline and sequence of synkinesis treatment strategies.

BACKGROUND: Synkinesis is commonly encountered after flaccid facial paralysis and can have a detrimental impact on a patient's life. First-line treatment of synkinesis is chemodenervation with botulinum toxin (Botox) and neuromuscular retraining. Surgical options include selective myectomy, selective neurectomy (SN), cross-facial nerve grafting, nerve substitution, and free gracilis muscle transfer. Data on surgical management of synkinesis using SN is limited.

All patients with postparalytic facial paralysis are at risk of developing synkinesis due to aberrant nerve regeneration. Synkinesis can result in smile dysfunction, tension, and eyelid aperture narrowing due to overactive and uncoordinated muscle activity. When the synkinesis causes an asymmetric smile, there are several treatment modalities including neurotoxin, neuromuscular retraining, and surgery. Modified selective neurectomy of the facial nerve is a treatment option that potentially can improve the smile mechanism by reducing the activity of counterproductive facial muscles while preser
