Management of sagittal synostosis: a solution to equipoise.

BACKGROUND: In the craniofacial surgery literature, there is a wide disparity of opinions regarding the appropriate treatment of nonsyndromic sagittal synostosis. With the lack of level 1 evidence to support a particular regimen, our study aims to elucidate the current state of practice among craniofacial surgeons with the hope of establishing a standard of care. METHODS: An internet-based survey was sent to 102 craniofacial surgeons in 14 countries on 4 continents. Data were collected regarding the following parameters: primary indication for surgery, preference of timing, and choice of oper

The developmental spectrum of proximal radioulnar synostosis.

OBJECTIVE: Proximal radioulnar synostosis is a rare upper limb malformation. The elbow is first identifiable at 35 days (after conception), at which stage the cartilaginous anlagen of the humerus, radius and ulna are continuous. Subsequently, longitudinal segmentation produces separation of the distal radius and ulna. However, temporarily, the proximal ends are united and continue to share a common perichondrium. We investigated the hypothesis that posterior congenital dislocation of the radial head and proximal radioulnar fusion are different clinical manifestations of the same primary develo

Is isolated sagittal synostosis an isolated condition?

BACKGROUND: Scaphocephaly is the most common type of craniosynostosis. In the medical literature, there is little information about the association of scaphocephaly and other congenital anomalies. PATIENTS AND METHODS: To determine the prevalence of genetic anomalies in scaphocephaly patients, a retrospective review of the medical charts of 30 consecutive patients was performed. A questionnaire was sent to parents to evaluate demographics. RESULTS: The male-female ratio was 2:1, 22% of patients were born at less than 37 weeks gestation, and 4% of the patients were twins. Nineteen percent of

Development of plagiocephaly in rabbits with unicoronal synostosis.

Human unicoronal synostosis results in plagiocephaly of the cranial vault due to predictable compensatory growth patterns of the contralateral coronal, sagittal, and ipsilateral squamosal sutures. The present study describes the development of plagiocephaly and tests compensatory growth predictions in a naturally occurring rabbit model of uncorrected unicoronal synostosis. Cranial vault and sutural growth data were collected from serial x-ray films in 70 normal rabbits and 19 rabbits with congenital unicoronal synostosis from 1.5 to 18 weeks of age. One-way analysis of variance results reveale

Tibiofibular synostosis: a cause of ankle disability.

A syndrome of ankle pain on weight-bearing while running due to post-traumatic ossification of the tibiofibular ligament is described. Pain is caused by failure of normal downward and lateral motion of the fibula. Treatment consists of complete excision of the synostosis, followed by cast immobilization for 3 weeks.

Cranial Vault Distraction in Nonsyndromic Sagittal Synostosis.

A multitude of operations exists for the treatment of patients with sagittal craniosynostosis presenting after 6 months of age, of which total cranial vault remodeling now provides the most reliable aesthetic outcome. As our national craniofacial center evolved and began to develop a comprehensive multi-disciplinary team to manage complex surgical cases, we offered cranial vault distraction as an alternative to more invasive surgery for late presentation nonsyndromic scaphocephaly. The authors conducted a retrospective review of all patients undergoing this procedure. An average distraction dis

What remains of non-syndromic bicoronal synostosis?

More and more genetic syndromes are associated with bicoronal synostosis (BCS), making non-syndromic BCS (NSBCS) a shrinking entity. However, the numerical importance and clinical impact of syndromic BCS (SBCS) versus NSBCS have not been much studied. We retrospectively reviewed our experience with BCS over the last four decades in order to compare prevalence trends in SBCS and NSBCS. 195 patients were treated for BCS during the period 1978-2017: 104 (53.3%) were syndromic, 24 (12.3%) showed clinical and/or familial features suggesting a syndrome, although without final diagnostic confirmation

Oblique facial clefting associated with unicoronal synostosis.

Oblique facial clefts are said to account for 0.25% of all clefts. Craniosynostosis is not known to be associated with oblique facial clefting, although craniofrontonasal dysplasia is well described. Craniosynostosis and oblique facial clefting presenting together in the same patient is a rare event. We present such a case, and detail the treatment plan to allow simultaneous correction of both deformities.

Ultrasonic prenatal diagnosis of coronal suture synostosis.

The present study was designed to assess the utility and validity of standard prenatal ultrasonography in identifying individuals with coronal suture synostosis by examining sutural abnormalities before the development of secondary craniofacial dysmorphologies. An Accusson 128 XP ultrasound machine was used to scan the coronal sutures of 31 twenty-five-day-old fetuses (term = 31 days) from four timed-pregnant New Zealand White rabbits with simple, nonsyndromic craniosynostosis. Each mother was sedated with general anesthesia, and the gravid uterine horns were exposed via a laparotomy procedure

Triangular Osteotomies for Y-shaped Metacarpal Synostosis.

Metacarpal synostosis is a rare congenital hand anomaly characterized by abnormal fusion between adjacent metacarpals. Although most commonly involving the fourth and fifth metacarpals, atypical fusion patterns may occur. We report a rare case of Y-shaped synostosis between the third and fourth metacarpals in a 17-year-old man and describe surgical correction using 3 triangular osteotomies. Realignment and stabilization were achieved with mini plates and screws. At 1-year follow-up, correction of angular deformity and digital shortening was maintained, with satisfactory functional and cosmetic

Squamosal Suture Synostosis: An Under-Recognized Phenomenon.

INTRODUCTION: The squamosal suture (SQS) joins the temporal to the parietal bones bilaterally and is a poorly described site of craniosynostosis. SQS fusion is thought to occur as late as the fourth decade of life and beyond; however, we have incidentally noted its presence among our pediatric patients and hypothesize that it may occur earlier in life and more frequently than previously believed. METHODS: A retrospective review of imaging performed on pediatric patients was completed to identify patients with SQS synostosis. This included a review of clinical notes as well as computed tomogra

Rotation flap distraction osteogenesis for unicoronal synostosis.

Unicoronal craniosynostosis is notoriously difficult to treat, with long-term studies demonstrating high rates of relapse and the need for reoperation using open fronto-orbital advancement. Applying the principles of distraction osteogenesis to cranial vault remodeling has demonstrated promising short-term results that compare favorably with traditional methods, with simultaneous correction of both frontofacial and endocranial morphology, along with significant increases in intracranial volume. Here, the authors demonstrate their technique for rotation flap distraction osteogenesis in the trea

Cerebrospinal fluid collections in sagittal suture synostosis.

PURPOSE: The premature fusion of the sagittal suture (scaphocephaly) may be associated with intracranial cerebrospinal fluid (CSF) dynamics alterations resulting in pericerebral effusion (PE) and/or ventriculomegaly. However, the etiology and development of such collections are not fully elucidated. The aims of this study are to clarify the characteristics of intracranial CSF collection in scaphocephaly, consider the role of surgery, and analyze the prognostic factor on CSF dynamics. METHOD: The pre- and postoperative cerebral imaging of 63 children (48 boys, 15 girls) with scaphocephaly who

Three-dimensional orbital dysmorphology in metopic synostosis.

BACKGROUND: Metopic synostosis is characterized by trigonocephaly, lateral supraorbital retrusion, and hypotelorism. Most phenotypic evaluations have focused on the forehead without much emphasis on the orbits. The study seeks to explore differences in orbital dysmorphology for metopic and control patients, along with different degrees of metopic synostosis. METHODS: Demographic and craniometric data were compiled. CT scans were digitized (Materialise) and metopic and control groups were compared. Degree of trigonocephaly was classified into moderate and severe cases based on endocranial bifr
