Atrioventricular reciprocating tachycardia involving twin atrioventricular nodes in patients with complex congenital heart disease.

INTRODUCTION: Histologic studies of autopsy specimens described the coexistence of two distinct AV nodes (so-called "Minckeberg sling" or "twin AV nodes") in specific congenital heart defects; however, the clinical electrophysiologic (EP) characteristics of twin AV nodes have not been characterized in detail. METHODS AND RESULTS: Since April 1993, a total of seven patients with complex congenital heart disease presented with AV reciprocating tachycardia suspected to be mediated by twin AV nodes. A common anatomic finding was AV discordance ([S,L,L] or [I,D,D]) with a malaligned complete AV ca

[AV reciprocating tachycardia with intermittent bundle branch block in latent Wolff-Parkinson-White syndrome].

This report deals with a patient with concealed Wolff-Parkinson-White syndrome in whom both narrow and wide QRS tachycardias were observed. The simultaneous occurrence of various QRS morphology during supraventricular tachycardia results in a challenging diagnostic ECG problem. The cycle length during tachycardia with left bundle branch block was longer than the cycle length during narrow QRS supraventricular tachycardia and with functional right bundle-branch block. Electrophysiologic studies revealed fixed V-A conduction time and increased V-A conduction during tachycardia with left bundle b

Incessant reciprocating atrioventricular tachycardia. Factors playing a role in the mechanism of the arrhythmia.

The case of a patient suffering from incessant supraventricular tachycardia is presented. The electrophysiological study showed the presence of an accessory atrioventricular (A-V) bundle with nodal-like properties and long conduction times. This structure was used as the retrograde arm of the tachycardia circuit. Tachycardia was intermittent at rest, but had a sustained character during slight exercise. Administration of atropine and isoproterenol failed to sustain the arrhythmia and spontaneous initiation during sinus rhythm was no longer observed. During handgrip exercise a sustained tachyca

Use of the esophageal lead in the diagnosis of mechanisms of reciprocating supraventricular tachycardia.

Recent studies have emphasized the role of concealed accessory pathways in reciprocating supraventricular tachycardia. Diagnosis has generally required multicatheter electrophysiologic study. We recorded esophageal electrograms during study in 16 patients with reciprocating tachycardia due to reentry using an accessory atrioventricular pathway, and in 12 patients with reciprocating tachycardia due to reentry in the AV node. The interval from onset of ventricular depolarization to earliest atrial activation (V-AMIN), earliest atrial activity on the esophageal lead (V-AESO), and high right atriu

Results of radiofrequency ablation in patients with clinically documented, but noninducible, atrioventricular node reentrant tachycardia and orthodromic atrioventricular reciprocating tachycardia.

Among 1,281 patients with symptomatic supraventricular tachycardia, 34 patients (2.7%) with presumed diagnosis of atrioventricular node reentrant tachycardia and orthodromic atrioventricular reciprocating tachycardia did not have inducible tachycardia in the electrophysiologic laboratory. Application of radiofrequency energy to the presumed arrhythmogenic sites could achieve a high success rate, with a low recurrence rate in these patients.

Role of extrastimulus site and tachycardia cycle length in inducibility of atrial preexcitation by premature ventricular stimulation during reciprocating tachycardia.

Preexcitation of the atria during reciprocating tachycardia (RT) by a premature ventricular complex occurring when the His bundle is refractory provides direct evidence of the presence of accessory atrioventricular (AV) connection. The impact of ventricular stimulation site and RT cycle length on inducibility of atrial preexcitation was assessed in 38 patients with RT utilizing a single accessory AV connection (right free wall in 5 patients, left free wall in 21 and posterior septal/paraseptal in 12). Extrastimuli were inserted at right ventricular (RV) apical, left ventricular (LV) septal and

[Effectiveness and pharmacodynamics of tobanum in patients with paroxysmal reciprocal atrioventricular tachycardia].

The antiarrhythmic effects and pharmacodynamics of tobanum were evaluated in 28 patients wist paroxysms of reciprocal atrioventricular tachycardia, by using transesophageal cardiac pacing. The agent given in a single dose of 10 mg produced antiarrhythmic effects in 70% patients with paroxysms of atrioventricular nodal tachycardia and in 40% with atrioventricular tachycardia in the presence of the Wolff--Parkinson-White syndrome. After administration of the drug, its antiarrhythmic effect occurred on an average 1.5 h and retained for 27 h.

[Functional state of atrioventricular conduction and inducibility of atrioventricular reciprocal tachycardia].

Programmed transesophageal electrical stimulation was used to examine 733 rural inhabitants aged 16 to 70 years. Conduction dissociation along the atrioventricular node was revealed in 65 (8.8%) of the examinees, out of them 12 had induced paroxysms of atrioventricular nodal tachycardia. Atrioventricular tachycardia involving the accessory anomalous pathways was provoked in 10 subjects. Among them 2 had apparent, 1, latent, and 7, obscure forms of the Wolff-Parkinson-White syndrome. The induced paroxysms of atrioventricular tachycardias were sustained in 45.5% of the cases. Only 50% of the sub

Reciprocal tachycardias using accessory pathways with long conduction times.

Three patients with reentrant tachycardia are described who had an accessory pathway with a very long conduction time that was incorporated in the tachycardia circuit. The accessory pathway was able to conduct in one direction only, in retrograde manner in two patients and in anteriograde manner in the remaining patient. Evidence is presented that reveals that in the first two patients the accessory pathway was septally located, had completely bypassed the normal atrioventricular (A-V) conduction system, had properties of decremental conduction, and had an atrial exit close to the coronary sin

Second degree block during reciprocal atrioventricular nodal tachycardia.

Of 67 patients with reciprocal atrioventricular (A-V) nodal tachycardia consecutively studied by programmed electrical stimulation of the heart, nine patients showed second degree block toward the ventricle and one patient toward the atrium during tachycardia. In four patients the occurrence of block was critically related to the prematurity of the test stimulus initiating the tachycardia. In three patients block developed following increase in rate of tachycardia. In two patients block could be elicited by introducing premature ventricular stimuli during tachycardia. Our observations indicate

"V-H-A Pattern" as a criterion for the differential diagnosis of atypical AV nodal reentrant tachycardia from AV reciprocating tachycardia.

BACKGROUND: During ventricular extrastimulation, His bundle potential (H) following ventricular (V) and followed by atrial potentials (A), i.e., V-H-A, is observed in the His bundle electrogram when ventriculo-atrial (VA) conduction occurs via the normal conduction system. We examined the diagnostic value of V-H-A for atypical form of atrioventricular nodal reentrant tachycardia (AVNRT), which showed the earliest atrial activation site at the posterior paraseptal region during the tachycardia. METHODS: We prospectively examined the response of VA conduction to ventricular extrastimulation dur

Severe cardiomyopathy due to permanent junctional reciprocating tachycardia: recovery after catheter ablation?-a case report.

BACKGROUND: Permanent junctional reciprocating tachycardia (PJRT) is a rare form of orthodromic atrioventricular reciprocating tachycardia involving a slowly conducting, decremental concealed accessory pathway. Its incessant nature may result in tachycardia-induced cardiomyopathy, which is reversible with timely rhythm control. CASE SUMMARY: A 41-year-old patient presented with progressive exertional dyspnoea and reduced exercise capacity. Continuous electrocardiogram monitoring revealed an incessant long RP narrow QRS tachycardia (heart rate 135 b.p.m.). Echocardiography demonstrated severe

Utility of Pre-Induction Ventriculoatrial Response to Adenosine in the Diagnosis of Orthodromic Reciprocating Tachycardia.

OBJECTIVES: This study sought to evaluate the utility of ventriculoatrial (VA) conduction patterns in response to adenosine in predicting inducibility of orthodromic reciprocating tachycardia (ORT). BACKGROUND: Adenosine is known to consistently block atrioventricular (AV) nodal conduction. We hypothesized that persistent VA conduction despite administration of adenosine would have a high predictive value for identifying the presence of a retrograde accessory pathway (AP) and associated ORT. METHODS: A total of 168 patients undergoing electrophysiological study for supraventricular tachycard

Cryoenergy catheter ablation: a new technique for treatment of permanent junctional reciprocating tachycardia in children.

INTRODUCTION: Permanent junctional reciprocating tachycardia (PJRT) is an infrequent form of reciprocating tachycardia, almost incessant from childhood and usually refractory to drug therapy. Radiofrequency catheter ablation currently is the first-line therapy for PJRT, but its application in the septal region may be associated with complications. In contrast, cryoenergy has several advantages, such as the ability to test the effects of ablation while the lesion is still forming, thus reducing the number of ineffective, useless, and potentially harmful lesions. The aim of this study was to inv

Outcomes of radiofrequency catheter ablation of atrioventricular reciprocating tachycardia in patients with congenital heart disease.

OBJECTIVES: Assessment of clinical outcomes of catheter ablation of atrioventricular reciprocating tachycardias in patients with congenital heart disease (CHD). BACKGROUND: Atrioventricular reciprocating tachycardias occur in patients with CHD and may be poorly tolerated. METHODS: Retrospective review of all 105 such ablations in 83 patients performed between 03/90 and 02/02 at one institution. RESULTS: The dominant arrhythmia mechanism was accessory pathway (70 patients, 84%), and the most common indications were drug-refractory tachycardia, life-threatening arrhythmia, and elective presur
