Compression neuropathies around joints in association with synovial cyst formation are rare events. A case of a posttraumatic ulnar nerve palsy in a patient with osteoarthritis of the elbow is presented. At operative exploration of the cubital tunnel, the ulnar nerve was found to be compressed by a synovial cyst. Cyst formation should be considered in determining the etiology of compression neuropathies in patients with posttraumatic and degenerative processes occurring around the joints.

Synovial cysts of various joints and tendon sheaths (knee, elbow, hip, proximal interphalangeal joints are well known. Cysts arising from the zygapophyseal joints however are rare, but not unknown. 3 patients with lumbosciatic pain, due to nerve root compression by a synovial cyst of the zygapophyseal joints are described. The age ranged from 54 to 77 years. There are 2 men and 1 woman. The symptoms and signs involved the fourth (1 patient) and fifth (1 patient) lumbal nerve root and the first sacral nerve root (1 patient). X-rays showed degenerative changes of the facet joints in 2 cases and

The lumbar intraspinal synovial cyst is an uncommon cause of low-back pain or "sciatica." However it is important to recognize these cysts so that correct surgical management is offered to the patient. Intraspinal synovial cysts can be accurately diagnosed by computed tomography (CT). The authors report ten cases of lumbar intraspinal synovial cysts (LISC) that highlight the clinical and radiologic features.

A 72-year-old man presented with several months of increasing lumbar pain, sciatica, lower extremity weakness, numbness in his buttocks and posterior thighs, burning sensations in his scrotum, and urinary incontinence. Myelogram-computed tomography scan demonstrated a high grade incomplete block at the T12-L1 level due to bilateral synovial cysts and simultaneously a high grade partial block at L4-L5 due to spinal stenosis. Laminectomy of the T-12 vertebra and partial laminectomy of the L-1 vertebra with excision of both synovial cysts and laminectomies of the L-4 and L-5 vertebrae with forami

BACKGROUND AND PURPOSE: This study evaluates the outcomes of arthroscopic surgery for recurrent wrist synovial cysts (SCs) in patients previously treated with open or arthroscopic surgery. The objective is to determine whether prior surgery makes the procedure more challenging and if results are comparable to those in first-time patients.
MATERIALS AND METHODS: A retrospective analysis included 22 patients who underwent surgery between 2019 and 2023, with a minimum follow-up of 6 months. Diagnosis was confirmed by clinical examination and magnetic resonance imaging (MRI). The same arthroscopi

Elbow synovial cysts are rare and can mimic more frequently encountered disorders such as lateral epicondylitis, presenting diagnostic challenges. This report describes a woman in her mid-40s with persistent pain and weakness in her right forearm due to a synovial cyst compressing the radial nerve at the Arcade of Frohse. Despite initial suspicions of lateral epicondylitis, deeper investigation using sonography confirmed the presence of a compressive synovial cyst. Ultrasound-guided aspiration of the cyst was performed, yielding clear synovial fluid and providing symptomatic relief. Post-proce

INTRODUCTION AND IMPORTANCE: Synovial cysts occur more frequently in the lumbar spine, especially at L4-5 level. Traditional open surgical management with posterolateral arthrodesis is the currently recommended treatment.
CASE PRESENTATION: This is a 64-year-old woman with no prior pathological history. Initial clinical and radiological assessment concluded that there was advanced right coxarthrosis with a degenerative lumbar spine. The patient therefore had a total hip arthroplasty secondarily presented an intermittent neurogenic claudication without sphincter disorders. The MRI concluded at

Lumbar spine synovial cysts develop from degenerated zygapophyseal joints. Symptomatic patients present with radicular pain and weakness or neurogenic claudication.1 In the absence of significant concomitant degenerative spondylolisthesis, symptomatic patients can be managed with a laminectomy and microsurgical resection of the cyst, without the need for instrumented fusion.2,3 In this video, we present the microsurgical resection of a left-sided L4-5 synovial cyst in a 68-yr-old man with radicular pain refractory to conservative management. The radiographical features, relevant surgical anato

Interspinous process devices (IPDs) have been developed as less-invasive alternatives to spinal fusion with the goal of decompressing the spinal canal and preserving segmental motion. IPD implantation is proposed to treat symptoms of lumbar spinal stenosis that improve during flexion. Recent indications of IPD include lumbar facet joint syndrome, which is seen in patients with mainly low-back pain. Long-term outcomes in this subset of patients are largely unknown. The authors present a previously unreported complication of coflex (IPD) placement: the development of a large compressive lumbar s

Lumbar synovial cysts are an uncommon cause of back pain and radiculopathy, usually manifesting with gradual onset of symptoms, secondary to involvement of the spinal canal. Rarely, intracyst hemorrhage occurs, and may acutely present as radicular - or even spinal cord - compression syndrome. Synovial cysts are generally associated with degenerative facets, although the pathogenesis has not been entirely established. We report a case of bleeding complication in a synovial cyst at L2-L3, adjacent to the right interfacet joint, causing acute pain and radiculopathy in a patient on anticoagulation

Synovial cysts are well-defined fluid filled cysts that can extend for a variable distance outside the joint space. These cysts can sometimes grow to a large size making its treatment challenging especially when associated with intra-articular pathologies. We describe a case of giant synovial cyst arising from the left knee and extending to the medial proximal calf in a 77-year old female who presented with knee joint pain, episodes of locking and calf mass occurring within 2 months of onset. Imaging studies also showed associated medial and lateral meniscal tears with medial compartment and p

A 51-year-old female presented with a rare case of synovial cyst at the cruciate ligament of the odontoid process associated with atlanto-axial dislocation, manifesting as a history of headache and numbness in her left extremities for 5 months, and progressive motor weakness of her left leg. Neuroimaging studies revealed a small cystic lesion behind the dens, which severely compressed the upper cervical cord, and atlanto-axial dislocation. The cyst was successfully removed via the transcondylar approach. C-1 laminectomy and foramen magnum decompression were also performed. Posterior craniocerv

Giant synovial cysts in patients with rheumatoid arthritis are well-recognized soft-tissue masses adjacent to the knee. Cases involving the elbow, hip, and other synovial joints have been reported as well. Regardless of location, these expanding, space-occupying lesions usually present with nonspecific symptoms of swelling and pain. Less commonly, the original presentation may be related to the secondary effects of the cyst on nearby anatomic structures. We present a case of a giant synovial cyst originating posteriorly in the knee, which, rather than dissecting distally into the calf, dissect

BACKGROUND: Here, we report a thoracolumbar (TL) junction synovial cyst (SC) causing a cauda equina syndrome (CES)/epiconus syndrome (ECS).
CASE DESCRIPTION: A 69-year-old male presented with a progressive parparesis/CES. When the magnetic resonance imaging revealed a T12-L1 epiconus/TL SC, a decompression was performed, resulting in symptomatic improvement (i.e., right-sided foot drop still resolving).
CONCLUSION: ECS attributed to TL SC warrants immediate magnetic resonance evaluations and surgical decompression.

Development of synovial cysts in the rigid thoracic spine is rare. Additionally, synovial cysts with compression of nerve roots typically cause subacute or chronic radiculopathy. We present a patient who had a new diagnosis of upper thoracic (T1-2) synovial cyst that caused acute paraplegia while hospitalized for therapies and surgical planning. The patient is a 56-year-old male with a history of congestive heart failure secondary to alcoholic cardiomyopathy. He presented with a progressive bilateral lower extremity discoordination, urinary incontinence, and altered perineal sensation. His exa

BACKGROUND: Hemorrhage is a rare presentation of spinal synovial cysts, which are usually located in the lumbar spine. They may cause an epidural hematoma and compression of the cauda equina. Infrequently, they may be located in the thoracic spine.
CASE DESCRIPTION: We present a case of sudden onset paraparesis caused by hemorrhage in a synovial cyst at a midthoracic level. Clinical, radiologic, and histologic findings are described.
CONCLUSIONS: This report describes a unique case of hemorrhagic synovial cyst at the mid-thoracic spine. These lesions are rarely located in the cervico-thoraci

Ligamentum flavum is generally resected with impunity when a laminectomy is performed; it is a strong ligament and its removal may not be inconsequential. We sought to examine the consequence of resection of ligamentum flavum as it pertains to the formation of synovial cysts. Following IRB approval, we retrospectively reviewed the charts of consecutive patients who underwent a laminectomy for any diagnosis during the years 2009-2010. Exclusions were made for patients undergoing resection of a synovial cyst, laminectomy done as part of a fusion, and microdiscectomy. A total of 201 laminectomies

Cervical synovial cysts are rare entities that are, most of the time, asymptomatic. They can cause nerve root or spinal cord compression, especially when acute haemorrhage or a marked increase in size occurs. Isolated unilateral hypoglossal nerve paralysis caused by compression of its cisternal segment is also an extremely rare condition. We report the case of a 51-year-old woman who presented with dysarthria and tongue fasciculation. MRI revealed an atlantoaxial synovial cyst that extended cranially through the hypoglossal canal and compressed the fibres of the left XII nerve on its cisternal
